Prediction of Hypertension Complications Risk Using Classification Techniques

  • Chronic diseases including hypertension and its complications are major sources causing the national medical expenditures to increase. We aim to predict the risk of hypertension complications for hypertension patients, using the sample national healthcare database established by Korean National Health Insurance Corporation. We apply classification techniques, such as logistic regression, linear discriminant analysis, and classification and regression tree to predict the hypertension complication onset event for each patient. The performance of these three methods is compared in terms of accuracy, sensitivity and specificity. The result shows that these methods seem to perform similarly although the logistic regression performs marginally better than the others.

  • Purpose: The purpose of this study was to examine self care behaviors according to the risk levels of vascular complications in elderly women with hypertension. Methods: The subjects of this study were 162 women living in the community who had been diagnosed with hypertension in clinics. The data were analysed by the SPSS 10.0 program using descriptive statistics, t-test, and ANOVA with Scheff$\dot{e}$ post-hoc test. Results: The average self care behavior score of the elderly women was 2.79. Total self care behavior was significantly different depending on the risk levels of vascular complications such as systolic blood pressure, diastolic blood pressure, ankle-brachial index, and the framingham point score. Among sub-scores of self care behaviors, exercise management was the poorest performance compared with other self care behaviors. Conclusion: This study proved the differences in self care behaviors according to the risk levels of vascular complications. To decrease the prevalence of vascular complication, it is necessary to develop programs specifically to enhance self care behaviors of elderly women with hypertension.

Effects of Comorbid Sleep Disorders on Cardiovascular Complications of Hypertension Among Patients With Newly-diagnosed Hypertension: An Analysis of the Korean National Health Insurance Service-National Sample Cohort

  • Objectives: This study investigated the effects of comorbid sleep disorders (SD) on the incidence of cardiovascular complications among newly-diagnosed hypertension (HTN) patients. Methods: As study population, 124 057 newly-diagnosed essential HTN patients aged 30 or older, without cardiovascular complications at diagnosis with HTN, were selected from the National Health Insurance Service-National Sample Cohort. The incidence of cardiovascular complications was calculated, Cox proportional-hazards regression model was used to analyze the risk of complications, and the population attributable fraction (PAF) for cardiovascular complications of having comorbid SD at HTN diagnosis was calculated. Results: Over 10 years, 32 275 patients (26.0%) developed cardiovascular complications. In HTN patients with comorbid SD at diagnosis of HTN, the incidence of cardiovascular complications (78.3/1000 person-years; 95% confidence interval [CI], 75.8 to 80.9) was higher than in those without comorbid SD (58.6/1000 person-years; 95% CI, 57.9 to 59.3) and the risk of cardiovascular complications was 1.21 times higher (95% CI, 1.17 to 1.25), adjusting for age, gender, income, area of residence, and comorbid diabetes mellitus. The PAF of having comorbid SD at diagnosis of HTN for the incidence of cardiovascular complications was 2.07% (95% CI, 1.69 to 2.44). Conclusions: Newly-diagnosed essential HTN patients aged 30 or older who had comorbid SD at the time of their HTN diagnosis had a higher incidence of cardiovascular complications than those without comorbid SD. Age, gender, income, area of residence, and comorbid diabetes mellitus had a significant effect on the incidence of cardiovascular complications. Approximately 2% of cardiovascular complications were found to occur due to the presence of SD.

Clinical Safety of Chemotherapy for Elderly Cancer Patients Complicated with Hypertension

  • Objective: To access the safety of chemotherapy for elderly cancer patients complicated with hypertension. Methods: Elderly cancer patients who were complicated with hypertension and treated by chemotherapy were recruited. All patients were treated by chemotherapy after an intervention on hypertension by psychotherapy, exercise guidance, salt regulation and nutrition support, therapy on hypertension, as well as prevention on hypertension associated complications. Results: In 68 eligible patients, two suspended chemotherapy because of adverse reactions and 4 because of disease progression. The remaining 62 patients completed chemotherapy smoothly based on good hypertension control. Conclusion: With effective control of blood pressure, chemotherapy for elderly cancer patients complicated with hypertension is generally safe.

Acute cardiovascular complications in patients with diabetes and hypertension: management consideration for minor oral surgery

  • Objectives: Medically compromised patients often fear required dental surgical procedures that can increase the risk of medical emergency when combined with reduced tolerance for stress. A stress reduction protocol (SRP) helps doctors minimize treatment-related stress and improves patient management with minimum complications. Diabetes and co-morbid hypertension carry 4-fold risk of aggravation of cardiovascular emergencies and 7.2-fold risk of mortality. Diabetic neuropathy can result in difficult diagnosis of myocardial infarction and reduces chances of surviving a myocardial infarction compared with a non-diabetic person. The aim of the study was to assess the feasibility of a protocol for management of patients having both diabetes and hypertension who required minor oral surgery to minimize the rate of cardiovascular emergencies. Materials and Methods: A prospective study was conducted in 140 patients having both diabetes and hypertension who required minor oral surgical procedures. A systematic approachable protocol was designed for management of such patients. Results: Among 140 patients, 6 patients (4.3%) had cardiovascular complications, while 3 patients (1 with syncope and 2 with hypertension) did not require any intervention other than observation. Two patients were managed with aspirin and nitroglycerin, and 1 patient had possible myocardial infarction (overall incidence 0.7%) with chest pain, S-T segment elevation on electrocardiogram, and troponin level of 0.60 ng/mL. Conclusion: The proposed protocol helps to improve management of patients having both diabetes and hypertension. We recommend that patients with uncontrolled diabetes and uncontrolled hypertension and/or patients having history of cardiovascular complication should be treated in a medical facility with a readily available cardiology unit. This facilitates prompt response to emergency and instant implementation of treatment, helping to reduce morbidity and mortality.

신규 고혈압 환자의 치료지속성과 고혈압 조절 여부가 심뇌혈관질환 합병증에 미치는 영향 (The Effects of Adherence and Hypertension Control on Complication among Newly Diagnosed Hypertension Patients)

  • Background: This study aimed to research on how adherence and blood control could make a difference when it comes to develop complications. Methods: The study's subjects were 255,916 patients who were newly diagnosed with hypertension in 2009 using data collected by National Health Insurance Cooperation. Patients are considered as a group under adherence if visit days and prescription days are more than 300 days. Patients are considered to have successfully controled their hypertension based on actual value measured by National Health Insurance Cooperation and the study takes a look at whether they were diagnosed with complications of cerebrocardiovascular disease in 2012. Chi-square test and logistic regression was used to analyze. Results: Patients who were able to control their hypertension show 0.80 times chance of developing cerebrovascular disease, and 0.89 times chance of developing cardiocerebrovascular disease. The group of adherence shows lower chance of developing complication in general than the group of non-adherence. Conclusion: The study revealed that hypertension's constant treatment could control the blood pressure and prevent complications. It is important that encourages patients to effort for persistent treatment for reducing complication.

병원에 입원한 고혈압 환자에 대한 조사연구(I) (Clinical Review of Hypertensive Patients Admitted in the Hospital)

  • As the seriousness of hypertension in adults is increasingly recognized, attention is being focussed on its control through continuous health supervisions. Complications arising from high blood pressure exceed those of many other adult health problems and affect the nursing needs of these patients. In order to contribute to effective nursing care planning and improved health supervision, 248 medical records of hypertensive patients admitted to Internal Medicine at Ewha Medical Center from January 1971 to August 1976 were examined. Results were as fellows: 1. The sample group averaged 5.0% of the total number of patients admitted to internal medicine during the period studied. The proportion increased slightly with each successive year. 2. Patients with hypertension averaged 31.4% of all patients with cardiovascular discease. This proportion was stable over the period. 3. Hypertensive patients were primarily in their 40s. The average age was 55.1 years. 4. Women were most frequently affected in their 60s and men in their 40s. 5. The bloo pressure of these in their 30s was not well controlled on discharge. Rates for those in their 30s averaged 151.5/107mmhg, for those in their 40s 161.5/110mphg, and for those in their 60s 169/100mmhg. 6. Complications increased with each age range from the 40s to the 60s for both men and women. 7. Complications related to systolic hypertension occurred most frequently when the rate exceeded 200mmhg. Cerebral vascular accident was the primary complication, presenting implications for nursing intervention. 8. Complications related to diastolic hypertension occurred most frequently when the rate was in the $110{\sim}129mmhg$ range. C.V.A. again was the primary complication needing nursing care planning. 9. Diets ordered for patients were low salt 79.%, salt free 4.19%, diabetic 6.1%, and protein free ('no protein') 2.0%. Caloric, salt end protein levels were not defined. Recommendations based on the findings were directed to diet and other therapies.

이침시술이 혈압에 미치는 영향 (The Effect of Auricular Acupuncture Therapy on Blood Pressure)

  • Objectives : Hypertension is very prevalent disease, and causes serious cardiovascular complications. Nowadays optimal hypertension treatment is emphasized to reduce the incidence of cardiovascular complications. Auricular acupuncture therapy is economical, safe and effective. Its clinical application is various, and it can be used to control blood pressure. So The effect of auricular acupuncture therapy was investigated. Daily variation of blood pressure during needle-embedded period and variation of blood pressure by blood pressure classification were observed. Methods : The auricular acupuncture points we used were Gangapjeom (降壓點), Sinmun (神門), Gyogam (交感), Sim(心), and Icheom (耳尖). Auricular acupuncture needles were embedded for three days. Blood pressure was checked four times per day and the mean obtained. Results : The following results were obtained : 1. During needle-embedded period, systolic and diastolic blood pressure decreased. Blood pressure decreased most significantly 2 days after treatment. 2. The change of blood pressure in the higher blood pressure group was more remarkable than that of the lower blood pressure group. Conclusions : Through this research, auricular acupuncture therapy is considered as an effective and safe method to lower blood pressure.

  • 고혈압은 소아 청소년시기에 발생하는 흔한 만성질환 중 하나이다. 고혈압을 치료하지 않으면 여러가지 소아의 장기에 악영향을 미칠 수 있다. 여기서는 고혈압이 중추신경계에 미치는 영향에 대해 알아 보았다. 혈압의 변화에 따라 대뇌가 받는 영향은 다른 장기에 비해 덜 하다. 그것은 대뇌 자동 조절 기전때문이다. 즉 혈압의 변화에 따라 뇌혈관의 저항이 변하기 때문에 뇌혈류량이 일정하게 유지 된다. 그러나 이런 자동 조절 기전의 항상성이 파괴되면 급성으로 또는 만성으로 급성 고혈압 뇌병증, 허혈성, 출혈성 뇌경색, 학습 장애와 인지 기능 장애 등이 발생한다. 이에 고혈압이 대뇌에 미치는 영향에 대해 숙지하고 빠른 시일 내에 적절한 치료를 하면 이런 합병증을 예방할 수 있다.
