Comparisons of BMI, Blood Lipid Profile, Blood Pressure and Nutrient Intakes by Serum Insulin Concentrations (정상 성인의 혈청 인슐린 농도에 따른 비만지수, 혈청 지질, 혈압 및 영양소 섭취상태 비교)

  • This study was conducted to figure out the differences of the BMI, blood lipids, blood pressure and nutrient intakes by serum insulin concentration among adults in Korea. Ninety-three subjects(male : n=37, female : n=56) participated in this study. The subjects were grouped by serum insulin concentrations into two groups-hyperinsulinemia group(n=17) and normoinsulinemia group(n=76). Anthropometric measurements, blood lipid profiles, blood pressure, oral glucose tolerance test, and daily nutrient intakes were analyzed. And serum glucose and insulin secretion pattern as shown through and oral glucose tolerance test were performed. BMI (p<0.05) and WHR(p<0.01) were significantly higher in hyperinsulinemic group than in normoinsulinemia group. There was no differences in fasting blood glucose level between groups, but total glucose area (p<0.001) and insulin glucose resistance(p<0.001) were significantly higher in hyperinsulinemia group than in normoinsulinemia higher in hyperinsulinemia group, but HDL-cholesterol(p<0.05), and systolic blood pressure(p<0.05) were significantly higher in hyperinsulinemia group, but HDL-cholesterol(p<0.001) was significantly lower in hyperinsulinemia group. They showed significant differences in energy, carbohydrate, potassium, riboflavin, niacin and dietary fiber intakes(p<0.05), the intake of those nutrients were low in normoinsulinemia group. Further investigation is necessary to determine the effects of amounts and types of carbohydrate and dietary fiber on serum insulin concentrations.

Effect of Silkworm Extract on Intestinal $\alpha$-Glycosidase Activity in Mice Administered with a High Carbohydrate-containing Diet (고탄수화물 식이투여 마우스에서 누에추출물이 소장내의 $\alpha$-glycosidase 활성에 미치는 영향)

  • The effects of silkworm extract on body weight, blood glucose and insulin levels, and an intestinal glycosidase activity were studied in high carbohydrate diet administered mice. Hyperglycemia and hyperinsulinemia represented in high carbohydrate diet administered mice disappeared when silkworm extract(50 mg) was added to 100 g diet for 10 week period. These results suggest that silkworm extract may resolve the insulin resistance by lowering serum insulin level resulting from inhibition of intestinal glycosidase activity. Silkworm extract coadministered with high carbohydrate diet to mice for 10 weeks significantly induced maltase, sucrase and lactase acivities, especially middle and distal portion. Further studies about regulatory mechanisms of glycosidase by silkworm extract are needed.

  • 비만 특히 복부비만은 여성의 임신 및 출산에 많은 악영향을 미친다. 비만이 가임력에 영향을 미치는 기전은 명확히 밝혀지지는 않았지만, functional hyperandrogenism과 insulin resistance에 수반된 hyperinsulinemia가 중요한 역할을 하리라 여겨진다. 또한 지방조직은 활발하게 대사 작용이 일어나는 장소로서 steroid hormone을 modification 시키고, 여러 가지 adipokine들을 분비시키는데 이런 지방조직의 분비물들은 생식계에 영향을 미친다. 비만이 생식계에 미치는 영향은 간략히 다음 표와 같이 정리할 수 있다 (Table 3). 특히 PCOS 환자에서 비만은 hyperinsulinemia에 의한 난소의 steroidogenesis 증가와 SHBG의 감소에 연관되어 hyperandrogenism 과 무배란의 원인으로 작용한다. 비만은 약물에 의한 배란유도 및 체외수정의 성공률 또한 떨어뜨리며, 임신이 되더라도 자연 유산, 조산, 임신성 당뇨, 임신성 고혈압 등의 위험은 높인다. 이와 같이 최근 급격히 증가하는 비만은 여성의 건강 특히 생식계에 많은 문제를 야기시킨다. 특히 청소년기의 비만은 성인이 된 뒤 PCOS로 발전하여 불임, 심혈관 질환 등과 같은 많은 합병증을 일으킬 수 있으므로 세심한 주의를 기울여야 하겠다.

Insulin Resistance in Obese and Non-obese Patients with Polycystic Ovarian Syndrome (다낭성 난소 증후군 환자에서 비만군과 비비만군에서의 인슐린 저항성)

  • This study was designed to investigate the relationship between insulin resistance and obesity in the pathogenesis of polycystic ovarian syndrome(PCO). Twenty-two women with PCO, of whom thirteen were non-obese with body mass index(BMI, kg/$m^2$) of <25 and nine were obese with BMI${\geq}$25 were studied. Eight non-obese control women and seven obese control women were studied. Serum concentrations of testosterone, lutenizing hormone(LH)/follicle-stimulating hormone(FSH) ratio, and insulin-like growth factor I (IGF-I) were found to be significantly higher(P<0.05) in PCO women compared with control women, which clearly is not related to obesity. Serum glucose, insulin, and C-peptide levels were measured during a 2-hour oral glucose tolerance test(OGTT). Non-obese and obese women with PCO both(P<0.05) compared with control women demonstrated significant hyperinsulinemia after OGTT. The degree of hyperinsulinemia was found to be significantly higher in the obese women with PCO compared with the non-obese women with PCO. We concluded that obesity may contribute to hyperinsulinemia, however may not playa central role in the pathogenesis of PCO.

Role of Hyperinsulinemia in Increased Risk of Prostate Cancer: A Case Control Study from Kathmandu Valley

  • Aim: To investigate the effect of hyperglycemia and hyperinsulinemia on prostate cancer risk. Materials and Methods: This hospital based study was carried out using data retrieved from the register maintained in the Department of Biochemistry of a tertiary care hospital of Kathmandu, Nepal between $31^{st}$ December, 2011 and $31^{st}$ October, 2013. The variables collected were age, serum cholesterol, serum calcium, PSA, fasting blood glucose, serum insulin. Analysis was performed by descriptive statistics and testing of hypothesis using Excel 2003, R 2.8.0, Statistical Package for the Social Sciences (SPSS) for Windows Version 16.0 (SPSS Inc; Chicago, IL, USA) and the EPI Info 3.5.1 Windows Version. Results: Of the total 125 subjects enrolled in our present study, 25 cases were of PCa and 100 were healthy controls. The mean value of fasting plasma glucose was 95.5 mg/dl in cases of prostatic carcinoma and the mean value of fasting plasma insulin was $5.78{\mu}U/ml$ (p value: 0.0001*). The fasting insulin levels ${\mu}U/ml$ were categorized into the different ranges starting from ${\leq}2.75$, >2.75 to ${\leq}4.10$, >4.10 to ${\leq}6.10$, > $6.10{\mu}U/ml$. The maximum number of cases of prostatic carcinoma of fasting insulin levels falls in range of > $6.10{\mu}U/ml$. The highest insulin levels (> $6.10{\mu}U/ml$) were seen to be associated with an 2.55 fold risk of prostatic carcinoma when compared with fasting insulin levels of (< $2.75{\mu}U/ml$). Conclusions: Elevated fasting levels of serum insulin appear to be associated with a higher risk of prostate cancer.

Effect of Persistant Mild Hyperglycemic Hyperinsulinemia on Development of Insulin Resistance in Rats (지속적인 경한 고혈당과 고인슐린증이 인슐린 저항성의 발생에 미치는 영향)

  • The effect of persistant mild hyperglycemic hyperinsulinemia on the development of the insulin resistance in rats was studied in vivo. Also, the characteristics of the insulin resistance compared with the insulin resistance of STZ diabetic rats. Persistant mild hyperglycemic hyperinsulinemic rat model was produced by ingestion of glucose polymer for 8 days. The glucose disappearance and infusion rate was measured by hyperinsulinemic euglycemic clamp technique at steady state of blood glucose and insulin levels. The clamped level of blood glucose was 100 mg/dl, and the clamped levels of insulin were $70{\mu}U/ml$ (physiologic condition) and $3000{\mu}U/ml$ (supramaximal condition). Hepatic glucose producticon rate was calculated using measured data. And the glycogen synthetic capacity of skeletal muscle(soleus) and liver was measured after 2 hours of hyperinsulinemic euglycemic clamp study. The glucose disappearance and glucose infusion rate in glucose polymer group was decreased in the both physiological and supramaximal insulin level compared to the rate of the normal control group. The rate of STZ diabetic group wase lowest at supramaximal insulin level among two another experimental groups. The hepatic glucose production rate of glucose polymer group was decreased compared to normal control but increased in STZ diabetic group. The glycogen synthetic capacity of skeletal muscle and liver of glucose polymer group was not significantly different from normal control group, but it was markdly decreased in STZ diabetic group. These results suggest that persistant mild hyperglycemic hyperinsulinemia may induce insulin resistance, but glycogen synthetic capacity is intact.

  • Visceral fat accumulation is known to be an evident clinical index for the insulin resistance related with obesity. Patients with excessive accumulation of visceral fat frequently suffered from metabolic disorder, such as hyperlipidemia, hypertension, and glucose intolerance. However, molecular mechanism for the pathogenesis of obesity-accompanied metabolic disorders has not been fully elucidated. It has been clarified that adipocytes in visceral fat area have different functions from subcutaneous fat area, and these differences might contribute the pathological significance of excessive accumulation of visceral fat for the accompanied insulin resistance and hyperinsulinemia.

Characteristics of the Anthropometry and Plasma Lipid Profiles of Patients with Simple and Combined Hypercholesterolemia (단순형과 혼합형 고콜레스테롤혈증 환자의 체위 및 혈장지질 특성)

  • Recent epidemiologic and clinical students have shown that plasma cholesterol and triglyceride levels are independent risk factors for coronary heart disease. However, there is not much data on the characteristics of anthropometry and lipid profiles of hypercholesterolemia patients with hypertiglyceridemia. In this study, 112 hypercholesterolemic subjects$(T.C{\ge}240mg/dl)$ were divided into two groups by their plasma triglyceride levels. We compared the anthropometric measurements and lipid profiles of the subjects between the two groups : the simples hypercholesterolemic group(SHC, TG<200mg/dl) and the combined hypercholesterolemic group$(CHC, TG{\ge}250mg/dl)$. The distribution of the subjects into the SHC and CHC groups was 36.6% and 47.3%, respectively. The frequency of the CHC patients decreased with age. The subjects in this group had higher weight, BMI, HWR, cricumferences of mid arm, waist, hip and thigh, and skinfold thicknesses of biceps and triceps than those of the SHC subjects. The difference of plasma total cholesterol level was mainly due to the difference of VLDL-C levels. These differences resulted in the CHC subjects having higher atherogenic indexes and T-C/HDL-C ratios than those of the SHC subjects. Also, the former had higher Apo-B and insulin levels than those the latter. However, blood pressure, fasting blood glucose and HDL-C levels were not significantly different between the two groups. These results suggest that hypercholesterolemic patients with hypertriglyceridemia have riskier lipid profiles for CHD than those of patients with normal triglyceridemia. They also indicate that CHC is closely associated with glucose resistance syndrome(obesity, hyperglycemia, hyperinsulinemia and hypertriglyceridemia), and more prevalent in young people.

Insulin Receptor Substrate Proteins and Diabetes

  • The discovery of insulin receptor substrate (IRS) proteins and their role to link cell surface receptors to the intracellular signaling cascades is a key step to understanding insulin and insulin-like growth factor (IGF) action. Moreover, IRS-proteins coordinate signals from the insulin and IGF receptor tyrosine kinases with those generated by proinflammatory cytokines and nutrients. The IRS2-branch of the insulin/IGF signaling cascade has an important role in both peripheral insulin response and pancreatic $\beta$-cell growth and function. Dysregulation of IRS2 signaling in mice causes the failure of compensatory hyperinsulinemia during peripheral insulin resistance. IRS protein signaling is down regulated by serine phosphorylation or protea-some-mediated degradation, which might be an important mechanism of insulin resistance during acute injury and infection, or chronic stress associated with aging or obesity. Under-standing the regulation and signaling by IRS1 and IRS2 in cell growth, metabolism and survival will reveal new strategies to prevent or cure diabetes and other metabolic diseases.

  • Non-insulin dependent diabetes mellitus (NIDDM) is characterized by hyperglycemia, hyperinsulinemia. and impaired insulin action. Insulin resistance is considered to be the underlying mechanism in the pathogenesis of type 2 diabetes. which also leads to dyslipidemia, hypertension. and obesity. Thazolidinediones are a class of oral insulin-sensitizing agents that improve glucose utilization without increasing insulin release. They significantly reduce glucose, lipid and insulin levels in rodent models of NIDDM and obesity, and recent clinical data support theri efficacy in obese diabetic patients. (omitted)
