Delaire 변법을 이용한 편측 불완전 구순열 환자의 기능적 일차 봉합술: 증례보고 (Functional Primary Closure of Unilateral Imcomplete Cleft Lip by Modified Delaire's Technique: Report of a case)

  • The outcome of primary surgery for cleft lip is judged by its effects on the quality of oro-facial function and development. Many surgical techniques have been tried to obtain better results, however, Delaire introduced a technique of functional closure of the lip and nose, based on the findings of no true hypoplasia in the tissues either side of the cleft. In a seven-month-old Asian male patient with unilateral incomplete cleft lip, we carried out the primary closure by modified Delaire's technique. With no alveolar bone graft, the vertical incision on the nasal base was omitted in this patient because of his acceptable symmetry of nose. Also, a small Z plasty was added on the non-cleft side. The V-shaped incisions, whose notch was located on each side of the red vermilion, were designed and beveled incisions were performed for the rehabilitation of lip length and thickness, considering the postoperative wound contracture. We assured that this modification of Delaire's technique could be applied for various cases of primary closure of incomplete cleft lip.

SWAT 모형을 이용한 낙동강 유역의 장기 유출에 따른 유사량 분석 (Analysis of Sediment Discharge by Long-term Runoff in Nakdong River Watershed using SWAT Model)

  • Sediment discharge by long-term runoff in the Nakdong River watershed should be predicted for the maintenance and management of the Nakdong River newly changed by the four major river restoration project. The data establishment by the analysis of runoff and sediment discharge using the long-term watershed model is necessary to predict possible problems by incoming sediments and to prepare countermeasures for the maintenance and management. Therefore, sediment discharges by long-term runoff in the main points of the Nakdong River were calculated using SWAT(soil and water assessment tool) model and the relations and features between rainfall, runoff, and sediment discharge were analyzed in this study. As a result of sediment discharge calculation in the main points of the Nakdong River and tributaries, the sediment discharge at the outlet of the Naesung Stream was greater than the Jindong Station in the Lower Nakdong River from 1999 to 2008 except the years with low precipitation. The sediment discharge at the Nakdong River Estuary Barrage (NREB) was corresponding to 20% of the Jindong Station which is located about 80 km upstream from NREB.

Optimum Soil Incorporation Time of Chinese Milk Vetch(Astragalus sinicus L) for its Natural Re-seeding and Green Manuring of Rice in Gyeongnam Province, Korea

  • To develop a natural re-seeding technology, the optimum soil incorporation time of Chinese milk vetch(CMV) plant residues, seed persistence in soil, biomass production and subsequent rice yield were determined from 2005 to 2007 in rice with CMV green manure in southern Korea. Generally, insufficient seedling stand was regenerated with CMV incorporation to the soil at 20 and 25 days after flowering(DAF), while high regeneration of greater than 400 seedlings per $m^2$ was observed at 35 and 40 DAF. High re-seeding stand of CMV incorporated at 35 DAF or later was related with high seed viability and heavy seed weight. Appreciable number of CMV seeds remained 4 month after burial in soil and a good number of CMV seedlings regenerated from this seed bank at rice harvest time in the fall. Based on the relationships among re-seeding number of CMV plants, seed weight and seed viability, CMV plants should be incorporated into the soil 35 DAF(May 30) or later when CMV seeds were sufficiently matured. The natural re-seeding stand for the 3-year trials was stable ranging from 437 to 700 plants per $m^2$ and the biomass production was sufficient to supply nitrogen for rice growth. The use of re-seeding CMV plant can produce similar rice yield like that of rice without CMV green manure.

Physical Dormancy in Seeds of Chinese Milk Vetch (Astragalus sinicus L.) from Korea

  • Freshly harvested seed of Chinese milk vetch (Astragalus sinicus L.; CMV) was strongly dormant because of hardseedness. Seeds of freshly harvested germinated only 8% while clipping the seed coat completely overcome the innate dormancy, which indicates inhibition of germination of the seed is mainly due to seed coat (87%). The dormant (intact) hard seeds did not imbibe water whereas the non-dormant (clipped) seeds took up rapidly. In natural environment condition, the hard seed coat dormancy was broken only after 5 months after seed harvest. To break such a strong seed coat dormancy, the chemical and heat treatments were effective. Concentrated sulfuric acid was more effective than dry heat and hot water treatments. Hot water treatment improved germination but the germination percentage was less than 41%. Treatments increased germination due to its effect on the seed coat integrity. A scanning electron microscope reveled that disruption of seed coat layers and subsequent development of numerous crack in the hilum region of the seed and on the seed coat surface of concentrated sulfuric acid treatment and formation of cracks in the dry heat treatments, respectively, were observed in the seed coat surface, which served as water entry points.

  • To make uniform pressure distributed over the contact surface was necessary to cold pad batch dyeing machine. In this study, to confirm characteristic of flexibility and the contact pressure distribution through experimental analysis of padder roll were controlled by hydraulic cell. When there were no load pressure only inner pressure, the value of displacement in the center of padder were greater than the end of the padder. The results of this study showed that the padder had the optimum value of inner pressure for uniform contact pressure distribution. Measuring the contact pressure in a padder system were driven by using a pre-scale film. Uniform contact pressure distribution of cell padder were a linearly with load pressure and inner pressure. When the load pressure was less than 8 tons, the inner pressure for the uniform contact pressure was 1~4 bar. The padder roll performance curves proposed in this study were available for practical production environments and various roll designs.

CCHE2D모형을 이용한 보 상류에 퇴적된 토사의 배출 효과 분석 - 상주보와 구미보를 중심으로 - (Numerical Analysis of the Sediment Pass-Through from the Sangju Weir and the Gumi Weir by using CCHE2D)

  • In this study, sediment pass-through (SPT) from the Sangju Weir and Gumi Weir in the Nakdong River is analyzed numerically using CCHE2D. Sediment delivery is analyzed to predict the sediment discharge rate in the Sangju Weir and Gumi Weir. The sediment discharge from the Sanju Weir is 4,381 tons in the inflow of $3,857m^3/s$, which are 2 year frequency discharge, for 48 hours, and the sediment discharge is 2,766 tons in the inflow of $1,500m^3/s$. The maximum sediment delivery occurs in the immediate downstream of the weir. The sediment discharge from the Gumi Weir is 3,216 tons in the inflow of $5,400m^3/s$, which are 2 year frequency discharge, for 48 hours, and the sediment discharge is 73 tons in the inflow of $3,857m^3/s$. The maximum sediment delivery occurs in the weir. Therefore, the effectiveness of sediment delivery is high in the condition of the 2 year frequency discharge and the gate in the weirs fully are opened.

댐 하류 관측 및 모의 유출량의 비교 분석 (Analysis of Observed And Simulated Runoff on Downstream of Reservoir)

  • 본 연구에서는 댐 하류지역의 물관리를 위한 기초자료를 제공하기 위해 금강수계의 댐 하류지역인 논산지점을 대상으로 기 개발된 수위-유량 곡선식에 의해 추정된 유출량과 RRFS에 의해 추정된 유출량을 실제 분석하고자하는 시점에서 도보측정에 의해 측정된 유출량과 비교하였다. 이러한 유출분석을 위해 유출분석의 기초자료인 논산유역의 면적강우량을 산정하고자 8개의 강우관측지점을 선정하고 티센계수를 구하였다. 각 방법별 유출량의 비교를 위해 도보측정에 의한 유출량은 2005년부터 2006년까지 각각 39회와 40회에 걸쳐 추정되었고 기 개발된 수위-유량곡선식에 의한 유출량은 국가수자원관리 종합시스템에서 제공하는 수위-유량곡선식을 사용하여 추정되었으며 RRFS에 의한 유출량은 매개변수의 민감도분석과 보정에 의해 추정되었다. 이들 방법에 의한 유출량 중에서 도보측정에 의한 유출량을 참값으로 가정하고 수위-유량곡선식에 의한 유출량과 RRFS에 의한 유출량, 각각에 대한 상대평균제곱근오차와 상대절대오차를 구하였다. 이러한 오차분석 결과에서 2005년과 2006년 모두 RRFS에 의한 유출량이 기 개발된 수위-유량 곡선식에 의한 유출량보다 도보 측정에 의한 유출량에 더 근접하는 좋은 결과를 나타내었다. 따라서 향후 댐 하류 주요지점을 대상으로 유출분석을 실시할 경우 본 연구와 같은 분석 절차를 거쳐야 할 것이다.

일단위 표준강수지수의 심도를 활용한 기상학적 가뭄 평가 (Evaluation of Meteorological Drought Through Severity of Daily Standardized Precipitation Index)

  • 가뭄에 대한 정의와 구분이 다양하게 존재하나 일반적으로 기상학적 가뭄으로부터 농업적 가뭄, 수문학적 가뭄, 사회경제학적 가뭄으로 전이되므로 강수량의 부족이 가뭄의 첫 번째 원인인 것은 자명하다. 최근까지 여러 가지 기상학적 가뭄지수가 개발되어 다양한 목적으로 세계 곳곳에서 활용되고 있으나, 그 중 적용사례가 가장 많은 것은 표준강수지수 (SPI; Standardized Precipitation Index)이다. 월단위로 계산되어지는 SPI는 우리나라와 같이 강수의 변동성이 큰 지역에서는 그 활용성이 떨어지는 경우가 있어 최근에는 일 단위 SPI를 산정하여 활용하는 경우도 있다. 그러나, 일단위 SPI는 가뭄기간 동안 가뭄단계가 'Extreme drought'에서 'Moderate drought'로 약해질 경우 가뭄 지속기간이 가장 긴 'Moderate drought' 단계에서 가뭄피해 및 체감하는 고통이 가장 클수 있어, 가뭄에 대응하거나 대국민 가뭄정보 전달에 한계가 있다. 가뭄에 대응하거나 가뭄정보 전달을 위해서는 가뭄이 지속되는 동안 가뭄단계가 높아질 필요성이 있다. 이에 본 연구에서는 가뭄사상의 특성 중 가뭄의 강도(Intensity)와 지속기간(Duration)의 특성을 모두 포함하는 가뭄의 심도(Severity)를 활용하여 기상학적 가뭄을 평가하였다. 일단위 SPI 값(강도)을 가뭄기간동안 누적하여 일단위 가뭄심도(SPI-S)를 산정하고 이를 통해 가뭄단계를 제안하였다. 또한 다양한 SPI 대상기간에 대해 최솟값을 취하는 Blended SPI에 대해서도 같은 방법으로 가뭄심도(SPIB-S)를 산정하고 가뭄단계를 적용하였다. 2001년, 2008-2009년, 2012년 가뭄사례에 적용한 결과 SPI-S(or SPIB-S)는 가뭄기간동안 단계적인 가뭄단계의 상승으로 당시의 가뭄상황을 잘 나타내었다. 이는 SPI-S(or SPIB-S)가 단계적인 가뭄대비와 대응 지수로 가뭄피해 경감에 활용도가 높을 것으로 판단되며, 가뭄상황을 지역민들에게 단계적, 일관적으로 전달할 수 있어 가뭄극복을 위한 시민참여를 유도하기에 유리할 것이다.

The Effect of Manual Physical Therapy on Improvement in the Range of Motion of Frozen Shoulder Patients: A Meta-Analysis of Cases in South Korea

  • Purpose: The main purpose of this meta-analysis was to identify the degree of effect size and variables for the impact of manual physical therapy on the improvement in the range of motion of frozen shoulder patients. Methods: This study collected 8 studies published between 1st January 2010 and 31st December 2020. The analysis of the results verified 49 effect size data and the random effect model was chosen. Results: First, the full case showed the largest mean effect size of 2.485 (p<0.001). Second, the size of the effect based on manual therapy and modality therapy showed an effect size of 4.178 (p<0.001). Third, the outcome group included 6 variables. The external rotation (2.818) variable group showed the largest mean effect size, followed by internal rotation (2.748), flexion (2.643), abduction (2.356), and adduction (2.356). Six outcomes were significant and the mean effect sizes of all the varied groups were above large size. The number of participants showed a 20 or less effect size of 2.478 (p>0.737). The number of intervention periods showed 4 weeks 20 or more effect size of 2.782 (p>0.294). Finally, the 'Trim and Fill' result confirmed that the calibration effect size was 1.471 (p<0.001). Conclusion: This study verified that manual physical therapy had a substantial effect on the improvement of the range of motion of patients with frozen shoulders and that the effects were dependent on the methods of outcomes.

Variation of Caffeic acid, Rosmarinic acid, Luteolin and Apigenin Contents in Perilla Germplasm

  • Perilla frutescens, which belonging to the Labiatae family, is widely cultivated oil crop and have been used traditional herbal medicine in East Asia such as Korea, China, and Japan. Especially, the leaves and the seeds of this species are important in Korean traditional cooking, as one of the popular garnish and food colorants. Numerous studies have revealed that the beneficial health effects of perilla are due to its several phytochemicals contents, such as rosmarinic acid, caffeic acid, luteolin, and apigenin. For this reason, increasing the content of phytochemicals in perilla hasbecome a major breeding objective. The genetic diversity of the rosmarinic acid, caffeic acid, luteolin, and apigenin content in perilla seed is poorly documented. We analyzed the rosmarinic acid, caffeic acid, luteolin, and apigenin content of 203 accessions of perilla germplasm by high performance liquid chromatography (HPLC). The rosmarinic acid and luteolin contents ofgermplasms were ranged from $15.7{\mu}g/g$ to $2717.1{\mu}g/g$ and from $1.6{\mu}g/g$ to $582.4{\mu}g/g$ respectively.
