Analysis of Medical Expenses for patients by Automobile Accident (자동차사고에 의한 환자의 진료비 성향 분석)

  • The present study aimed to compare and analyze the particulars of and difference in medical expenses and their composition ratio for 2,026 patients hospitalized at 2 types of medical institutions (443 at a clinic and 1,583 at a hospital) in Jeonju-si, Jeollabuk-do under coverage of automobile insurance during 2009. From the analysis, it became evident that there were differences in individual characters of inpatients covered by automobile insurance, and details, composition and composition ratio of medical treatment for them depending on types of medical institutions. There was no difference, however, between the total and average medical expenses per capita even when markup rate was applied to the automobile insurance by types of hospitals. The ratio of radiotherapy and physical therapy in the composition of medical expenses was found to be extraordinarily high in clinics compared to that of hospitals. The composition ratio of the fixed cost including charge for hospitalization also turned out to be fairly high in all medical institutions.

A Study on the Provision of TPN for Hospitalized Patients (입원환자의 TPN 공급현황에 대한 연구)

  • In Korea, implementation of nutrition support guidelines has been limited due to strict health insurance reimbursement policies as well as the lack of consensus on the best approach to TPN management. We examined the impact of TPN provision to hospitalized patients where NST (nutrition support team ) consultations were not requested by their primary physicians. The study showed the followings : 1. The median dutation of TPN provision was 8 days, but many patients were on TPN for less than 1 week. 2. The intake of energy and protein were less than the patient's requirements 3. Lipid emulsion was not provided to the most TPN patients. In conclusion, the role of NST should be expanded and studies are needed not only on TPN formulations which are suitable to Koreans but also on the cost-effectiveness of NST activities. TPN policies and protocols should be established based on the needs of each hospital.

Longitudinal Study of Cancer-Related Fatigue in Patients with Colorectal Cancer

  • Background: Dynamic changes of cancer-related fatigue (CRF) among hospitalized patients with colorectal cancer were determined. Materials and Methods: This longitudinal, descriptive study involved 96 hospitalized patients with colorectal cancer, all recruited from a tertiary general hospital in Guangzhou, China. Patients completed questionnaires three times to assess the degree of fatigue, and measurement points were within one week of admission, at 2 to 3 days after surgery and 3 weeks after surgery. Results: Significant differences among the three measurement points (p<0.01) were observed. The scores of fatigue in the second survey were the highest, followed by the third and first surveys. Colon cancer patient scores were higher than those of rectal cancer patients with a significant difference (p<0.05). Colorectal patients experienced different degrees of fatigue at different periods during hospitalization. Conclusions: This study highlights the importance of interventions that are carefully tailored to patients based on the characteristics at different periods to alleviate fatigue.

  • Consultation with the patient and doctor is very important in the examination. However, if the consultation cannot be done directly, such as corona virus, it is difficult for the doctor to determine the patient's condition more accurately. Recently, an image counseling system has been developed based on the Internet, but in the case of heart disease, remote medical counseling cannot be performed because it is not possible to stethoscope the heart sounds remotely. In order to solve this problem, it is necessary to develop an interactive mobile robot capable of remote medical consultation, and a doctor and a patient should be able to set a planting sound during consultation and transmit it in real time. In this paper, we developed a robot that can remotely control a medical counseling robot to move to a hospital room where patients are hospitalized, and to consult a patient in the room remotely from a doctor's office. A remote medical imaging stethoscope system for real-time heart sound transmission is presented. The proposed system is a kind of P2P communication that transmits video information, audio information, and control signal independently through webRTC platform, so that there is no data loss. Consults and sees doctors in real time and finds it more effective than traditional methods for patient security. The system implemented in this paper will be able to perform remote medical care in the place where the spread of diseases between humans like the recent corona 19 as well as the remote medical care of heart disease patients in the future.

A Study for Dietetic Practitioner's Job Analysis : labor Time Spent and Staffing Need Indices (3 차 의료기관에서의 영양업무 분석 : 업무수행 시간 및 적정인원 산출)

  • This study was designed to analyze the dietetic practitioner's job in the over 600-ed hospitals in korea and to assess their labor time spent and staffing need indices. The actual time spent and expected labor time spent on dietitians' activities were investigated and the proper dietic staffing needs in the hospitals was also calculated. A job analysis questionnaires were developed and mailed to 20 hospitals. Completed questionnaires were received form 12 hospitals for a response rate of 60%. The followings are summary of the results. 1. The jobs dietitians at the hospitals were classified into the following 7 areas, direct patient care, indirect patient care, therapeutic patient care, education & counseling, meeting & research, delay & movement, and administration & food services. 2. The actual time spent on dietetic practice was 48.6 hours and expected labor time spent was 99.2 hours, Therefore, the proper time required to conduct classified jobs was 2.1 times higher than the time spent. Especially, the time required for performing clinical nutrition services including direct patient care, indirect patient care, therapeutic patient care was significantly higher than the time needed. 3. The average times required for the direct patient care was 1334.6min, for the indirect patient care was 796.3min, for the therapeutic patient care was 1634.5min, for the education & counseling was 265.2min, for the meeting & research was 366.7min, for the delay & movement was 327.3min, and for the administration & food services was 1170min. The staffing need indices was 12.3. As a conclusion, the standardized job descriptions for the dietitian to carry out their job at the hospital should be established. And the clinical dietitians as nutrition professionals have to be recruited to provide systematically hospitalized patients with medical nutrition therapy at each hospital.

  • Objectives: This study was performed to evaluate the effect of acupuncture and traction on Spasmodic Torticollis patient. Methods: The patient was hospitalized Dept. of Oriental Rehabilitation Medicine, College of Oriental Medicine, Dong-Guk University from 19th, May 2005 to 24th, August 2005, diagnosed as Spasmodic Torticollis and treated with Acupuncture, Traction and so on. Results and Conclusions: Symptom of patient such as Spasmodic Torticollis was improved after above treatment. So it is suggested that oriental medical treatment are effective in the Spasmodic Torticollis.

Analysis Study about the Status and Satisfaction of Korean Medical Treatment in Long-term Care Hospital in Sejong (세종시 소재 일개 요양병원내 한방치료의 현황 및 만족도 분석 연구)

  • Objectives : The purpose of this study was to analyze the status of inpatients in long-term care hospital and the satisfaction of Korean medical treatment. Methods : These analyzed data were collected from patients who were hospitalized in long-term care hospital in Sejong from Jan. 1. 2017 to Dec. 31. 2017. To analyze the status and the satisfaction of inpatients, we calculated the data of inpatients and conducted a survey. Results : Preference for acupuncture treatment was 95.6% and preference for numbers of Korean medical treatment more than two times per week was 40%. In addition, confidence in Korean medical treatment was 4.31, improvement after Korean medical treatment was 4.07 and general satisfaction was 4.58 with five-point scale. Conclusions : Most of the patients in long-term care hospital were satisfied with Korean medical treatment.

A Report of Elderly Gravida Suffered from Traffic Accidents during Pregnancy (임신 중 교통사고로 내원한 고령 임산부의 치료경과 보고)

  • Objectives : The objective of this study is to report a treated case with elderly gravida who suffered from two-times of traffic accidents during pregnancy. Methods : The patient, 39-year-old, was hospitalized to care pain and symptoms caused by traffic accidents during pregnancy. The patient was treated with herbal medicine, acupuncture, moxibustion, cupping therapy and physical treatment during the admission period. We investigated the clinical management of Korean obstetrics and gynecology in an elderly gravida during pregnancy. Results & Conclusions : Despite of hard conditions such as elderly gravida and two-times of traffic accidents, pregnancy was well maintained through medical treatment. It is suggested that medical treatment of Korean medicine is effective to manage pregnancy of symptoms after accident.

The Effects of Smartphone Application to Educate Patient on Patient Safety in Hospitalized Surgical Patients (위해사건 예방을 위한 수술 환자 교육용 스마트폰 애플리케이션의 효과)

  • Purpose: This study was designed to evaluate a patient safety application in the prevention of adverse event among surgical patients. Seventy three surgical patients with thirty six of them using the patient safety application and the rest (N=37) were provided educational booklet. Further, the instrument would measure patients' right to know, knowledge about patient safety, and attitude toward patient safety. Methods: The patient safety application was developed by the ADDIE along with input from experts, patients and an extensive literature review. Data were collected from 7 September through 20 October 2015. Results: The experimental group had significantly higher scores in patients' right to know (t=2.01, p=.024), knowledge on patient safety (t=3.80, p<.001) and attitude toward patient safety (t=2.74, p=.004) than those of the control group. Conclusion: The patient safety application developed using Smartphone could be an effective tool enhancing patient involvement in preventing adverse events that may occur to patients. Further studies are recommended with diverse subjects with varying medical conditions.

Statistical Study of Conservative Treated Herniated Intervertebral Lumbar Disc (요추 추간판탈출증 환자의 보존적 치료에 대한 통계적 연구)

  • Objectives : The object is to study a clinical effect regarding the conservative treatment of the patient who was diagnosed as the herniated Intervertebral lumbar disc(HIVD) and hospitalized in the oriental medical hospital and clinical study about the type of the HIVD. Methods. The clinical studies were done on 65 cases (hospitalized in Dept. of Oriental Rehabilitation Medicine, jaseng on oriental medical hospital from June 2001 to May 2002) who complained of low back pain or sciatica and diagnosed as HIVD on CT(computerized tomography) or MRI(magnetic resonance imaging). They were given both oriental conservative treatment and exercise during the hospitalization period. We investigated and observed 20 items about sex, age, weight, height, occupation, smoking. obesity, duration of disease, clinical symptom and treatment duration in basis of medical recording which was drawn up at patient hospitalization. We classified HIVD with 4 types(Bulging. Protruded. Extruded, Sequestered) and decided the case which simultaneously had 2 overs with Mixed type. The treatment evaluation standard was classified with 4 branches; Excellent. Good, Fair and Poor. Results & Conclusions : The treatment result of over 'fair' is bulging type 85%, protruded type 92.7%, extruded type 100% and the mixed type was 85.1 %. The effect was good in order of bulging, mixed, protruded and extruded type. It appeared most plentifully with 30 people in L4-5 and L5-S 1 disc herniation type.
