지역사회획득폐렴으로 대학 병원에 입원한 성인의 사망률과 관련된 위험인자 (Risk Factors for Mortality in Community-Acquired Pneumonia Patients Admitted to a Referral Hospital)

  • 배 경: 지속적인 노력에도 지역사회폐렴에 의한 사망률은 증가하고 있으며 서구에서는 이에 대한 적절한 접근 및 예후 평가를 위해 예후 인자들을 확인하려 하였고 종합적인 접근을 위해 Pneumonia Severity Index(PSI) 등의 지표를 개발하였다. 하지만 국내에서는 단순 지표들만을 이용한 연구에 그치고 있어 PSI를 이용하여 폐렴으로 입원한 환자의 예후에 종합적인 접근을 할 수 있는지 확인하고자 하였다. 방 법: 2002년 1월부터 2005년 1월까지 지역사회폐렴으로 입원한 179명의 환자들을 대상으로 생존군/사망군 그리고 일반병실 입원환자/중환자실 입원 환자 사이를 비교하였으며, 각 군에 대해 Pneumonia Severity Index를 분석하였다. 결 과: 사망군과 생존군 사이에는 평균 수축기 혈압, 평균 이완기 혈압, 평균 맥박수, 평균 호흡수, 평균 체온, albumin, LDH, total cholesterol, HDL, PT, aPTT, hemoglobin, blood urea nitrogen(BUN)에서 유의한 차이가 있었다. (p<0.05) 일반병실과 중환자실 입원환자 사이에는 평균 맥박수, pH, $pCO_2$, $pO_2$, $SaO_2$, 혈청 총 단백질, 알부민, 혈청 총 칼슘, LDH, 총 콜레스테롤, HDL, PT, aPTT, hemoglobin, blood urea nitrogen(BUN)에서 유의한 차이가 있었다. (p<0.05) Pneumonia Severity Index의 경우 class I에서 사망률이 가장 낮고 class가 증가할수록 사망률이 증가하였으며 class V에서 사망률이 가장 높았다. 결 론: 다양한 지표들을 이용하여 지역사회폐렴으로 입원한 환자들을 평가할 수 있으나, Pneumonia Severity Index(PSI)를 이용하여 종합적인 접근을 할 수 있으며 이를 국내 환자에게 유용하게 이용할 수 있을 것으로 본다.

  • The successful treatment of community-acquired pneumonia requires appropriate, empirical antimicrobial therapy. The etiology and antimicrobial susceptibility of major pneumonia pathogens can differ by country. Therefore, the ideal treatment guidelines for community-acquired pneumonia should be based on the studies performed in each country. We developed a treatment guideline for community-acquired pneumonia for immunocompetent adults in Korea. This guideline was developed by the joint committee of the Korean Society for Chemotherapy, the Korean Society of Infectious Diseases, and the Korean Academy of Tuberculosis and Respiratory diseases.

Predictive Factors of Methicillin-Resistant Staphylococcus aureus Infection in Elderly Patients with Community-Onset Pneumonia

  • Background: Methicillin-resistant Staphylococcus aureus (MRSA) infection is a severe and life-threatening disease in patients with community-onset (CO) pneumonia. However, the current guidelines lack specificity for a screening test for MRSA infection. Methods: This study was retrospectively conducted in elderly patients aged ${\geq}65years$, who had contracted CO-pneumonia during hospitalization at the Jeju National University Hospital, between January 2012 and December 2014. We analyzed the risk factors of MRSA in these patients and developed a scoring system to predict MRSA infection. Results: A total of 762 patients were enrolled in this study, including 19 (2.4%) with MRSA infection. Healthcare-associated pneumonia (HCAP) showed more frequent MRSA infection compared to community-acquired pneumonia (4.4% vs. 1.5%, respectively; p=0.016). In a multivariate logistic regression analysis, admissions during the influenza season (odds ratio [OR], 2.896; 95% confidence interval [CI], 1.022-8.202; p=0.045), chronic kidney disease (OR, 3.555; 95% CI, 1.157-10.926; p=0.027), and intensive care unit admission (OR, 3.385; 95% CI, 1.035-11.075; p=0.044) were identified as predictive factors for MRSA infection. However, the presence of HCAP was not significantly associated with MRSA infection (OR, 1.991; 95% CI, 0.720-5.505; p=0.185). The scoring system consisted of three variables based on the multivariate analysis, and showed moderately accurate diagnostic prediction (area under curve, 0.790; 95% CI, 0.680-0.899; p<0.001). Conclusion: MRSA infection would be considered in elderly CO-pneumonia patients, with three risk factors identified herein. When managing elderly patients with pneumonia, clinicians might keep in mind that these risk factors are associated with MRSA infection, which may help in selecting appropriate antibiotics.

Evaluation of the Quality of Care among Hospitalized Adult Patients with Community-Acquired Pneumonia in Korea

  • Pneumonia is an important cause of morbidity and mortality. Since 2014, the Health Insurance Review and Assessment Service (HIRA) has assessed the overall quality of care among hospitalized adult patients with community-acquired pneumonia (CAP) provided by all medical institutions in Korea. A committee of the Korea Academy of Tuberculosis and Respiratory Diseases developed the hospital inpatient quality measures set for CAP consisting of eight core measures and five monitoring measures. The composite measure score was calculated. The medical records of hospitalized adult patients ages 18 years or more with CAP from October to December 2014 were evaluated. The data of 523 hospitals (42 tertiary hospitals [8.0%], 256 general hospitals [49%], and 225 hospitals [43.0%]) and 15,432 cases (tertiary hospitals, 1,673 cases [10.8%]; general hospitals, 8,803 cases [57.1%]; hospitals, 4,956 cases [32.1%]) were analyzed. We found large variations among institutions in terms of performance of care measures for CAP. For the composite measure score, the mean value was 66.7 (tertiary hospitals, 98.5; general hospitals, 79.2; hospitals, 43.8). Despite significant differences in measure scores between tertiary, general hospitals and hospitals, no significant differences were found in mortality between hospitals. Further studies are needed to determine the care measures appropriate for CAP.

지역사회획득 폐렴 환자의 중증도 보정 재원일수 분석 (A Study on analysis of severity-adjustment length of stay in hospital for community-acquired pneumonia)

  • 본 연구의 목적은 지역사회획득 폐렴 입원 환자의 재원일수의 변이를 분석하기 위해 중증도 모형을 개발하였다. 2004~2006년 퇴원손상환자 조사자료 중 지역사회획득 폐렴환자 5,353건을 연구대상으로 하였으며, 재원일수의 차이분석은 t검정, 분산분석을 실시하였고, 중증도 보정 재원일수 예측 모형은 데이터마이닝 기법을 이용하였다. 여자에 비해 남자, 연령이 많을수록, 의료급여, 응급실 경유 환자의 재원일수가 긴 반면, 병원사망 여부에 따라서는 유의한 차이가 없었다. 개발된 의사결정나무 모형을 이용하여 예측 재원일수를 산출하고 표준화한 값을 비교한 결과 타지역 진료여부에 따라서 재원일수의 차이는 없는 반면, 보험유형과 지역별로 재원일수의 변이가 존재하는 것으로 나타났다. 환자 특성과 중증도를 통제하고 나타난 재원일수의 변이는 공급자 요인으로 설명될 수 있는데, 진료행태나 의료자원에 대한 후속 연구가 필요한 것으로 보인다. 본 연구는 행정 데이터를 이용하여 중증도 모형을 개발하고 변이를 확인하였다는 점에서 활용의 효용성을 높이는 데 기여할 것으로 사료된다.

  • Background: High cortisol levels are frequently observed in patients with severe infections are of prognostic value in sepsis. The aim of this study was to evaluate the clinical usefulness of serum cortisol in assessment for the severity of community-acquired pneumonia (CAP). Methods: This study analyzed the results of 52 CAP subjects admitted in Changwon Fatima Hospital between July 2008 to May 2010. Total serum cortisol, infection markers such as C-reactive protein (CRP), procalcitonin (PCT) and CURB (Confusion, Uremia, Respiratory rate, Blood pressure)-65 were examined retrospectively. Results: In clinically unstable subjects on admission day 4, baseline serum cortisol, CURB-65, and CRP were elevated significantly compared to those of stable subjects. Area under curve (AUC) of cortisol, CRP, and CURB-65 from ROC curves were 0.847, 0.783, and 0.724 respectively. In the subjects with serum cortisol ${\geq}22.82{\mu}g/dL$, CRP, PCT, CURB-65 score, and mortality were significantly elevated. Conclusion: These findings suggest that measurement of serum cortisol in early stage may provide helpful information in the assessment of CAP severity.

  • Mycoplasma pneumoniae(M. pneumoniae) is a primary pathogene of the respiratory tract in children. Most studies of such pneumonia involve a group of admitted patients in hospital, usually with major medical illness. We retrospectively studied the epidemiologic and clinical feature of 105 patients with serologically proven M. pneumoigic pneumonia treated at Gwang-Yang Hospital during a epidemic period of Jun. 1993 to Dec. 1993. All cases of pneumonia developed in this period were also reviewed and compared with serologically proven group. The results were as follows. 1. There were 63 males and 42 females, and the male /female ratio was 1.5 : 1. 2. More than half(57%) of cases belonged to 5-9 years of age group, and mean age was 6.5 years old. 3. Mean age was steadily decreased as prevalence of mycoplasmal pneumonia had been subsided. 4. The date of index case was June 26th, and that of median case was 3 months after the index case. 5. A major determinant of the outbreak seemed to be the population density rather than the population size. 6. Recurrence and serious complication were not observed in this period. 7. All cases of pneumonia developed in this period might be attributed to M. pnemoniae. These epidemiologic and clinical characteristics would contribute to the prevention and diagnosis of mycoplasmal pneumonia.

Healthcare-Associated Pneumonia among Hospitalized Patients: Is It Different from Community Acquired Pneumonia?

  • Background: The increasing number of outpatients with multidrug-resistant (MDR) pathogens has led to a new category of pneumonia, termed healthcare-associated pneumonia (HCAP). We determined the differences in etiology and outcomes between patients with HCAP and those with community-acquired pneumonia (CAP) to clarify the risk factors for HCAP mortality. Methods: A retrospective study comparing patients with HCAP and CAP at Jeju National University Hospital. The primary outcome was 30-day mortality. Results: A total of 483 patients (208 patients HCAP, 275 patients with CAP) were evaluated. Patients with HCAP were older than those with CAP (median, 74 years; interquartile range [IQR], 65-81 vs. median, 69 years; IQR, 52-78; p<0.0001). Streptococcus pneumoniae was the major pathogen in both groups, and MDR pathogens were isolated more frequently from patients with HCAP than with CAP (18.8% vs. 4.9%, p<0.0001). Initial pneumonia severity was greater in patients with HCAP than with CAP. The total 30-day mortality rate was 9.9% and was higher in patients with HCAP based on univariate analysis (16.3% vs. 5.1%; odds ratio (OR), 3.64; 95% confidence interval (CI), 1.90-6.99; p<0.0001). After adjusting for age, sex, comorbidities, and initial severity, the association between HCAP and 30-day mortality became non-significant (OR, 1.98; 95% CI, 0.94-4.18; p=0.167). Conclusion: HCAP was a common cause of hospital admissions and was associated with a high mortality rate. This increased mortality was related primarily to age and initial clinical vital signs, rather than combination antibiotic therapy or type of pneumonia.

  • Cryptococcal pneumonia usually occurs in immunocompromised patients with malignancy, acquired immune deficiency syndrome, organ transplantations, immunosuppressive chemotherapies, catheter insertion, or dialysis. It can be diagnosed by gaining tissues in lung parenchyma or detecting antigen in blood or bronchoalveolar lavage fluid. Here we report an immunocompetent 32-year-old male patient with diabetes mellitus diagnosed with cryptococcal pneumonia after a ultrasound-guided percutaneous supraclavicular lymph node core needle biopsy. We treated him with fluconazole at 400 mg/day for 9 months according to the guideline. This is the first case that cryptococcal pneumonia was diagnosed from a percutaneous lymph node biopsy in South Korea.

  • Background: It is difficult but important to differentiate between bacterial and viral infections, especially for respiratory infections. Hence, there is an ongoing need for sensitive and specific markers of bacterial infections. We investigated novel biomarkers for discriminating community acquired bacterial pneumonia from 2009 H1N1 influenza A infections. Methods: This was a prospective, observational study of patients with community acquired bacterial pneumonia, 2009 H1N1 Influenza A infection, and healthy controls. Serum samples were obtained on the initial visit to the hospital and stored at $-80^{\circ}C$. We evaluated CRP (C-reactive protein), PCT (procalcitonin), LBP (lipopolysaccharide-binding protein) and copeptin. These analytes were all evaluated retrospectively except CRP. Receiver operating characteristic curve (ROC) analyses were performed on the resulting data. Results: Enrolled patients included 27 with community acquired bacterial pneumonia, 20 with 2009 H1N1 Influenza A infection, and 26 who were healthy controls. In an ROC analysis for discriminating community acquired bacterial pneumonia from 2009 H1N1 influenza A infection, areas under the curve (AUCs) were 0.799 for CRP (95% Confidence interval [CI], 0.664~0.934), 0.753 for PCT (95% CI, 0.613~0.892) and 0.684 for LBP (95% CI, 0.531~0.837). Copeptin was not different among the three groups. Conclusion: These findings suggest that serum CRP, PCT and LBP can assist physicians in discriminating community acquired bacterial pneumonia from 2009 H1N1 influenza A infection.
