상급종합병원에서의 간호·간병통합서비스 병동과 일반병동 간호사의 근무환경, 역할갈등, 직무 배태성 비교 (A Comparison of Nursing Work Environment, Role Conflict, and Job Embeddedness of Nurses Working in Comprehensive Nursing Care Service Wards and General Wards in a Tertiary Hospital)

  • Purpose: The objective of this study was to compare and analyze the work environment, role conflict, and job embeddedness between comprehensive nursing care service (CNCS) ward nurses and general ward nurses. Methods: This descriptive research study involved 70 CNCS ward nurses and 69 general ward nurses working at an advanced general hospital in Seoul. Data were collected using the structured questionnaire from March 27 to April 14, 2019 and analyzed with the SPSS 24.0 program. Results: The work environment of the CNCS ward nurse was higher than that of the general ward nurse (t=4.38, p<.001), and the role conflict of the CNCS ward nurse was lower than that of the general ward nurse (t=-2.09, p=.038). However, job embeddedness did not show any statistically significant difference (t=0.22, p=.824). Conclusion: The results of this study show that the introduction of CNCS ward has shown improvement in the work environment and strengthened the establishment of the roles in their team, while maintaining the job embeddedness of nurses. These results indicate that improvements in the work environment, such as nurse staffing and material support, would contribute to the qualitative enhancement of nursing and that it would need to extend the introduction of CNCS wards.

호스피스 병동에서 시행되는 말기 암 환자의 DNR (Do-Not-Resuscitate) 동의 (DNR (Do-Not-Resuscitate) Order for Terminal Cancer Patients at Hospice Ward)

  • 배경: 보편적으로 호스피스 병동에서 말기 암 환자에 있어서, DNR 동의가 흔하게 취득되고 있다. 그러나 말기 암 환자에 대한 현황과 실태 분석에 관한 국내 연구는 아직 드문 현실이다. 최근 저자 등은 보호자가 DNR 동의를 거부하여 심폐소생술 후 인공호흡기 치료를 시행하여 2개월 간 중환자실에서 치료 후 사망한 환자를 경험하면서 지금 까지 진행된 DNR 동의의 현 실태와 앞으로 시행될 DNR 동의의 보완점을 마련하기 위해 본 연구를 시작하였다. 방법: 2003년 1월부터 6월까지 6개월 동안 성빈센트병원 호스피스 병동에 입원한 말기 암 환자 60명을 대상으로 후향적으로 의무기록과 DNR 동의서를 조사하였다. 대상 환자들의 나이, 성별,진단명, DNR 동의 시간, 사망까지의 시간, DNR 동의에 참여한 보호자, DNR 결정 당시 환자 상태, 사망장소, DNR 결정 당시의 치료와 DNR 결정 전후 치료의 변화 등을 조사하였다. 치료 단계는 3단계로 분류하였다. 결과: 중앙 연령은 66세($31{\sim}93$세) 였고 남자가 31명, 여자가 29명이었다. 폐암 12명, 위암 12명, 담낭암 및 담도암 7명, 대장암 6명, 췌장암 4명, 기타 19명이었다. DNR 동의서에 서명한 사람은 아들이 22명, 배우자가 19명, 딸이 16명, 기타가 3명이었다. 이 중 환자가 DNR 동의서에 동의한 경우는 한 명도 없었다. 60명 중 30명이 입원 시에, 30명은 입원 기간 중에 DNR 동의서에 서명하였다. 입원 기간 중에는 증상의 악화 19명, 활력 증후 변화 4명, 다기관 기능부전 3명, 기타 상태 4명 등으로 DNR이 결정 되었다. DNR 동의 후 사망까지의 시간은 13명이 5일 이내에 사망하였다. 사망 장소는 60명 중 한 명을 제외하고는 모두 본원이었다. DNR이 시행되었을 당시 치료 단계는 2명을 제외하고 1단계였고 2단계와 3단계가 각각 1명씩이었다. 결론: 환자의 존엄성과 권리라는 측면에서 DNR 동의의 환자 참여가 국내에서도 신중하게 고려되어야 하겠다. 또 이를 위해 DNR 동의의 의미, 경과, 동의 철회 등의 사항이 포함된 문서화된 동의서에 의해서 환자와 보호자에게 설명되어야 하겠다.

중정이 계획된 병동부 바닥면적과 간호동선 평균거리 비교 연구 (A Comparative Study of Floor Area and Average Distance of Nurse Working of Planned Courtyard in Ward)

  • Purpose: The courtyard in the ward can light and ventilate in the central space and corridor of the ward. It is possible to improve the disadvantages of the existing double-corridor ward. Also, there is the advantage in that patients are able to contact with nature indoors and cause positive activities. So, the purpose of this study is quantitatively analyze the floor area and the average distance of nurse working of the courtyard in ward. Methods: The subjects are converted through the same criteria setting. And it investigate and analyze the influence of the courtyard in the ward through the analysis of the spatial composition. Result: The factors that affected the floor area increase are the planning courtyard, the corridor type and the dispersion of the medical rooms. The average distance of nurse working is more influenced by the nursing management system of the hospital than by the courtyard. So, It can improve according to the position and number of nurse station. Implications: It can be used as a primary data for courtyard plan in the ward for positive patient environment plan considering the floor area, average distance of nurse working.

정신병원 입원 환자가 인지하는 의료서비스 품질이 만족도와 충성도에 미치는 영향 (The effect of mental hospital inpatient's perceived service quality on satisfaction and customer loyalty)

  • The purpose of this study is to identify the inpatient service quality at mental hospitals and to examine the effect of service quality on patient's overall satisfaction and customer loyalty. Data collection was done through conducting a survey of inpatients in three mental hospitals located in Chungnam, Korea using self administered questionnaire. The questionnaire included the modified version of 'service satisfaction scale for psychiatric patients' invented by Chul Kwon Kim and other members, which consists of five dimension of service quality: staff attitude, treatment quality, ward environment, access/cost and ward rule. Total 236 questionnaires were gathered and 219 were used for analysis. The data was analyzed by using version SAS9.2 and path analysis model was applied to test our hypothesis. As a result, the four factors of service (staff attitude, treatment quality, ward environment and ward rule) were extracted, which counted for 62.89% of the common variance. Moreover, Cronbach Alpha showed relatively high internal consistency of answer, all exceeding 0.6. According to the study, ward environment and treatment quality turned out to have direct influence on the patient's satisfaction. In addition, staff attitude and overall satisfaction directly influenced the customer loyalty while ward environment and ward rule indirectly had influence using overall satisfaction as a medium. In conclusion, in order to enhance the satisfaction and customer loyalty in mental hospitals, efforts in improving environmental service quality in particular is highly demanded.

서울지역 공공의료기관 간호사의 병동과 병동 외 구분에 따른 직무스트레스와 피로 (Job Stress and Fatigue between Ward Nurses and Non-ward Nurses in Public Medical Institution, Seoul)

  • Objectives: The study was conducted to understand job stress and fatigue conditions by dividing nurses in a polyclinic-level public medical institution, Seoul with more than 600 beds into ward nurses and non-ward nurses and to comprehend sub-areas of job stress that affect fatigue. Methods: A survey was conducted from August 18 2014 to September 12 2014, so 216 cases were analyzed by using PASW statistics 18.0. Results: Job stress of ward nurses is significantly high in the psychological burden of nursing service area and medical limit. Fatigue of ward nurses is also higher. As a result of multiple Linear regression, nursing service area affects fatigue of ward nurses and there is no significant influence factors in non-ward nurses. Conclusion: Therefore, mental health education and interest of hospital in nursing service area are more needed for ward nurses with high job stress and fatigue among nurses.

일부 대학부속병원 외래간호사와 병동간호사의 업무 스트레스와 직무만족도 비교 (A Comparative Study on Job Stress and Satisfaction between Ward Nurses and Outpatient Nurses)

  • Purpose: This study was to compare job stress and job satisfaction between ward nurses and outpatient nurses, and to identify the correlation between job stress and job satisfaction. Methods: The subjects of this study were 200 ward nurses and 85 outpatient nurses in G and I University Hospital in Incheon. The data were collected in March 2010 and analyzed with t-test and Pearson correlation coefficient. Results: There was no significant difference in overall job stress between two groups, but slightly higher stress level for ward nurses. In sub-area of job stress, 'nursing duty' was primary for both groups. And in 'nursing duty' (t=2.05, p=.041), 'conflict with patients' (t=2.73, p= .007), and 'working environment' (t=3.37, p<.001) ward nurses showed significant higher level of stress than outpatient nurses. For job satisfaction, outpatient nurses showed higher level significantly (t=-2.98, p=.003). And both groups of nurses' job satisfaction was negatively correlated with job stress significantly (r=-.31, p<.001). Conclusion: It will be possible to reduce job stress and promote job satisfaction level for both groups of nurses by adding staffs, and specifically for ward nurses by improving facilities for relax and appropriate supply, and for outpatient nurses by managing interpersonal stress and reducing side tasks.

입.퇴원 수속창구 중앙화와 분산화에 따른 이용자의 만족도와 재이용 의사 (The Study of Comparison Satisfaction and Re-use Intention between Central and Ward Reception Desk Users)

  • The purpose of this study is to design strategic hospital service based on each hospital's features. For this study, an assessment was conducted by 398 in-patents of one university hospital located in Seoul. The self-questionnaires, which were investigated from Oct. 15th to 29th in 2008, compared central reception desk with ward reception desk in satisfaction and re-use rate of patients. The major results of this study are as follow. First, according to each reception desk user, they have different satisfaction of it. As for the staff kindness, admission procedure, discharge procedure and manner of staff, those made patients be gratified as well. Second, when it comes to the intention of re-use, there were no significant features between them. Only convenience in admission and discharge procedure, however, was an attractive factor for the recommendation. Third, this study found out the reasons for higher re-use rate of central reception desk users. Regarding service, they were contented with the time for test and treatment. As for the hospital service, they would like to re-use this hospital because of convenient steps of paying interim fee and getting certificates. Forth, this study found out the reasons for higher re-use rate of ward reception desk users. As a point of hospital service view, they responded that respected privacy, hospital facility and general service were good for staying. As for the manner of staff, they mentioned nurses and staff in charge and whole staff members were kind. When it comes to the procedures of patient management, steps of discharge and paying interim fee were convenience. In conclusion, the results of this study suggest that providing a ward reception desk service can boost the satisfaction and re-use rate of in-patients. Furthermore, this strategic management method would be good for not only cutting the moving line but also efficient in-patient care system. These results can be used for the strategic hospital marketing field, as well. Even though this study has a limitation of the targeted populations which were only in a ward reception desk running hospital, it can say that having competitiveness in satisfaction of hospital service is good for promoting and differencing each hospital. Consequently, whole general management system would be adjusted first for differencing each hospital; however, this sort of additional factor should be concerned as well. I expect that this study would give meaningful data for designing strategic and differencing marketing method to lots of hospitals.

종합병원 병동부 간호 이동 동선을 고려한 가시 특성 분석 모델에 관한 연구 (A Study on the Visibility Analysis Model for the Ward of General Hospital Considering Nurse's Circulation)

  • The path and visibility of nurses in general hospital wards have been treated as architectural planning factors. However, the analysis approach of existing studies shows limitations that only fixed physical elements are considered without considering the behavior of users using space. Consider factors for analysis of ward and models based on this study model. Select a case hospital to apply the analysis technique and conduct the nurse questionnaire and route survey of the hospital. Establish a framework for analysis model applied with the path of nurse movements. The analysis model applies to the case hospital. The analysis results are aggregated to derive design suggestion for reference to the spatial improvement of the ward. Visible visibility to observe the bedside in the nursing station, visibility to observe the beds in the nurse's path, and visibility to observe patients moving in the nurse's path were derived from visual access frequency and exposure frequency. The survey of nurses' movements at the site allowed the nurses to calculate the distance required to move. Reflecting the path of nurse movement, a model was presented for a comprehensive analysis of nursing distance and nursing visibility, which could lead to improvement in the observation and visibility of nurses and the layout of patient rooms or day rooms.

환자 중심적 개념에 따른 병동부의 건축계획에 관한 이론적 연구 (A Study on the Architectural Planning of Ward According to the Concept of Patient-centered)

  • The architect do not have a data about planning patient-centered hospital, do not reflect a demand of patient at the planning step. This study arrange what is concept of patient-centered, recently concept of patient-centered hospital is importantly raised, at centering around a ward that a life of patient is the most influenced. This study make clear character of patient-centered hospital at home, the problem and demand of patient through the P.O.E. The concept of patient-centered was disregarde, when the hospital was planned, in spite of being aimed hospital. At home, the shape of ward did not be developed, did not be corresponded with a demand of patient. But now proposition, which is satisfied psychologic, physical, mental demand, should be attempted.

삼각형병동의 결합유형에 관한 건축계획적 연구 (A Study on the Architectural Planning of Joint Pattern in the Triangular Type Ward)

  • The purpose of this study is to present the research data to improve the quality of triangular ward in general hospital in Korea. Drawing documents analysis were used to examine the condition of ward. This study in focused on patient room and atrium according to connection type of triangular ward, allocation of NS, module and direction of patient room. The results of this study is as follows; Triangular ward has two joint pattern(conner and conner, side and side). Conner and conner joint pattern is good at planning of lighting than side and side joint pattern. Triangular ward can has different module system in each side. Allocation of NS is important factor which is focused on patient relationship or ward control.
