Impact of Maternal Uncertainty on Coping: Experiences from a General Hospital Pediatric Ward (입원 환아 어머니의 불확실성이 대처에 미치는 영향: 일 개 종합병원 소아과병동을 중심으로)

  • Purpose: The aim of this study was to test whether maternal uncertainty and the general characteristics of mothers and children influenced maternal coping. Methods: In this cross-sectional study, 190 mothers whose children had been admitted to the pediatric ward of a general hospital completed self-report questionnaires during their children's hospitalization. The questionnaires assessed the general characteristics of the mothers and children, maternal uncertainty, and maternal coping. Multiple regression analysis was used to test the research model. Results: In the multivariable model, help from one's spouse (t=3.10, p=.002), religion (t=2.68, p=.008), overall ambiguity (t=2.64, p=.009), and family income (t=2.33, p=.021) were associated with higher coping scores. Conclusion: This research model presents possible guidelines for pediatric nurses to provide comprehensive and accurate information on children's illnesses and treatments for mothers of children hospitalized in general hospitals. In particular, nurses should pay more attention to mothers who are not receiving help from their spouses, are not religious, and have a low family income.

Effects of Noise Block on Anxiety and Vital Sign of Patients with Gynecologic Laparoscopic Surgery in Operating Room (수술실의 소음차단이 산부인과 복강경 수술 환자의 불안과 활력징후에 미치는 영향)

  • Purpose: This study was to examine the effects of noise block on anxiety and vital sign of gynecologic laparoscopic surgery. Methods: The data were collected from March to May 2011. Participants were sixty patients with gynecologic laparoscopic surgery, divided into 30 of experimental group and 30 of control group at C University hospital located in I city. The day before surgery, demographic data, trait-state anxiety and vital signs were measured at ward. After noise block, the data were measured using VAS anxiety and vital signs before anesthesia and in recovery room. And then state anxiety and vital signs were measured in ward after surgery. The data were analyzed by $x^2$-test, t-test, repeated measured ANOVA and Bonferroni comparison method using SPSS/WIN 19.0. Results: After conducting noise block program, the experimental group showed significant decrease in state anxiety and heart rate compared to those of the control group. But there were not significant differences in VAS anxiety, systolic pressure and diastolic pressure between two groups. Conclusion: This program can be regarded as an effective nursing intervention for the management of anxiety with gynecologic laparoscopic surgery.

A Study on the Scattered Dose in Portable Chest Radiography (portable 흉부촬영시 공간산란선량에 관한 연구)

  • The purpose of this study is to survey the present status of portable radiography and the result of free space scattered dose rate when taking a radiography at the general hospital or the university hospital in Taejon city. The results were as follows; 1. The number of cases using portable radiography for three years increased to averages 16.2%, 7.7% per year from January 1st in 1996 to December 31st in 1998. 2. The average of distance of adjacent patients was 219.1 cm at the ward. 3. For portable chest radiography, the free space scattered dose rate was 10.5 mSv/hr at 50 cm distance, 1.8 mSv/hr at 100 cm distance, and 0.2 mSv/hr at 200 cm distance. Therefore, in case of portable chest radiography at the ward, the average of distance of adjacent patients is 219.1 cm, so it does not have influence on the adjacent patients. But during the portable radiography, a guardian who is close to the patient, doctor, nurse and radiologic technologists has to set up the shield to prevent from the unnecessary radiation or the distance should be as great as possible from the mobile X-ray equipment.

Protocol Development of Cardiopulmonary Resuscitation Nursing Tasks targeting Patients with Ventricular Fibrillation Generation (심실세동 환자의 심폐소생술 간호업무 프로토콜 개발)

  • Purpose: In order to improve resuscitation rate of CPR through providing qualitative nursing while performing CPR in hospital ward and to enhance quality of emergency nursing in the ward, the author embarked on the research as a way of developing ventricular fibrillation protocol about CPR. Method: Collected data were analyzed by using the SPSS/PC14.0 program while the routes of final protocol developed in this research are as follows. Result: Based on analysis results of literature study and CPR electronic hospital records, a total of 22 items and 53 specific contents were confirmed through the gathering of opinions from panels, and the allotment of roles and tasks with a standard of 2 nurses was designated. As the result of specialists' verification on validity, the final protocol composed of a total of 23 items and 45 specific contents was confirmed. At the result of the pertinency evaluation of confirmed protocol, it was evaluated as relatively pertinent with its average score 2.89-3.32. Conclusion: The protocol developed in this research is seen as to contribute to nurses participating in CPR to avoid the overlapping of tasks and to develop CPR through effective teamwork of medical teams by presenting clear roles and tasks.

A Study on the Remodeling Construction Execution Strategy of General Hospitals in Korea (국내 종합병원의 리모델링 공사수행전략에 관한 연구)

  • The construction can proceed in different ways according to the acquired profitability of the hospital during the construction and to the features of departments or areas. This study is an analysis of remodeling construction processes to resolve major tasks of remodeling. The remodeling strategies gained from this study can be summed up as follows: 1) Remodeling work in hospitals involves the acquire relocation of space through extensive area renovations and then moving back to the space, and lastly working on the empty space. Thus, it is more advantageous in terms of construction work to demolish the existing buildings than to acquire the relocation space through extensions or renovations. That is, demolition after the maximum utilization of the existing buildings is the most desirable in terms of space availability. 2) The construction methods for remodeling are two: a method of carrying out construction by dividing the plane areas into several individual ones and of working on it floor by floor. In case of ward areas, and the outpatient area, the construction proceeds after securing the relocation space and partially setting construction areas in order to minimize the decrease in profitability due to the smaller number of beds and treatment rooms during construction. If the outpatient diagnosis/ treatment area and the supply area relocate together with the ward areas, there may be extra expenses. Thus, doing construction by area, while partially operating those areas or after relocating the whole areas.

Status of Medical Care Psychiatric Quality Assesment in Busan (부산광역시 정신의료기관 적정성 평가지표 실태)

  • Objectives: The study aimed to compare the indicators of infrastructure of the whole country with those of Busan through National Medical Care Psychiatric (hospital level) Quality Assesment and to provide the basic evidence for presenting vision and strategies of mental health policy. Methods: National Medical Care Psychiatric (hospital level) Quality Assesment Data was used from 2011 to 2015. A total of 8 indicators were analyzed including facility and workforce. Results: The median value of the capacity per ward was decreasing with years both whole country and Busan. The number of beds out of total inpatient occupancy was improved over the years. There was no clear change in the number of inpatients per toilet in the hospitalized ward, but the indicator was decreasing obviously. Therefore, the median value of whole country and Busan was same with 10.2 in 2015. Conclusions: We confirmed that indicators for infrastructure were improving in a generally positive change over the years. The infrastructure is related to the patient-centered treatment environment, and the workforce is related to the quality of care. Therefore, the structural area should be continuously evaluated and improved.

The Job Performance, Importance, and Difficulty in Surgical Clinical Practice Nurses using Importance-Performance Analysis (중요도-수행도 매트릭스 분석 기법을 활용한 외과계 임상전담간호사의 업무 수행도, 중요도, 난이도)

  • Purpose: The purpose of this study was to describe the job performance, importance, and difficulty in Surgical Clinical Practice Nurses (SCPNs), and to identify the priority of job tasks using Importance-Performance Analysis (IPA). Methods: One hundred SCPNs were recruited from two university hospitals and two general hospitals during November 2021 to April 2022. A total of 97 data were analyzed excluding 3 data due to missing responses. The data were analyzed by two groups, with Ward-Based SCPNs (W-SCPNs) and Operation Room-Based SCPNs (OR-SCPNs) according to different characteristics of job tasks. Results: There were differences of job performance between W-SCPNs and OR-SCPNs. As a result of the IPA, tasks of 'working practices in the ward' for W-SCPNs and those of 'surgery and treatments' for OR-SCPNs were included in the first quadrant (keep up the good work). 'Resarch' and 'leardership' were included in the third quadrant (low priority). Conclusion: Since there are differences in job tasks between W-SCPNs and OR-SCPNs, health care institutions should consider the differences in defining the scope of job performance. Furthermore, it is necessary to develop customized continuing education program based on our findings of IPA.

Statistical Analysis Based on ICT for the Satisfaction and Service Evaluation of Patients Admitted to a Nursing Care Integrated Service Ward (간호간병통합서비스 병동 입원 환자의 만족도와 서비스 평가를 위한 ICT 기반 통계분석)

  • The Nursing Care Integrated Service is an inpatient service provided in a ward equipped with all kinds of professional nursing services, nursing environment improvement, and patient safety management through team nursing staff placement through appropriate nursing staff placement. The subjects of the study were 92 patients who agreed to understand and participate in the research purpose as the inpatient ward of a general hospital in Gyeonggi Province, for the study method, frequency, percentage, mean, standard deviation, t-test, ANOVA were applied using SPSS/WIN 21.0 program and the post test was Scheffe test. In addition, the correlation between nursing satisfaction and service evaluation was analyzed by Pearson's correlation. This paper is meaningful in that it reaffirms the importance of satisfaction and presents basic data for improving the quality of service of nursing care integrated service ward inpatients.

The Emotional Labor Status and Improvement Plans of Nurses Working in the Integrated Nursing Service Ward: Applying Focus Group Interviews (간호⋅간병통합서비스 병동 간호사의 감정노동 파악 및 개선방안: 초점집단인터뷰 적용)

  • Purpose: The purpose of this study was to investigate the status of emotional labor of nursing personnel working in comprehensive nursing service ward and to suggest the way service improvement can be achieved. Methods: A total of 28 nurses working in comprehensive nursing service ward were divided into four groups to conduct focus group interview. All interviews were recorded and transcribed after the interview to perform data analysis in the order of data classification, topic categorization, and keyword derivation. Results: The five categories of subjects and relating keywords drawn from the focus group interviews are as follows: 1) Emotional labor experience: suppressing emotions, expressing emotions or actions that are different from reality, 2) Situations of emotional labor: verbal abuse and assault, sexual harassment, personal needs and errands, 3) Responses to emotional labor: responding directly, responding directly, receiving senior's help, using the organizational system, persevering, 4) Problems caused by emotional labor: work exhaustion, job change intention, job stress, 5) Protection plan against emotional labor: manual or education for nurses, education for patients and carers, compensation, tough sanctions though system strengthening. Conclusion: This study shows that although nurses working in comprehensive nursing service ward generally experience high levels of emotional labor, the problem solving of them relies mainly on personal response. Therefore, it is necessary to develop various measures to protect nurses in an organizational level response, thus to improve the comprehensive nursing service system.

A Study on the Architectural Planning of the Planning Characteristics in the Ward History (병동의 발전과정에 수용된 계획요소에 관한 건축계획적 연구)

  • Wards are the biggest part in hospital buildings and the flow of medical personnels and materials is an important factor in the ward design. The evaluation method of the wards and is esscential for the optimum design considering function, environment, patient psychology, etc. To achieve this goal, factors for evaluation of wards were introduced and then each factor was systematically analyzed. A evaluation method are developed from studied factors and are applied to existing wards to check the reliability. The selected evaluation factors are limited to the privacy of patient, the functional efficiency of operation, and the economy of wards.
