의료서비스 향상을 위한 병원 행정전략의 우선순위 분석 (Analytical Study on the Priority in Hospital Administrative Strategy for Improvement of Medical Service)

  • 본 연구는 의료 환경의 변화로 병원의 경쟁력을 갖추기 위하여 병원의 행정전략에 대한 변화의 필요성이 있으며, 의료서비스 향상을 위해서 어떤 전략이 가장 중요한지 제시하여, 의료서비스 향상을 위한 병원 행정전략에 기초자료를 제공하고자 한다. 이를 위해서 병원행정 분야와 관련된 행정학 및 경영학 박사급 전문가 20명을 선정하였고, 자료분석방법은 AHP 방법으로 상대적 중요도를 측정하였다. 이에 대한 연구결과는 다음과 같다. 첫째, 측정영역에서 의료서비스 향상을 위한 병원 행정전략에 대한 상대적 중요도는 의료진 요인(1순위), 제반시설 요인(2순위), 직원서비스 요인(3순위), 접근편의 요인(4순위) 순으로 결과가 나타났다. 둘째, 복합가중치 분석 결과, 의료진 전문성(1순위), 의료진 다양성(2순위), 의료장비수준(3순위) 순으로 상대적 중요도가 측정되어 의료서비스 향상을 위한 병원 행정전략은 의료진의 전문성 확보, 의료진의 다양성, 그리고 의료장비 수준을 높이는 것이 가장 중요함을 알 수 있다. 이러한 결과가 의료서비스를 향상시키기 위한 병원의 행정전략 마련 시에 유용한 기초자료로 활용될 수 있을 것이며, 또한 향후 병원 행정전략 마련 시에 다각적인 요소들을 파악해야 할 것이다.

병원종사자들의 직무가치와 직장가치 인식 간의 관계에 조직성과 자각상태의 조절효과 (Moderating Effect of Organization Performance Recognition on the Relationship between Job Value and Organization Value of Hospital Employees)

  • 본 연구는 병원종사자들의 심리적 속성과 태도적 속성을 고려하여 조직차원에서 효과적인 인적자원 관리를 위한 시사점을 제공할 목적으로 시행하였으며, 연구방법은 부산지역에 소재한 종합병원 4개, 병원 5개, 요양병원 7개, 정신병원 7개의 의료기관에 종사하고 있는 근로자들을 대상으로 2019년 5월 2일부터 5월 25일까지 자료를 수집하여, 직무가치 인식과 직장가치 인식 간의 관계에 조직성과에 대한 자각상태의 조절효과를 살펴보기 위하여 평균중심화방법을 이용하여 3단계 모형 조절회귀분석을 실시하였다. 조절회귀분석 결과, 직무가치와 직장가치 간의 관계에 조직성과 자각상태의 조절효과가 있는 경우를 구체적으로 살펴보면, 첫째, 근무의료기관 종류가 병원인 경우의 간호사들은 고객대응 노력수준과 병원 이미지에 대한 자각이 조절효과가 있었다. 둘째, 근무의료기관 종류가 병원인 경우의 행정직 종사자들은 고객대응 노력수준에 대한 자각은 부정적인 조절효과가 있었으며, 병원 이미지에 대한 자각은 긍정적인 조절효과가 있었다. 셋째, 근무의료기관 종류가 요양병원인 경우의 간호사들은 성장과 경쟁력에 대한 자각은 긍정적인 조절효과, 병원 이미지에 대한 자각은 부정적인 조절효과가 있었다. 즉, 병원종사자들의 직무가치 인식과 직장가치 인식 간의 관계에 조직성과에 대한 자각상태의 조절효과는 근무의료기관 종류와 직종에 따라 다르다는 것을 알 수 있었다.

중국 중의약대학의 외국인 연수프로그램 현황에 대한 고찰연구 (Review of International Programs of Chinese Medicine University in China)

  • Objectives: This study aimed to survey and report on the international programs for Chinese Medicine in China. Methods: Online research was conducted based on a survey of official websites of 25 universities of Chinese medicine. In certain situations, we used e-mail or phone calls to get more detailed information. Results: Among 25 universities of Chinese medicine, 22 operate international programs for Chinese medicine. The main contents of the programs are acupuncture, moxibustion, tuina, Chinese materia medica, cosmetology or qigong, and an average 400 foreign students finish each program yearly. China has maintained the lead in international education of traditional Oriental medicine, and has already established a systematic and remarkable infrastructure for globalization of Chinese medicine. Conclusions: This study can inform the development of strategy in the process of raising the competitiveness of Korean medicine in the world market.

치의학 교육 수출의 전략과 과제 (Strategies and Tasks of Exporting Dental Education)

  • Recently, education is regarded as a service item and university education services are being exported abroad. In the form of educational export operations, export of educational contents and curriculum, specific consultancy projects for foreign institutes, installation of local educational facilities, or attracting foreign students are being carried out. Korea has the potential and competitiveness to export dental education. The advantages of Korea's dental education services, such as dental equipment and materials, excellent education programs, and high-quality human resources, will enable the export of education services in various ways. Establishment of educational infrastructure and educational programs for overseas dental students, export of educational consulting items, clinical training programs for foreign dentists abroad, invitation for international clinical workshops, dispatch of faculty members, exchange student programs can be considered as exporting dental education service items. Therefore, in a long-term perspective, it is necessary to establish differential and appropriate educational export plans.

  • One field that the information society has recently taken much interest in and has continuously implemented is medical services. Health tourism services which emphasize today's patient-oriented management rather than the past hospital-oriented management are highly value-added. As interest in physical fitness increases in keeping with such trends of the society at large as the declining birth rate and increasing life expectancy, and the medical market is opened, the added value of medical services that attract and manage domestic and foreign patients from the global market and satisfy their diverse desires with domestic medical competitiveness such as excellent medical staff and technology is on the spotlight. Hereupon, this study intends to suggest the possibility of developing competitive domestic medical services into a highly value-added industry in keeping with changing environment.

재무분석을 통한 한방병원의 경영성과 분석 - 재무비율 및 투자효율을 중심으로 (An Analysis of the Financial Performance of Korean Medicine Hospitals in Korea: Focusing on Financial Ratios and Investment Efficiency)

  • Objectives: This study investigated the financial performance of Korean Medicine hospitals in Korea in order to understand the current status of hospital management and improve its efficiency. Methods: Financial statements of 24 medical corporations, 19 juridical foundations and 18 school hospitals from 2016 to 2018 were obtained from the secondary data published by the Health Insurance Review and Assessment Service, the National Tax Service and the Korea Advancing Schools Foundation. Financial performance was measured on 6 dimensions: liquidity, profitability, activity, growth, cost and productivity (investment efficiency) by analyzing 8 financial indicators: Liability to Total Assets, Net Profit to Patient Service Revenues, Total Assets Turnover, Growth Rate of Patient Service Revenues, Operating Expenses to Patient Service Revenues, Value Added to Patient Service Revenues, Value Added to Total Assets, and Value Added to Personnel Expenses. Results: Korean Medicine hospitals showed lower Liability to Total Assets, Liquidity and Value Added to Total Assets than Western Medicine hospitals did. They also showed higher Value Added to Patient Service Revenues and Value Added to Personnel Expenses than Western Medicine hospitals did. They also showed higher Value Added to Patient Service Revenues and Value Added to Personnel Expenses than those of Western Medicine hospitals do. The net profit decreased significantly (-50.8%) in 2018 whereas Patient Service Revenues increased (6.9%) for the same period due to Operating Expenses increase and Non-Operating loss. Conclusions: These findings suggest that the Korean Medicine hospital sector in Korea needs to improve liquidity and financial structure and to enhance profitability by reducing Personnel Expenses and generating Non-operating revenues in order to improve its investment efficiency and competitiveness.

데이터마이닝 기법을 활용한 의료보험 진료비청구 삭감분석시스템 개발 및 구현에 관한 연구 (A Study on the Development and Implementation of a Data-mining Based Prototype for Hospital Bill Claim Reduction System)

  • 경제의 세계화와 지식정보화 사회로의 진입과 함께 초래된 경영환경의 급속한 변화는 의료기관들에게도 경쟁력강화를 위한 변신을 강요하게 되었다. 다시 말하면, 의료기관들은 선진 의료기술의 확보, 환자들에 대한 서비스제고와 함께 경영의 효율성 증대라는 세가지 목표를 동시에 달성해야만 하는 상황에 놓이게 된 것이다. 본 연구는 의료기관들이 당면하고 있는 이러한 세가지 과제 중 병원의 경영효율성 증대를 위한 한가지 대안으로 진료비 청구삭감의 빈도 및 발생 가능성을 낮추기 위한 해법의 마련이 시도되었다. 진료비청구삭감이란 의료기관들이 환자들에 대한 의료서비스에 대한 진료비 중 의료보험으로 인해 환자들이 감면 받은 진료비를 건강보험심사원에 청구하면, 심사원이 의료기관의 청구내역의 적정여부를 심사하여 적정하지 않은 내용에 대한 청구금액을 삭감하는 제도를 이른다. 청구금액에 삭감이 발생하면 해당 의료기관의 수입이 감소하는 것은 물론 원인분석이나 재청구 작업등에 비용과 인력이 이중으로 투입되게 되어 의료기관의 경영에 부담을 주게 되고, 이러한 상황이 빈발하게 되면 해당 의료기관에 대한 환자와 건강보험심사평가원의 신뢰에 문제가 발생하게 된다. 그러므로, 효과적인 진료비 청구삭감분석시스템에 의한 사전대비의 필요성이 높아지게 되는 것이다. 이를 위하여 본 연구에서는 진료비 청구삭감분석을 위한 프로토타입의 개발이 시도되었다. 프로토타입은 데이터마이닝 기법 중 연관분석 알고리즘을 적용하여 개발되었으며, 이렇게 개발된 프로토타입을 D의료원에서 10개월간 발생한 실제 진료데이타를 사용하여 성능을 시험하였다.

일 지역 상급종합병원 입원환자의 만족도 연구 (A study of inpatient satisfaction levels at general hospitals)

  • 본 연구는 병원에서 제공하는 입원 서비스에 대한 입원환자 만족도를 분석하여 보다 나은 의료서비스를 제공하는데 활용하고, 의료서비스를 향상시켜 타 병원과의 경쟁에서 우위에 설 수 있도록 기초자료를 제공하고자 본 연구를 시도하였다. G시에 소재한 지방 상급종합병원의 입원환자를 대상으로 2011년 9월19일부터 9월 30일까지 설문조사를 실시하여 320부를 최종 분석한 결과, 첫째, 입원환자 서비스 만족도는 총 4점 만점에 평균 3.25점이였고, 둘째, 일반적 특성과 의료인서비스 만족도 결과는 진료과, 건강상태, 입원횟수에서, 병원 환경. 입 퇴원 절차 및 기타서비스 만족도에서는 연령, 진료과, 건강상태, 입원횟수에서 통계적으로 유의하였다. 셋째, 입원환자 서비스 만족도간의 상관관계에서는 통계적으로 유의미한 정(+)의 상관관계를 보였고, 넷째, 입원환자 만족도에 미치는 요인을 분석한 결과 의사서비스와 기타서비스가 통계적으로 유의미한 정(+)의 영향을 미치는 것으로 나타났다. 결론적으로 입원환자 만족도를 높이기 위해서는 환자와 접촉이 상대적으로 많은 진료과의 인력을 충분히 확보하고 입원기간 동안 환자가 만족 할 수 있는 다양한 프로그램 개발과 운영이 요구되며, 그에 따른 효과를 검증할 수 있는 후속 연구가 필요하다.

병원 조직의 진성리더십 유형이 조직몰입과 조직시민행동에 미치는 영향에 관한 연구 : LMX의 매개효과를 중심으로 (A Study on the Effect of Authentic Leadership of Hospital Organization on Organizational Commitment and Organizational Citizenship Behavior: Focusing on Mediating Effect of LMX)

  • Purpose - This study is to clarify the structural relationship between the direct supervisor and organizational commitment and organizational citizenship behavior through mediation of the Leader-member exchange (LMX) in the hospital organization, and to confirm the mediating effect of the LMX. Through this study, we will provide basic data on the aspects of HR to enhance organizational immersion and organizational civic behavior in hospital organizations, and present suggestions on academic contribution and effective organizational operation measures. Research design, data, and methodology - This study is to investigate the relationship between authentic leadership and OC and OCB of the organization member, which is proposed to secure more active and high competitiveness in the recent changing medical environment, with multi-regression and B&K(1986) method. For this purpose, 653 employees of senior hospitals in Seoul and Kyeong-In area were surveyed using structured questionnaires. Each questionnaire was composed of 7-point scale, and each hypothesis was testified by multiple leaner regression and by BK method for testifying mediator effect. Result - As a result of the analysis, it was found that the male and non-medical staff had higher recognition level than the medical staff in the LMX, organizational commitment and organizational citizenship behavior. In the senior management or more than 5 years' experience group, organizational commitment and organizational citizenship behavior were recognized higher than the group of subordinate managers. Second, authentic leadership has a positive influence on LMX, organizational commitment and organizational citizenship behavior, and LMX has a significant positive impact on organizational commitment and organizational citizenship behavior. In addition, as a result of analyzing LMX mediating effect in the relationship of authentic leadership and organizational effectiveness by BK method, LMX had partial mediating effects between authentic leadership and organizational citizenship behavior, and between authentic leadership and organizational commitment. Conclusion - Results of the study confirmed that all the members of the hospital organization that were lacking served as positive factors for the relationships of authentic leadership and LMX, and authentic leadership and organizational effectiveness. It is expected that the hospital organization will be able to suggest such as the organizational approach to expect high organizational effectiveness centering on authentic leadership ability as psychological capital.

병원 간호인력의 간호활동량에 관한 연구 (An analysis on nursing activity in a hospital)

  • The management of nursing resource is very important. that is because nursing staff accounts for 30-40% of total staff in a hospital and nurses provide patoents with attentive service for hours daily. The continuous turnover of nursing staff. however, impedes the quality-oriented nursing care, which will consequently leads to the loss of human and material resource in competitive society. This study aimed to calculate nurse's activity and compare the activity amount based on career experience. Futhermore it aimed to find factors which would influence 'quality weighed direct nursing activity amount'. Questionnaires and check lists for this study were distributed to nurses in a suburban hospital outside of Seoul from October 20 to November 14. 1997. The nursing activities were calculated according to professionality. independency and working hour. And then it were accumulated by quality score. The collected data was analyzed by statistical methods as t-test, ANOVA, correlation, multiple regression. The results of this study were as follows ; Firstly, carrel' experience had no influence on the quality weighed nursing activity amount. But the quality level of service of skilled nurses was higher than that of new nurses. Secondly, career was a variable affecting the quality in nursing service. So career was positively related to the quality of nursing care. Patient's disease severity and number of patient were positively correlated with weighed nursing activity amount. But job satisfaction was negatively correlated v:ith the amount. Thirdly, the independent variables which had significant influence on the weighed nursing activity amount were disease severity and the number of patients, The severity score and number of patient were directly proportional to the weighed nursing activity amount. This results indicated· that weighed nursing activity amount was influenced by the number of patient and patient's disease severity. The quality score of nursing services for experienced members is higher than that of new staff. But both new and skilled staff showed no difference in the quality weighed nursing activity amount. Internal and. external environment influences nursing activities. The quality of nursing services is very important factor in nursing activity. Therefore nursing managers should make an effort to improve nursing care quality through continuous research. Also they should try to maintain experience nurses and assign nursing staff appropriately with patient's severity and other relevant factors being considered. The quality-improved nursing care in the hospital will strengthen hospital's competitiveness.
