Objectives: Radiotherapy is an effective treatment in the initial management of T1 glottic carcinoma, but local recurrent or residual tumor growth is found in approximately 10% of the patients. Even in recurrence or residual tumor, in highly selected cases, conservation surgery with preservation of a portion of the larynx is feasible. So we investigated the efficacy of salvage vertical partial laryngectomy for recurrent glottic carcinoma after irradiation. Material and Methods: Retrospectively we reviewed a consecutive series of 10 patients treated by conservative vertical partial laryngectomy of the larynx for radiation-failure, recurrent T1 glottic squamous cell carcinoma treated at Severance Hospital from 1994 to 2002. Results: Local recurrence developed in 4 patients (40%). Two patient was salvaged by further totallaryngectomy. Successful salvage was achieved finally in 8 (80%) of the 10 patients. There was no operative mortality. Postoperative perichondritis was developed in two patients. One was resolved by conservative treatment, but the other has underwent the total laryngectomy. Conclusion: A vertical partial laryngectomy in case of recurrent glottic carcinoma after a course of irridation should be the therapy of choice when stringent criteria are observed.

Background/Aims: Gastric varices (GVs) are a major cause of upper gastrointestinal bleeding in patients with liver cirrhosis. The current treatments of choice are balloon-occluded retrograde transvenous obliteration (BRTO) and the placement of a transjugular intrahepatic portosystemic shunt (TIPS). We aimed to compare the efficacy and outcomes of these two methods for the management of GV bleeding. Methods: This retrospective study included consecutive patients who received BRTO (n=157) or TIPS (n=19) to control GV bleeding from January 2005 to December 2014 at a single tertiary hospital in Korea. The overall survival (OS), immediate bleeding control rate, rebleeding rate and complication rate were compared between patients in the BRTO and TIPS groups. Results: Patients in the BRTO group showed higher immediate bleeding control rates (p=0.059, odds ratio [OR]=4.72) and lower cumulative rebleeding rates (logrank p=0.060) than those in the TIPS group, although the difference failed to reach statistical significance. There were no significant differences in the rates of complications, including pleural effusion, aggravation of esophageal varices, portal hypertensive gastropathy, and portosystemic encephalopathy, although the rate of the progression of ascites was significantly higher in the BRTO group (p=0.02, OR=7.93). After adjusting for several confounding factors using a multivariate Cox analysis, the BRTO group had a significantly longer OS (adjusted hazard ratio [aHR]=0.44, p=0.01) and a longer rebleeding-free survival (aHR=0.34, p=0.001) than the TIPS group. Conclusions: BRTO provides better bleeding control, rebleeding-free survival, and OS than TIPS for patients with GV bleeding.
Objectives: The purpose of this study is to demonstrate empirically through a specialist AHP analysis what factors should be more important in the development of the public health care system. In addition, we will use Analytic Hierarchy Process (AHP) method for experts to achieve research purpose. Methods: The data analysis method of this study is as follows. First, we set up three metrics in order to measure the relative importance between the factors to be improved for the development of the public health care system and each of the sub-factors. A total of nine measurements (items) were set by combining the three measurement criteria for each measurement index. Second, the relative importance and priority analysis use the AHP analysis. Third, the subjects of this study were 15 experts in the field of public health care. The statistical processing was performed using the Expert Choice 2000 statistical program. Results: In order to development of the public health care system, experts ranked the most important as improvement in the systematic aspect of public health care (56%) as the first priority. Next, the relative importance analysis of the measurement items considering the multiple-weights of the sub-factors is as follows. The strengthen institutional improvement (revitalization of secondary public function hospital) was the number one, strengthen cooperation between agencies was the second, and Re-establishing the role of local public health care system was the third place. Conclusions: Considering the relative importance, factors that are considered to be important in the first place may not be improved as the best policy alternative due to limitations in spatial, temporal, financial, and institutional aspects. In this case, we suggest that we should choose the best policy alternative by using prioritization considering relative weights.

Last year, the elderly population aged 65 years and over exceeded 14.2 percent of the total population, and Korea has entered the ageing society. At the same time, social interest in improving the health level and quality of life in the elderly has also increased. In particular, pain is one of the most common diseases experienced by middle-aged and elderly people and is one of the factors that closely affect the quality of life. The purpose of this study was to analyze the factors affecting the pain level and pain region of Korean medicine users and the use of Korean medicine in the Korean middle aged and elderly using the 6th Korean Longitudinal Study of Ageing (KLoSA) panel. As a result, the main influencing factors of Korean medical care were subjective factors such as gender, subjective health status, and education level. In particular, half of the middle-aged and elderly who used Korean medicine experienced back and knee pain, and generally complained of low level of pain. This study suggests the need for research and development of Korean medicine that can control and relieve the pain of the middle-aged and elderly.

Depending on the location of the lung tumor, the choice of treatment technique should be considered when treating the Cyberknife. The 4DCT images of 18 lung cancer patients were analyzed, and location error values were extracted through application program. The evaluation result was lower than the average position error only in the upper and the inner. These results suggest that the Vertebral tracking technique is effective when it is close to the pulmonary attachment or near the vertebral body, and the Synchrony technique is effective at other positions. In the future, we would like to study cyber knife treatment technique according to the position of the tumor as well as the volume of the lung and the respiratory cycle.

Objective : There is a lack of knowledge regarding whether decompression is necessary in treating patients with epidural spinal cord compression (ESCC) grade 2. The purpose of this study was to compare the outcomes of minimally invasive surgery (MIS) without decompression and conventional open surgery (palliative laminectomy) for patients with hepatocellular carcinoma (HCC) spinal metastasis of ESCC grade 2. Methods : Patients with HCC spinal metastasis requiring surgery were retrospectively reviewed. Patients with ESCC grade 2, medically intractable mechanical back pain, a Nurick grade better than 3, 3-6 months of life expectancy, Tomita score ${\geq}5$, and Spinal Instability Neoplastic Score ${\geq}7$ were included. Patients with neurological deficits, other systemic illnesses and less than 1 month of life expectancy were excluded. Thirty patients were included in the study, including 17 in the open surgery group (until 2008) and 13 in the MIS group (since 2009). Results : The MIS group had a significantly shorter operative time ($94.2{\pm}48.2minutes$ vs. $162.9{\pm}52.3minutes$, p=0.001), less blood loss ($140.0{\pm}182.9mL$ vs. $1534.4{\pm}1484.2mL$, p=0.002), and less post-operative intensive care unit transfer (one patient vs. eight patients, p=0.042) than the open surgery group. The visual analogue scale for back pain at 3 months post-operation was significantly improved in the MIS group than in the open surgery group ($3.0{\pm}1.2$ vs. $4.3{\pm}1.2$, p=0.042). The MIS group had longer ambulation time ($183{\pm}33days$ vs. $166{\pm}36days$) and survival time ($216{\pm}38days$ vs. $204{\pm}43days$) than the open surgery group without significant difference (p=0.814 and 0.959, respectively). Conclusion : MIS without decompression would be a good choice for patients with HCC spinal metastasis of ESCC grade 2, especially those with limited prognosis, mechanical instability and no neurologic deficit.
Background Thoracodorsal vessels (TDVs) and internal mammary vessels (IMVs) have both been widely employed as recipient vessels for use in free muscle-sparing transverse rectus abdominis myocutaneous (MS-TRAM) flaps. However, whether TDVs or IMVs are preferable as recipient vessels for autologous breast reconstruction with a free MS-TRAM flap remains controversial. The purpose of this study was to compare the clinical outcomes when TDVs were used as recipient vessels to those obtained when IMVs were used as recipient vessels for autologous breast reconstruction with a free MS-TRAM flap. Methods A retrospective matched-cohort study was performed. We retrospectively reviewed data collected from patients who underwent a free MS-TRAM flap for autologous breast reconstructions after mastectomy between March 2003 and June 2013. After a one-to-one matching using age, 100 autologous breast reconstructions were selected in this study. Of the 100 breast reconstructions, 50 flaps were anastomosed to TDVs and 50 to IMVs. Patient demographics and clinical outcomes including operation time, length of hospital stay, postoperative complications, and aesthetic score were compared between the two groups. Results No statistically significant differences were found between the two groups in patient demographics and clinical outcomes, including the complication rates and aesthetic scores. There were no major complications such as total or partial flap loss in either group. Conclusions The results of our study demonstrate that both TDVs and IMVs were safe and efficient as recipient vessels in terms of the complication rates and aesthetic outcomes.

Recommendation Systems is the top requirements for many people and researchers for the need required by them with the proper suggestion with their personal indeed, sorting and suggesting doctor to the patient. Most of the rating prediction in recommendation systems are based on patient's feedback with their information regarding their treatment. Patient's preferences will be based on the historical behaviour of similar patients. The similarity between the patients is generally measured by the patient's feedback with the information about the doctor with the treatment methods with their success rate. This paper presents a new method of predicting Top Ranked Doctor's in recommendation systems. The proposed Recommendation system starts by identifying the similar doctor based on the patients' health requirements and cluster them using K-Means Efficient Clustering. Our proposed K-Means Clustering with Content Based Doctor Recommendation for Cancer (KMC-CBD) helps users to find an optimal solution. The core component of KMC-CBD Recommended system suggests patients with top recommended doctors similar to the other patients who already treated with that doctor and supports the choice of the doctor and the hospital for the patient requirements and their health condition. The recommendation System first computes K-Means Clustering is an unsupervised learning among Doctors according to their profile and list the Doctors according to their Medical profile. Then the Content based doctor recommendation System generates a Top rated list of doctors for the given patient profile by exploiting health data shared by the crowd internet community. Patients can find the most similar patients, so that they can analyze how they are treated for the similar diseases, and they can send and receive suggestions to solve their health issues. In order to the improve Recommendation system efficiency, the patient can express their health information by a natural-language sentence. The Recommendation system analyze and identifies the most relevant medical area for that specific case and uses this information for the recommendation task. Provided by users as well as the recommended system to suggest the right doctors for a specific health problem. Our proposed system is implemented in Python with necessary functions and dataset.

Purpose: (i) To evaluate the biologic properties of a bi-layered 1-ethyl-3-(3-dimethylaminopropyl)carbodiimide hydrochloride-cross-linked collagen membrane (CCM) in vitro. (ii) To assess the efficacy of CCM for localized bone regeneration in vivo. Materials and Methods: Biodegradation of CCM compared to a native collagen membrane (NCM) was assessed in vitro. In vivo, twelve male New Zealand White rabbits were used. Four calvarial, circular defects (diameter 8 mm) were created in each animal. The sites were randomly allocated to i) CCM+biphasic calcium phosphate (BCP) (CCM-BCP group), ii) CCM alone (CCM), iii) BCP alone (BCP) and, iv) negative control (control). Animals were sacrificed at 2 (n=6) and 8 weeks (n=6). Outcome measures included: micro-computed tomography (μCT) analysis (total augmented volume [TAV], new bone volume) and histomorphometry (total augmented area [TAA], newly formed bone, remaining membrane thickness [RMT]). Result: CCM was more resistant to degradation than NCM. μCT analysis showed CCM-BCP (196.43±25.30 mm3) and BCP (206.23±39.13 mm3) groups had significantly (P<0.01) larger TAV than the control (149.72±12.28 mm3) after 8 weeks. Histomorphometrically, CCM-BCP group (17.75±5.97 mm2) had significantly (P<0.01) greater TAA compared to the CCM group (7.74±2.25 mm2) and the control (8.13±1.81 mm2) after 8 weeks. After 8 weeks, RMT was reduced by 67%. Conclusion: CCM can be a favorable choice of barrier membrane when performing guided bone regeneration (GBR) in localized bone defects. CCM has better resistance to degradation than the natural collagen membrane, in vitro. In vivo, CCM provides an advantageous integration of prolonged barrier function and biocompatibility for GBR.

Background and Objectives Vocal polyp is one of the most common benign diseases of vocal fold caused by overuse of voice. Laryngeal microsurgery is the first treatment of choice for vocal polyp. However, surgery has many risks such as side effects of general anesthesia, injury of tooth and psychological burden. And we often experience reduction of vocal polyps without surgical procedure. The purpose of study is to evaluate the effect of non-surgical treatment such as vocal hygiene education and proton pump inhibitor (PPI) in patients with vocal polyp. Materials and Method We performed retrospective study for seventy-three patients of vocal polyp who treated with non-surgical modalities such as vocal hygiene education and PPI over three months. Treatment outcomes and risk factors such as age, sex, polyp size, position, symptom duration, presence of laryngopharyngeal reflux (LPR) symptoms, smoking history, voice abuse history and vocal hygiene education were evaluated by comparison between polyp size improved group and non-improved group. Results 5.5% of enrolled patients showed complete response and 23.3% showed partial response without surgery. Polyp size improved group significantly carried out more practice of vocal hygiene education treatment than the non-improved group (p=0.040). And the presence of LPR symptoms [hazard ratio (HR) 3.368, confidence interval (CI) 1.055-10.754, p=0.040] and not performing of vocal hygiene education (HR 3.664, 95% CI 1.078-12.468, p=0.038). Conclusion Vocal hygiene education can be a useful treatment option when making a decision to treat with vocal polyp.
