A feasibility study of audio-video communication application using mobile telecommunication in inter-hospital transfer situations (병원 간 전원 상황에서 이동통신망을 이용한 음성화상정보통신 애플리케이션의 유용성에 관한 연구)

  • Purpose: The aim of this study was to evaluate the usefulness of video communication with medical staff located at a remote location, through a communication application connected to a long term evolution (LTE) mobile communication network in a moving ambulance. Methods: In this study, we recruited patients who were transferred by mobile intensive care unit ambulance from one hospital to another. In the moving ambulance, the information of the patient was transferred to a physician using the application in real time. Recorded video files were evaluated by emergency physicians with experience in video direct medical control. Results: A total of 18 patients were evaluated, and the average score was 5.9 out of a possible 9. It was expected that applying the use of the technology to actual clinical sites would enable the sites to provide assistance. Conclusion: In this study, we confirmed the possibility of benefiting from the clinical field when using the video-audio communication application which is connected to the remote location in real time through the current LTE mobile communication network in the ambulance.

A Study of The Number of Patients per Physician for a Day at Hospital and Its Relating Factors (병원급 의료기관 의사 1인당 일일 진료환자 수와 관련요인)

  • The purpose of this paper is to find the factors affecting the number of patients per physician for a day at hospital and compare them with the results of previous studies. We used data of Korean national patients survey that had been carried out by central government and local health centers from October 27th to November 11th in 1999. Descriptive analyses were performed to overview the general characteristics of our study subject. Also, we tried to find the factors affecting the number of patients per physician for a day at hospital through t test and correlation analysis. Factors affecting the number of outpatients per physician for a day at hospital are location, training type, ownership, No. of physicians, No. of beds per physician, No. of physicists per physician, No. of nursing aids per physician and No. of medical technicians per physician. While factors affecting the number of inpatients per physician for a day at hospital are location, ownership, No. of physicians, No. of beds per physician, No. of pharmacists per physician, No. of nurses per physician, No. of nursing aids per physician and No. of administrative auxiliary personnels per physician. The most correlated variable with outpatients per physician for a day at hospital is No. of medical technicians per physician while the most correlated variable with inpatients per physician for a day at hospital is No. of beds per physician. A more detailed investigation is needed for verifying factors affecting the number of patients per physician for a day at hospital and other medical institutions.

  • RFID is estimated to have great potential for hospitals to reduce various costs, reconsider patients' safety, and provide high quality medical service. As the ubiquitous technology allows the medical market to expand, medical centers all around the country are fiercely competing against one another. In order to increase the hospital's competitive edge by reducing IT expenses and concentrating on medical practice, the need to establish a u-Hospital using advanced IT technology is rapidly rising. This paper implements an RFID based u-Hospital system by using ubiquitous computing technology to design a location tracking device for all surgical patients, which can secure the patients' safety and increase the quality of medical service through interlocking hospital information systems.

  • Objectives : The purpose of this study was to investigate the location of LI20 and ST3 by facial measurement according to Sasang Costitution, Sex and Age. Methods : To collect the photograph of face, we used the face measurement apparatus, and then analysed whether ST3 was located out of the nasolabial sulcus or not. Results : ST3 was located at the lateral to the nasolabial sulcus in 94.9%; 92.1% of Soyangin, 97.7% of Taeumin, 94.4% of Soeumin 92.3% of male, and 92.3% of female according to Sasang Constitution and sex. According to age distribution, the highest percentage(100%) was appeared at $16{\sim}20$ age, and the lowest(91.8%) was appeared at $70{\sim}74$ age. Conclusion : This study suggests that actual measurement would be nessesary with classical literature to determine the location of acupuncture point, and we need more study in other race.

Percutaneous Thrombin Injection Based on Computational Fluid Dynamics of Femoral Artery Pseudoaneurysms

  • Objective: To analyze the computational fluid dynamics (CFD) of femoral artery pseudoaneurysm (FAP), identify a suitable location and timing for percutaneous thrombin injection (PTI) based on this analysis, and report our clinical experience with the procedure. Materials and Methods: CFD can be used to analyze the hemodynamics of the human body. An analysis using CFD recommended that the suitable location of the needle tip for PTI is at the center of the aneurysm sac and the optimal timing for starting PTI is during the early inflow phase of blood into the sac. Since 2011, seven patients (three male and four female; median age, 60 years [range, 43-75 years]) with FAP were treated with PTI based on the devised suitable location and time. Prior to the procedure, color Doppler ultrasonography was performed to determine the location and timing of the thrombin injection. Results: The technical success rate of the PTI was 100%. The amount of thrombin used for the procedure ranged from 200 IU to 1000 IU (median, 500 IU). None of the patients experienced any symptoms or signs of embolic complications during the procedure. Follow-up CT images did not reveal any embolism in the lower extremities and showed complete thrombosis of the pseudoaneurysm. Conclusion: Based on our study of CFD, PTI administered centrally in the FAP during early inflow, as seen on color Doppler, can be an effective technique.

  • A RFID technology that process information of tag attaching in physical object by a point technology that enable this Ubiquitous computing through radio frequency is noted. One of our most important, location tracking for object in RFID system. With this in mind, proposed EPCglobal network gives the location tracking and location services. but, RFID system has the drawbacks. That is special construction and components be positively necessary for RFID system. The service difficult to integrate other service and it increases costs considerably. This thesis gives location service using RFID based on SIP in hospital.

The Radiological Findings of the Catheters Inserted 10cm Cephaladly in Epidural Space (경막외강내 두측으로 10cm 삽입한 카테터의 X-선상 소견)

  • We have inserted epidural catheter for single or continuous injection of a drug for epidural analgesia. It is important to localize the tip of epidural catheter in appropriate site to acquire the most effective analgesia. In epidural block, we observed course and location of the tip of epidural catheter. Subject: 70 patients were divided into group I(non-injection of saline group during catheter insertion) and group II(injection group during catheter insertion). Group I included cervical(n=20), thoracic(n=10), and lumbar(n=20) epidural group. Group II, cervical(n=10), and lumbar(n=10) epidural group. Method: 19G FlexTip $Plus^{TM}$ Epidural Catheter ($Arrow^{(R)}$) was inserted 10cm cephaladly in epidural space with(group II) or without(group I) saline flushing. We observed course and location of the tip of epidural catheter by C-arm image intensifier during injection of contrast media ($Omnipaque^{(R)}$). Result: In group I, the number of tips of epidural catheters located within 2 cm from inserted site were: cervical 14/20(70%), thoracic 2/10(20%). lumbar 16/20(80%). In thoracic epidural blocks, tips of epidural catheters were more cephaladly located than with cervical and lumbar epidural blocks. With cervical epidural blocks, the number of tips of epidural catheters located within 2 cm from insertion site were less in group II than group I (20% vs. 70%). But no significant differences were noted between group I and group II with lumbar epidural block(90% vs. 80%). The number of tips of epidural catheters located around a predicted site were: cervical 2/20(10%), thoracic 4/10(40%), lumbar 0/20(0%) in group I, and cervical 2/10(20%), lumbar 1/10(10%) in group II. Conclusion: It was impossible to predict the exact location of tips of epidural catheters by measuring the inserted length without epidurogram. With many cases, tips of epidural catheters were located around the insertion site in lumbar epidural blocks, and in some cases around the predicted site in thoracic epidural blocks. The results suggests that epidural block should be done at a point near the required band of analgesia.

Gender Differences in Paediatric Patients of the Swiss Inflammatory Bowel Disease Cohort Study

  • Purpose: Gender differences in paediatric patients with inflammatory bowel disease (IBD) are frequently reported as a secondary outcome and the results are divergent. To assess gender differences by analysing data collected within the Swiss IBD cohort study database since 2008, related to children with IBD, using the Montreal classification for a systematic approach. Methods: Data on gender, age, anthropometrics, disease location at diagnosis, disease behaviour, and therapy of 196 patients, 105 with Crohn's disease (CD) and 91 with ulcerative or indeterminate colitis (UC/IC) were retrieved and analysed. Results: The crude gender ratio (male : female) of patients with CD diagnosed at <10 years of age was 2.57, the adjusted ratio was 2.42, and in patients with UC/IC it was 0.68 and 0.64 respectively. The non-adjusted gender ratio of patients diagnosed at ${\geq}10$ years was 1.58 for CD and 0.88 for UC/IC. Boys with UC/IC diagnosed <10 years of age had a longer diagnostic delay, and in girls diagnosed with UC/IC >10 years a more important use of azathioprine was observed. No other gender difference was found after analysis of age, disease location and behaviour at diagnosis, duration of disease, familial occurrence of IBD, prevalence of extra-intestinal manifestations, complications, and requirement for surgery. Conclusion: CD in children <10 years affects predominantly boys with a sex ratio of 2.57; the impact of sex-hormones on the development of CD in pre-pubertal male patients should be investigated.

Assessment of Korean Hospitals Management Using Dupont Analysis (듀퐁 분석을 통한 한국 병원계의 경영 현황 분석)

  • Purposes: The hospitals needs to generate a minimum profit, in order to perform its own role such as providing high-quality medical services. The demand for hospital management is increasing, as the social demands are diversified and the financial transparency is emphasized. The purpose of this study is to compare hospitals management based on Dupont Identity, by various hospital classification. Methodology: This study is based on '2016 Statistics for Hospital Management' provided by the Korea Health Industry Development Institute. The hospitals were classified according to the scope of care, the type of establishment, the location, and the number of beds. We analyzed the general and financial characteristics of over 337 hospitals using the method of Dupont Identity. Findings: Net profit margin (PM) has the biggest impact on return of equity (ROE). By the number of beds, general hospital with 160-299 beds have the highest return on equity (ROE). By location, hospitals in local municipalities have higher return on equity than hospitals in urban municipalities. According to the type of establishment, public hospitals have lower business performance, and although they invest more than private hospitals. Practical Implications: This study can inspire interest and provide understanding in hospital management and financial structure, by analyzing through an intuitive indicator named Dupont identity. It is possible to provide basic data for hospital management methods for each financial elements, in order to increase the profitability of hospitals.

A Case of Lingual Thyroglossal Duct Cyst Treated by CO2 Laser via Transoral Approach (경구강 CO2 레이저를 이용하여 치료한 설갑상설관 낭종 1예)

  • A lingual thyroglossal duct cyst(LTGDC) is a rare congenital anomaly that account for only 0.5% to 2% of total thyroglossal duct cyst. LTGDC is frequently associated with respiratory problem in infants and pharyngeal foreign body sensation or dysphagia in adults. Because of its location and characteristics, lingual thyroid, dermoid cyst, and vallecular cyst should be included in differential diagnosis. Standard treatment for thyroglossal duct cyst is sistrunk's operation, but in terms of LTGDC, because of its location and cosmetic reasons, different kinds of treatments such as electrical cauterization, $CO_2$ laser, Robort surgery via transoral approach have been introduced. Recently authors encountered 21 years old woman with LTGDC and the mass was removed successfully via transoral approach using $CO_2$ laser. We report the clinical course with review of the literature.
