Clinical application of gonadotropin-releasing hormone analogs in children and adolescents

  • Although the increasing incidence of central precocious puberty (CPP) in Korea has recently raised public concerns about health and growth problems, there are many areas of uncertainty regarding the pathogenesis, diagnosis, and management of CPP. In this paper, we review the definition of precocity, the assessment of CPP, and the hormonal abnormalities that support the diagnosis. In addition, we review the practical guidelines regarding the clinical use of gonadotropin-releasing hormone analogs in children with CPP. Indications for treatment, determination of dosage, monitoring during treatment, and discontinuation of therapy are discussed.

Resistance to Thyroid Hormone Syndrome Mutation in THRB and THRA: A Review

  • Resistance to thyroid hormone syndrome (RTH) is a genetic disease caused by the mutation of either the thyroid hormone receptor-β (THRB) gene or the thyroid hormone receptor-α (THRA) gene. RTH caused by THRB mutations (RTH-β) is characterized by the target tissue's response to thyroid hormone, high levels of triiodothyronine and/or thyroxine, and inappropriate secretion of thyroid-stimulating hormone (TSH). THRA mutation is characterized by hypothyroidism that affects gastrointestinal, neurological, skeletal, and myocardial functions. Most patients do not require treatment, and some patients may benefit from medication therapy. These syndromes are characterized by decreased tissue sensitivity to thyroid hormones, generating various clinical manifestations. Thus, clinical changes of resistance to thyroid hormones must be recognized and differentiated, and an approach to the practice of personalized medicine through an interdisciplinary approach is needed.

한방 난임 치료 시범사업에 참가한 여성의 임신 성공에 영향을 미치는 요인에 관한 연구 (A Study on Factors Influencing Pregnancy in the Pilot Project for Korean Medical Treatment for Subfertility)

  • Objectives: This study aims to identify associated factors in female infertility treatment using herbal medicine, acupuncture and moxibustion. Methods: Acupuncture and moxibustion treatments were performed with intake of herbal medicines (Jokyungjongoktanggagam-bang, Sutaehwangagam-bang) for six menstrual cycles in 32 subfertile women. The variables selected for initial analysis were age, duration and type of infertility, frequency of previous In Vitro Fertilization (IVF) and live births, weekly frequency of sexual intercourse, number of treatment cycles, endometrial thickness on MCD15 and hormonal parameter (anti-mullerian hormone, follicle-stimulating hormone, luteinizing hormone, estrone). Results: Six patients became pregnant (18.75%) and seven patients withdrew. Factors influenced pregnancy were age (${\leq}35$ years), a shorter duration of infertility (<6 years), frequency of IVF (${\leq}3$), anti-mullerian hormone level (>1 ng/ml), and number of treatment cycles(${\leq}3$ cycles). Conclusions: Korean medicine is a useful and optimized treatment option for women with younger age, fewer previous IVF cycles, shorter infertile duration and normal anti-mullerian hormone level at first 3 cycles of treatment.

조현병 입원 환자에서의 갑상샘 기능이상과 증상 심각도, 치료 반응과의 관계 (Association between Thyroid Dysfunction and Severity, Treatment Response in Schizophrenic Inpatients)

  • Objectives Thyroid hormone deficiency during the neurodevelopmental period can impair brain development and induce psychiatric symptoms. This study examined the association between thyroid dysfunction and the severity of symptoms in schizophrenia patients, and the treatment response of patients with schizophrenia. Methods Three hundred thirty-eight schizophrenia patients, with no prior history of thyroid disease or taking medication associated with it, were studied. We assessed the blood thyroid hormone level, the Brief Psychiatric Rating Scale (BPRS) scores on the day of admission and discharge, admission period, dose of administered antipsychotics, and the number of antipsychotic combinations. The collected data were subsequently analyzed using the Kruskal-Wallis test and Pearson's chi-square test. Results The percentage of schizophrenia patients who presented with abnormal thyroid hormone level was 24.6%. High total triiodothyronine (TT3) (p = 0.003), low TT3 (p = 0.001), and high free thyroxine (fT4) (p < 0.001) groups showed a higher BPRS score on admission than did the normal thyroid hormone group, while thyroid stimulating hormone (TSH) levels were not significantly correlated with the severity of symptoms. Furthermore, thyroid hormone was not associated with the treatment response assessed by the rate of BPRS score reduction, admission days, use of clozapine, and dose of antipsychotics. Conclusions The TT3 and fT4 hormone levels were significantly associated with the severity of symptoms in schizophrenia patients. These relations suggested that thyroid dysfunction may be associated with the severity of schizophrenia. And hence, further analysis of the results of the thyroid function test, which is commonly used in cases of psychiatric admission, is required.

녹용의 물 추출액 투여가 난소를 절제한 흰쥐의 여성호르몬 변화에 미치는 영향 (The effect of water extract of antler on serum level of female hormone in ovariectomized rats)

  • The effect of water extract of antler on serum level of female hormones was investigated in ovariectemized rats. Sprague-Dawley rats were ovariectomized(ox). After a further 40 days the animals were administrated with water extract of antler (625mg/kg daily) for 10, 20 and 30 days, respectively. Serum level of estradiol, progesterone, leutenizing hormone and follicle stimulating hormone were measured. Significant increase of serum estradiol level was elicited at 20 and 30 days after treatment with water extract of antler, respectively, Associated with the increase of serum estradiol level, there was a concomitant decrease in serum follicle stimulating hormone level. Serum progesterone level was also significantly increased at 30 days after treatment with water extract of antler. Although serum leutenizing hormone level of ox rats treated with water extract of anther was slightly lower than that of untreated ox rats, the decrease was not significant.

Efficacy and safety of growth hormone treatment for children born small for gestational age

  • Recombinant growth hormone (GH) is an effective treatment for short children who are born small for gestational age (SGA). Short children born SGA who fail to demonstrate catch-up growth by 2-4 years of age are candidates for GH treatment initiated to achieve catch-up growth to a normal height in early childhood, maintain a normal height gain throughout childhood, and achieve an adult height within the normal target range. GH treatment at a dose of $35-70{\mu}g/kg/day$ should be considered for those with very marked growth retardation, as these patients require rapid catch-up growth. Factors associated with response to GH treatment during the initial 2-3 years of therapy include age and height standard deviation scores at the start of therapy, midparental height, and GH dose. Adverse events due to GH treatment are no more common in the SGA population than in other conditions treated with GH. Early surveillance in growth clinics is strongly recommended for children born SGA who have not caught up. Although high dose of up to 0.067 mg/kg/day are relatively safe for short children with growth failure, clinicians need to remain aware of long-term mortality and morbidity after GH treatment.

  • This study was conducted to abserve the effects of cortisone acetate, estrogenic hormone and tocopherol on the antibody tresponse to both pullorum suspects and negatives of The increase of antibody titer was shown in a few birds after the treatment of either cortisone acetate or estrogenic hormone. There wuas no change in antibody titer of chickens treated with tocopherol.

성장호르몬 치료의 최신 지견 (Current use of growth hormone in children)

  • Since the advent of growth hormone(GH), children with a wide variety of growth disorders have received GH treatment. In GH deficiency(GHD), Turner syndrome, chronic renal failure, children born small for gestational age, Prader-Willi syndrome, and idiopathic short stature, the therapeutic effects and safety profile of GH are reviewed. GH therapy has been clearly shown to improve height velocity and final adult height in a variety of pediatric conditions in which growth is compromised irrespective of GHD. Early initiation and individualization of GH treatment has the potential to normalize childhood growth. The supra-physiological doses of GH have been shown to increase height velocity during childhood and final height in non-GHD conditions. Adverse events during GH therapy are uncommon and often not drug related. However continued surveillance into adult life is crucial, especially in children receiving supra-physiological doses or whose underlying condition increases their risk of adverse effects.

  • The growth effects of newly developed recombinant human growth hormone (rHGH), were compared with those of Biotropine. For the effective evaluation, we examined the increasing rate of body weight and the thickness of tibial epiphysis as criteria of growth effects on hypophysectomised female rats treated with varing concentration of rHGH for 4 days. rHGH treated groups showed significant body weight gain which was less evident in Biotropine and vehicle treatment group. In tibial epiphyseal test, rHGH also showed clear effects compared to Biotropine and vehicle treatment group. Above findings indicate that newly developed rHGH has better effects of growth stimulation on female rats than Biotropine does.
