Study on Perceived Symptoms and Preventive Behaviors during Asian Dust storms among Children, Adults and Elders (황사 기간 동안에 어린이, 성인, 노인에서의 자각증상과 예방행동에 대한 조사연구)

  • Purpose: Recently there are scientific evidences for the health effects of Asian dust-storms. Particularly in 2002, the daily average of $PM_10$ exceeded 600 and 700 $\mu{g}/m^3$ in March and April respectively. We examined the effects of Asian dust‐storms on perceived symptoms and preventive behaviors among children, adults, and elders. Method: The subjects of this study were 425 children, 444 adults and 60 elders. A questionnaire survey was carried out on the children and the adults by teachers and on the elders by interviewers to obtain information on demographic variables, perceived symptoms and preventive behaviors, previous respiratory disease, environmental tobacco smoke, and proximity between the house and the road during Asian dust-storms. We analyzed using descriptive statistics, $X^2$ test and multiple logistic regression models. After adjustment for potential confounders, we estimated the odds ratios and 95% confidence intervals of the children and the elders for perceived symptoms and preventive behaviors. Result: The elders had a significantly higher prevalence of respiratory symptoms than the other groups. The children had a significantly higher prevalence than the adults in getting URI (1.51), coughing (1.68), rhinorrhea (1.46), fever (2.39) and medication for allergy or asthma (1.90). The children had better behaviors than the elders in closing windows (3.57), taking less outdoor recreational activity (2.59) and staying home (2.60). Conclusion: This study suggested that children and elders are susceptible to the effects of Asian dust on health. Health educators should inform these populations about the influence of Asian dust and provide health education for preventive behaviors.

Main Reasons for Choosing to Stay in a Paid Elderly Residential Facility (유료 노인주거복지시설에 거주하게 된 이유에 관한 연구)

  • This study has been performed to explore main reasons for staying in paid elderly residential care facilities among the residents. In this study, five males and seven females aged 65 to 82 were asked about main reasons for staying in the facilities. The answers of the qualitative interview were drawn as follows; first, some residents had suddenly decided to move into the facility due to critical life events such as bereavement, serious illness, or supporting problems. Second, some residents had taken a long period of considerations on whether staying home or in the facilities, because their health and family supports gradually diminished over the long life span. On the other hand, a few of them voluntarily chose to stay at the facilities, because they recognized supervised group activities, nutritionally well-planned meals, and health care as major inconveniences. In this study, suggestions were made as follows: first, guiding and settlement programs should be developed for the residents of the sudden events, particularly at early stage of admission. Second, intensive nursing care units should be in separation from healthy independent groups, so that the residents are relieved from stressful contacts with extremely ill patients in the residential area.

Development and Effects of Nocturnal Emission and Menstruation Education Program using CAI for Korean Elementary School Children (CAI (컴퓨터 이용학습)를 활용한 후기 학령기 아동의 몽정과 월경 교육 프로그램 개발 및 효과)

  • Purpose: The purpose of this study was to develop a nocturnal emission and menstruation education program using CAI for Korean elementary school children. Methods: The research design was methodological study a ISD model 4 fields into sub contents in each field of the nocturnal emission and menstruation by CD titles. And to verify the effect of the education program, performance evaluation of the educational program for structured questionnaire was conducted on 120 late school-age children on June, 2010. Results: The results of this study were as follows; Through this, developed program was proved its effectiveness in enhancing knowledge level on nocturnal emission and menstruation in late school-age children(t=14.03, p<.001, t=11.52, p<.001). Conclusion : Accordingly, this program is expected to be an educational program to be used in various educational institutes, communities, and home as well as self-study that allows children themselves to study repeatedly, choosing the contents they want, whenever they hope as an educational program on nocturnal emission and menstruation in school-age children. In addition, it is suggested that various fields of programs should develop in consideration of early sexual maturity.

Obesity Rate and Sexual Knowledge in Menarche and Non-Menarche among 6th Grade Elementary School Girls (초등학교 6학년 여학생 월경군과 비월경군의 비만도와 성지식)

  • The purpose of this study was to investigate the level of obesity and sexual knowledge between age-matched 168 postmenarcheal and 168 premenatcheal girls among 6th grade elementary school girls in Busan. Anthropometric measurements were taken for height and body weight of subjects for obesity and a questionnaire for sexual knowledge representing 5 categories was developed by researcher. For the statistical analysis of the collected data, frequency, Chi-square, t-test, ANOVA, and Pearson correlation were performed by SPSS WIN Vesion 12.0 program. These measurements of menarcheal grils were significantly higher than those of non-menacheal girls in height, body weight, BMI and obesity level. The sexual knowledge was low, expecially they didn't know well about sexual disease and contraception. There was no significant difference between two groups, but there was significant difference among obesity level, obesity group had sexual knowledge than others. And there were significant positive relations between the menarche and the obesity. The most important findings in the study showed that is needed the systemic and concrete sexual education in home, school and community, and especially by the professionals in the sex education field.

Confirmatory Analysis of Infant Temperament Inventory (WBL; What my baby is like) (영아기질 측정도구(WBL)의 신뢰도 및 타당도 평가)

  • This study was conducted to test the reliability and validity of "What my baby is like(WBL)" that was developed by Pridham et al. for measuring infant temperament. 37 subjects were participated four times-1, 3, 6, and 12months, and 146 cases were analyzed. The construct validity for the scale was tested by factor analysis with four factor loading solution based on the previous study. One item was excluded because total-item correlation was too low, so that the number of total items were eighteen. The analysis yielded four well defined factors: amenability and persistence, activity, reactivity, and adaptability. These four factors explained 57.62% of the variance in the infant temperament. The criterion related Validity was confirmed by the relationship between the infant temperament and HOME, and between the infant temperament maternal confidence in childrearing. The reliability of the scale was determined with Cronbach's alpha coefficient and Guttman split-half coefficient. Cronbach's alpha was .86, for 18 items, and Guttman split-half cofficient was .81. In subscale analysis, Cronbach's alpha was .56~.84. In conclusion, WBL showed a high degree of validity and reliability, suitable in Korean infants for measuring temperament. In the further study, it would be necessary to refine the concept of temperament and to analyze the correlation between WBL and other temperament inventory.

A Study on the Current Status and Services of Energy Performance Certificates in Healthcare Building - Focused on UK and Korea (의료기관의 에너지효율등급 현황과 서비스에 관한 연구 - 영국과 한국을 중심으로)

  • Purpose: Medical center has consumed the energy about twice the average value of building types. So, many countries have tried sustainability and energy reduction for building types through eco friendly certificate. This study investigate the eco friendly building certificate and energy performance certificate focusing in UK and Korea, and search the characteristics of status and services of certificates. Methods: BREEAM openbook and EPC open data have been studied in UK, and green building performance certificate and EPC data in korea have been analyzed. Results: UK has eco friendly building certificate focusing on healthcare, and EPC shows the standard, benchmark reference value, so lead to the CO2 emission reduction. Primary health center and hospital show the higher value due to the dwelling unit. like a patient bed. Korea doesn't have green building certificate yet on healthcare, and EPC shows not the reference data, but the primary energy consumption. Korean cases just like in UK, clinic, public health, and outpatient center show the lower value compared to medical center, nursing home and general hospital. Implications: In Korea, it is needed for energy efficiency to have reference data information like a standard and benchmarking value for new design and construction.

Effect of Home-based Rehabilitation on Balance and Gait Function in Patient With Stroke: A Systematic Review and Meta-analysis

  • Stroke is one of the most common disabilities experienced by the elderly in the community. However, stroke progresses to a chronic level, patients are discharged from medical institutions and eventually no longer receive therapeutic interventions at home. In this systematic review, we compared home-based rehabilitation (HBR) with comparison for patients with stroke. Literature published in Cumulative Index for Nursing and Allied Health Literature (CINAHL), Embase, Physiotherapy Evidence Database (PEDro), PubMed, and Google Scholar were reviewed. A total of 1,158 studies were initially retrieved. After reading the full texts, 11 articles were included in the systematic review. Quality assessment of the included studies was conducted using Risk of Bias (RoB) 2.0, and Egger's regression test was used to evaluate publication bias. Data analysis was performed using the R studio software (R Studio). According to the quality assessment using RoB 2.0, three studies were evaluated as low risk, two as of some concern, and three as high risk. The overall effect size was moderate (0.309). The value of the balance function was a small effect size (0.201), while the value of the gait function was a moderate effect size (0.353). The values were small and moderate effect (0.154, 0.411) for the chronic and subacute conditions, respectively. According to the Egger's regression test, no publication bias was observed. The findings of this study indicate that HBR resulted in the greatest improvement in gait function in patients with subacute stroke compared to those with chronic stroke. Therefore, the application of this intervention to patients with stroke in the community is recommended.

Research Trend of the Healthcare and Medical Care for Elders in the Journal of the Korea Gerontological Society (한국노년학의 보건·의료·건강영역 연구동향)

  • In this study we review the selected articles on elderly health and medical care published in the Journal of the Korea Gerontological Society (JKGS) in the last 40 years, and make suggestions for future research directions for gerontological health and medical care issues. Of all the 40 year publications from volume 1 (1980) to 38 (2017), we first examined the 30th anniversary review on the subject of gerontological nursing care and healthcare policies published in JKGS from 1980 (vol. 1, No. 1) to 2008 (vol. 28, No. 2), and reviewed recent 237 researches of this decade (out of all 655 articles from 2008, vol. 28, No. 3 to 2017, vol. 38, No. 4). We could find the following trends. Firstly, the analysis of the primary authors in the past 10 years revealed that those in public health, nursing and other health-related including physical education areas have dealt the subjects focusing on physical health while those in social welfare mostly on mental health. That is, physical health has been the prime subject of researches in the health and medical care area. Secondly, in the same period quantitative researches were accounted for 89.9%, which is similar to the trend of the first 30 years 81.5 %. On the other hand, qualitative studies were only 11 cases and the focus group interview were the most frequently used method comprising 33.3% among them. Thirdly, the non-experimental researches in the past 10 years comprise 65.4%, which was 82.7% in 1980 2008 period, indicating the increasing trend in experimental researches to deal with the issues in medical and healthcare fields. Lastly, the subjects of the researches were mostly the elders who are healthy, residents of city areas, or home dwellers, and 60% of them were over 65 years old in the past 10 years while the proportion was 42.7% in the previous review period. 81.6 % of the researches in the past 10 years was dealing both genders, slightly decreased trend compared to 88.5% of the previous review period. This study reveals that the researches in non-experimental physical health remains the main stream of JKGS despite the efforts by some researchers to diversify the methods and subjects. Systematic and in-depth researches employing multidisciplinary, qualitative, longitudinal and meta-analytical approaches are called for to guide the gerontolgical health issues with preventive and proactive perspectives.

Effects of Support from Spouse and Family on the Mental Health of Marriage Immigrant Women (결혼이주여성의 배우자지지, 가족지지가 정신건강에 미치는 영향)

  • By identifying the effects of support from spouse and family on the mental health of marriage immigrant women, this study was proposed in obtaining basic data for a mental health intervention program. This study is a descriptive correlation study conducted on 129 marriage immigrant women as subjects. The result of the study showed differences in the following: for anxiety, depending on their country of origin, length of marriage, family structure, marriage structure, visit back home, and Korean language skill; for depression, depending on their spouse's education level, providing financial support for family back home, and Korean language skill; for somatization disorder, depending on their spouses' education level, area of residence, and occupation. Also, anxiety and depression showed a negative correlation with support from spouse and family, and somatization disorder showed a negative correlation with support from family. The main variable for mental health was support from family; for anxiety, the main variables were country of origin, length of marriage, and marriage structure; for somatization disorder, the main variables were spouse's education level, area or residence, and occupation. Based on the results of this study, a variety of mental health programs helpful for immigrant women in adapting to their multi-faceted and complex life in a different culture and nursing interventions for improving support from family would need to be developed.

Effects of the Nurse-led Discharge Education on Symptom Experience and Self-Care Compliance in Patients with Chronic Obstructive Pulmonary Disease (일대일 퇴원교육이 만성폐쇄성 폐질환자의 증상경험 및 자가간호수행에 미치는 효과)

  • Purpose: This study was conducted to examine the effects of the nurse-led one-on-one discharge education on the levels of symptom experience and self-care compliance among patients with chronic obstructive pulmonary disease (COPD) who had not previously participated in an education or respiratory rehabilitation program. Methods: This study used a quasi-experimental pre-and post-test design with nonequivalent control group. Sixty seven hospitalized patients (34 for experimental and 33 for control groups) were recruited from in-patient units of a general hospital from August to November, 2010. The one-on-one education session of symptom management and daily self-care was administered to experimental group for 50 minutes along with a follow-up phone call. Self-administered questionnaire was administered at the pretest and at 1 month after discharge. Results: The subjects were 67.43(${\pm}10.24$)years old in average. The mean differences of symptom experience (t=3.39, p=.001) and self-care compliance (t=-38.13, p<.001) in the experimental group was significantly higher than those of the control group. Conclusion: Nurse-led one-on-one discharge education was effective in reducing the level of symptom experience and enhancing self-care compliance at home within one month after discharge. Therefore, this form of one-on-one education provided by nurses might be applicable effectively for COPD patients who do not have access to respiratory rehabilitation programs.
