The Effects of Task Oriented ADL Exercise in Different Environments on ADL in The Persons with Chronic Stroke (환경에 따른 과제 지향적 일상생활동작 훈련이 만성 뇌졸중 환자의 일상생활에 미치는 영향)

  • PURPOSE: The purpose of this study was to investigate the effects of task oriented ADL exercise in different environments in patients with chronic stroke. METHODS: A total of 28 patients with hemiplegia resulting from stroke were included in this study. The patients were randomized into two groups. The control group(n=14) was received neurodevelopment therapy + task oriented ADL exercise and experimental group(n=14) was received neurodevelopment therapy + home based task oriented ADL exercise for 30 minutes twice per week during 6weeks. A task oriented ADL exercise pretest and postest design was used examine the change of FIM(Functional Independent Measure) and K-MBI(Korea-Modified Bathel Index) at the completion of 6 weeks task oriented ADL exercise. RESULTS: After 6 weeks, the experimental group compared with control group showed a significant improvement (p<.05) in FIM and K-MBI scores. CONCLUSION: The results indicate that the home based task oriented ADL exercise improves functions in the persons with chronic stroke.

  • Purpose: This study was to evaluate and compare the effectiveness of the Resistance Exercise Program on pain, stiffness, ADL and bone mineral density in Arthritis Patients at home. Methods: Forty-two patients participated in this study and they were divided randomly into two groups (exercise and control group). Two subjects dropped during the study period. The exercise group had a structured training which was 50 minutes in duration, five times per week for a period of 16 weeks and the control group without any scheduled treatment. Data were gathered for 16 weeks (May 21, 2007- September 10, 2007) and were analyzed by using SPSS Win 12.0. Results: The pain, stiffness, ADL, balance and bone mineral density in the exercise group were improved significantly after the Resistance Exercise Program. However, a significant improvement on the above mentioned variables was not observed in the control group.

Effectiveness of Upper Extremity Exercise and Bandage on the Edema and ROM of Patients with Lymphedema (상지운동과 붕대법이 상지림프부종 환자의 부종과 관절가동범위에 미치는 영향)

  • Purpose : The purpose of this study was to examine the effect of upper extremity exercise and Bandage on the edema and range of motion. Complex decongestive physiotherapy was one of the latest methods to deal with the limitation of range of motion and the edema that were the prime problems of patients with lymphedema. Methods : Twenty patients undergone mastectomy were participated in this study voluntarily. They had lymphedema on upper extremity and partial limitation of range of motion. The subjects had been treated with upper extremity home exercise and complex decongestive physiotherapy with Bandage for 4 weeks. The measure of these patients with upper extremity edema was included: the volume, arm circumfenence and range of motion. These were measured two times: before the treatment, after 4 weeks of the treatment. Results : The upper extremity edema of patients treated home exercise group and complex decongestive physiotherapy group with Bandage was definitely decreased (p<0.05). Moreover, the upper volume of those who were the same condition also significantly shrank and patients' arm circumferences of upper arm breathtakingly diminished as well (p<0.05). But home exercise group patients' arm circumferences of forearm was not remarkable(p>0.05). However, patients' range of motion who were treated with upper extremity exercise were evidently increased when compared to that of the before treatment (p<0.05). Conclusion : Complex decongestive physiotherapy and bandage performed by physical therapist increased the ROM of upper arm and reduced the edema in patients with lymphedema.

The Effects of an Exercise Program on Joint Angles, Blood Pressure, and Self-efficacy in Elderly Community-dwelling Adults (유연성 운동프로그램이 노인의 관절각도, 혈압 및 자기효능감에 미치는 효과)

  • Purpose: The purpose of this study was to develop and evaluate an exercise program for elderly community-dwelling adults. The program was intended to improve joint flexibility and self-efficacy and to reduce blood pressure. Methods: A quasi-experimental, nonequivalent control group pre-post test design was used to evaluate the program. Twenty-six subjects were enrolled in the experimental group, and twenty-seven subjects were enrolled in the control group. The program was carried out between March 6 and May 4, 2006. The experimental group subjects participated in the exercise program once a week for eight weeks and received a phone call weekly to encourage self-exercise at home. Results: After program participation, the experimental group subjects showed significantly higher flexibility in the right shoulder joint and higher self-efficacy than subjects in the control group. Systolic and diastolic blood pressures were also significantly lower in the experimental group subjects. However, there were no significant differences in knee joint flexibility between the two groups after the intervention. Conclusion: The eight-week exercise program was effective in decreasing blood pressure and improving joint flexibility and self-efficacy in older adults. This study suggests that this exercise program could be utilized as an effective independent nursing intervention modality in elderly persons.

An Effect of Aquatic Exercise through Home Nursing Care Demonstration Program for Patients having Rheumatoid Arthritis (가정간호 시범사업을 통한 류마티스 관절염 환자의 수중운동의 효과)

  • This study was undertaken to evaluate the effect of long-term(6 Month) aquatic exercise through home nursing care demonstration on body weight, body fat, pain, muscle strength, self-efficacy and quality of life. Twenty-one women with rheumatoid arthritis were enrolled in the study. The sample was restricted to women in order to increase homogeneity. All patients were stable medication regimens in rheumatism center. The data was collected from October, 1994 to November, 1995. The data were analyzed Mean, Standard Deviation, $x^2$ Test, Mann-Whitney U-Wilcoxon Rank Sum W Test, Wilcoxon Matched-pairs signed-ranks test using SPSS $pc^+$ program. The results of this study were as follows : 1. There was no significant difference between the control and the experimental group on body weight. 2. There was no significant difference between the control and the experimental group on body fat. 3. There was no significant difference between the control and the experimental group on pain(RAI). 4. There was no significant difference between the control and the experimental group on muscle strength. 5. There was no significant difference between the control and the experimental group on quality of life. But in the experimental group, finding from additional analysis showed significant difference in the score of quality of life between pre and post experiment. 6. There was no significant difference between the control and the experimental group on self-efficacy. The findings in this study were not support the positive effect of long-term aquatic exercise in women with rheumatoid arthritis. A further study is necessary to determine the pure effect of aquatic exercise except the effect of home nursing care.

Effects of a Short-term Home-based Pulmonary Rehabilitation Program in Patients with Chronic Lung Disease (만성폐질환자를 위한 가정 호흡재활 단기 프로그램의 효과)

  • Purpose: The purpose of this study is to exam the effects of a short-term pulmonary program on lung function, exercise tolerance, and quality of life in chronic lung patients. Method: Randomized controlled pre-post test design was used. The outcome measures were forced expiratory volume in one second (FEV1, % predicted), 6 min walking distance (6MWD), Borg score after 6MWD, and Chronic Respiratory Disease Questionnaire (CRDQ). Experimental group performed the 4-week home-based pulmonary rehabilitation program composed of inspiratory muscle training, upper and lower extremity exercise, relaxation, and telephone visit. Patients in control group were only given education about self-management strategies. Thirty four patients with moderate-to-severe respiratory impairment were recruited, and 28 patients (19 in experiments, 15 in control) completed the study. Result: Significant improvements in lung function, exercise tolerance, and health related quality of life were found only in the experiment group. Conclusion: This study yielded evidence for the potential and beneficial effects of home-based pulmonary rehabilitation program in patients with moderate to severe chronic lung disease. The program could be adequately utilized for improvement of health related quality of life in chronic lung patients.

Effects of Home Exercise Program on the Improvement of Balance in the Elderly (가정운동 훈련프로그램이 노인의 균형증진에 미치는 영향)

  • The purpose of this study was to investigate the effects of home exercise program on the improvement of balance in the elderly. Subjects were twenty members living in Daegu (10 males, 10 females), between 65 and 81 years of age. The subjects for the experimental group were participated in the home exercise program for 8 weeks, between April 2001 and June 2001. The results of this study were as follows: After the exercise program, balance ability by CTSIB in the experiment group improved with no statistical significance. On the other hand, balance ability by OLST was significantly improved in the condition with open eyes than dosed eyes. Balance ability by OLSTR was improved 37.2% (p<0.001) on the hard surface with open eyes and improved 41.1% (p<0.01) on the soft surface with open eyes. Balance ability by OLSTL improved 37.1% (p<0.001) on the hard surface with open eyes and improved 72.5% (p<0.001) on the soft surface with open eyes. Balance ability by BBT also improved 1.7% (p<0.05).

The Effect of Case Management for Clients with Hypertension.DM Registered in Customized Home Visiting Health Care Services (일 지역 맞춤형 방문건강관리사업의 고혈압.당뇨병 사례관리 효과분석)

  • Purpose: To analyze the effects of customized home visiting health services on the health and health behaviors of clients with hypertension (n=107) and diabetes mellitus (DM: n=67). Methods: A one group pre and post-test research design was used. The subjects were registered in a customized 8-week, interventional, home visiting health services available in Daegu. Data was collected from November 17, 2008 to January 23, 2009. Analyses involved descriptive statistics, $x^2$ test and paired t-test. Result: Hypertension control rate was improved 25.2% and DM control rate was improved 3.0%. There were significantly beneficial hypertension-related differences in BP, health belief, health knowledge and health behaviors including performance of 10 min of moderate exercise, diet, BP monitoring and medication. Significantly beneficial DM-related changes included glucose, health belief, health knowledge and health behaviors including performance of 10 min of moderate exercise and glucose monitoring. But there were no significant hypertension-related differences in health belief (barrier) and health behavior including drinking and exercise length/frequency. Also, no significant DM-related differences were evident in health belief (barrier) and health behaviors including drinking, smoking, exercise length/frequency, diet and medication. Conclusion: Customized home visiting health service can provide effective, but not complete. Whether these benefits are maintained in the longer term is unknown.

Effect of the Fall Prevention Program(EPP) on gait, balance and muscle strength in elderly women at a nursing home (낙상예방 프로그램이 양로원 여성노인의 보행, 균형 및 근력에 미치는 영향)

  • Purpose: To determine the effect of the Fall Prevention Program(EPP) on gait, balance and muscle strength in elderly women at a nursing home. Method: The subjects of this consisted of 38 elderly women between the ages of 70 to 89 years living at a nursing home located in Seoul. Each of the experimental group and control group was composed of 19 subjects. The subjects in experimental group have participated in FPP for the 8 weeks which consisted of exercise, education and foot care. They started to exercise for 40 minutes per session, 3 sessions a week during the 1st week at 40% of age adjusted maximum heart rate. From the 2nd week to the 4th week, they increased the duration of exercise to 50 minutes per session and the intensity to 60% of age-adjusted maximum heart rate. They participated in 50 minutes at 60% of age-adjusted maximum heart rate from the 5th week to the 8th week. Each exercise session consisted of 10 minutes of warming-up exercise, 30 minutes of conditioning exercise and 10 minutes of cooling-down exercise. They participated in education for 20 minutes per week from the 1st week to the 4th week. Then they participated in a 30-minute foot care program per week from the 5th week to the 8th week. Gait, balance and muscle strength for each subject were measured before and after FPP. Gait was evaluated by step length, step width, gait speed and walking distance. Balance was measured by the duration of standing on one leg with their eyes closed and open each, and a get-up and go test. Grip strength was measured by hand dynamometer. Hip extensor and flexor strength, knee extensor and flexor strength and ankle plantarflexor and dorsiflexor strength were measured by manual muscle tester. Data was analyzed using SPSS form Windows. t-test and Chi square test were utilized as a homogeneity test. Repeated measure ANOVA was used to test the effect of FPP. Result: 1) Step width significantly decreased, and step length, gait speed and walking distance significantly increased in the experimental group compared with the control group after FPP(p<0.005). 2) There was no significant change in standing time on one leg with their eyes closed after FPP. The standing time on leg with their eyes open and the time of "get-up and go" significantly decreased in the experimental group compared with the control group after FPP(p<0.005). 3) Muscle strength-grip strength, hip extensor and flexor strength-significantly increased in the experimental group compared with the control group after FPP(p<0.005). 4) There was no significant difference of frequency of fall between the experimental group and control group during the period of FPP. Conclusion: These results suggest that FPP can increase gait, balance and muscle strength of elderly women at a nursing home.

Effects of 12 weeks of home-based exercise program in patients with ankylosing spondylitis (강직성 척추염 환자에 대한 12주간의 가정기반 운동 프로그램의 효과)

  • This study was performed to provide detailed and comprehensive information on inflammation-related blood indicators, joint range of motion, pain scale, and psychological indicators by patient characteristics by performing a 12-week home-based exercise program for ankylosing spondylitis patients. For the purpose of this study, 10 patients with ankylosing spondylitis were selected by age (30s vs. 40s vs. 50s), gender (male vs. female), and duration (less than 5 years vs. 5 years or more). The home-based exercise program was a combination of aerobic exercise and Pilates-based resistance exercise, and was performed 4 times a week for 12 weeks at an intensity of 50-70% of maximal heart rate (MHR). As a result, after 12 weeks of home-based exercise intervention, the blood C-reactive protein (CRP) concentration of patients with ankylosing spondylitis decreased (-35.6%, p=.002), and the blood inflammation level was improved, and each joint (hip, lumbar, cervical) improved mobility (p<.05). In addition, the bath ankylosing spondylitis disease activity index (BASDAI) was decreased by -67% (p=.001) and the visual analogue scale (VAS) was decreased by -64.8% (p=.001), stiffness and pain has been alleviated. In particular, as the degree of depression decreased by -65.5% (p=.001) and the degree of anxiety by -55.2% (p=.003), 12 weeks of home-based exercise improved not only physical changes but also psychological factors. On the other hand, there was no difference in exercise effect according to age, gender, and disease duration in ankylosing spondylitis patients (p>.05). These results suggest that the 12-week home-based exercise applied in this study can be an effective exercise program that can be universally used for ankylosing spondylitis patients regardless of patient characteristics.
