Development of Educational Program for Hospice Smart Patient Service Provider (호스피스 스마트 환자 서비스 제공자를 위한 교육과정 개발)

  • Purpose: The purpose of the study was to develop an educational program reflecting the educational needs of Hospice Smart Patient service providers. Method: The description, goal, curriculum, method, and process evaluation of the educational program were constructed based on Modified Tyler-type Ends-Means Model followed by the analysis of current curriculum and needs of service providers. Results: The curriculum was constructed based on hospice volunteer program currently offered in Korea and the recommendations of hospice service volunteers and experts. A total of 90 hr was required to complete the curriculum that was composed of 'Introduction to cancer', 'Treatment and treatment complications of cancer', 'Post-treatment nutritional care', 'Helpful information', 'Introduction to hospice and palliative care', 'Comprehension of life and death', 'Holistic hospice and palliative care', 'How to communicate as a smart patient', 'Hospice and ethics', 'Pediatric hospice', 'Bereavement management', and 'Clinical practicum'. Conclusion: It is necessary to implement the developed educational program and evaluate its effectiveness, as well as making the service available to a greater number of cancer patients.

The Experience of Life of the Physically Handicapped Adolescents (지체장애 청소년의 삶의 과정)

  • The purpose of this study was to construct a grounded theory as the basis for developing future nursing interventions by describing and analyzing the holistic lives of the physically handicapped adolescents. The subjects were 10 physically handicapped adolescents who were attending a rehabilitation school located in Kyunggi region and could understand and respond to the questions. The aim and purpose of the study were explained to the subjects, and only those who accepted to participate in the study were selected as the actual study subjects. The study data were collected from July to November 2000. The investigators provided detailed explanations to the subjects about keeping the anonymity of the subjects name and confidentiality of the information provided by the study subjects. The subjects received in-depth interviews by the investigators. Each interview took about 40 minutes to 1 hour and 30 minutes, and each subject received interviews with the range of one to 3 times. All interviews were recorded under the permission of the subjects. The investigators formed field notes after finishing each interview. In the process of data analysis, 'realizing the difficulty due to physical handicap' was found to be the causal condition, whereas' acceptance of physical handicap' was identified as the context. Then, we identified' awaring as the valuable being' as the core phenomenon, 'support from family and social group and spiritual help' as the intervening condition, 'effort for adaptation' as the strategy, and' finding the meaning of life' as the consequence. Through the theoretical model derived from this study, an important aspect the lives of physically handicapped adolescences was identified that' awaring as the valuable being' help them cope better and find the meaning of life in spite of suffering they experience.

Experiences of Ego Integrity Recovery in Elderly Cancer Patients: Grounded Theory Approach (노인 암환자의 자아통합감 회복 경험: 근거이론 접근)

  • Purpose: This study was conducted to derive a substantive theory on lived experiences of elderly cancer patients. Methods: The data were collected from February to March 2018 through in-depth personal interviews with 14 elderly cancer patients. The collected data were analyzed based on Corbin and Strauss's grounded theory. Results: The core category was "the journey to find balance in daily lives as a cancer patient by recovering disturbed ego integrity." The core phenomenon was "shattered by suffering from cancer," and the causal conditions were "physical change" and "limitations in daily life." The contextual conditions were "decreased self-esteem," "feelings of guilt toward the family," and the sense of "economic burden." The participants' action and interaction strategies were "maintaining or avoiding social relations," "seeking meaning of the illness," "falling into despair," and "strengthening the willingness to battle the cancer." The intervening conditions were "support from health care providers and family," "dissatisfaction with health care providers," "spiritual help from religion," and "the improvement or worsening of health conditions." The consequences were "having a new insight for life," "living positively along with cancer illness," and "the loss of willingness to live." A summary of the series of processes includes the "crisis stage," "reorganizing stage," and the "ego integration stage." Conclusion: This study explored the holistic process of ego integrity impairment and the recovery experience of elderly cancer patients. This study is expected to be used as a basis for the development of nursing interventions that can support patients when coping with all stages of their cancer illness trajectory.

Impact of Anger Expression Style and Occupational Stress on the Risk of Cardiovascular Disease in Middle-aged Male Workers (중년남성 근로자의 분노표현양식과 직무 스트레스가 심혈관질환 발생위험에 미치는 영향)

  • Purpose: This study aimed to investigate the factors affecting cardiovascular disease in middle-aged male workers. Methods: The participants were 137 male office workers over 40 years old and under 60 years old working in small- and medium-sized workplaces from three southern provinces of Korea. Data were collected through self-reported questionnaires and health screening reports from the 2014 National Health Examination. The influencing factors included general characteristics of participants, anger expression style, and occupational stress. Data were analyzed using t-test, ANOVA, and regression analysis with SPSS 22.0. Results: The mean score of risk of cardiovascular disease was 6.73±4.69 and there were significant differences in exercise (t=2.13, p=.035) working time (t=-2.15, p=.034). Logistic regression analysis showed that, when adjusted for exercise and working time, the probability of becoming under a cardiovascular disease risk was 21% higher for those who anger-in (OR=1.21, 95% CI=1.02~1.44, p=.027) and 12% higher for those perceived occupational stress (OR=1.12, 95% CI=1.01~1.24, p=.030). Conclusion: The results suggest developing the strategies for middle-aged male workers to encourage exercise and to decrease occupational stress, as well as an appropriate anger expression style to improve holistic aspect of health considering their demographic characteristics.

A Study of the Lived Experiences of Clients Receiving Long-Term Hemodialysis (장기 혈액투석 수혜자들의 생활경험에 관한 연구)

  • The purpose of this study was to construct a grounded theory as the basis for nursing intervention by describing and analysing the holistic lived experiences of clients receiving long-term hemodialysis. The subjects of this study were fifteen persons receiving regular hemodialysis regimen at artificial kindey treatment centers in two different university hospitals, and who were able to participate in conversation and were available for long and dup interviews. Eight of the subjects were male and seven were female and their ages ranged from 30's to 60's. The length of the hemodialysis experience ranged from two months to six years. The collection and analysis of data were done in accordance with the grounded theory methodology of Strauss & Corbin. The method to collect the data mainly depended. on long and deep interviews, participant observation and focused group interviews and the equipment used to collect data were a portable tape recorder and field notes. The study is summarized as follows : 1. The meaning of holistic lived experiences of clients receiving long -term hemodialysis was found to be uncertainty. which was identified as the core category. 2. The main categories following the core category were found to be shock, ambiguity, social support and quality of life. 3. Through the main category the type of behavior newly formed by clients receiving long-term hemodialysis was found to be as follows. That is to say, in the circumstances of shock caused by the identified fact and the ambiguity of hemodilysis they formed a quality of life based on social support, which was found to be a kind of chaotic phenomenon. 4. The lived experiences of clients receiving long-tern hemodialysis was found to include nine categories : emotional shock, feelings of isolation, burden, unclearness, dependency, help from others, coping strategies, maintenance of self-esteem and transitional life. 5. The intervening factors influencing each category are as follows : 1) The factors influencing 'emetional shock' were found to be set age, the level of knowledge received in advance, locus of control, the period of struggle against the disease before hemodialysis and whether any serious illness existed. 2) The factors influencing 'feelings of isolation' were found to be religion and the length of the hemodialysis experience. 3) The factors influencing 'burden' were found to be sex, economic situation, employment status and the length of the hemodialysis experience. 4) The factors influencing 'unclearness' were found to be sex, age, religion. economic situation, the length of the hemodiaysis experience, whether they had had a transfusion and whether there were any complications. 5) The factors influencing 'help from others' were found to be religion. economic situation, past experiences and whether family members lived together. 6) The factors influencing 'coping strategies' were foung to be age, level of education, experiences of illness and locus of control. 7) The factors influeruing 'maintenance of self-esteem' were found to be the length of the hemodialysis experience and self-actualization. 8) The factors influencing 'transitional life' were found to be age, religion, economic situation, employment status. locus of control. past experiences and whether there was a plan for a kidney transplant.

Meta-analysis on the Effectiveness of Pulmonary Rehabilitation Program on Exercise Capacity/Tolerance and General Health Status (만성 폐쇄성 폐질환 환자를 위한 호흡재활 중재가 운동 능력 및 내구성, 일반적 건강상태에 미치는 효과에 대한 메타분석)

  • Purpose: This study was conducted to combine the effects of pulmonary rehabilitation program (PRP) on exercise capacity/tolerance and general health status of COPD patients based on the primary research results examined the effects of PRP, Method: Seventeen studies were selected by the sampling criteria established to include the studies that reported enough statistics necessary to conduct meta-analysis. Result: According to the study results, the most effective indicators for exercise capacity/tolerance were exercise time (such as cycling time or treadmill walking time) and ground walking distance within given time (6 minutes or 12 minutes), whereas effects on such indicators as VE and VO$_2$ were not statistically significant. PRP induced significant effect on patients' general health status, frequently measured by physical, psycho-emotional, and holistic indicators, the enhancement on psycho-emotional dimension resulted from PRP was more prominent than those of the other dimensions. From the results, it was noted that the place where PRP was given and the contents of PRP exercised their influence on the outcome variables. Which body part was trained was also one of the important factors that influence on the patients' perception of dyspnea during exercise as well as on exercise capacity/tolerance. Conclusion: PRP including exercise training significantly improved the exercise capacity and general health status of COPD patients.

A Study on the Establishment of Clinical Nurse Specialist (우리나라 전문간호사제도 개선방안에 관한 연구)

  • Our medical care system is trying to diversify in order to meet the client's needs, and to adjust to a medical environment which is changing very rapidly. Because current nursing theory and practice focus on holistic care, health care management, education, and research, contrary to the traditional emphasis on only assisting a physician, more autonomy and specialization for the implementation of nursing are required. Considering these trends and actual needs, the category of clinical nurse specialist should be established as soon as possible. In order to develop strategies for implementing this new professional specialty, the authors conducted a field survey and literature review of the current system in Korea. As a result, various obstacles and constraints were discovered as follows : 1) There are few accredited educational programs for the training of CNS's. 2) Several hospitals already have staff designated as clinical nurse specialist (CNS) even though the term CNS is not yet standardized or adopted in nationwide. 3) The role of the CNS is not clearly understood by the medical societies, or even nursing societies. A nurse who works in specific nursing areas such as central supply, kidney dialysis, intensive care, coronary care, etc. for a long time, considers herself /himself a CNS. Based upon the above findings, the following alternatives are recommended. 1) The role of the CNS should be defined according to specified functions and authority : professional autonomy ; counselling and educating patients and their familes, nurses, and even other medical personnel ; research on improvement of nursing ; and management of the nursing environment including medical resources, information, and cases. 2) the qualification of CNS should be attained only by a nurse who has an RN license and clinical experience of more than 3 years in a specific nursing field: passes a qualifying examination; and contributes to the professional development of peers, colleagues, and others. A master's degree should only be optional, because of the insufficient of graduate programs which are well designed for the CNS. 3) The CNS should initially be a head nurse rather than line staff in order to deal with as wide an experience base as possible. 4) The nursing specialty could be divided into two areas such as a clinical field and a community field. The clinical field could then be categorized by the Styles' classification such as diseases and pathogenics, systems, ages, acuity, skills/techniques, and function/role ; the community field could be classified according to work site.

A Case Analysis of Home Health Care for Cesarean Postpartum Women and Their Newborns (제왕절개분만 산욕부와 신생아의 가정간호 사례분석)

  • This study was done to provide a basis for home health care management for women following Cesarean delivery. Furthermore it was initiated as an possible application of home health care in the future. In this study, client selection criteria was developed by the researcher and assessment tools for home health care, recording system and problem oriented recording system were revised from Jun's(1993) methods. The selection criteria tool for home health care for women who had a Cesarean delivery was structured and consisted of five areas : physical status, functional status, psychological-emetional status, educational needs status and environmental status. The structured assessment tool consisted of general items, obstetric history, past medical history, methods of feeding, medications taken before ad-mission, laboratory results, discharge instructions, discharge medications, family tree, economic status, environmental status, a map, health assessment of women and their newborns. The visit note consisted of the date : nursing problems : nursing process including initial assessment : nursing goals : visit plan : health status of the postpartum women and their newborn : nursing diagnoses : nursing implementation evaluation : summary : next visit plan and revision. The problem oriented recording system consisted of the date, problem numbers, nursing diagnoses, problem appearance date, problem resolution date. The results of the research are as follows : The seven cases having had a Cesarean delivery were discharged on an average on the 5th day after the Cesarean birth. The total number of home visits was 13. According to Cordon's functional health patterns the total possible nursing diagnoses was 34 diagnoses for the methers and their newborns. Among the 34 diagnoses, there were 13 diagnoses in the health perception /management pattern, 7 in the psychosocial health perception / management pattern, 8 in the psychosocial self-perception, 2 in the nutrition / metabolism pattern of physical function, 2 in the knowledge deficit of newborn management, anxiety related to newborn management, knowledge deficit related to disease process of new-born, anxiety related to disease process of newborn anxiety related to prognosis of baby's condition, knowledge deficit related to newborn jaundice each appeared once. The changes in the number of nursing diagnoses was related to not the number of visits but to the number of nursing diagnoses decreasing. The con-tent of the home health care was categorized ac-cording to assessment, direct care, counseling, education, family care. The recommendation based on the results of this research are Home health care nurses for Cesarean postpartum women and their neonates requires comprehensive knowledge of pregnancy, delivery, and the postpartum period and of the neonate so that they can provide appropriate care and holistic views. Most of cases terminated after the second visit, this outcome may be related to the subjects being discharged on the 5th day after delivery. Therefore, study done with earlier discharge after delivery may have different outcome. It is very hard to assess psychological aspects that need follow-up and to develop communication channels.

A Study on Perceived Discomfort in Patients Following Cardiac Catheterization (심도자 검사 환자의 불편감에 관한 연구)

  • Cardiac cathererization has become a routine diagnostic procedure indicated for evaluation of a wide varity of cardiac conditions. Patients are admitted to the coronary care unit after cardiac catheterization. These conscious patients report feeling uncomfortable in the CCU, but no studies have been done on the effects of bed rest, sand bags on the femoral puncture site and restricted mobility for 4 to 12 hours or longer postprocedure. The main objective of this thesis is to provide basic data to nursing on interventions which de-crease the uncomfortableness experienced by patients in the CCU following cardiac catheterization. In this phenomenological study, the various discomforts felt by the patients were collected and classified. The study subjects were a convenience sample of 29 patients who were admitted to the CCU of a general hospital in Inchon following cardiac catheterization. They were conscious, so they were able to communicate without difficulty. The data were collected over an U days period from July 21, to October 14, 1994. The subjects were interviewed using unstructured open questions and the interviews were tape recorded with the patient's permission The data were analyzed using the Van Kaams phenomenological method. Reliability and validity were exammed by two professor of nursing science, one head nurse, one staff nurse and one cardiologist. The results of the study are summarized as follows ; 1. The 129 descriptive statements by the postcardiac catheterization patients of discomfort were organized into 19 themes. 2. The 19 themes were divided into 3 categories ; physical, psychological, and environmental aspects. 3. The problems concerning the physical aspect were the discomfort of restriction of movement, dysuria, medical devices, pain in the puncture site, symptom is related to the procedure of cardiac catheterization, headache and dizziness, leg painand tingling sensation, and chest pain. The problems concerning the psychological aspect were regret resulting from dependency, economic burden, dissatisfaction with medical personnel, dissatisfaction with medical service system, anxiety about the result of the procedure, concern about the prognosis, loneliness, and concern over treatment procedure. The problems concerning the environmental aspect were influence from neighboring patients, noise, and maladaptation to environmental change. The necessity for holistic care which satisfies physical, psychological, and environmental need must be emphasized in order to solve these discomforts.

Factors of the -Taegyo- of korean Pregnant Women -Self Care of Pregnant Women Based on Oriental Folk Behavior- (한국 임부의 태교요인 -동양 민속행위적인 임부의 자가관리-)

  • All human health behavior is deeply rooted one's beliefs or value system usually encompassed within the culture in which they live. The Taegyo, based on Oriental folk medicine, is defined as the behavior and self care of pregnant women administered for herself and her fetus(unborn child). Taegyo is believed to be desirable, effective, and healthy behavior by most of Korean pregnant women. It is essential in our contemporary culture, to ascertain what the components of Taegyo are and to integrate them into current, western nursing care, particularly in the area of prenatal care. 910 Korean women were the subjects of this study, who were in a gestation period of pregnancy between 10 weeks to three months postpartum. The subjects were selected by clustered smpling from 10 representative cities in Korea. Data was collected from February 10 to March 30 1995 by a constructed Questionnaire which consisted of 95 items. The questionnaire was developed through three steps such as content analysis, calculation of content validity index, and pretest. Data was analyzed by descriptive statistics and rotated matrix factor analysis with pc-SAS. The mean age of the subjects was 28.9 years : 36.5% of them were employed and the mean income per month was about 2000 dollars. The component of Taegyo was clustered into five factors such as fetus psychological stability(equity), fetus personality development, maternal-fetal interaction, fetus intellectual development and physical health promotion. The variance of each factor were 23.7%, 8.3%, 4. 7%, 4.1%, and 3.3% respectively in that order. The Eigen value of each factor was 13.03, 4.57, 2.60, 2.23, and l.83 respectively. It was found that the Taegyo is an unique and holistic self care behavior of Korean pregnant women. Therefore it has been concluded that this study has broadened the understandability of the implications the Taegyo. It is suggested that further studies on the effects of the Taegyo are needed to provide a scientific basis for professional maternity nursing.
