어류피부의 조직학적 구조와 병변 (The histological structure and the pathologic lesions of teleost skin)

  • 어류의 피부는 각종 환경성 저해인자에 대한 최외측 방어장벽으로써, 어체내의 생리적인 기능을 유지하는 데 중요한 역할을 담당하기 때문에, 피부의 상태는 각종 질병에서 매우 중요하다 하겠다. 어류는 형태학적으로나 생리학적으로 수중의 환경에 적응되어 있기 때문에, 육상 포유동물과는 또다른 다수의 어류 특이의 조직학적 소견을 갖고 있다. 그러므로, 피부에서 일어나는 병리조직학적인 소견에 대한 보다 정확한 해석을 가하기 위하여는 이러한 조직학적 특성을 잘 이해해 둘 필요성이 있다. 정상 환경하의 어류피부는 육상 고등동물에 비하여 한계적 장벽으로써 그 생리학적 요구에 보다 미묘히 적응되어 있다. 그러나, 이러한 피부의 적응상은 오히려 인공적인 사육시설이나, 수질의 오염등에 의하여 더욱 피부의 항상성은 쉽게 파괴될 수 있다. 피부색조의 변화, 궤양등과 같은 소견은 임상적으로 빈번히 접하게 되지만, 이와 같은 가시적 병변이 유도되기 까지는 다양한 생리학적 또는 병리학적 반응이, 피부의 각층에서 일어날 수 있다. 지금까지, 경골어류의 피부에 대한 조직학적 정보를 기술한 참고서는 그 수에 있어 빈약할 뿐 아니라, 정보의 체력적 정리가 부족하다. 게다가, 병리조직학적 기술도 체계적인 실험적 연구에 의하기 보다는 임상 병리학적인 소견에 근거 기술하고 있는 것이 대부분이다. 그러므로, 여기에서는, 새로운 조직학적 정보를 기술하기 보다는, 지금까지 정설화된 피부에 대한 조직학적 및 병리조직학적 소견의 일반을 모아 정리하고자 하였다.

Clinicoradiological Characteristics in the Differential Diagnosis of Follicular-Patterned Lesions of the Thyroid: A Multicenter Cohort Study

  • Objective: Preoperative differential diagnosis of follicular-patterned lesions is challenging. This multicenter cohort study investigated the clinicoradiological characteristics relevant to the differential diagnosis of such lesions. Materials and Methods: From June to September 2015, 4787 thyroid nodules (≥ 1.0 cm) with a final diagnosis of benign follicular nodule (BN, n = 4461), follicular adenoma (FA, n = 136), follicular carcinoma (FC, n = 62), or follicular variant of papillary thyroid carcinoma (FVPTC, n = 128) collected from 26 institutions were analyzed. The clinicoradiological characteristics of the lesions were compared among the different histological types using multivariable logistic regression analyses. The relative importance of the characteristics that distinguished histological types was determined using a random forest algorithm. Results: Compared to BN (as the control group), the distinguishing features of follicular-patterned neoplasms (FA, FC, and FVPTC) were patient's age (odds ratio [OR], 0.969 per 1-year increase), lesion diameter (OR, 1.054 per 1-mm increase), presence of solid composition (OR, 2.255), presence of hypoechogenicity (OR, 2.181), and presence of halo (OR, 1.761) (all p < 0.05). Compared to FA (as the control), FC differed with respect to lesion diameter (OR, 1.040 per 1-mm increase) and rim calcifications (OR, 17.054), while FVPTC differed with respect to patient age (OR, 0.966 per 1-year increase), lesion diameter (OR, 0.975 per 1-mm increase), macrocalcifications (OR, 3.647), and non-smooth margins (OR, 2.538) (all p < 0.05). The five important features for the differential diagnosis of follicular-patterned neoplasms (FA, FC, and FVPTC) from BN are maximal lesion diameter, composition, echogenicity, orientation, and patient's age. The most important features distinguishing FC and FVPTC from FA are rim calcifications and macrocalcifications, respectively. Conclusion: Although follicular-patterned lesions have overlapping clinical and radiological features, the distinguishing features identified in our large clinical cohort may provide valuable information for preoperative distinction between them and decision-making regarding their management.

한국의 메기과 (Siluridae) 어류 2종의 상악 수염의 조직학적 특성 (Characteristics of the Histological Structure of the Mandibular Barbels of Two Species of Catfish (Siluridae) from Korea)

  • 한국의 메기과 (Siluridae) 어류의 수염을 조직학적으로 조사하였다. 메기, Silurus asotus의 상악 수염은 연골성 중축을 감싸는 진피와 표피로 구성 되었다. 표피층에는 점액세포, 과립형세포 및 미뢰가 있었다. 세포질이 약한 호염기성을 보이는 점액세포와 호염기성을 보이는 미뢰는 각각 표피의 표면과 표피 말단 부위에 위치 하였다. 표피와 진피는 기저막으로 구분되며 진피는 소성결합조직으로 구성 되었으며 신경세포와 혈관이 내재하고 있었다. 수염의 가장 내부에 연골성 중축이 존재 하였으며 근육층으로 둘러싸여 있었다. 미유기, S.microdorsalis의 상악 수염 구조는 메기의 상악 수염 구조와 비교시 별다른 차이는 보이지 않았으나, 상악 상부와 하부에서의 표피 % 두께 및 상악 상부와 중부에서의 미뢰 수에 있어 미유기는 메기에 비해 차이를 보였다. 본 연구결과 메기와 미유기의 상악 수염은 딱딱하고 굴절성형(stiff and flexible type)으로 파악 되었다.

  • Primary nasal type natural killer (NK)/T cell (NKTC) lymphoma, a specific form of malignant lymphoma, has a higher geographic incidence in Oriental, Mexican, and South American populations than the Western population. In Koreans, it comprises 9-12% of all cases of non-Hodgkin's lymphoma. This type of lymphoma has also been named as angiocentic lymphoma and lethal midline granuloma because the most common site is the upper airway area and its clinical aggressiveness presents with a necrotic and destructive pattern. NKTC lymphoma can also be detected in different organs (testis, spleen, parotid gland, skin, gastroinstinal tract, central nervous system, lungs, bone marrow, etc.) other than the upper airway including the oral cavity. The lymphoma detected in the oral cavity shows various destructive and inflammatory changes, similar to the signs of inflammation and infection from periodontitis and pulpal disease, making a diagnosis difficult with just the clinical signs. For early detection, clinical, radiological, and pathological examinations are required. This report describes the clinical, radiological and histological characteristics with a case report for the early detection of NKTC lymphoma in the oral cavity.

다람쥐 부고환의 조직학적 및 조직화학적 연구 (Histological and histochemical studies of the squirrel epididymis)

  • In order to investigate the morphological characteristics of epididymal duct of the squirrel, the histological and histochemical studies were carried out. The results obtained were summarized as follows: The epididymal duct can be divided into 9 segments by histological and histochemical features. Segments 1 to 5 were located in the head, segments 6 and 7 in the body, and segments 8 and 9 in the tail of the epididymis. The apical cells were numerous in the segment 1. Clear cells which has a compact, deeply staining nucleus and a characteristically clear cytoplasm were scattered in the epithelium throughout the duct. Interepithelial clear cells which had PAS-positive granules tended to increase in number caudally. Strong PAS-positive reaction was detected at the intralumen of the segments 3,8 and 9. Acid phosphatase activity was relatively high in the basal cytoplasm of the segment 7, and then in the supranuclear region of the segments 8 and 9. Alkaline phosphatase activity was weakly positive or negative except the segments 3 and 4. ATPase activity was strong in the free surface of the epithelium in the head and the entire cytoplasm in the body and tail, a,nd SDH activity was generally weak except for the body where it was more intense.

  • 수간(樹幹) 둘레를 철사(鐵絲)로 결체(結締)함에 따라 인위적(人爲的)인 피해(被害)를 받으므로써 형성(形成)된 아까시나무(Robinia pseudoacacia L.) 종양재(腫瘍材)의 조직학적(組織學的) 특성(特性)을 고찰(考察)하고 아울러 정상재(正常材)와의 그 특성(特性) 차이(差異)를 구명(究明)하기 위하여 본(本) 연구(硏究)를 실시(實施)하였다. 종양재(腫瘍材)는 일부(一部) 관공(管孔) 및 대부분(大部分)의 목섬유(木纖維)와 방사조직(放射組織)이 정상배열(正常配列)에서 이탈(離脫)된 불규칙적(不規則的)인 배열(配列)을 이루고 있는 점, 횡단면상(橫斷面上)의 관공(管孔)이 다소 방사방향(放射方向)으로 신당(伸張)되어 있는 점, 선단(先端)이 굴곡(屈曲)된 목섬유(木纖維)가 방사단면상(放射斷面上)의 방사조직(放射組織) 사이에서 가끔 관찰(觀察)되는 점, 방사조직(放射組織)의 폭(幅)과 높이가 더 큰 점, 방사단면상(放射斷面上)의 목섬유(木纖維)가 서로 교착배열(交錯配列)을 이루고 있는 점, 추재부(秋材部)의 복합관공(複合管孔) 출현(出現) 빈도(頻度)가 더 큰 점, 젤라틴목섬유(木纖維)의 발달(發達)이 더 일반적(一般的)인 점 및 농색(濃色)의 검(gum)상물질(狀物質)이 국부적(局部的)으로 목섬유(木纖維), 방사조직(放射組織), 축방향유세포(軸方向柔細胞)의 내강(內腔)에 발달(發達)되어 있는 점에 있어서 정상재(正常材)와의 조직(組織) 특성(特性) 차이(差異)를 나타내었다.

실리콘비삽입물 표면의 석회화: 부위별 석회침착물의 특성 (Calcification on the Surface of Silicone Nasal Implants: Regional Properties of Calcification Deposits)

  • The silicone rubber implants are widely used in plastic surgery because of various advantages; however, calcification in surface of implant(as a chemical resistance) may transform or destroy the high molecular biomaterial when it stays too long within the human body. The purpose of this study is to determine the relationship between calcification and the histological disparities of the tissues surrounding the area adjoining the silicone nasal implant by examining the regional characteristics of calcium deposits in the silicone nasal implant via elemental analyses using EDX(energy-dispersive X-ray analysis) and ultrastructural analyses using SEM(scanning electron microscopy). The subjects of the study were 19 silicone nasal implants removed by revision rhinoplasty, all displaying calcification. According to the tissue characters, the implant surface was divided into 4 zones with the rhinion as the basis. For each zone, elemental and ultrastructural analyses were performed. Elemental analysis revealed that the calcium deposits consisted of Ca and P only. There were no statistically significant disparities among the ratios between Ca and P according to the zones. Ultrastructural analysis showed acellular mineral-like deposits coalesced to create amorphous deposits in all zones; however, in zones 1 and 3(more pressurized zones by periosteum or nasal bone), additional flaky cylinder-shaped calcium deposits were detected. Thus, it seems that the histological disparities in the surrounding tissues do not affect the components and their proportions in the calcification process. However, it can be inferred that the physical environment due to the histological disparities in the surrounding tissues affects the ultrastructures of calcium deposits.

Association of The IDH1 C.395G>A (R132H) Mutation with Histological Type in Malay Brain Tumors

  • Background: Brain tumors, constituting one of the most deadly forms of cancer worldwide, result from the accumulation of multiple genetic and epigenetic alterations in genes and signaling pathways. Isocitrate dehydrogenase enzyme isoform 1 (IDH1) mutations are frequently identified in primary brain tumors and acute myeloid leukemia. Studies on IDH1 gene mutations have been extensively performed in various populations worldwide but not in Malaysia. This work was conducted to study the prevalence of IDH1 c.395G>A (R132H) hotspot mutations in a group of Malaysian patients with brain tumors in order to gain local data for the IDH1 mutation profile in our population. Methods: Mutation analysis of c.395G>A (R132H) of IDH1 was performed in 40 brain tumor specimens by the polymerase chain reaction-restriction fragment length polymorphism method (PCR-RFLP) and then verified by direct sequencing. Associations between the IDH1 c.395G>A (R132H) mutation and clinicopathologic characteristics were also analyzed. Results: The IDH1 c.395G>A (R132H) mutation was detected in 14/40 patients (35%). A significant association was found with histological tumor types, but not with age, gender and race. Conclusions: IDH1 is frequently mutated and associated with histological subtypes in Malay brain tumors.

Clinical Features of Eosinophilic Esophagitis: A Single Center Experience in Ecuador

  • Purpose: Data on eosinophilic esophagitis (EoE) in South America is scarce. Moreover, no studies are available in Ecuador. We evaluated the clinical, endoscopic, and histological characteristics of Ecuadorian children with EoE. Methods: Medical records of 2,711 children who underwent upper gastrointestinal endoscopy (UGE) between 2009 and 2020 at Hospital Metropolitano de Quito, Ecuador were reviewed. Esophageal mucosal biopsies were obtained from 72 patients and the features of 35 children with EoE were described. EoE was diagnosed when there were more than 15 eosinophils in the esophagus, per high power field. Results: EoE was diagnosed in 35 children (9.4±4.5 years) with a male predominance (74%). Abdominal pain (51.4%) and vomiting (31.4%) were dominant symptoms. A history of allergic diseases was noted in 47.1% of the children, which mainly included allergic rhinitis (37.1%) and atopic dermatitis (11.4%). The most common endoscopic findings were furrowing (82.9%) and edema (74.3%). All patients were initially treated with proton-pump inhibitors (PPIs). Those who did not respond to PPIs received steroids (5.7%) and diet therapy (5.7%), and five patients were referred to an allergist. Clinical and histological resolution was observed in 65% of the patients who underwent a second UGE after 6-8 weeks of PPI. Conclusion: Our study describes the clinical features of pediatric EoE in Ecuador. This is the first retrospective study in Ecuador that describes the clinical, endoscopic, and histological manifestations of EoE in a small pediatric population. Almost half of the children who underwent a biopsy had EoE.

Evaluation of P57, P53 and Ki67 Expression in Meningiomas

  • Objective : We conducted this study with the aim of predicting the biological behavior of meningiomas, and determining the benefits of associating histological subtype and grade with the expression of proliferative markers and tumor suppressor proteins. Methods : The study included 29 patients with primary intracranial and intraspinal meningioma diagnosed in the pathology laboratory of Konya City Hospital between January 2014 and December 2020. Clinicopathological characteristics of the patients including parameters such as age and gender were obtained from the hospital records. Histopathological findings were obtained by re-evaluating the preparations stained with Hematoxylin-Eosin, which were extracted from the archive, and by evaluating new sections obtained from paraffin blocks of patients stained with Ki67, p53, and p57 immunohistochemical stains. Results : A moderate correlation was found between tumor size and Ki67 proliferation index (PI) (p=0.003, r=0.530). There was no significant difference between grade I and grade II tumors in terms of p53 (p=0.184) and p57 (p=0.487) expressions. There were higher levels of Ki67 PI in grade II tumors. The histological subtypes of the tumor had no significant difference with Ki67 PI (p=0.018), p53 (p=0.662), and p57 (p=0.368) expressions. Conclusion : In order to obtain more definitive results, there is a need for studies, which are conducted with a greater number of patients and in multiple centers, and in which a long prospective follow-up is planned. The combination of histological, surgical, and imaging markers could make a more sensitive tool for predicting recurrence, and this could also be tested in future studies.
