Radiotherapy of Para-Aortic Node Metastases in Carcinoma of the Uterine Cervix (자궁경부암에서 대동맥 임파절 전이의 치료)

  • Forty one patients with para-aortic node metastases from carcinoma of the uterine cervix treated with radiotherapy at Department of Radiation Oncology, Yonsei University, College of Medicine from January 1982 to December 1987 were retrospectively analyzed. Eleven patients were diagnosed at the time of diagnosis of carcinoma of the cervix (early diagnosis) and 30 patients were diagnosed during follow up period after definitive radiotherapy of primary site (late diagnosis). The most important factors affecting the survival in this study were time of diagnosis and dose of irradiation. Overall 5 year actuarial survival rate of 41 patients was $25.7\%$. Five year survival rate for early diagnosis was $60.3\%$, but late diagnosis was $16.9\%$. And survival rate for high dose (over 4000 cGy) radiation group and low dose radiation group were $42.2\%\;and\;8.9\%$ respectively The most leading cause of death was para-aortic node failure, so early diagnosis and maximum palliation with full dose radiotherapy (over 4000 cGy) is necessary to improve the survival and the quality of life.

Comparison of the anesthetic effects of 2,2,2-tribromoethanol on ICR mice derived from three different sources

  • This study was conducted to compare the anesthetic effects of 2,2,2-tribromoethanol (TBE, $Avertin^{(R)}$) in ICR mice obtained from three different sources. TBE (2.5%) was intraperitoneally injected at three doses: high-dose group (500 mg/kg), intermediate-dose group (250 mg/kg), and low-dose group (125 mg/kg). Anesthesia time, recovery time, end-tidal peak $CO_2$ ($ETCO_2$), mean arterial blood pressure, heart rate, oxygen saturation ($SpO_2$), body temperature, pH, $PCO_2$, and $PO_2$ of the arterial blood were measured. Stable anesthesia was induced by all doses of TBE and the anesthesia time was maintained exhibited dose dependency. No significant differences in anesthetic duration were found among the three different strains. However, the anesthesia time was longer in female than in male mice, and the duration of anesthesia was significantly longer in female than in male mice in the high-dose group. The recovery time was significantly longer for female than male mice in the intermediate- and high-dose groups. In the ICR strains tested, there were no significant differences in the mean arterial blood pressure, $SPO_2$, arterial blood $PCO_2$, and $PO_2$, which decreased after TBE anesthesia, or in heart rate and $ETCO_2$, which increased after TBE anesthesia. In addition, body temperature, blood biochemical markers, and histopathological changes of the liver, kidney, and lung were not significantly changed by TBE anesthesia. These results suggested that ICR mice from different sources exhibited similar overall responses to a single exposure to TBE anesthesia. In conclusion, TBE is a useful drug that can induce similar anesthetic effects in three different strains of ICR mice.

Uptake efficiencies of PCB 153 in fathead minnows through food chain of sediment-midge-fish

  • Uptake efficiencies of PCB 153 in fathead minnow through food chain of sediment-midge-fish were evaluated. Contaminated fish food, the midge Chironomus plumosus was prepared by exposing to sediments with PCB 153. We could harvest the midges with body PCB 153 levels of ∼ 1.0 mg/g and ∼ 10.0 mg/g, respectively, in 2 wk of exposure. PCB 153 level in fish fed with midge of 10.0 mg/g PCB 153 (high-dose group) reached its highest at 11.2 mg/g after 30 d of exposure. However, PCB level in fish fed with midge of 1.0 mg/g PCB 153 (low-dose group) kept increasing following first order rate kinetics until the end of exposure (38 d). When the fish food was changed to the uncontaminated ones, the fish body PCB levels were stabilized in ∼ 3 wk. The uptake efficiency in high-dose fish group was 37%, whereas low-dose group was 55%. Uptake efficiencies in fathead minnows were notably lower than that of pike (∼ 70%). This finding suggests that the uptake efficiency of this PCB congener may depend on the amount of the PCB in diet.

  • Radioactive iodine($^{131}I$) treatment reduces recurrence and increases survival in patients with differentiated thyroid cancer. However, it is important in terms of radiation safety management to measure the radiation dose rate generated from the patient because the radiation emitted from the patient may cause the exposure. Research methods, it measured radiation dose-rate according to the elapsed time from 1 m from the upper abdomen of the patient by intake of radioactive iodine. Directly comparing the changes over time, high dose rate sensitivity and efficiency is statistically significant, and higher chamber than GM counter(p<0.05). Low dose rate sensitivity and efficiency in the chamber had lower levels than gm counter, but not statistically significant(p>0.05). In this study confirmed the characteristics of calibrated ionization chamber and GM counter according to the radiation intensity during high-dose radioactive iodine therapy by measuring the accurate and rapid radiation dose rate to the patient explains, discharged patients will be reduced to worry about radiation hazard of family and others person.

  • This thesis shows about the meaning of treatment rate increasing, the current treated level and the reason of low treatment rate and increasing methods. 1. Treatment rate incresing means high treat level within short time, keeping treatment effect for a long time as well as raising treatment rate. 2. The current by diseases each others completed treatment rate of oriental medicine is 14.0% to 89.7%$(mean:\;{\pm}40.0%)$. Therefore the rate is show too low. 3. The reasons of low treatment rate; low academic level of oriental, academic limitation, clinic and prevention problem of oriental medicine, lack of medical approch suitable for current diseases and symptoms, mostly incurrable diseases using oriental medicine, lack of preventive education, disappropriate medical service and nonspecialty of the treatment, etc. 4. The next methods for incresing the treatment rate must be improved; such as accurate establishment of process that diagnosis symptoms and treats them, system research of microdiagnosis, positive treatment with medicine and nonmedicine method at the same time, appropriate subdivision and actualization of clinical basic research, research of dose and response, diversity of treatment methods and forms, development of treatment service and prevention based on health level, enormous change as cure medicine and opening-up of new disease field, specialization of medical examination, reinforcement of public medical part and herbal drugs use with same origin, mental and pysical stability of patients, accurate extract and oral drinking ways, etc.

Effects of Low Dose γ-Radiation on Photosynthesis of Red Pepper (Capsicum annuum L.) and the Reduction of Photoinhibition (저선량 γ선 조사가 고추의 광합성과 광 스트레스 경감에 미치는 효과)

  • The effect of low dose $\gamma$ radiation on photosynthesis and the reduction of photoinhibition in red pepper plant was investigated. The seedling height leaf width and leaf length of pepper were stimulated in plants grown from seeds irradiated with the low dose of 4 Gy. The $O_2$ evolution in the 4 Gy irradiation group was 1.5 times greater than in the control. To investigate the effect of low dose $\gamma$ radiation on response to high light stress, photoinhibition was induced in leaves of pepper by illumination of high light (900 $\mu mol/m^2/s$). Pmax was decreased with increasing illumination time by 20% in the control, while hardly decreased in the 4 Gy irradiation group. The photochemical yield of PSII, estimated as Fv/Fm, was decreased with increasing illumination time by 50% after 4 hours while Fo did not change. However, Fv/Fm in the 4 Gy irradiation group was decreased by 37% of inhibition, indicating that the photoinhibition was decreased by the low dose $\gamma$ radiation. Changes in the effective quantum yield of PSII, $\Phi_{PSII}$, and 1/Fo-1/Fm, a measure of the rate constant of excitation trapping by the PSII reaction center, showed similar pattern to Fv/Fm. And NPQ was decreased after photoinhibitory treatment showing no difference between the control and the 4 Gy irradiation group. These results showed the positive effect of low dose $\gamma$ radiation on the seedling growth and the reduction of photoinhibition.

High Dose of FGF-2 Induced Growth Retardation via ERK1/2 De-phosphorylation in Bone Marrow-derived Mesenchymal Stem Cells

  • Fibroblast growth factor (FGF)-2 is one of the most effective growth factors to increase the growth rate of mesenchymal stem cells (MSCs). Previously, we reported that low dose of FGF-2 (1 ng/ml) induced proliferation of bone marrow-derived mesenchymal stem cells (BMSCs) through AKT and ERK activation resulting in reduction of autophagy and senescence, but not at a high dose. In this study, we investigated the effects of high dose FGF-2 (10 ng/ml) on proliferation, autophagy and senescence of BMSCs for long term cultures (i.e., 2 months). FGF-2 increased the growth rate of BMSCs in a dose dependent manner for a short term (3 days), while during long term cultures (2 months), population doubling time was increased and accumulated cell number was lower than control in BMSCs when cultured with 10 ng/ml of FGF-2. 10 ng/ml of FGF-2 induced immediate de-phosphorylation of ERK1/2, expression of LC3-II, and increase of senescence associated ${\beta}$-galactosidase (SA-${\beta}$-Gal, senescence marker) expression. In conclusion, we showed that 10 ng/ml of FGF-2 was inadequate for ex vivo expansion of BMSCs because 10 ng/ml of FGF-2 induced growth retardation via ERK1/2 de-phosphorylation and induction of autophagy and senescence in BMSCs.

Dose Intensity of Oxaliplatin in 5-Fluorouracil and Leucovorin Regimens in Pretreated Metastatic Colorectal Cancer (5-Fluorouracil, Leucovorin과 병용 투여된 Oxaliplatin의 Dose Intensity가 재발된 전이성 대장암 치료에 미치는 영향)

  • Studies of oxaliplatin, 5-fluorouracil and leucovorin in pretreated metastatic colorectal cancer showed that oxaliplatin dose intensity is important prognostic factor for objective response rates and progression-free-survival (PFS). To evaluate response rates, PFS and toxicity according to oxaliplatin dose intensity, we retrospectively analyzed data from patients with metastatic colorectal cancer received oxaliplatin,5-fluorouracil, leucovorin regimens. Sixty-three patients were reviewed in this study, 42 patients received low dose intensity oxaliplatin (LDI: $\leq85\;mg/m^2/2wks$) and 21 patients high dose intensity oxaliplatin (HDI: $>85\;mg/m^2/2wks$). Objective responses occurred in 10 $(47.7\%)$ HDI patients and 9 $(21.4\%)$ LDI patients (p = 0.014). Median PFS was 24.7 weeks in HDI group, with $45.1\%$ of HDI patients progression free at 6 months, and 20.5 weeks in LDI group, with $33.5\%$ of LDI patients progression free at 6 months (p = 0.344). Increased oxaliplatin dose intensity was not associated with neutropenia, thrombocytopenia, neuropathy, nausea and vomiting. This study showed that oxaliplatin dose intensification significantly improves objective response rate in pretreated metastatic colorectal cancer without increasing severe toxicity.

Remote Afterloading High Dose Rate Brachytherapy AMC EXPERIANCES (원격조정 고선량 근접 치료)

  • Remote afterloading high dose rate brachytherapy (HDRB) is a new technology and needs new biological principle for time and dose schedule. Here, authors attempt to evaluate the technique and clinical outcome in 116 patients, 590 procedures peformed at Asan Medical Center for 3 years. From Sep. 1989 to Aug 1992, 471 procedures of intracavitary radiation in 58 patients of cervical cancer and 26 of nasopharyngeal cancer,79 intraluminal radiation in 12 of esophageal cancer, 11 of endobronchial cancer and 1 Klatskin tumor and 40 interstitial brachytherapy in 4 of breast cancer, 1 sarcoma and 1 urethral cancer were performed. Median follow-up was 7 months with range $1\~31$ months. All procedures except interstitial were performed under the local anesthesia and they were all well tolerated and completed the planned therapy except 6 patients. 53/58 patients with cervical cancer and 22/26 patients with nasopharynx cancer achieved CR. Among 15 patients with palliative therapy, $80{\%}$ achieved palliation. We will describe the details of the technique and results in the text. To evaluate biologic effects of HDRB and optimal time/dose/fractionation schedule, we need longer follow-up. But authors feel that HDRB with proper fractionation schedule may yield superior results compared to the low dose rate brachytherapy considering the advantages of HDRB in safety factor for operator, better control of radiation dose and volume and patients comfort over the low dose brachytherapy.

Graves' Disease Patients with Large Goiters Respond Best to Radioactive Iodine Doses of at Least 15 mCi: a Sonographic Volumetric Study

  • Background and Objectives: Radioactive iodine therapy (RAI) is an important treatment modality of Graves' disease (GD), but there is still not a consensus on the optimal dosage regimen. We studied the treatment success rate of different RAI doses, and examined which clinical markers were useful for determining the optimal RAI dosage for successful therapy in Korean patients. Materials and Methods: We retrospectively studied 123 patients with GD treated with RAI between 2004 and 2014 at Chonnam National University Hwasun Hospital. The responder group was defined as patients who developed hypothyroidism requiring levothyroxine replacement following RAI, regardless of the RAI dosage. Results: A total of 54 patients (43.9%) became hypothyroid after the first dose, and 31 needed two to four additional doses to achieve hypothyroidism. In the responder group as a whole (85 patients), the mean total dose of RAI was $15.5{\pm}7.0mCi$ and the mean thyroid volume (TV) was $35.4{\pm}23.4mL$. When divided into low dose (<15 mCi, n=46) and high dose (${\geq}15mCi$, n=39) responder groups, TV was significantly lower in the low-dose responder group ($25.7{\pm}11.4$ vs. $48.4{\pm}31.3$, p<0.001). The optimal cut-off TV for the low-dose responder group was <32.37 mL (sensitivity 80.9%, specificity 76.7%). Conclusion: TV had significant effects on the outcome of RAI in GD patients. The optimal fixed RAI dose for Korean GD patients with a large goiter (${\geq}33mL$) should be at least 15 mCi to achieve the best outcome.
