Abdominal Hypertension after Abdominal Plication in Postbariatric Patients: The Consequence in the Postoperative Recovery

  • Background Abdominoplasty with abdominal plication increases intra-abdominal pressure (IAP) and has been previously associated with limited diaphragmatic excursion and respiratory dysfunctions. Many factors found in abdominoplasties and among postbariatric patients predispose them to a higher occurrence. This study aims to evaluate the impact of abdominal plication among postbariatric patients, assess whether the plication increases their IAP, and analyze how these IAP correlate to their postoperative outcome. Methods This prospective study was performed on all patients who underwent circumferential Fleur-De-Lis abdominoplasty. For this intended study, the IAP was measured by an intravesical minimally invasive approach in three stages: after the initiation of general anesthesia, after a 10-cm abdominal wall plication and skin closure, and 24 hours after the procedure. Results We included 46 patients, of which 41 were female and 5 were male. Before the bariatric procedure, these patients had an average maximum weight of 121.4 kg and an average maximum body mass index of 45.78 kg/m2; 7 were grade I obese patients, 10 were grade II, and 29 were grade III. Only three patients were operated on with a gastric sleeve and 43 with gastric bypass. We presented six patients with transitory intra-abdominal hypertension in the first 24 hours, all of them from the grade I obesity group, the highest presented was 14.3 mm Hg. We presented 15% (7/46) of complication rates, which were only four seroma and five dehiscence; two patients presented both seroma and wound dehiscence. Conclusion Performing a 10-cm abdominal wall plication or greater represents a higher risk for intra-abdominal hypertension, slower general recovery, and possibly higher complication rate in patients who presented a lower degree of obesity (grade I) at the moment of the bariatric surgery.

Evaluation of models for estimation of genetic parameters for post-weaning body measurements and their association with yearling weight in Nellore sheep

  • Objective: The objective of this study was to obtain (co) variance components and genetic parameter estimates for post-weaning body measurements such as body length (BL), height at withers (HW), and chest girth (HG) recorded at six (SBL, SHW, and SHG), nine (NBL, NHW, and NHG) and twelve (YBL, YHW, and YHG) months of age along with yearling weight (YW) in Nellore sheep maintained at livestock research station, Palamaner, Andhra Pradesh, India and also the association among body measurements with YW was studied. Methods: Data on 2,076 Nellore sheep (descended from 75 sires and 522 dams) recorded between 2007 and 2016 (10 years) were utilized in the study. Lambing year, sex of lamb, season of lambing and parity of dam were included in the model as fixed effects and ewe weight was kept as a covariate. Analyses were conducted with six animal models with different combinations of direct and maternal genetic effects using restricted maximum likelihood procedure. Best model for each trait was determined based on Akaike's information criterion. Results: Moderate estimates of direct heritability were obtained for the studied traits viz., BL (0.02 to 0.24), HW (0.31 to 0.49), and CG (0.08 to 0.35) and their corresponding maternal heritability estimates were in the range of 0.00 to 0.07 (BL), 0.13 to 0.17 (HW), and 0.07 to 0.13 (CG), respectively. Positive direct genetic and phenotypic correlations among the traits and they ranged from 0.07 (YBL-YW) to 0.99 (SBL-SHG, SHG-YW, and NBL-YBL) and 0.01 (SBL-YBL) to 0.99 (NBL-NHG), respectively. Further, the genetic correlations among all the body measurements and YW were positive and ranged from 0.07 (YW and YBL) to 0.99 (YW and SHG). Conclusion: There was a strong association of chest girth at six months with YW. Further, it is indicated that moderate improvement of post-weaning body measurements in Nellore sheep would be possible through selection.

Elemental Composition of the Soils using LIBS Laser Induced Breakdown Spectroscopy

  • Laser induced breakdown spectroscopy (LIBS) technique has been used for the elemental composition of the soils. In this technique, a high energy laser pulse is focused on a sample to produce plasma. From the spectroscopic analysis of such plasma plume, we have determined the different elements present in the soil. This technique is effective and rapid for the qualitative and quantitative analysis of all type of samples. In this work a Q-switched Nd: YAG laser operating with its fundamental mode (1064 nm laser wavelength), 5 nanosecond pulse width, and 10 Hz repetition rate was focused on soil samples using 10 cm quartz lens. The emission spectra of soil consist of Iron (Fe), Calcium (Ca), Titanium (Ti), Silicon (Si), Aluminum (Al), Magnesium (Mg), Manganese (Mn), Potassium (K), Nickel (Ni), Chromium (Cr), Copper (Cu), Mercury (Hg), Barium (Ba), Vanadium (V), Lead (Pb), Nitrogen (N), Scandium (Sc), Hydrogen (H), Strontium (Sr), and Lithium (Li) with different finger-prints of the transition lines. The maximum intensity of the transition lines was observed close to the surface of the sample and it was decreased along the axial direction of the plasma expansion due to the thermalization and the recombination process. We have also determined the plasma parameters such as electron temperature and the electron number density of the plasma using Boltzmann's plot method as well as the Stark broadening of the transition lines respectively. The electron temperature is estimated at 14611 °K, whereas the electron number density i.e. 4.1 × 1016 cm-3 lies close to the surface.

Unconventional shunt surgery for non-cirrhotic portal hypertension in patients not suitable for proximal splenorenal shunt

  • Backgrounds/Aims: Proximal splenorenal shunt (PSRS) is a commonly performed procedure to decompress portal hypertension, in patients with refractory variceal bleed, especially in non-cirrhotic portal hypertension (NCPH). If conventional methods are hindered by any technical or pathological factors, alternative surgical techniques may be required. This study analyzes the effectiveness of various unconventional shunt surgeries performed for NCPH. Methods: A retrospective analysis of NCPH patients who underwent unconventional shunt surgeries during the period July 2011 to June 2022 was conducted. All patients were followed up for a minimum of 12 months with doppler study of the shunt to assess shunt patency, and upper gastrointestinal endoscopy to evaluate the regression of varices. Results: During the study period, 130 patients underwent shunt surgery; among these, 31 underwent unconventional shunts (splenoadrenal shunt [SAS], 12; interposition mesocaval shunt [iMCS], 8; interposition PSRS [iPSRS], 6; jejunal vein-cava shunt [JCS], 3; left gastroepiploic-renal shunt [LGERS], 2). The main indications for unconventional shunts were left renal vein aberration (SAS, 8/12), splenic vein narrowing (iMCS, 5/8), portalhypertensive vascular changes (iPSRS, 6/6), and portomesenteric thrombosis (JCS, 3/3). The median fall in portal pressure was more in SAS (12.1 mm Hg), and operative time more in JCS, 8.4 hours (range, 5-9 hours). During a median follow-up of 36 months (6-54 months), shunt thrombosis had been reported in all cases of LGERS, and less in SAS (3/12). Variceal regression rate was high in SAS, and least in LGERS. Hypersplenism had reversed in all patients, and 6/31 patients had a recurrent bleed. Conclusions: Unconventional shunt surgery is effective in patients unsuited for other shunts, especially PSRS, and it achieves the desired effects in a significant proportion of patients.

Physicochemical Composition and Heavy Metal Contents on the Sediment of Kwangyang Bay (광양만의 퇴적물에 대한 이화학적 조성 및 중금속 함량)

  • For the purpose of surveying the physicochemical composition of sediment collected from Kwangyang Bay, the percentage of water loss, COD, $H_2S$, grain size and 10 heavy metals were studied at 17 sites. During the surveying period, the changes of the percentage of water loss were appeared $35.5\;{\sim}\;53.8%$. COD and $H_2S$ were showed $3.8\l{\sim}\;12.9\;mg/g$, and $0.1\;{\sim}11.4\;{\mu}g/g$, respectively, In composition of grain size on the sediment, percentages of grain sizes under $74\;{\mu}m$ were varied from 40.5% to 86.7% and above $74\;{\mu}m$ were varied from 11.5% to 43.0%. From the spatial distribution of heavy metal using contour map, we can suppose some heavy metal discharges which affect sediment of Kwangyang Bay, It was estimated that Shinpung creek, Ssang-bong creek, and draining area of sewange treatment plant were the main discharge among the heavy metal output sources. By comparison between present study and heavy metal guideline of nonpolluted sea sediment that is provided by EPA, US, it was showed that the contents of Pb and Hg were acceptable but contents of Mn, Zn, Cu, Fe, As, and Cr were higher than those of EPA guideline.

Other Processed Products, Monitoring and the Exposed Dose Assessment of Heavy Metal, the Illegal Compounds (기타가공품의 중금속, 부정유해물질 모니터링 및 노출량 평가)

  • This study was performed for both evaluating the safety of other processed products and providing basic information for making the general standard for contaminants in the category of other processed products. We analyzed the contents of three heavy metals, thirty six anti-impotence drugs and their analogues, three anti-obesity drugs and their analogues, twenty eight steroid drugs and their analogues, collecting in Incheon Metropolitan City. Any illegal compound was not detected in those products. However the contents of lead, cadmium and mercury of those products were at the range of 0.001-13.390 mg/kg, 0.03-1.231 mg/kg and 0.001-0.650 mg/kg respectively. Because there are no standards of heavy metals against other processed products, we compared the analytical results with relevant standards of both S. Korea and foreign countries. As a result, two products exceeded the relevant standards of lead, and other two products exceeded the relevant standards of mercury. The relative hazards compared to PTWI of FAO/WHO (Codex), Seafoods-pajeonmix, Perilla seed powder exceeded PTWI standards 0.214. The compulsory standards of each food product are determined by the category of the products. Because there is no standard of heavy metals in the category of other processed products in S. Korea, any food products registered as other processed product by manufacturer are free with those standards. Abusing similar problems on the categorization of food products could cause consumers' health problem. To prevent these problems, detail regulations on the categorization of food products have to be introduced.

Study on characteristics of specific hazardous substances in the industrial wastewater effluent (사업장 방류수 중 특정수질유해물질 배출 특성 연구)

  • In this study, 165 wastewater discharge facilities in 10 business types were investigated with regard to 24 specific hazardous substances that included heavy metals, VOCs, CN, and phenol in the Gwangju city. Cu in the range from from 0.008 to 35.420 mg/L was detected in all business types and the detection rate was 46.8 %. Other heavy metals, such as Cd, As, Hg, Pb, and Cr+6 were detected as well. However, their detection rates ranged between 0.6 and 1.8 %. CN and phenol were detected in one and five facilities, respectively. 12 species of VOCs were detected: chloroform 80.6 % (0.42 to 81.60 μg/L), benzene 16.4 % (1.49 to 3.31 μg/L), trichloroethylene 11.5 % (1.78 to 6.02 μg/L), 1,1-dichloroethylene 10.3 % (1.23 to 5.89 μg/L), and dichloromethane 8.5 % (0.28 to 968.86 μg/L) in the detection rate order. The concentration of VOCs was detected in trace amounts, except for dichloromethane that exceeded the effluent quality standard in three business types, namely, metal manufacturing, food industry, and car washing facility. Chloroform was detected in all business types, where 24.88 μg/L were detected in the laundry business and 53.41 μg/L in the water supply business; the mean concentration of chloroform in these two business types was higher than elsewhere. Therefore, for the disposal of non-degradable specific hazardous substances in industrial wastewater, it is necessary to introduce physical and chemical processes, such as activated carbon adsorption, fenton oxidation, ozone treatment, as well as photocatalyst and the UV radiation.

Purification and Enzymatic Characteristics of the Bacillus pasteurii Urease Expressed in Escherichia coli (Escherichia coli에서 발현된 Recombinant Bacillus pasteurii Urease의 정제 및 효소학적 특성)

  • The gene coding for urease of alkalophilic Bacillus pasteurii had been cloned in Escherichia coli previously. The urease protein was purified 63.1-fold by TEAE-cellulose, DEAE-Sephadex A-50, Sephadex G-150 and Sephadex G-200 chromatographies with a 7.3% yield from the sonicated fluid of the E. coli HB1Ol(pBUll) encoding B. pasteurii urease gene. The ureases of E. coli (pBUll) and B. pasteurii possessed as a $K_m$ for urea, 42.1 mM and 40.4 mM, respectively. They hydrolyzed urea with $V_{max}$ of 86.9$\mu$mol/min and 160$\mu$mol/min, respectively. Both ureases were composed with four subunits (Mrs 67,000) and a subunit (Mr 20,000). The molecular weight of both native enzymes was Mr 280,OOO$pm$10,000 determined by gel filtration chromatography and Coomassie blue staining of the subunits. The optimal reaction pH of both ureases were pH 7.5. The ureases were stabled in pH 5.5-10.5. The optimal reaction temperature of both ureases were $60^{\circ}C$, and the ureases were stable for an hour at $50^{\circ}C$, 40min at $60^{\circ}C$ and 10 min at $70^{\circ}C$ The activity of both enzymes were inhibited completely by $Ag^{2+}$, $Hg^{2+}$, $Zn^{2+}$, $Cu^{2+}$, and were inhibited 60% by CoH, 30% by $Fe^{2+}$ and 10% by $Pb^{2+}$. However it was increased by the addition of $Sn^{2+}$, $Mn^{2+}$, $Mg^{2+}$ at concentration of $1{\times}10^{-3}$M. Both ureases were inhibited completely by p-CMB and acetohydroxamic acid. The urease expressed in E. coli (pBU11) was inhibited 70% by SDS. The urease of B. pasteurii was inhibited 40% by hydroxyurea, whereas the recombinant urease of E. coli strain was inhibited 17%. Both enzymes were not inhibited by cyclohexanediaminetetraacetic acid (CDTA) and ethylendiaminetetraacetic acid (EDTA).

A Study on Nutrient Intakes and Blood Parameters of Adult Men and Women with Metabolic Syndrome (대사증후군을 가진 성인남녀의 영양소 섭취상태와 혈액성상에 관한 연구)

  • The purpose of this study is to evaluate the nutrient intake and selected blood parameters of adults with metabolic syndrome (MS) and to provide data in forming a dietary guideline for the prevention of chronic diseases. Subjects were recruited and divided into two groups according to the NCEP-ATP III criteria and WHO Asia-Pacific Area criteria for obesity. MS group was defined as subjects who have three or more risk factors and control group was defined as those with two or less of the risk factors. The average age, height, weight, body mass index (BMI) were 58.8 years, 158.0 cm, 66.3 kg, $26.5\;kg/m^{2}$, respectively, in the MS group; and 58.4 years, 158.9 cm, 59.6 kg, $23.5\;kg/m^{2}$, respectively, in the control group. The weight and BMI in the MS group were significantly higher than those in the control (p<0.001). There was no significant difference in the food and nutrient intake between the MS and control group. Male subjects in the MS group showed significantly higher intake of mushrooms than those in the control (p<0.05). Egg consumption in the MS group was significantly lower than those in the control (p<0.01). Consumption of vegetables and fiber was significantly lower for female subjects in the MS group than those in the control (p<0.05). Serum GPT, AI and WBC count in the MS group (27.8 IU/L, 3.7, $5964.2\;{\mu}/L$) were significantly higher than those in the control (22.6 IU/L, 3.2, $5250.0\;{\mu}/L$; p<0.01, p<0.001, p<0.01). In conclusion, consuming fiber and vegetables may prevent and reduce metabolic syndrome in adult men and women, and this study demonstrates the need for proper dietary management for them.

Heavy Metal Contents and Safety Evaluation of Commercial Salts in Seoul (서울시 유통 소금의 중금속 함량 및 안전성 평가)

  • This study was to investigate the heavy metal content of 55 commercial salts in the Seoul area. There were 22 types of solar sea salt, 17 types of processed salt and 16 types of reworked salt. Looked at another way, there were 22 types of domestic salt and 33 types of salt imported from France, the U.S., Japan, Australia, New Zealand, and Argentina. The samples were measured using both a mercury analyzer and an Inductively-Coupled Plasma Optical Emission Spectrometer (ICP-OES). The average heavy metal contents for commercial salts were Pb $0.281{\pm}0.344$, Cd $0.035{\pm}0.221$, Cr $0.364{\pm}0.635$, Cu $0.182{\pm}0.313$, As $0.046{\pm}0.062$, Ni $0.155{\pm}0.247$, Al $5.753{\pm}10.746$, Co $0.028{\pm}0.211$ and Hg $0.001{\pm}0.001$ mg/kg. The leads were detected highly in solar sea salt rather than in processed salt or reworked salt. Also chrome, arsenic and nickel were found more in processed salt. There were large differences in aluminum content between imported solar sea salt and processed salt. Aluminum was highly detected in French products, showing that salt can be affected by regional differences. The weekly average intakes of Pb, Cd, Cr, Cu, and Hg from commercial salt were 1.652% (0.000~6.754), 0.372% (0.000~7.214), 3.177% (0.000~26.279), 0.008% (0.001~0.049), and 0.031% (0.000~0.094) respectively compared with Provisional Tolerable Weekly Intakes established by the Joint FAO/WHO Expert Committee for the evaluation of food safety. The content of heavy metals from commercial salts was determined to be at safe levels.
