Recent Domestic and International Trends in Clinical Research of Herbal Medicine Treatment for Crohn's disease (크론병의 한약 치료에 대한 최근 국내외 임상연구 동향)

  • Objectives The purpose of this study is to analyze the trend of herbal medicine treatment, treatment's effect and stability for Crohn's disease by reviewing clinical studies published over the past 5 years. Methods 22 articles which were published from 2017 to August, 2021 were obtained from the ScienceON, Research information sharing service (RISS), Koreanstudies Information Service System (KISS), Oriental medicine advanced searching integrated system (OASIS), China national knowledge infrastructure (CNKI), Pubmed. We analyzed the literature in regards to the treatment methods and results. Results There were 6 case reports, 1 controlled clinical trial, and 15 randomized controlled trial and in most cases, herbal medicines and Western medicines were administered in combination. The pathology of damp-heat due to spleen deficiency was the most common, and Sijunzi-tang and Shenling Baishu san were the most commonly used. As a result of the treatment, it was found that the combined administration of the herbal medicine and the Western medicine was more effective than the administration of the Western medicine alone. Conclusions Herbal treatment of Crohn's disease induced high cure rate safely than when only western treatment was applied. Future studies on safety and studies that can suggest specific guidelines for the combined administration of western and herbal medicines are needed.

Efficacy of Combination Treatment of Herbal Medicine and Western Medicine for the Treatment of Type 1 Diabetes in children : Systemic Review and Meta-analysis (소아 1형 당뇨의 한약과 양약 병용 치료에 대한 체계적 문헌 고찰 및 메타분석)

  • Objectives The purpose of this study is to investigate the clinical studies on the effect of herbal medicine in type 1 diabetes in children and to seek better approach of herbal medicine to treat type 1 diabetes in children. Methods This study researched randomized controlled trials through various databases in the world about herbal medicine treatments in type 1 diabetes in children. Results 10 out of 337 studies were selected and analyzed. All studies were conducted in China. All studies were using herbal medicines, as an adjunctive treatment to the main regimen. As a result, the integrated Chinese medicine and western medicine lowered FPG by -1.56 mmol/L and 2hPG by -1.94 mmol/L on average, respectively. The HbA1c also decreased by -1.11% in the treatment group compared to the control group. Total efficacy of the treatment was 1.21 times more effective in the treatment group than in the control group. Conclusions Based on the results of the studies, it seems that the herbal medicine for the treatment of type 1 diabetes in children will be effective as a combination with conventional medicines. Further research is needed to prove the findings of this observatoional studies.

Review of Clinical Researches for Herbal Medicine Treatment on Nocturnal Enuresis in Children (소아 야뇨의 한약치료에 대한 임상연구 동향 -중의학 논문을 중심으로-)

  • Objectives The purpose of this study is to evaluate the efficacy and safety of herbal medication for the treatment of nocturnal enuresis in children by analyzing randomized controlled trials conducted in China. Methods We searched literatures from China National Knowledge Infrastructure published up to 19 January, 2018. Selected literatures were collected and analyzed in order of publication year, and then demographic information, treatment method, duration of illness, duration of treatment, follow-up period, outcome measurement and adverse events. Results A total of 34 studies were selected for the analysis. In most studies, the total efficacy of the treatment group was reported to be higher than that of the control group, and the recurrence rate and complete cure rate were also found to be more effective in the herbal medicine treatment group. The most frequently used medical herb was Alpiniae Fructus (益智仁), and the use of medical herbs belong to tonifying yang (補陽藥), tonifying qi (補氣藥), and astringing essence strengthening collapse medicine (澁精縮尿止帶藥) were relatively high. The adverse events rate for the herbal medicine treatment group were significantly lower than those in the Western medicine treatment group. Conclusions This study showed that a herbal medicine treatment can be effective and safe option for treating pediatric nocturnal enuresis. However, additional well-designed clinical studies need to be performed to establish a basis.

Review of Korean Medical Treatment in Hyperlipidemia - Focusing on Treatment of Korean Herbal Medicine (본초학적 접근을 통해 본 고지혈증의 최근 연구동향)

  • The purpose of this study was to review the direction of Korean medicine treatment in hyperlipidemia, focusing on Korean herbal medicine and to suggest sources for drug development in treatment of hyperlipidemia. We searched 115 articles which had been published from 1997 to 2009 in journals in Korean Oriental Medical Society and its sub-societies. Those articles were selected from the search engine of web site of each journal with the key words 'hyperlipidemia'. Excluding 21 articles in no relation to herbs, we finally reviewed 94 articles. There are many studies of 'hyperlipidemia' in Korean Medicine. Analyzing the articles, we found that the medicinal effects of the herbs used in recent studies and patents coincides with treatment of hyperlipidemia in Korean medicine. Although many herbs in articles is granted patent, it doesn't proceeds to develop herbal drug. It requires to study in treatment of hyperlipidemia in Korean medicine and then to develop herbal drug.

The Study on Acute and Subacute Toxicity and Sarcoma-180 Anti-cancer Effects of Triglii Semen Herbal-acupuncture (파두약침(巴豆藥鍼)의 급성(急性) 아급성(亞急性) 독성실험(毒性實驗) 및 Sarcoma-180 항암효과(抗癌效果)에 관(關)한 실험적(實驗的) 연구(硏究))

  • Objective: The purpose of this study was to investigate the acute and subacute toxicity and sarcoma- 180 anti-cancer effects of Herbal acupuncture with Triglii Semen in mice and rats. Method: Balble mice were injected intraperitoneally with Triglii semen Herbal acupuncture for $LD_{50}$ and acute toxicity test. Sprague Dawley rats were injected intraperitoneally with Triglii semen Herbal acupuncture for subacute toxicity test. The Triglii semen Herbal acupuncture was injected on Chung-wan(CV12) of mice with S-180 cancer cell line. Results: 1. In acute toxicity test, the $LD_{50}$ value was $7.49{\times}10^3$ml, 0.30ml/kg.2. The body weights of mice treated with Triglii semen Herbal acupuncture increased during the acute toxicity test. 3. In acute toxicity test of serum biochenrical values of mice, total protein was decreased in treatment groups I, 2 and 3, albunrin was decreased in treatment groups 2 and 3 compared to the control group. GOT was increased in treatment group I and Alk. Phosphatase was increased in treatment groups 1,2 and 3 compared to the normal group(p<0.05). 4.ln subacute toxicity test, severe tissue injury was found in lung and liver. 5. In subacute toxicity test, the body weight was decreased in treatment groups I and 2 compared to the normal group and the weight of liver. lung and kidney were increased in treatment groups 1, 2 and 3 compared to the normal group.(p<0.05) 6. In subacute toxicity test, RBC, HGB and HCT were decreased in treatment groups 1 and 2 compared to the normal group. MCV was increased in treatment group1 compared to the normal group, MCH was increased in treatment groups 1 and 2 compared to the control group in complete blood count test.(p<0.05) 7. In subacute toxicity test, total protein was decreased in treatment groups 1 and 2 compared to the nonnal group, BUN was increased in treatment groups 1 and 2 compared to the nonnal group, creatinine and uric acid were decreased in treatment groups 1 and 2 compared to the normal group, glucose was increased in treatment group 2 compared to the nonnal group, triglycelide was decreased in treatment groups I and 2 compared to the normal group, total cholesterol was increased in treatment groups 1 and 2 compared to the control group. GOT was decreased in treatment group 2 compared to the normal and control group, AIk. Phosphatase was increased in treatment group 1 compared to the normal and control group.(p<0.05) 8. Median survival time was 17days in treatment group 2 for S- 180 cancer cell treated with Triglii semen Herbal acupuncture. 9. Natural killer cell activity was insignificant for S-180 cell treated with Triglii semen Herbal acupuncture.(p<0.05) 10. lnterieukin-2 productivity was decreased for S-180 cell treated with Triglii semen Herbal acupuncture compared to the normal and control group.(p<0.05) Conclusion: According to the results, we can conclude Herbal-acupuncture with Triglii semen caused toxicity, and caused no effects in S-180 cancer cell.

Effects of distilled Phaseoli Semen rubra Herbal-Acupuncture on lipid composition, liver function, antioxidant capacity and molecular biological aspects in obese rats induced high fat diet (적소두증유약침(赤小豆蒸溜藥鍼)이 비만(肥滿)쥐의 지질구성(脂質構成), 간장기능(肝臟機能), 항산화효과(抗酸化效果) 및 분자생물학적(分子生物學的) 특성(特性)에 미치는 영향(影響))

  • Effects of Phaseoli Semen rubra Herbal-acupuncture at zusanli(ST-36), Quchi(LI-11) and Sanyinjiao(Sp-6) on lipid composition, liver function, oxidative capacity and molecular biological aspects were investigate in high fat diet induced obese rats. Forty male Sprague-Dawley rats weighing about 400g were divided into 4 groups according to body weight and raised four weeks with control, zusanli(ST-36), Quchi(LI-11) and Sanyinjiao(Sp-6) Herbal-acupuncture groups. 1. Plasma total cholesterol and LDL-cholesterol showed a low values in the (ST-36) and (LI-11) Herbal-acupuncture groups and HDL-cholesterol showed a high values in the (ST-36) Herbal-acupuncture groups. 2. Plasma triglyceride and glucose showed a low values in the (ST-36) and (LI-11) Herbal-acupuncture groups. 3. The contents of plasma free fatty acids showed a tendence to decrease in the Herbal-acupuncture groups, however in the Herbal-acupuncture groups, the values showed no significantly different. 4. The activities of AST and ALT showed no significantly different in all treatment groups. 5. The contents of plasma ${\beta}-lipoprotein$ and free fatty acids showed a tendency to decrease in the Herbal-acupuncture groups compared to those of control group. In the Herbal-acupuncture groups, the values of (ST-36) and (LI-11) Herbal-acupuncture groups showed a low in the acupuncture groups. 6. Liver total cholesterol and triglyceride showed a low values in the (ST-36) and (LI-11) Herbal-acupuncture groups. 7. Contents of plasma TBARS showed a low values in the (ST-36) and (LI-11) Herbal-acupuncture groups, however contents of liver TBARS showed no significantly different among treatment groups. 8. Values of liver glutathione peroxidase and catalase activity showed a tendency to increase in the (ST-36) and (LI-11) Herbal­acupuncture groups. Values of liver super oxide dismutase activity showed a high in the (ST-36) Herbal-acupuncture groups compared to those of other groups. 9. Expression of apo-B mRNA in liver cell showed a low in (ST-36) Herbal-acupuncture groups, however expression of apo-E mRNA showed a high in Herbal-acupuncture groups and $TNF-{\alpha}$ mRNA in adipose cell showed no different among all the treatment groups. Expression of Leptin mRNA showed a tendance to decrease in (ST-36) Herbal-acupuncture group. 10. Histological character of liver, those of (ST-36) Herbal-acupuncture group showed a good, however other treatment groups showed slight vasodilation and slight fat accumulation. These results indicated that Phaseoli Semen rubra Herbal-acupuncture at (ST-36) and (LI-22) suppressed adipose tissue mass and lipid peroxidation and increased antioxidant system.

Comparative Study of Effects of 'Intramuscular Bee Venom Herbal Acupuncture' and 'Intracutaneous Bee Venom Herbal Acupuncture' in Knee Osteoarthritis Patients (퇴행성 슬관절염 환자에서 근육 내 봉독약침과 피내 봉독약침의 효능 비교연구)

  • Objectives : This study was designed to compare the effects on Korean Western Ontario and McMaster Universities Osteoarthritis Index(KWOMAC), 36-ltems Short-Form Health Survey(SF-36), Visual Analog Scale(VAS) and Nine Point Scale between 'Intramuscular Bee Venom Herbal Acupuncture' and 'Intracutaneous Bee Venom Herbal Acupuncture' in knee osteoarthritis patients for 4weeks. Methods : All the voluntary subject were diagnosed as knee osteoarthritis. Patients were randomly assigned either to two groups : Intramuscular bee venom herbal acupuncture treatment group(IM-BV group) or Intracutaneous bee venom herbal acupuncture treatment group(IC-BV group). The IM-BV group received bee venom herbal acupuncture intramuscularly on muscles closest to the pathologic site on ultrasonography. The IC-BV group received bee venom herbal acupuncture intracutaneously on $LE_{110}$, $LE_{201}$, $ST_{35}$, $ST_{36}$ and $LR_8$. Bee venom herbal acupuncture was applied twice a week for 4 weeks by an oriental medical doctor at Kyung-hee Oriental Medical Hospital. Both groups were checked of pathologic site by ultrasonography before treatment and follow up after four-week treatment. The patients were assessed by KWOMAC and SF-36 before and after four-week treatment. The changes in patients' pain was measured by VAS before treatment and after one-, two-, three- and four-week treatment. The patients' overall treatment outcome assesment was measured by Nine Point Scale after four-week treatment. Results : The results were as follows ; 1. The IM-BV group showed significant improvement on total, pain, stiffness and physical function in KWOMAC, and the IC-BV group showed significant improvement on total and physical function in KWOMAC after 4weeks compared to the pre-treatment. In KWOMAC, the IM-BV group showed more improvement on average compared to the IC-BV group, but there were no significant difference between two groups. 2. The IM-BV group showed significant improvement on VAS after one-week treatment. The IC-BV group showed significant improvement on VAS after two-week treatment. But there were no significant difference between two groups. 3. The IM-BV group showed significant improvement on physical functioning(PF) and bodily pain(BP) in SF-36, and the IC-BV group showed significant improvement on vitality(VT) and bodily pain(BP) in SF-36 after 4weeks compared to the pre-treatment. But there were no significant difference between two groups. 4. 88.2% of IM-BV group and 93.3% of IC-BV group graded their improvement as 'Fair' or better on Nine Point Scale. But there were no significant difference between two groups. Conclusions : This study suggests that in the treatment of knee osteoarthtitis patients, effects of 'Intramuscular Bee Venom Herbal Acupuncture' were not different from 'Intracutaneous Bee Venom Herbal Acupuncture' statistically. But both treatment showed effects on pain and physical function in knee osteoarthtitis patients.

Screening of Plants with Inhibitory Activity on Cellular Senescence

  • In this study, the effect of plant extract on the senescence action and cell survival rate in two types of cells, in which aging was derived by adriamycin, was analyzed to find the materials for suppressing cell senescence from natural resources. The results are as follows. For human umbilical vein endothelial cells (HUVECs), the fruit of Physalis angulata L. and the aerial part of Synurus deltoides (Aiton) Nakai showed excellent cell-senescence inhibition activities in a treatment concentration-dependent manner, demonstrating the high possibility for utilization as a material for prevention and treatment for vascular diseases. The water extract from the root of Polygonatum odoratum var. pluriflorum for variegatum Y. N. Lee showed potent cell-senescence inhibitory effect for human dermal fibroblasts (HDFs). Thus it is considered that the additional study on the plant needs for elucidating the possible utilization as material for skin health improvement.

Assessment of Herbal Treatment in Appetite Improvement of Anorexia Children Using Korean Children's Eating Behaviour Questionnaire (K-CEBQ) (한국어판 아동식습관 설문지를 사용한 식욕부진 소아의 식욕 개선 평가)

  • Background Children's Eating Behaviour Questionnaire (CEBQ) is a parents-targeted questionnaire to measure eating behavior of children, developed in United Kingdom and verified validity and reliability in several countries. Validity of Korean version of CEBQ (K-CEBQ) was verified in 2009. Objectives The study was conducted to assess the appetite improvement after herbal treatment in children who have anorexia. Methods The parents of the 26 children(anorexia 11, non-anorexia 15; $79.54{\pm}38.17$ months) who visited one university hospital was surveyed using K-CEBQ from April 2010 to April 2011. It was re-surveyed at intervals of 3 months. Differences between the anorexia group and the non-anorexia group were analyzed by Mann-Whitney U test, and comparisons of pre-treatment and post-treatment in anorexia group and non-anorexia group were verified by Wilcoxon signed-rank test. Results Depends on the presence of anorexia in child, Slowness in eating/Satiety responsiveness (SE/SR), Enjoyment of food (EF), Emotional undereating (EUE) and total scores were significantly different ($p$<0.05). Also, there were significant difference in SE/SR and total scores before and after the herbal treatment in anorexia group (p<0.05), and all scores tended to increase a few. However there were no significant difference before and after the herbal treatment in non-anorexia group, and the scores except for Desire to drink (DD) and Emotional overeating (EOE) were decreased. Conclusions Herbal treatment in children who have anorexia has improved their appetite in terms of eating sensitivity.

A Review of Clinical Researches for Herbal Medicine Treatment on Vitiligo (백반증의 한약 치료에 대한 임상 연구 고찰)

  • Objectives The purpose of this study is to analyze clinical studies on the efficacy and safety of herbal medicine in vitiligo by analyzing randomized controlled trials (RCTs). Methods Three electronic databases including the PubMed, EMBASE, and Cochrane library were used to search for randomized controlled trials, by using specific key words and criteria up to January 4th, 2020. Data in regards to years of publication, nation, demographic information, disease characteristics, duration of diseases, treatment methods, treatment period, outcome measures, results and adverse events were collected for this study. Results A total of 11 randomized controlled trials were selected and analyzed. The total effective rate of the treatment group treated with herbal medicine was significantly higher than that of the control group. In the other outcome measures, the treatment group also showed statistically significant differences in improving the outcome measures compared to the control group, or showed similar treatment effects to the control group. The most commonly used herbal medicines were Carthami Flos (紅花), Angelicae Gigantis Radix (當歸), Angelica dahurica Bentham et Hooker f. (白芷), Astragali Radix (黃芪), Glycyrrhizae Radix et Rhizoma (甘草), Salviae Miltiorrhizae Radix (丹參), Persicae Semen (桃仁), Araliae Continentalis Radix (獨活), Tribuli Fructus (白蒺藜), Psoraleae Semen (補骨脂) etc. Hardly any severe adverse events were reported from the trials selected. Conclusions Based on the results of the clinical studies, herbal medicine treatment could be an effective and safe option for vitiligo treatment and symptom improvement.
