  • Objectives : Combined treatment (CT) of oriental herbal medicine and prescribed drugs is now being increasingly used among cancer patients around the world. However, in Korea, clinical information on the frequency, efficacy and safety of CT among cancer patients has not yet been thoroughly reported. This study aimed to identify the status and adverse effects of CT for the management of cancer patients. Methods : A questionnaire and medical record survey by oriental medical doctors or physicians were performed at two oriental medical hospitals and one general hospital. Of the initial 400 in-patients, 368 participated in this survey, representing a response rate of 92.0%. Results : Among cancer patients in oriental medical hospitals, the proportion of CT was 45.9%. In contrast, the proportion of CT in the general hospital was only 0.6%. The proportion of CT among breast cancer patients (20) and gastric cancer patients (35) were 85.0% and 51.4%, respectively. The proportion of CT among cancer patients was high in younger, female or married patients groups. 10 respondents (11.1%) among 90 cancer patients experienced several adverse effects including nausea, fatigue, etc. Conclusions : This study suggests that many more patients in oriental medical hospitals than general hospitals use combined treatment of oriental herbal medicine and prescribed drugs for management of cancer. Therefore, medical professionals should provide comprehensive and up-to-date clinical information about potential benefits and risks of CT to cancer patients in Korea.

  • Background: Chronic kidney disease (CKD) is affective a large portion of the world population prompting the need for extensive healthcare resources such as lifelong dialysis or kidney transplantation. The beneficial effect of conventional therapy in controlling the CKD progression remains a challenge due to their relative efficacy, safety, and accessibility. On the other hand, Unani medicine provides a therapeutic regimen that consists of a combination of treatment from rehabilitation to herbal pharmacotherapy. Methods: Two cases of chronic kidney disease were treated with dietotherapy, regimenal therapy ('bukhoor aam') and oral herbal drugs for 2-3 weeks. Endpoints of evaluation were symptoms and signs of the CKD, kidney function test, urine albumin, urine RBC, hemoglobin and liver function test. Result: Notable improvement was observed in the endpoints. Conclusion: Unani treatment was observed preliminarily beneficial in the treatment of chronic kidney disease. Rigorous pharmacological and clinical studies should be performed to warrant their efficacy and safety in CKD individuals.

  • Objectives : HT042 is a combination of three herbal extracts from the roots of Astragalus membranaceus, the stems of Eleutherococcus senticosus and the roots of Phlomis umbrosa, which has been demonstrated to increase longitudinal bone growth rate. The aim of this study was to assess the safety of HT042 after repeated oral administration. Methods : A 14-day repeated oral dose toxicity study was conducted using male and female Sprague-Dawley rats. HT042 was administered orally at repeated doses of 500, 1,000 and 2,000 mg/kg/day for 14 days. Clinical signs and mortality were observed daily, whereas body weight and food consumption were recorded weekly throughout the experiment. At the end of the study, blood was taken from the posterior vena cava for hematology and serum biochemistry. All organs of the body surface, subcutis, head, thoracic cavity, and abdominal cavity were observed grossly. Then, the internal organs were removed and weighed. Results : No death occurred and no significant changes in clinical sign, body weight, food consumption and serum biochemistry parameters were observed in male and female rats over the study period. Although there were some alterations in hematologic and necropsy findings, and organ weights, these changes were not considered toxicologically significant. Conclusions : These results suggest that the 14-day repeated administration of HT042 does not produce any significant oral toxicity at doses of up to 2,000 mg/kg/day in male and female rats under the present experimental conditions.

  • Herbal medicines have been used since ancient times as medicines to treat and relieve the symptoms of many different human diseases. However, so far, relatively few herbal medicines have been evaluated scientifically to prove their safety, potential benefits and effectiveness. This study was conducted to provide the groundwork for improving the current registration regulations for herbal medicines in distinction from synthetic medicines. The study was performed based on the literature research and individual interviews with 5 experts who had extensive experience in registration of herbal medicines. When compared with synthetic drugs, herbal medicines exhibit some marked differences, namely the active principles are frequently unknown, standardization, stability and quality control are not easy, they are usually mixtures of complex compounds. Second, the current regulations for herbal medicines are reviewed by comparison of foreign regulation systems like the one in China. The regulation requirements of herbal medicine in China are in distinction from synthetic drugs. The authors conclude that new registration requirements for the herbal medicines should be changed as follows; the toxicity and efficacy data should be submitted as mixed herbal preparation and the documents and other research on the reproduction and generation toxicity need to be shown for the proof of reproduction and generation toxicity, if needed.

  • Objectives : About 13% of the medicines used by traditional korean medicines(TKM), are called animal medicines and are derived from non-herbal sources such as animals and insects. However, the clinical use of these preparations from animal medicines is often based on tradition and belief, rather than on evidence of toxicity and efficacy. As a result, animal medicines containing toxin have caused serious problems from injecting patients with venom. Here, various venoms frequently used as TKM were reviewed in terms of their instinct toxity and tried to estimate their safety classification. Methods : The estimation of safety classification was based on human equivalent dose(HED)-based MOS (margin of safety) and clinical dose applied for patients. Results and Conclusions : Except that of snake venom due to no clinical dose, they were evaluated as class 3 for bee venom, class 4 for cantharidin, toxin from blister beetle, and class 1 for venom from scolopendrid. In conclusion, animal medicines showed a wide range of safety classification from class 1 to class 4. This wide range is estimated to result from extremely limited applications of each venom for patients because of their strong toxicity. However, it should be cautious for application in clinics since animal medicines can produce anaphylactic reactions particularly after veinous administration even with a tiny amount of venom.
