Effect of Hwangryunhaedoktang on Contact Hypersensitivity and Passive Cutaneous Hypersensitivity in Mice

  • During the screening program to discover antiatopic agents from herbal formulas, we investigated the inhibitory effect of Hwangryunhaedoktang (HT) on passive cutaneous anaphylaxis and oxazolone-induced mouse ear dermatitis. HT significantly inhibited PCA reaction in mice at doses of 50 and 200 mg/kg with inhibitory activity of 31 and 53%, respectively. HT at concentration of 0.05 and 0.1% inhibited ear swelling by 23 and 46% at 16 days in oxazolone-induced mouse ear dermatitis. Both HT with anti without human intestinal microflora showed potent inhibitory activity on the ${\beta}$-hexosaminidase release induced by IgE. Based on these findings, HT may be a usable agent for skin disorder contact dermatitis.

태음인 처방의 본초 조합 용례 분석 - 중요 고문헌을 중심으로 - (Analysis of the prescription for persons of Taiyin constitution (太陰人 Tae-eum) in the herbal formulas and cases found in classic texts)

  • Purpose : This study compares prescriptions in the 1901 edition of the Donguisusebowon (東醫壽世保元) and the 1901 edition of 24 Revised Tae-eum constitution prescriptions (新定太陰人病應用要藥二十四方) (1901PCDT) with medical herbs found in classical texts. This study also examines whether to include patterns which compose medical herbs for the person of Tae-eum constitution (太陰人). Method : The prescriptions for Tae-eum-in in the Shinchukbon and the ingredients of medical herbs of the prescriptions of the Complete Works of Zhang Jingyue (景岳全書), Secret Works of Universal Benefit (廣濟秘笈), Treasured Mirror of Eastern Medicine (東醫寶鑑), Comprised Edition for Formulas (方藥合編), Protecting Essence for Longevity (壽世保元), Introduction to Medicine (醫學入門), New Book for Saving People (濟衆新編 ENGLISH), and Compendium of Local Medicinals and Formulas (鄕藥集成方) were input into a database. The prescriptions were examined and combinations of medical herbs used to treat the person of Tae-eum constitution were noted. Result : Among the prescriptions for the person of Tae-eum constitution, similar examples of the 17 prescriptions of Kudzu Decoction to Relieve the Muscles (葛根解肌湯), Regulate the Stomach and Coordinate the Purity Decoction (調胃升淸湯), Clear the Heart and Lotus Pip Decoction (淸心蓮子湯), Decoction for Stabilizing Asthma with Ephedra (麻黃定喘湯), Profuse Heat and Sparse Cold Decoction (熱多寒少湯), Decoction for Coordinating Qi with Kudzu (葛根承氣湯), Major Decoction for Coordinating Qi with Kudzu (葛根大承氣湯), Minor Decoction for Coordinating Qi with Kudzu (葛根小承氣湯), Decoction for Dispersing the Exterior with Ephedra (麻黃發表湯), Boost the Lung Essence Decoction (補肺元湯), Major Supplementing Decoction with Deer Antler Velvet (鹿茸大補湯), Boosting Black Essence Pill (拱辰黑元丹), Honeylocust Fruit and Rhubarb Decoction (皂角大黃湯), Kudza and Duckweed Decoction (葛根浮萍湯), Sweet Flag and Polygala Powder (石菖蒲遠志散), Liriopis and Polygala Decoction Powder (麥門冬遠志散), and Cattle Gallstone Formula to Clear the Heart (牛黃淸心元) were found in important ancient literature and Dried Chestnut and Holotrichia Decoction (乾栗蠐螬湯) and Dried Chestnut and Tree of Heaven Root Bark Decoction (乾栗樗根皮湯) are the combination originally set by Lee Jema. Conclusion : Because 70.8% of the prescriptions in 1901PCDT could be found in literature which were pervasive in the period of the Joseon Dynasty (the period of Ming and Qing), it corresponds with Lee Jema's view that medical men in the Song, Yuan, and Ming Dynasties disclosed half of the prescriptions for Tae-eum person by studying these texts more.

류마티스 관절염에 대한 한약의 면역학적 연구동향 (Recent Trends of Immunologic Studies of Herbal Medicine on Rheumatoid Arthritis)

  • Objective : Rheumatoid arthritis is an autoimmune disease that pathogenesis is not fully understood and one of the most intractable musculoskeletal diseases. The concern in the immunopathogenesis of rheumatoid arthritis has been increased since 1980's and many immunotherapeutic agents including disease-modifying antirheumatic drugs (DMARDs) were developed and became the mainstay of treatment of rheumatoid arthritis. However, the cure of the disease has hardly been achieved. In oriental medicine, rheumatoid arthritis is related to Bi-Zheng(痺證), that presents pain, swelling, andlor loss of joint function as major clinical manifestations, and also known to be deeply involved in suppression of immune function related to weakness of Jung-Ki(正氣). The herbal medicine, empirically used, could be a potential resource of development of new immunotherapeutic agents for rheumatoid arthritis. Methods : We developed a search strategy using terms to include "rheumatoid arthritis and herbal medicine" combined with "Chinese medicine" and/or "Oriental medicine". The search was focused on experimental studies of herbal medicine (January 1999 to May 2004), which is known to have effects on immune function of patients with rheumatoid arthritis. Computerized search used Internet databases including KISS and RISS4U (Korea), CNKI (China), MOMJ (Main Oriental Medicine Journal, Japan), and PubMed. The articles were selected from journals of universities or major research institutes. Results : The literature search for experimental studies on effects of herbal medicine on immunity of rheumatoid arthritis retrieved a total of 21 articles (Korea; 8, China ; 12, Japan ; 1). Of 21 articles, 10 were related to single-drug formula, 2 to drug interaction, and 9 to multi-drug formula. Single-drug formula was mainly used for aqua-acupuncture and researches on active components. Studies of drug interaction emphasized harmony of Ki-Hyul(氣血) and balance of Han-Yeul(寒熱). Multi-drug regimen was mainly found among formulas for Bo-Ki-Hyul(補氣血) and Bo-Sin(補腎). Conclusion : Studies on rheumatoid arthritis were performed both in vitro and in vivo in vitro study, LPS-stimulated splenocytes and synoviocytes were treated with herbal medicine, resulting in proliferation and activation of immune cells and suppression of cytokine activities in vivo study CIA animal model demonstrated that herbal medicine decreased antibody production and improved function of immune cells. In cellular and molecular study herbal medicine showed profound effects on the level of mRNA expression of certain cytokines related to immune function. This study revealed that herbal medicine has significant immune modulatory action and could be used for recovery of immune dysfunction of rheumatoid arthritis patients.

東醫寶鑑 中 羌活이 主藥으로 配伍된 方劑의 活用에 대한 考察 (A Study on the Applications of Ostericum Radix Mainly Blended Prescription in Dongeuibogam)

  • This report describes 90 prescriptions related to the use of Ostericum Radix(OR) blended as the main ingredient in the Dongeuibogam.

    The following conclusions were reached through investigations on the prescriptions that use OR as a key component.

    Prescriptions where OR was used as one of the monarch(drug) utilized for 27 therapeutic purposes in total.

    It was searched that the frequency of prescriptions in the therapeutic purposes were as follows. The chapter of eyes is 22.2%, the wind is 13.4%, the child 11.1%, the cold is 8.9%, the head is 6.7%, the malaria disease 4.5% in the order.

    Prescriptions that utilized OR as the main ingredient were most used for treating diplopia, facial nerve palsy, headache and a fever in the whole body. These were also used for treating 55 different types of diseases.

    The dosage of OR in formulas was ranging from 2pun(about 0.75g) to 2don(about 7.5g). However, 1don(about 3.75g) was most used for clinical application.

    We can find out that according to herbs or prescriptions blended with itself, OR made a variety of functions to eliminate cold with wind, humidity, pain, and convulsions from the body. And also, Bangpungtang was the most useful base prescription which used OR as the main component.

  • Objectives : Examined the editing system and the characteristics of "Clinical Formula Science of Korean Medicine" to establish a basis for compilation system of the common formula science textbook for colleges of Korean medicine nationwide. Methods : Comprehended the "Clinical Formula Science of Korean Medicine" published by professor Hyeongok(1911-1987) in 1985 and analyzed its configuration system. Results : "Clinical Formula Science of Korean Medicine" has made it possible to understand the formula systematically by establishing and classifying 24 kinds of treatment method [the three treatment methods of cold-dampness, dryness-heat and interior deficiency and its subdivision, 19 kinds of treatment method (stomach, phlegm, eum, yang, spirit, essence, ki, blood, blood stasis, exterior, interior, half-exterior half-interior, water, wind, cold, heat, dampness) combined with reducing and vomiting] and main formula. These can be considered as professor Hyeongok's own method of classification, which is distinctive from the existing formula science system. Also, it suggests that a Korean medicine doctor should not use a biased formula science that is limited to some schools but apply the formula widely and synthetically because it is believed to pursue perfection in cure if the main formulas related to a physical disorder, pregnancy and childbirth, invigorating and purging five viscera, and Four-Constitution Medicine are added. Conclusions : "Clinical Formula Science of Korean Medicine" is seen as a formula science book which is to be consulted when compiling a common formula science textbook for colleges of Korean medicine by forming a Korean formula science system that is distinctive from that of China.

LC-ESI-MS/MS를 이용한 계지탕 중 주요 성분 분석 (Quantitative Determination of the Bioactive Marker Components in Gyeji-tang Using LC-ESI-MS/MS)

  • A traditional herbal formula, Gyeji-tang has been used to treat the early colds, headache, chills, and fever in Asian countries. In this study, we were performed simultaneous determination of the 14 bioactive marker compounds, gallic acid, spinosin, paeoniflorin, albiflorin, liquiritin apioside, liquiritin, 6'''-feruloylspinosin, liquilitigenin, coumarin, cinnmamic acid, benzoylpaeoniflorin, cinnamaldehyde, glycyrrhizin, and 6-gingerol in Gyeji-tang using an ultra-performance liquid chromatography-electrospray ionization-mass spectrometry (UPLC-ESI-MS/MS). Analytical column was used a Waters Acquity UPLC BEH $C_{18}$ analytical column ($2.1{\times}100mm$, $1.7{\mu}m$) and maintained at $45^{\circ}C$ with a flow rate of 0.3 mL/min. The mobile phase consists of 0.1% (v/v) formic acid in water and acetonitrile with gradient elution. The MS analysis was conducted using multiple reaction monitoring in the positive and negative modes by a Waters ACQUITY TQD LC-MS/MS system. The calibration curves of 14 bioactive marker compounds showed linearity with correlation coefficients ${\geq}0.9798$. The limits of detection and quantification values were in the range of 0.11-6.66 ng/mL and 0.34-19.99 ng/mL, respectively. As a result of the analysis using the established LC-MS/MS method, the amounts of tested 14 compounds in the lyophilized Gyeji-tang sample were detected up to $85.7{\mu}g/g$. These results may be useful for quality assessment of a traditional herbal formulas.

조선 중기의 비증(痺症)에 대한 연구: 승정원일기(承政院日記)를 중심으로 (A Study of Bi-Jeung in the Mid-Chosun Dynasty: Based on the Seungjeongwon Ilgi)

  • Objectives Through the clinical records of Seungjeongwon Ilgi, we reviewed the usage of Bi-Jeung to know the concept of the word and studied therapeutic strategies for managing Bi-Jeung. Methods We investigated the clinical records of the mid-Chosun dynasty containing the key word "Bi" from electronic database (Seungjeongwon Ilgi). Results Of 4,039 records, 249 articles thought to have medicinal value were lastly selected. We subdivided the cases into 13 categories according to time, the connection of contents and the change of associated symptoms. "Bi" was not used alone but used in combination with body parts or other symptoms. Etiological causes of "Bi" involved dampness, phlegm, fire, heat and qi disorders. We suggested that "Bi" of the mid-Chosun dynasty meant a symptom group mainly of sensory impairment and additionally pain or motor disturbance. Among the 22 herbal medicine formulas used, 15 were based on internal medical pattern identifications and 7 were symptomatic treatments. Acupuncture and moxibustion therapy were primarily applied to adjacent acupoints. In addition, External therapies were used together, such as washing therapy, plaster therapy, cupping therapy and thermotherapy. Conclusions "Bi" principally indicated sensory impairment on limbs and the main etiological cause was considered to be dampness. Herbal medicine, acupuncture, moxibustion and external therapies were used to cure Bi-Jeung.

Anti-osteoarthritic effects of a combination of pomegranate concentrate powder, Eucommiae cortex and Achyranthis radix in rats

  • Objectives: We examined the effects of a mixed formula consisting of dried pomegranate concentrate powder (PCP) and the aqueous extracts of Eucommiae cortex (EC) and Achyranthis radix (AR) in rats with surgically induced osteoarthritis (OA). Methods: Two weeks after OA-inducing surgery, a PCP:EC:AR 5:4:1 (g/g) combination or single formula was orally administered. Changes in body weight, knee thickness, maximum knee extension angle, bone mineral density of the knee joints, femoral and tibial articular surfaces, and compressive strength of the femoral and tibial articular cartilage (AC) were assessed, along with the prostaglandin E2 level, 5-lipoxygenase, matrix metalloproteinase (MMP)-2 and MMP-9 activity, and chondrogenic gene mRNA expression in the femoral and tibial AC with the synovial membrane (SM). In addition, the number of cleaved poly(ADP-ribose) polymerase, cyclooxygenase and tumor necrosis factor-${\alpha}$-immunoreactive cells in the femoral and tibial AC with SM were monitored, and the rate of cell proliferation was determined with a 5-bromo-2'-deoxyuridine uptake assay. Results : The signs of surgically induced OA in rats were significantly inhibited by both PCP, EC and AR combined and single formulas. In particular, the combination formula-treated OA model rats showed dose-dependent, significantly increased inhibitory activity against all tested criteria compared with single formula-treated rats. Conclusions: Taken together, our results suggest that the combination formula synergistically increased the anti-OA effects of its components through anti-inflammatory and chondrogenic activity in rats with surgically induced OA. In addition, 200, 100 and 50 mg/kg combination formula treatments showed dose-dependent inhibitory activity against all of the tested criteria.

  • Objective: This case report presents the effects of traditional Korean medical treatment for a patient with symptoms of the upper burner due to Beçhet's disease. Methods: A 42-year-old male patient was treated with traditional Korean herbal medicine (herbal formulas and patent medicines), acupuncture, and cupping therapy. Neck, ocular, and right wrist pain and xerostomia were assessed daily with a numeric rating scale (NRS) and grading assessment tool for subjective symptoms. Results: For a total of 20 days over two hospitalizations, the patient's neck pain was relieved from NRS 4 to 1, the ocular pain from NRS 5 to 1, the right wrist pain from NRS 7 to 1, and the xerostomia from grade 8 to 1. Conclusion: These results show that Beçhet's disease symptoms of the upper burner can be relieved with traditional Korean medical treatment, although additional well-designed studies are required to confirm these findings.
