Objective: Nowadays the combined use of herbal extracts and western medicines has been prevalent, but concern about its risk is also increasing. Even though the importance of clinical trials is well recognized, there have been only a few studies on the combined use of herbal extracts and western medicines. This study was aimed to examine which herbal extracts and antiplatelets or anticoagulants are most commonly prescribed together to inpatient ischemicstroke patients and investigate the combined prescription rate. Methods: We investigated the most frequently prescribed herbal extracts from two different sources. First, we chose herbal medicine extracts from 2008 Traditional Korean Medicine Utilization Status. Then, among patients who were admitted to Kyung-Hee Oriental Medical Center diagnosed with ischemic stroke, we found patients who were administered with these herbal medicine extracts and among these patients, we investigated how many were concomitantly administered with antiplatelets or anticoagulants. Second, we chose other herbal medicine extracts that were most often prescribed within Kyung-Hee Oriental Medical Center and found patients who were administered with these herbal medicine extracts, then investigated how many of them were concomitantly administered with specific antiplatelets or anticoagulants. Results: The most commonly prescribed herbal medicine extracts among ischemic stroke patients were Ojeok-san, Bojungikgi-tang, Sochungryong-tang and Samsoeum. About 46 to 69 percent were concomitantly administered with either specific antiplatelets or anticoagulants. Conclusions: The most often prescribed herbal medicine extracts in inpatient ischemic stroke patients are Ojeok-san, Bojungikgi-tang, Sochungryong-tang, and Samsoeum. Among patients who were prescribed with Ojeok-san, Bojungikgi-tang, Sochungryong-tang, Samsoeum, patients who were concomitantly administered with either antiplatelets or anticoagulants were about 46% to 69%.

The aim of this study was to investigate whether a novel formulation of an herbal extracts has an inhibitory effect on obesity. To determine its anti-obesity effects, we performed anti-obesity-related experiments in vitro and in vivo. Thus, our present study was carried out to evaluate the anti-obesity effect of herbal extracts using a high fat diet (HFD)-induced obese mouse model and 3T3-L1 adipose cells. The effects of each herbal extracts on lipid accumulation in 3T3-L1 cells were examined using Oil Red O staining. Results showed that treatment with each herbal extracts at $10{\sim}100{\mu}g/ml$ had no effect on cell morphology and viability. Without evidence of toxicity, herbal extracts treatment decreased lipid accumulation compared with the untreated adipocytes controls as shown by the lower absorbance of Oil Red O stain. Futhermore, compared with control-differentiated mature adipocytes, each herbal extracts significantly inhibited lipid accumulation in mature 3T3-L1 adipocytes. In the HFD-fed obese mice, body weight, liver weight and white adipose tissue weights were significantly reduced by mixture of herbal extracts administration in mouse skin. Futhermore, we found that mixture of herbal extracts administration suppressed serum triglyceride (TG), and total cholesterol (TCHO) in HFD-induced obese mouse model. The mixture of herbal extracts of permeability was estimated by measuring the transepithelial electrical resistance (TEER) value in pig skin. The optimized formulations of herbal extracts (Test 3 formulation) showed skin permeation. However, test 1 formulation containing essential oil as enhancer showed maximum skin permeation. After confirming the enhanced skin permeability, in vivo studies were performed to assess whether skin irritation potential on the basis of a primary irritation index (PII) in rabbit skin. Reactions were scored for erythema/edema reactions at 24 h, 48 h and 72 h post-application. It was concluded that the test 1 formulation was not irritation (PII = 0). The present study suggests that the test 1 formulation might be of therapeutic interest with respect to the treatment of obesity.
Objectives : This study aims to investigate the acute oral toxicity and safety of Samsoeum (Shensuyin) extracts and fermented Samsoeum extracts. Methods : For that objective, we used ICR mice. ICR mice were administerd orally with dosage of 1250mg/kg, 2500mg/kg and 5000mg/kg of Samsoeum extracts and fermented Samsoeum extracts. Results : We daily examined number of deaths, clinical signs, body weights and gross findings for 2 weeks. 1. The results of acute oral toxicity using ICR mice showed that LD50 of value over 5000 mg/kg. 2. Samsoeum extracts and fermented Samsoeum extracts not affect on bodyweight gross findings of ICR mice. 3. The results of Serum chemistry analysis and Complete Blood Count(CBC) through the autopsy were showed normal range values. Conclusions : Samsoeum extracts and fermented Samsoeum extracts did not show any toxic effects in ICR mice. And oral LD50 value was over 5000mg/kg in ICR mice and it is very safe for ICR mice.

Objectives : The aim of this study was to examine anti-thrombotic effect of traditional herbal extracts in a rat model of ferric chloride ($FeCl_3$)-induced carotid arterial thrombosis. Methods : Thirty minutes prior to a 35% $FeCl_3$ application, Sprague Dawley(SD) rats were injected with the 10 types of traditional herbal extracts (100mg/kg, intraperitoneal injection), respectively. The effect of these herbal extracts was examined for time to occlusion(TTO) using the Laser doppler flow meter and measured for thrombus weight (TW) in $FeCl_3$-induced thrombosis model. Results : In the TTO, Salvia miltiorrhiza (Sm, $2.30{\pm}0.28$ min, p<0.001) and Santalum album (Sa, $2.19{\pm}0.19$ min, p<0.001) showed significantly delayed TTO more than twice compared with Saline-treated group. Cnidium officinale (Co), Psoralea corylifolia (Pc), Scutellatia baicalensis (Sba), Panax notoginseng (Pn), Angelica tenuissima (At), Scrophularia buergeriana (Sbu), Rhus verniciflua (Rv) and Picrasma quassioides (Pq), except for Rhus verniciflua (Rv) also meaningfully impeded TTO more than one fold. In addition, Salvia miltiorrhiza, Santalum album, Cnidium officinale, Psoralea corylifolia and Scutellatia baicalensis significantly reduced TW more than 10% compared with Saline-treated group. Especially, Salvia miltiorrhiza and Santalum album showed the most excellent anti-thrombotic effect among the 10 herbal extracts tested on the restoration of altered TTO and TW. Conclusions : These results suggest that Sm and Sa extracts have outstanding anti-thrombotic effect in $FeCl_3$-induced thrombosis model and is potentially useful as herbal medicines for the treatment and prevention of thrombosis.
Objectives : Herbal formulas are complicated to analyze and difficult to compose because of its mixed, complex features. So we discussed about how to compose herbal formulas effective and efficient by analyzing various effectiveness of each herbal extracts. Methods : To evaluate the effectiveness of herbal formula, anti-oxidative and anti-inflammatory effectiveness were mathematically analyzed. DPPH, superoxide anions, Nitric Oxide scavenging activity was measured to evaluate the effectiveness of 7 herbal extracts. And next, cytotoxic activity of extracts on RAW 264.7 cells were measured using MTS assay. To asses anti-inflammatory effect, nitric oxide and $PGE_2$ production were measured. Based on these anti-oxidative and anti-inflammatory experiment result, mathematical analyzation were carried out with constOptm function, and determined efficacy-maximizing ratio. Results : This mathematical analysis based formula showed significantly outstanding effectiveness than other formulae. And estimated tendency of anti-inflammatory effectiveness was matched with real effectiveness. Conclusions : So, mathematical analysis can be available to evaluate and estimate the effectiveness of herbal formulae.

This study was carried out to investigate the effects herbal extracts on the skin inflammatory reactions. Among the herbal ingredients of herbal extracts, ethanol extract of Glycyrrhizae Radix showed potent radical scavenging activity, more than 86$\%$ at a concentration of 0.01$\%$, tested by DPPH(I,1-diphenyl-2-picryl-hyrazyl) method. And ethanol extract of Aurantii Immaturus Fructus inhibited the lipopolysaccharide-induced release of nitric oxide(NO), 99$\%$ at 50㎍/㎖, by the macrophage RAW 246.7 cells. In addition, the ethanol extracts of Sophorae Radix, Glycyrrhizae Radix showed antibacterial activities on Propionibacterium acnes that causes acne, Sophorae Radix, Glycyrrhizae Radix and Aurantii Immaturus Fructus on Corynebacterium xerosis that cause osmidrosis axillae, Glycyrrhizae Radix and Aurantii Immaturus Fructus on Staphylococcus aureus that was reported to cause impetigo and atopic disease. Taken together, I expect that herbal extracts may be used as a drug for treatment on skin inflammation.
Various herbal medicinal extracts were examined for the development of cosmetic products with anti-oxidative and anti-wrinkle activity. First, total polyphenol contents and DPPH radical scavenging activities of herbal medicinal extracts were measured. Most herbal samples, except for DW extracts of Portulaca oleracea, Caesalpinia sappan, Taraxacum platycarpum, Carthamus tinctorius, and 70% EtOH extracts of Taraxacum platycarpum and Carthamus tinctorius, showed DPPH radical scavenging activity over 80% at a concentration of $400{\mu}g/m{\ell}$. SOD-like antioxidant activity of DW extracts of Syzygium aromaticum, DW extracts of Eriobtrya japonica and 70% EtOH extracts of Sophora japonica was measured as 40%, 35% and 80%, respectively at a dry matter concentration of $50{\mu}g/m{\ell}$. In elastase inhibition assay, DW extracts of Lycium chinense ($50{\mu}g/m{\ell}$) and 70% EtOH extracts of Areca catechu ($50{\mu}g/m{\ell}$) showed 50% and 40% of inhibition, respectively. At a concentration of $1.250{\mu}g/m{\ell}$, DW extracts of Lycium chinense and 70% EtOH extracts of Areca catechu showed 10% and 30% of collagenase inhibition, respectively. Skin and lotion samples were prepared using the two herbal extracts of high anti-wrinkle activity: Lycium chinense extract and Areca catechu extract. The storage stability of skin and lotion containing each of the selected herbal extracts was evaluated. pH and viscosity were used as stability indicators for the stability test under different storage temperatures and freeze-thaw cycle conditions. The skin and lotion containing each of DW extract of Lycium chinense and 70% EtOH extract of Areca catechu was showed high pH and viscosity stability. The skin and lotion containing DW extracts of Lycium chinense showed relatively higher stability than the skin and lotion containing 70% EtOH extract of Areca catechu, at cycle chamber and freeze-thaw conditions. In summary, these results indicated that cosmetics containing DW extract of Lycium chinense were relatively stable, and this herbal extract could be used as a stable functional cosmetic material.

Aster koraiensis Nakai (A. koraiensis) which has been used as a food and medicinal plant in the past, is valuable as functional food material. Therefore, the aim of this study was to determine the antioxidant properties and major phenolics of A. koraiensis extracts with different ethanol concentrations (0, 50, 70, and 100% aqueous ethanol solution). When ethanol concentration in the extraction solvent was increased, extraction yield decreased; 34.2, 23.2, 21.0, and 5.5% in 0, 50, 70, and 100% ethanolic extracts, respectively. Total phenolics content and antioxidant activities of extracts were increased in an ethanol concentration-dependant manner. The major phenolics in the extracts were chlorogenic acid (21.264~58.666 mg/g), isochlorogenic acid A (10.432~145.353 mg/g), and isochlorogenic acid C(0.239~13.148 mg/g), and these phenolic contents were higher in 70 and 100% ethanolic extracts than other extracts. Significant correlations were observed between ethanol concentration of extraction solvent, antioxidant properties, and major phenolics. These results indicated that the optimal ethanol concentration for extraction was 70%.
Objectives : This study was performed to find out the correlation between the use of herbal extracts and the Sasang constitution of patients in Japan. Methods : We recruited 315 subjects who had been visiting the Center for Kampo Medicine for the treatment, Keio University in Tokyo, Japan. They answered SSCQ-P(Sasang Constitution Questionaire for Patients) questionnaire, a sasang constitution specialist interviewed them and diagnosed the sasang constitution. And the 80 herbal extracts subject's having taken were classified into four type Sasang constitutional herbal formulas according to "Donguisusebowon", the ingredients in a formula that have different role(sovereign, minister, assistant and courie) and effect and indication of formula. Results : 1. Among the total 315 subjects, the number of Taeyangin, Soyangin, Taeeumin, and Soeumin was 3(1.0%), 85(27.0%), 90(28.6%), and 137(43.5%). 2. Among the 80 herbal extracts subject's having taken, the number of each Soyangin, Taeeumin, and Soeumin herbal formulas was 30, 7 and 43. 3. 1 Soyangin and 2 Soeumin herbal formulas were prescribed in 3 Taeyangin subjects group. 29 Soyangin, 4 Taeeumin, 52 Soeumin herbal formulas were prescribed in 85 Soyangin subjects group. 39 Soyangin, 6 Taeeumin, 45 Soeumin herbal formulas were prescribed in 90 Taeeumin subjects group. 51 Soyangin, 6 Taeeumin, 80 Soeumin herbal formulas were prescribed in 137 Soeumin subjects group. Conclusions : The Soeumin rate in Japan was significantly higher than that in Korea. There was no significant correlation between the use of herbal extracts and the Sasang constitution in Japan.
Ganoderma lucidum, an oriental polypore fungus and medicinal mushroom, has a long history of use for promoting health and longevity in Korea, China, and other Asian countries. This study was aimed at determining the anti-inflammatory activity and mechanism of action of Ganoderma lucidum in murine macrophage RAW 264.7 cells. Ganoderma lucidum was extracted with ethanol and freeze-dried. The anti-inflammatory effect (nitrite production) of Ganoderma lucidum extracts was tested using a nitric oxide (NO) colorimetric assay. Semi-quantitative reverse transcription polymerase chain reaction (RT-PCR) was performed to quantify the mRNA expression of cytokines including tumor necrosis factor-${\alpha}$ ($TNF-{\alpha}$), interleukin $(IL)-1{\beta}$, and IL-6. Western blotting was performed to measure the expression levels of inflammation-related proteins, such as inducible nitric oxide synthase (iNOS), cyclooxygenase-2 (COX-2), nuclear factor kappa B ($NF-{\kappa}B$) p65, and phosphorylated $NF-{\kappa}B$ p65. The NO colorimetric assay showed that NO production increased with the treatment of lipopolysaccharide in (LPS)-activated RAW 264.7 macrophages and decreased with the cotreatment of Ganoderma lucidum extracts and LPS. Ganoderma lucidum extracts repressed the mRNA expressions of cytokines, which were increased after the LPS treatment. In addition, Ganoderma lucidum extracts inhibited the LPS-induced expression of iNOS and COX-2 and the LPS-induced phosphorylation of $NF-{\kappa}B$ p65. These results suggest that the Ganoderma lucidum extracts exert an anti-inflammatory activity by inhibiting $NF-{\kappa}B$ related proteins and cytokines.
