체외 수정과 한약 치료 중재에 대한 임상연구 동향 - Pubmed를 중심으로 - (Clinical Research of the Effect of Traditional Herbal Medicine with In Vitro Fertilization and Embryo Transfer in Pubmed)

  • Objectives: The purpose of this systematic review is to assess the effects of herbal medicine with in vitro fertilization-embryo transfer (IVF-ET) based on the current evidence. Methods: Eligible randomized controlled trials (RCT) searched from Pubmed which compared a combination of herbal medicine and IVF with IVF alone were included. Results: Sixteen trials, in which 2025 women involved, were included in this review. The review results showed that the effect of traditional herbal medicine can improve the clinical pregnancy rate (herbal medicine intervention: 30.36~63.64%, Control: 9.38~47.5%) and rate of high quality oocytes and embryos. The mechanism may be through regulating follicular fluid to improve microenvironment for oocytes which would lead to a successful embryo implantation. Conclusions: This analysis showed that combination of IVF and traditional herbal medicine used in the included trials improve clinical pregnancy rate and IVF success. During in vitro fertilization, TCM can regulate the microenvironment in the follicular fluid to mature the oocyte, improve the quality of the embryo, and help the development and implantation of the embryo. Further large randomized placebo controlled trials are needed to confirm the effectiveness of traditional herbal medicine with concurrent IVF.

단미 한약과 합성 혈압약의 상호작용 (Interactions between herbal medicines and synthetic antihypertensive drugs)

  • Objective : Many patients take antihypertensive drugs as well as herbal medicines at the same time in order to treat other symptoms or to keep their well-being. In this study, interactions between herbal medicines and synthetic antihypertensive drugs were analyzed. Methods : To investigate the interaction between herbal medicines and synthetic antihypertensive drugs, three electronic databases, including OASIS, Mediline and Sciencedirect were searched. Experimental and clinical studies on the interaction between herbal medicines and antihypertensive drugs were independently reviewed and included. Results : Analyzing selected studies, twenty herbs were found to interact with antihypertensive drugs. Herbs found to increase the antihypertensive effect were Panax ginseng, Carthamus tinctorius, Magnolia officinalis, Silybum marianum, Scutellaria baicalensis, Schisandra chinensis, Sophora flavescens, Piper nigrum, Curcuma longa, Ginkgo biloba, Juncus effuses and Hydrastis canadensis. In contrast, Commiphora myrrha, Rhodiola rosea, Hypericum perforatum, Eurycoma longifolia, and Daturae metel were found to inhibit the antihypertensive effect. Stephania tetrandra could increase or decrease the effect depending on the type of antihypertensive drug. Epedria sínica was suspected of pharmacodynamic interaction with antihypertensive drug. Glycyrrhiza uralensis has been reported to have serious side effects in combination with antihypertensive drugs. Conclusion : These results imply that when used in combination with herbal medicines and synthetic antihypertensive drugs, proper doses and herbs which are to avoid need to be informed to the patients. Despite concerns about interactions between herbal medicines and synthetic drugs, related research is very limited. More systematic researches are needed to give information on patient safety as well as to guide clinical practice.

Helicobacter pylori infection에 대한 한약과 합성의약품 병용투여 효과에 대한 메타분석 및 체계적 문헌고찰 : PubMED를 중심으로 (Meta-analysis and Systematic Review on the Effects of Herbal Medicine and Synthetic Drugs in Helicobacter pylori Infection: Focusing on PubMED)

  • Objectives : In clinical practice, there are many cases of co-administration of herbal medicine and synthetic drugs. This study tried to identify whether the combined administration of herbal medicine and synthetic drugs including amoxicillin increases Helcicobacter pylori eradication rate compared to the single administration of synthetic drugs or the combined administration of synthetic drugs and placebo herbal medicine through systematic review. Methods : Relevant randomized controlled trials were searched in PubMED database. The risk of biases was assessed through the Cochrane Risk of Bias criteria. Three reviewers were extracted the characteristics and outcomes of each study. Meta-analysis of eradication rate and adverse event was conducted. Results : Four RCTs were selected. In meta-analysis, the combination of herbal medicine and synthetic drugs showed eradication effect, but it was not statistically significant (Odds Ratio [OR] 0.46; 95% confidence interval [CI] 0.17 to 1.24; p=0.13; I2=56%) than administration of synthetic medicine alone. Combination of herbal medicine and synthetic medicine did not increased the incidence of adverse event(OR 1.07; 95% CI 0.72 to 1.59; p=0.68, I2=0%) compared to single administration of synthetic medicine. Conclusion : Although no significant difference was observed between the two groups in the eradication rate and the incidence rate of adverse events, it was difficult to draw a clear conclusion due to the heterogeneity between studies and the low quality of reporting. A number of studies that have overcome these limitations in the future will lead to definite conclusions.

소아 1형 당뇨의 한약과 양약 병용 치료에 대한 체계적 문헌 고찰 및 메타분석 (Efficacy of Combination Treatment of Herbal Medicine and Western Medicine for the Treatment of Type 1 Diabetes in children : Systemic Review and Meta-analysis)

  • Objectives The purpose of this study is to investigate the clinical studies on the effect of herbal medicine in type 1 diabetes in children and to seek better approach of herbal medicine to treat type 1 diabetes in children. Methods This study researched randomized controlled trials through various databases in the world about herbal medicine treatments in type 1 diabetes in children. Results 10 out of 337 studies were selected and analyzed. All studies were conducted in China. All studies were using herbal medicines, as an adjunctive treatment to the main regimen. As a result, the integrated Chinese medicine and western medicine lowered FPG by -1.56 mmol/L and 2hPG by -1.94 mmol/L on average, respectively. The HbA1c also decreased by -1.11% in the treatment group compared to the control group. Total efficacy of the treatment was 1.21 times more effective in the treatment group than in the control group. Conclusions Based on the results of the studies, it seems that the herbal medicine for the treatment of type 1 diabetes in children will be effective as a combination with conventional medicines. Further research is needed to prove the findings of this observatoional studies.

Carthamus tinctorius seeds-Taraxacum coreanum combination attenuates scopolamine-induced memory deficit through regulation of inflammatory response and cholinergic function

  • BACKGROUND/OBJECTIVES: There is growing interest in herbal medicines for managing age-related diseases, such as Alzheimer's and Parkinson's. Safflower seeds (Carthamus tinctorius L. seeds, CTS) and dandelions (Taraxacum coreanum, TC) are widely used to treat bone- or inflammation-related diseases in Oriental countries. This study investigated the protective effect of the CTS-TC combination on scopolamine (Sco)-induced memory deficits through inflammatory response and cholinergic function. Moreover, marker components such as serotonin, N-(p-coumaroyl) serotonin, N-feruloylserotonin, chlorogenic acid, and chicoric acid in the CTS-TC combination were analyzed for their potential benefits on memory function. MATERIALS/METHODS: Water extracts of CTS, TC, and the CTS-TC combination at various ratios (4:1, 1:1, and 1:4) (100 mg/kg) were orally administered to mice for 14 days. Sco (1 mg/kg) was intraperitoneally injected into the mice before each behavioral test. T-maze and novel object recognition tests were conducted to monitor behavioral changes after the treatment. Western blotting was performed to detect protein expression. In addition, the presence of 5 biomarkers, serotonin, N-(p-coumaroyl) serotonin, N-feruloylserotonin, chlorogenic acid, and chicoric acid, was analyzed using high-performance liquid chromatography (HPLC). RESULTS: Behavioral tests showed that the CTS-TC combination enhanced memory function in Sco-injected mice. Inflammation-related proteins (inducible nitric oxide synthase, cyclooxygenase-2, and glial fibrillary acidic protein) were downregulated after treatment with the CTS-TC combination. The acetylcholinesterase protein expression was also downregulated. HPLC analysis revealed that N-feruloylserotonin and chicoric acid were the predominant components, followed by N-(p-coumaroyl) serotonin, chlorogenic acid, and serotonin. CONCLUSION: These findings suggest that the CTS-TC combination protects against Sco-induced memory deficits by inhibiting inflammatory responses and cholinergic dysfunction. N-feruloylserotonin and chicoric acid, along with N-(p-coumaroyl) serotonin, chlorogenic acid, and serotonin, might be biomarkers for the CTS-TC combination, and their effects on memory protection warrant further study.

사염화탄소 유도성 급성 간 손상 모델에서 레몬밤과 민들레 잎 추출물의 혼합 비율에 따른 간 보호 효능 연구 (Study on the hepatoprotective effects of lemon balm and dandelion leaf extract combination in carbon tetrachloride-mediated liver injured mice)

  • Objectives : Present study investigated the hepatoprotective effects of various combinations of lemon balm and dandelion (LD) leaf extract on carbon tetrachloride ($CCl_4$)-induced acute liver injury. Methods : Mice were orally treated with 200 mg/kg of LD extracts [1:1, 1:2, 1:4, 1:6, 1:8, 2:1, 4:1, 6:1, or 8:1 (weight : weight)] for 7 days, and then intraperitoneally injected with $CCl_4$ (0.5 mL/kg). Silymarin (100 mg/kg) was used as reference drug. Body weight gain, relative liver weight, serum biochemistry, histopathologic analyses, and hepatic antioxidant system were examined to elucidate the fittest combination ratio of LD extract. Results : All varying combinations of LD extract significantly increased body weight gain and decreased relative liver weight by $CCl_4$. In addition, LD extract reduced serum aspartate aminotransferase and alanine aminotransferase. Histopathological analyses indicated that LD extract significantly decreased histological activity index score, degenerated hepatocytes, and infiltrated inflammatory cells induced by $CCl_4$. Moreover, LD extract reduced lipid peroxidation, and attenuated the reduction of hepatic glutathione, superoxide dismutase, and catalase by $CCl_4$. Although there were no statistical differences in body weight gain between silymarin- and LD extract-treated groups, administration of 1:1, 2:1, and 4:1 combination of LD extract exhibited more favorable hepatoprotective effects than silymarin. Especially, 2:1 combination of LD extract showed the most potent hepatoprotective effects. Conclusion : Of various combinations of LD extract tested, present results suggest that 2:1 combination of LD extract would be a promising herbal formulation to protect liver from oxidative stress.

기허가 한약제제를 대상으로 한 인지장애 치료 유효 약물 탐색 - 인지장애 임상연구를 위한 예비연구 (Exploring the Effective Herbal Prescription for Cognitive Disorder Treatment among Licensed Herbal Medicines in Korea - A Preliminary Study for Clinical Trial of Cognitive Disorders)

  • It is necessary to investigate the efficacy of commercial Korean herbal medicine for cognitive disorder. The purpose of this study is to select candidates among licensed herbal medicines that are expected to be effective in the treatment of cognitive disorders and can be used in future clinical trial. From laboratory experiments, we first found individual single herbal drugs that could be effective for cognitive disorder, and then through experts recommendation, selected five priorities of single herbal drug and obtained the information of the best herb as a combination of each herbal drug. To derive the final herbal prescriptions, we searched the KFDA drug information system for licensed herbal medicines containing each drug and its combination. As a result of laboratory experiments and experts recommendation, we found that the five effective single herbal drugs for cognitive disorders. They are Ginseng Radix, Acori Graminei Rhizoma, Cyperi Rhizoma, Coptidis Rhizoma, Pinelliae Rhizoma, Hoelen cum Pini Radix, and Rehmanniae Radix Preparata(Pinelliae Rhizoma, Hoelen cum Pini Radix, and Rehmanniae Radix Preparata were tied for $5^{th}$). And licensed herbal prescriptions derived from the planned search are Palbohoichoon-tang, Taehwa-hwan, Bosim-hwan, and so on. Among these, in consideration of the feasibility of research and possibility of success in development, Yukgunja-tang, Samhwangjichul-hwan can be selected as future study subjects. Through experimental studies and expert recommendations, we have derived herbal prescriptions that can be effective in treating cognitive disorders from licensed herbal medicines.

아토피 치료 처방의 본초 활용 분석 (Analysis on the herbal combinations in Korean medicine for atopic dermatitis)

  • Objective : The aim of this study was to search and analyze the herbal combinations in formulas used to treat atopic dermatitis in Korean medicine. Methods : Articles on the atopy were searched for from among those registered on the OASIS up to December, 2015. After the articles were reviewed, information on both internal and external formulas for atopy was constructed. Then, the herbal combination in the formulas was analyzed by using frequency distribution, network analysis, and data mining. Results : The combination of 'Saposhnikovia Radix - Schizonepetae Spica' and 'Scutellariae Radix - Phellodendri Cortex ' was mostly used in both internal and external formulas to treat atopy in articles. By using the network analysis and data mining, twelve combinations including ' (Saposhnikovia Radix, Ponciri Fructus Pericarpium) - (Platycodi Radix, Schizonepetae Spica) ' were discovered. Five effective combinations were also searched for external formulas.Conclusions : This study could help researchers to analyze the formulas in various ways. Moreover, the herbal combination in atopy formulas could be used to search for atopy formulas in other databases or make a new prescription.

Herbal Medicine for Coronavirus Disease 2019 (COVID-19) in Children: a Literature Review

  • Objectives Few studies to date reviewed herbal medicines prescribed to pediatric patients with coronavirus disease 2019 (COVID-19). We aimed to summarize and analyze these herbal medicines. Methods Nine databases were searched for studies published up until November 24, 2020. Nine case reports and one case series involving 17 pediatric patients were included in the study. The clinical symptoms, interventions, outcomes, pattern identification, composition, and frequency of herbal medicine administration were analyzed. Results The symptoms of pediatric patients with COVID-19 treated with herbal medicines were mild or moderate. Herbal medicines have been used to alleviate symptoms, such as fever and cough. No adverse events were reported. All of the patients who received herbal medicines were discharged from the hospital after symptomatic relief. Pattern identification revealed that the lungs were the most affected organs. Epidemic toxins, wind, dampness, and heat were the primary etiological factors. More pediatric patients with COVID-19 were prescribed a combination of herbal and Western medicines than herbal medicines alone. The frequently used herbs include Glycyrrhizae Radix et Rhizoma, Lonicerae Flos, Scutellariae Radix, Armeniacae Semen, Citri Reticulatae Pericarpium, and Astragali Radix. Conclusions Our review can serve as a reference for the treatment of COVID-19 in children.

녹용(鹿茸), 홍화자(紅花子) 단일 및 혼합 물 추출물( 抽出物)의 파골세포(破骨細胞) 분화(分化) 억제(抑制)와 골흡수(骨吸收) 억제(抑制) 효과(效果) (Inhibitory Effects of Water Extract of Cervi parvum cornu, Carthami tinctorii fructus and Their Combination on Osteoclast Differentiation and Bone Resorption)

  • Cervi parvum cornu (Deer Antler) and Carthami tinctorii fructus (Also known as Carthami seed) are widely used for treating osteoporosis and rheumatoid arthritis. In this study, We found out that the water extract of Cervi parvum cornu(WECPC), Carthami tinctorii fructus(WECTF) and their combination have effects of suppressing the RANKL-induced osteoclast differentiation. We assayed mRNA expression levels of NFATc1, c-Fos, TRAP and GAPDHS from bone marrow macrophages(BMMs) by means of RT-PCR. Similarly, the protein expression levels of NFATc1, c-Fos, MAPKs and $\beta$-actin in cell lysates were analyzed by means of Western blotting. then we determined the anti-osteoporotic effects of WECPC, WECTF and their combination using Lipopolysaccharide (LPS)-induced bone-loss mouse. WECPC, WECTF and their combination showed remarkable inhibition on RANKL-treated osteoclast differentiation without cytotoxicity. WECPC suppressed degradation of I-${\kappa}B$. WECPC, WECTF and their combination down-regulated the induction of c-Fos and NFATc1 by RANKL. Lastly, in vivo data showed that WECPC, WECTF and their combination rescued the bone erosion by LPS treatment. Thus, these results demonstrate that WECPC, WECTF and their combination can be efficacious remedies for bone-loss diseases such as osteoporosis and rheumatoid arthritis.
