The Effect of Sympathetic Ganglion Block on Hyperhidrosis (다한증 환자에서 교감신경절 차단의 임상적 고찰)

  • Two hundred and eighty eight patients suffering from excessive sweating of palms, soles and axillae etc., visited our Neuro-Pain clinic from November 1991 to March 1996. The sex ratio was 1:1.2. the third decade of age was the major age group. the onset time of hyperhidrosis was prepubertal period (in 95.1% of them). the provocative factors fo excessive sweating were tension and stress from interpersonal relationship. they had the family history (30.9%) and the past history treated with herb medication (56.9%), medicine (30.6%), operation (1.4%), and no treatment (39.6%). We treated 113 patients by sympathetic ganglion block with pure alcohol. the average times of thoracic sympathetic ganglion block were 2.1 (left), 2.4 (right) and those of lumbar sympathetic ganglion block were 1.2 (left), 1.6 (right). Average admission period was 14.7 days. Recurrence rare was 7.1%. Most longstanding effective period was 45 months. We conclude from our results that sympathetic ganglion block is one of the most effective treatments for hyperhidrosis owing to its simple technique and low recurrence rate.

Anti-cancer Activity of Korean Local Plant Extracts Inducing Apoptosis in Various Carcinoma Cells (암세포 특이적 세포 사멸을 유도하는 자생식물 추출물의 항암 효과)

  • Thirty five methanol extracts from 19 natural local plants, which have been used as traditional anti-cancer medicine, were prepared. They were analyzed the cytotoxic effects on primary fibroblast cells and carcinoma cells. The root extract of Solanum nigrum were highly toxic in both cell lines with $IC_{50}$ values of less than $0.01{\mu}g/{\mu}l$, and 26 of 35 extracts were toxic in all cells with $IC_{50}$ values of $0.1{\sim}2{\mu}g/{\mu}l$. Three extracts including the fruit extracts of Solanum nigrum and Morus alba had no cytotoxic activity in both cell lines. Five of 35 extracts were highly toxic in cancer cells than in primary cells. Because primary cells were more resistant on these extracts, the five extracts were selected for anti-cancer agent candidates. Apoptosis or programmed cell death has an essential role in chemotherapy-induced tumor cell killing. Recently, inducers of apoptosis have been used in cancer therapy. When two of 5 cancer cell-specific cytotoxic extracts (Ulmus parvifolia and Zelkova serrata) were treated in concentration of $0.02{\sim}0.1{\mu}g/{\mu}l$, apoptosis were increased at 3-5 times in cancer cell lines. Finally, the apoptotic effects of these extracts were confirmed by cleavages of both poly-(ADP-ribose)-polymerase and caspase-3 as apoptotic markers. In this report, we suggested that two of 35 medicinal herb extracts can be useful anti-cancer drug candidates inducing apoptosis in several carcinoma cell lines.

Screening of Herbal Medicines for Recovery of Acetaminophen-induced Hepatotoxicity

  • This study was conducted to quantitatively evaluate the recovery effects of herbal medicines on acetaminophen-induced hepatotoxicity. In the present study, the recovery effects of 251 herbal medicines on THLE-2 cells that had been damaged by acetaminophen were evaluated using an MTS assay. THLE-2 cells were cultured in 96-well plates and then pretreated with or without 60 ${\mu}M$ acetaminophen (${IC}_{50}$ value: 35.84) for 1 hr. Next, different herbal medicines were added to the wells, after which the cells were reincubated at $37^{\circ}C$ for 24 hr. After first round of screening, the candidate herbal medicines were selected based on a recovery rate of greater than 40% and their efficacy were then determined by dose response kinetic analysis. Among these extracts, 8 herbal medicines (Terminalia chebula, Pueraria lobata, Acronychia laurifolia, Lopatherum gracile, Oroxylum indicum, Cynanchum atratum, Senecio scandens, and Sophora flavescens) had a strong recovery effect on acetaminophen-induced damage in THLE-2 cells. Dose response non-linear regression analysis demonstrated that Senecio scandens showed the best recovery rate (98%), and that its ${EC}_{50}$ was 19.54 ng/mL. Additional studies of these herbal medicines should be conducted to determine if they possess novel therapeutic agents for the prevention or treatment of liver disorders.

A Way to Avoid Muscular Fibrosis in the First Dorsal Interosseous Muscle after Acupuncture Injection Therapy

  • Fibrosis of skeletal muscle following acupuncture is an iatrogenic disorder. The present case illustrates a patient with a unilateral fibrotic formation on a thumb muscle after acupuncture injection therapy with red sage. The patient in the present case was a counter-terrorism police officer with right-handedness; he noted a palpable nodule three months after injection therapy at his left first dorsal interosseous in which the acupuncture point LI4 (He Gu) is located. He also found a reduction in the strength of his left pinch grip that noticeably affected his left handgun marksmanship. However, being ambidextrous in single-hand pistol shooting is an essential requirement for counter-terrorism police officers. Based on the patient's medical history and claims, no underlying disease or trauma was found to be associated with his current complaint. During physical examination, a fibrotic formation in his left first dorsal interosseous muscle was visualized by using diagnostic ultrasound; also, as confirmed with dynamometry, the strength of his left pinch grip was significantly lower than that of the right counterpart. Because acupuncture injection therapy has three components, antiseptic practices, the mechanical action of syringe insertion, and the pharmacological effect of the sterile herb extract, any one of the components may have contributed to the present adverse event. The first dorsal interosseous muscle is small in dimension and rather vascular; thus, it is not an ideal site for intramuscular injection. When a clinician needs to treat a patient by performing acupuncture at the LI4 acupoint and injecting a herbal extract simultaneously, the clinician should only mechanically stimulate the LI4 acupoint while injecting the herbal medicine into the LI14 (Bi Noe) acupoint on the same meridian, the LI14 acupoint being located in the distal portion of the deltoid muscle and being fairly close to the universally agreed upon site on the upper arm for safe administration of an injection.

Effects of Shihoieokgan-Tang with Other Herbs and Formulae Pretreatment on the Change of Corticosterone Induced by Immobilization Stress in Mice (시호억간탕(柴胡抑肝湯) 등 약물(藥物)의 처치(處置)가 구속(拘束) 스트레스로 유발(誘發)된 생쥐의 혈(血) 중(中) Corticosterone 변화(變化)에 미치는 영향(影響))

  • To investigate the effect of Qi promoting-agents, clearing heat of deficiency type-agent, clearing heat of excessive type-agent, warming interior part of body-agent and Shihoieokgan-Tang against the immobilization stress, Cypri Rhizama, Auranti pericarpium, Aucklandiae Radix and Bupleuri Radix, Soyo-San, Hwangyeonhaedok-Tang, Geongangbuja-Tang and Shihoieokgan-Tang was administered intragastically 1hr prior to immobilizaiton stress and the change of corticosterone level has been measured by the flurometric method. The results obtained are as follows: 1. Qi promoting-agents(Cypri Rhizama, Auranti pericarpium, Aucklandiae Radix and Bupleuri Radix) showed mildly reducing effect or did not shown the reducing effect against the elevated coritocostereone level induced by the immobilization stress in the mice. 2. Hwangyeonhaedok-Tang, Gungangbuza-Tang, and Soyo-San did not show the reducing effect against the elevated corticostereone level induced by the immobilization stress in the mice. 3. Shihoieokgan-San was effective against the elevated corticosterone level induced by the immobililzation stress at 1g/kg, 3g/kg respectively. Consequently these data proved that Qi pomoting-agents as single herb(Cypri Rhizama, Auranti pericarpium, Aucklandiae Radix and Bupleuri Radix), clearing heat of deficiency type-agent(Soyo-San), warming interior part of the body(Gungangbuza-Tang), clearing heat of excessive type-agent(Hwangyeonhaedok-Tang) have no effect to decrease the elevated corticosterone level in mice exposed to the immobilization stress. Therefore our study revealed that Shihoieokgan-San which has the effects of herbs and formulae mentioned above decreased the elevated corticosterone level in mice exposed to the immobilization stress significantly.

Potential mechanism of anti-diabetic activity of Picrorhiza kurroa

  • Picrorhiza kurroa Royle ex Benth. (Scrophulariaceae) is a traditional Ayurvedic herb known as Kutki. It is used as a remedy for diabetes by tribes of North Eastern Himalayan region of India. Present study was conducted to explore the mechanism of antidiabetic activity of standardized aqueous extract of Picrorhiza kurroa (PkE). PkE (100 and 200 mg/kg/day) was orally administered to streptozotocin induced diabetic rats, for 14 consecutive days. Plasma insulin levels were measured and pancreas of rat was subjected to histopathological investigations. Glucose transporter type 4 (GLUT-4) protein content in the total membrane fractions of soleus muscle was estimated by Western blot analysis. Plasma insulin level was significantly increased along with concomitant increase in GLUT-4 content of total membrane fractions of soleus muscle of diabetic rats treated with extract. There was evidence of regeneration of ${\beta}$-cells of pancreatic islets of PkE treated group in histopathological examinations. PkE increased the insulin-mediated translocation of GLUT-4 from cytosol to plasma membrane or increased GLUT-4 expression, which in turn facilitated glucose uptake by skeletal muscles in diabetic rats.

The Effect of Artemisia Capillaris Crude Juice Extract on $CCl_4$ Induced Liver Damage in Dogs (인진쑥이 사염화탄소 투여로 유발된 개의 간 손상 회복에 미치는 영향)

  • Artemisia is a major edible vegetable in Korea and it has traditionally been used as a herbal medicine for the treatment of coughing, abdominal pain, indigestion, bleeding, jaundice, chronic liver disease and diabetes. However the biological and pharmacological actions of the herb have not been studied well. Recently it is known to possess antibacterial, antihelmintic and antifertility activities. But the effect of Artemisia capillaris extract on carbon tetrachloride($CCl_4$) induced liver damage in dogs have not been reported yet. This study was designed to investigate the effect .of Artemisia capillaris crude juice extract on $CCl_4$ induced liver damage in dogs. 30 clinically healthy dogs were divided into 2 groups: crude Artemisia capillaris juice treated group(CEC group) and carbon tetrachloride($CCl_4$) administerd group. The results are as follows: I. The degree of increase in AST activity and ALT activity in CEC group was lower than that in $CCl_4$ group and the recovery in CEC group was faster than that in $CCl_4$ group. 2. Changes of ALP concentration in CEC group were significant(P < 0.05) but changes of Total-bilirubin concentration were not significant(P < 0.05) in both groups. 3. The recovery of GGT concentration in CEC group was faster than that in $CCl_4$ group. 4. Hematological changes other than MCHC were significant(P < 0.05) in CEC group only and changes of GSH and Met-Hb concentration were significant(P < 0.05) in $CCl_4$ group.

Analysis on Factors Affecting Intake Behavior of Yaksun of Adults in Daegu and Gyeongbuk Region (대구.경북지역 성인의 약선 섭취 행동에 영향을 미치는 요인 분석)

  • The purpose of this study was to investigate perception and awareness of yaksun and willingness to eat yaksun in the future, and to examine the factors affecting intake behavior of yaksun of adults in Daegu and Gyeongbuk region. The data were collected in April, 2011 through a self-administered questionnaire. Chi-square test, t test, and binary logistic regression were conducted by SPSS Windows V.19.0. The results were as follows. Almost sixty-six percent of the respondents answered that they knew yaksun. The perception level of yaksun for the intake respondents was higher than that of non-intake respondents. Additionally, many respondents agreed to varied menu development and convenient-food development of yaksun. Intake respondents showed higher level of agreement for the varied menu development of yaksun than non-intake respondents. On the other hand, four fifths of the respondents showed that they would be willing to eat yaksun in the future. The level of willingness to eat yaksun in the future for the intake respondents was higher than that of non-intake respondents. Additionally, the respondents who knew yaksun showed higher level of willingness to eat it than those who did not know it. Finally, the result of binary logistic regression analysis for intake behavior of yaksun showed that gender, educational level, and the awareness of yaksun were significant. On the basis of these results, not only the promotion of yaksun to consumers but also the development of convenient foods, varied menus for different age groups, and menus with cheap prices are needed to raise yaksun popularity.

A Survey on Use of Unconventional Treatments and Related Variables of Pain in Patients with Rheumatic Disease (류마티스 질환자의 비전통적 치료 이용과 통증 관련요인)

  • This study was designed to explore use of unconventional treatments and identify related variables of patients with rheumatic disease. One hundred fifty rheumatic patients were recruited from two university based rheumatic centers according to selection criteria. Collected data were analyzed using SAS program through with a structured questionnaire. T-test, ANOVA, and Scheffe's test were adopted. The results were as follows: 1. 77.7% of the subjects were over forties and mean age was 48.5 years. 74.0% were women and 73.2% were living with their spouse. 82.0% of them was good or moderate or good economic status, 38.3% was university graduates. 2. 58.0% of the subjects had rheumatoid arthritis, and mean duration of suffering rheumatic disease was 6.5 years. 89.3% of them had joint pain and their mean pain score was 4.07. Most painful joints were hip(58.0%), finger(42.0%), wrist(42.0%), and the number of painful joint was 5.4. 70% of the subjects using unconventional treatments, the most common treatments using them was herb medicine(42.7%) and acupuncture(36.7%). 3. Level of pain was different according to subject's age, educational background, diagnosis, number of painful joints, and using unconventional treatments or not. It is recommended that use of a larger sample to understand more about unconventional treatments and pain of patients with rheumatic disease. More research is needed to consider developing individual nursing interventions for their well-being and quality of life.

Antioxidant Activity and Physicochemical Characteristics of Tangerine Peel Tea Prepared with Citrus unshiu Cultivated in Cheju (제주산 진피를 이용한 단일 침출차의 가공 특성 및 항산화성)

  • To investigate the physiological activities and development of tangerine peel tea, we examined the antioxidant effect of tangerine peel tea and determined the physicochernical characteristics including proximate composition, mineral contents, color values and sensory evaluation. The wet weight composition of tangerine peel tea was $61.6\pm0.3mg$ of crude carbohydrate, $15.0\pm20mg$ of crude lipid, $8.3\pm2.1mg$ of crude Protein and $7.6\pm0.1mg$ of crude ash. The pH of tnagerine peel tea decreased with increasing preparation temperature. The Hunter color values (L and a values) generally increased with increasing preparation temperature. Total phenolic contents of tangerine peel tea were $16.0 mg\%\;at\;60^{\circ}\;18.9mg\%\;at\;80^{\circ}C\;and\;20.9m\%\;at\;100^{\circ}C$. As the preparation temperature of tangerine peel tea increased, the radical scavenging activity (DPPH) also increased obviously. In the sensory evaluation, the tangerine peel tea prepared with 1 g at $80^{\circ}C$ obtained the best result with high scores in overall acceptability.
