생간탕(生肝湯)의 효능(效能)에 관(關)한 임상적(臨床的) 연구(硏究) (A Clinical Study on the Effects of Saenggan-tang)

  • In order to study the effect on liver diseases, Saenggan-tang has been applied to 26 patients of chronic hepatitis, 9 patients of liver cirrhosis, 5 patients of acute hepatitis, 8 patients of alcoholic liver diseases, 1 patient of hepatoma total 49 patients visiting the first internal medicine department of the hospital of Oriental Medicine, Kyung Hee University. The Saenggan-tang was taken every 4 weeks (ecept acute hepatitis every 2 weeks). 1. Saenggan-tang had decrease on activity of SGOTF SGPT, with the passage of time, 4, 8, and 12 weeks, showing statistically significant effect. 2. Saenggan-tang revealed sharp decrease on levels of total bilirubin and alkaline phosphatase no showing significance in relation to a few cases 3. Serum protein total and albumin levels were in normal limit before or after Saenggan-tang treatment 4. Triglyceride level was lowered remarkably after Saenggan-tang treatment, showing significance in alcoholic liver disease group whereas no significance in chornic hepatitis Judging from above results, it is proved that Saenggan-tang has curative effect of liver diseases such as acute, chronic hepatitis, liver cirrhosis, alcobolic liver diseases and so on.

뇌경색 한방치료중 독성간염(약물유인성 간염)으로 추정되는 간기능 손상의 1례 (One Case of The Hepatic Injury Suggested Toxic Hepatitis(Drug Induced Hepatatis) in the Treatment of Oriental Medicine for Cerebral Infarction)

  • Although there are a few reports concerning the hepatic injury of herbal medicine in western medicine, there is no reports concerning the hepatic injury of herbal medicine in oriental medicine. We experienced one case of the hepatic injury suggested toxic hepatitis (drug induced hepatitis) in the treatment of oriental medicine for cerebral infarction. We make a reports for the progress of treatment. One patient of Rt hemiparesis diagnosed cerebral infarction in Brain MRI was administered Taeumin Yuldahanso-tang, treated with physical therapy and improved in the state of Rt hemiparesis(Gr 2/3,4). In the course of treatment, the patient intermittently complained of general weakness, dizziness, pericephalic discomfort(頭不淸), nausea, both leg weakness so we changed Taeumin Yuldahanso-tang to Soyangin Dokwhaljiwhang-tang. Alter that, the patient complained of general weakness, abdominal discomfort, heat and fever($38.4^{\circ}C$). We recognized the elevated total bilirubin(1.7mg/dl), serum transaminase(AST534U/L ALT720U/L), serum gammg glutamyl transpepridase (GGT106mg/dl) and Alk phosphatase124U/L. In the abdominal ultrasonogram there is hepatomegaly (16.5cm). We supposed hepatic injury suggested toxic hepatitis(induced hepatitis) of herbal medicine. After we administered Saeng gan gunbi-tang for 2days and Alk phosphatase and GGT inadequately was elevated. After that, we stopped administering medicine and the serum transaminase, total bilirubin, serum gamma glutamyl transpeptidase and Alk phosphatase level was decreased. In the abdominal ultrasonogram there was no signal. In the case, we supposed that the changing Sasang constitution and Sasang herbal medicine may induce hepatic injury. We are suggesting the necessity of a test for the hepatic injury of herbal medicine.

Hep G2 세포에서 간염제1탕의 에탄올에 의한 세포독성 억제효과 (The Effect of Hepatitis Treatment-Tang No.1 on Ethanol-Induced Cytotoxicity of Hep G2 Cells)

  • Object : Hepatitis Treatment-tang No.1 has been used for the treatment of Liver disease and Jaundice. Long-term EtOH exposure leads to immunoregulatory and detoxification impairment. This study aimed to determine the relationship between TNF-${\alpha}$ production and expression, and EtOH-induced cytotoxicity on Hep G2 cells. Method : Cells were incubated with EtOH in the presence or absence of HT. The cells were tested after 24 hours and, again, after 48 hours. Cytoviability and TNF-${\alpha}$ release were analyzed by MTT assay and enzyme linked immunosorbent assay (ELISA), respectively. After 24 hours of EtOH exposure, the cytoviability decreased, and the release of TNF-${\alpha}$ was increased. Increased amounts of TNF-${\alpha}$ contribute to EtOH-induced cytotoxicity. The Anti-TNF-${\alpha}$ antibody almost abolished it. Interestingly, EtOH-induced cytotoxicity and TNF-${\alpha}$ production were inhibited by HT. Moreover, when HT was used in combination with the anti-TNF-${\alpha}$ antibody, there was a marked inhibition of EtOH-induced cytotoxicity. Results : These results suggest that HT may prevent the cytotoxicity through partial inhibition of the TNF-${\alpha}$ secretion.

급성 B형간염 환자에 대한 치험 1례(例) (A Case Report of Acute Hepatitis B)

  • A 36 year-old female patient with acute hepatitis B was treated with modified Chunggangunbi-tang. The patient complained of right arm and numbness, chest pain and discomfort, pain in the right chest, fever, athalpia, indigestion, fatigue, pruritus, jaundice and other minor problems. Two weeks later, symptoms has gone from severe to mild and transaminase level was lower. The medicine was prescribed for four more weeks and symptoms disappeared. The transaminase level fell to within normal range with no side effect. The Chunggangunbi-tang showed desirable effect on indigestion and more rapid recovery of liver function than previous reports on treatment for hepatitis. Finally results from clinicopathological examinations(about AST, ALT, total bilirubin, direct bilirubin, etc.) were promising. o we hope that this clinical study is helpful in treating patients with hepatic disease. Result suggests that oriental medical therapy is useful in treating acute hepatitis type B. More study and development of approach and application of this treatment for acute hepatitis type B are necessary.

알코올성 간염 환자의 불면과 피로 개선에 대한 한양방 병용치료 치험 1례 (A Case Report of Insomnia and Fatigue in Alcoholic Hepatitis Patients Treated with Korean Medicine and Western Medicine)

  • Objectives: This study aimed to report a case that showed improvements in the symptoms of patients with alcoholic hepatitis without any indication of deterioration of the disease. Methods: Western medicine with Urusa tablets and Godex capsules and Korean medicine therapeutic approaches, including Shihogayonggolmoryo-tang, acupuncture, and moxibustion, were administered to a patient during the period of treatment. Blood tests were used to determine levels of aspartate aminotransferase (AST), alanine aminotransferase (ALT), AST/ALT ratio, alkaline phosphatase (ALP), gamma glutamyltransferase (GGT), total bilirubin, direct bilirubin, and total cholesterol. Fatigue was measured using the numeric rating scale (NRS), and the patient's total sleeping time was checked, daily. Results: After the combined treatment, the AST/ALT ratio and the AST, ALT, ALP, GGT, total cholesterol, and direct bilirubin levels were decreased. Through Oriental medicine for the purpose of improving symptoms, NRS of fatigue decreased from 10 to 5, and the amount of sleeping time increased from 2 to 5 hours. Conclusions: The herbal medicine had no effect on the hepatoprotective drugs such as Urusa tablets and Godex capsules used to treat alcoholic hepatitis, and no adverse reaction from the combined administration was observed. To reduce fatigue and insomnia in patients with alcoholic hepatitis, it might be helpful to combine Western medications with Korean medicine treatments, including acupuncture, moxibustion, and Shihogayonggolmoryo-tang.
