  • Hepatitis C virus (HCV) has become a major public health issue and is prevalent in most countries. HCV infection starts frequently without clinical symptoms, and progresses in the majority of patients (70 to 85%) to persistent viremia and chronic hepatitis including cirrhosis and hepatocellularcarcinoma (HCC) [1]. Alcohol is one of the independent cofactors accelerating the development of HCC in chronic hepatitis C patients. This is of great interest because a synergy between excessive alcohol intake and HCV infection has been documented in the development of HCC in chronic hepatitis C patients [2]. The aim of this study is to investigate liver changes in ethanol feeding HCV-transgenic (Tg) mouse and to establish an animal model system. (omitted)

한국(韓國)에 만연(蔓延)하고 있는 만성간염(慢性肝炎)의 자연병력(自然病歷) (Natural History of Chronic Hepatitis in Korea)

  • Korea is an endemic area of chronic hepatitis in the world. Liver cirrhosis and liver cell carcinoma, presumed to be related to such chronic hepatitis, are the major causes of death in this country. The purpose of this study is disclosing the sources of chronic hepatitis in Korea establishing its histologic characteristics, disclosing the patterns of progression in chronic hepatitis, delineating its prognosis and finally speculating its etiology. The study group was composed of 183 patients with biopsy-proven acute icteric viral hepaticis, 32 patients with biopsy- proven anicteric hepatitis and 260 patients with biopsy- proven chronic hepatitis. These patients submitted to long-term follow-up by means of liver needle biopsy and/or clinicolaboratory evaluation. The period of follow-up ranged from two months to 18 years. The histological features of the initial biopsy specimens of chronic hepatitis permitted a division of the cases cases into the following five types: Type I. Persisting portal hepatitis : so called persisting hepatitis 43 Type II. Chronic inactive hepatitis with incomplete strand septal fibrosis. This type has thin fibrotic septation in addition to Type I with portal sclerosis 38 Type III. Chronic active periportal hepatitis(CAPH) : so called aggressive hepatitis, characterized by marked piecemeal necrosis. This type has been subdivided further into three groups: AB and C on the basis of histologic features. A CAPH without cirrhosis 15 B CAPH with cirrhosis 99 C CAPH with diffuse acinus type parenchymal nodules; characterized by rosette-forming micronodules 21 Type IV. Subacute hepatic necrosis; characterized by multilobular and/or bridging necrosis. 14 Type V. Persisting lobular hepatitis; characterized by spotty necrosis, which looks very similar to acute viral hepatitis. Such histologic changes should be persisted for more than six months 30 In Korea the main source of chronic hepatitis is the anicteric type. Of the chronic hepatitis observed in the hospital, Type IIIb was the most frequent in its incidence and occasionally exhibited development of hepatocellular carcinoma, but the mortality was highest in Type IIIc during the period of follow-up. Histologic characteristics of these five types suggest a spectrum of chronic hepatitis in Korea from an early and mild stage to advanced and fatal cirrhosis, which is occasionally associated with primary hepatic cell carcinoma. It seems that Type IV can be followed by flare-up of various stages of acute and chronic hepatitis with HBsAg and that many cases of liver cirrhosis prevalent in Korea occur through such an active process of Type IV. The etiology is not established, but in Korea it is mainly related to HBsAg.

Hepatitis C Viral Infection in Children: Updated Review

  • Hepatitis C virus (HCV) infection is a major medical challenge affecting around 200 million people worldwide. The main site of HCV replication is the hepatocytes of the liver. HCV is a positive enveloped RNA virus from the flaviviridae family. Six major HCV genotypes are implicated in the human infection. In developed countries the children are infected mainly through vertical transmission during deliveries, while in developing countries it is still due to horizontal transmission from adults. Minimal nonspecific and brief symptoms are initially found in approximately 15% of children. Acute and chronic HCV infection is diagnosed through the recognition of HCV RNA. The main objective for treatment of chronic HCV is to convert detected HCV viremia to below the detection limit. Children with chronic HCV infection are usually asymptomatic and rarely develop severe liver damage. Therefore, the benefits from current therapies, pegylated-Interferon plus ribavirin, must be weighed against their adverse effects. This combined treatment offers a 50-90% chance of clearing HCV infection according to several studies and on different HCV genotype. Recent direct acting antiviral (DAA) drugs which are well established for adults have not yet been approved for children and young adults below 18 years. The most important field for the prevention of HCV infection in children would be the prevention of perinatal and parenteral transmission. There are areas of focus for new lines of research in pediatric HCV-related disease that can be addressed in the near future.

Impact of Chronic Hepatitis B and Hepatitis C on Adverse Hepatic Fibrosis in Hepatocellular Carcinoma Related to Betel Quid Chewing

  • The pathogenesis of hepatocellular carcinoma (HCC) related to habitual betel quid (BQ) chewing is unclear. Risk of HCCis increased with adverse hepatic fibrosis. This study aimed to assess the impact of chronic viral hepatitis on adverse hepatic fibrosis in HCC related to BQ chewing. This hospital-based case-control study enrolled 200 pairs of age- and gender-matched patients with HCC and unrelated healthy controls. Serologic hepatitis B surface antigen (HBsAg), antibodies to hepatitis C virus (anti-HCV), ${\alpha}$-fetoprotein (AFP), and surrogate markers for significant hepatic fibrosis were measured. Information on substance-use habits was obtained with a questionnaire. By analysis of surrogate markers for hepatic fibrosis, the prevalence of significant hepatic fibrosis in patients chewing BQ was between 45.8% and 91.7%, whereas that for patients without BQ chewing was between 18.4% and 57.9%. The difference was significant (P <0.05 for each surrogate marker). Multivariate analysis indicated that cirrhosis with Child-Pugh C (odds ratio (OR) = 3.28; 95% confidence interval (CI), 1.29-8.37), thrombocytopenia (OR = 3.92, 95% CI, 1.77-8.68), AFP >400 mg/L (OR = 2.21, 95% CI, 1.05-4.66) and male gender (OR = 4.06, 95% CI, 1.29-12.77) were independent factors associated with habitual BQ chewing. In conclusion, adverse hepatic fibrosis and severe liver damage play important roles in the pathogenesis of BQ-related HCC, which could be aggravated by chronic hepatitis B and hepatitis C. BQ-cessation programs and prevention of chronic HBV/HCV infection are needed to prevent HCC related to BQ chewing.

RIA, Recombinant Immunoblot Assay 및 Nested RT-PCR에 의한 C형 간염바이러스 검출 (Detection of Hepatitis C Virus by RIA, Recombinant Immunoblot Assay and Nested RT-PCR)

  • Hepatitis C virus (HCV) is one of the important human pathogen that can cause acute and chronic hepatitis, liver cirrhosis and hepatocellular carcinoma. Recently, the third generation radiation immuno assay (RIA) method has been developed as a very sensitive test to detect anti-HCV antibody. However, false positive is the problem with RIA test. To solve this the RIA results were compared to those of 5-antigen recombinant immunoblot assay (5-RIBA) and reverse transcription-polymerase chain reaction (RT-PCR). Among 12,767 serum samples tested from clinic visitors, total 275 (2.2%) samples were antibody positive by RIA. RIBA was performed with 148 RIA positives cases but among them was shown eighty five was antibody positive and sixty three (42.6%) was negative result. However, nested RT-PCR test was shown also carried out with 43 positive, 6 intermediates and 25 negatives of RIBA. As a result of the nested RT-PCR results, HCV antigen were detected in RIBA positive, 33.3% (2/6) RIBA intermediate and 12% (3/25). Clinical syndrome of all 148 patients as a with chronic active hepatitis (46.0%), cirrhosis (18.9%), hepatocellular carcinoma (8.1%) and others (27.0%) and they were positive in reaction by RIA test. But RIBA positive patients with 34.9% of chronic active hepatitis, 18.6% of cirrhosis, 4.6% of hepatocellular carcinoma and 41.9% of others were detected to be positive case by nested RT-PCR.

Serotyping in Patients with Chronic Hepatitis C

  • To determine the clinical usefulness of Immuno Blot test, 160 samples from the patients with chronic HCV infection were analyzed. And serotyping and line probe assay were performed to evaluate the distribution of hepatitis C virus genotypes in Korean isolates. In this group, as a result of genotyping type 1 band 2a, the serotype I and II were the most common source of HCV infection. There were no significant difference in response to the alpha-interferon HCV infection treatment with the subtype 1 b or 2a. And the serotypes of NS4 peptides were compared with the genotypes to evaluate their clinical usefulness. Among 49 cases studied for genotypes and serotype, genotype 1 b, 1 b/2b, 2a, 2a/2c and 2b were 51.0%, 2.0%, 34.6%, 8.1% and 4.0%, respectively. The serotypes I and II were 57.1% and 42.8%, respectively; they were matched with genotypes in 85.7% and seemed to be easy to perform. To monitor their performing progress or treatment response, serotype test was made before the genotype test. The Result showed that there was no significant difference in response to the alpha-interferon HCV infection treatment with the subtype 1 b or 2a in Korea.

만성 B형 간염 환자에서 인터페론 치료 후 녹내장 발생 여부의 전향적 연구 (Prospective Study on the Development of Glaucoma after Interferon alfa Therapy for Chronic Hepatitis B)

  • 목 적: 만성 C형 간염 환자에서 인터페론 치료후 녹내장이 발생하였다는 보고가 전세계적으로 1례 있어왔다. 최근 저자들도 15세된 만성 B형 간염 환자에게 인터페론 투여 후 녹내장이 생긴 예를 경험한 바 있어 만성 B형 간염 환자에서 인터페론 치료와 녹내장 발생과의 연관성을 알아보고자 본 연구를 전향적으로 시행하였다. 방 법: 1998년 2월부터 1999년 7월까지 인하대학교 병원에서 만성 B형 간염으로 진단 받고 인터페론 알파를 투여받은 9명의 환아(남아 4명, 여아 5명)을 대상으로 전향적으로 연구하였다. 인터페론 투여 전, 투여 3개월 후, 6개월 후 안과적 검사를 통하여 시력검사, 안압, C/D비, 시야검사를 측정 비교하였다. 결 과: 1) 대상환아들의 평균 연령은 $11.7{\pm}4.1$세, 투여기간은 6개월이었고 평균 투여량은 5백만 단위였다. 2) 각 환아들의 인터페론 투여 전 시력, 안압, C/D비를 투여 3개월 후, 6개월 후에 비교하였을 때 유의한 변화는 없었다. 3) 시야검사에서는 시야결손은 한 명도 발생하지 않았다. 결 론: 만성 B형 간염 환아에게서 인터페론 치료 후 녹내장이 발생하여, 이후 9명의 만성 B형 간염 환아를 대상으로 인터페론 치료 후 안압증가 여부 등을 전향적으로 조사하였지만 유의한 차이는 없었다. 하지만 인터페론 투여 후 녹내장 및 안과질환 발생 가능성에 대해 지속적인 관심이 필요할 것으로 보인다.

만성 C형 간염에 대한 ${\alpha}$-interferon 치료 후 ALT치의 변화 (Effect of ${\alpha}$-Interferon Treatment on Serum ALT Levels in Patients with Chronic Hepatitis C)

  • HCV에 감염된 환자중 반이상은 만성간염 환자로 이행되며 이중 20% 정동서는 간경변증 환자가 된다고 하며 그 외 간세포암 발생과의 관련성도 높다고 한다. 혈청으로 인한 각종 감염경로를 차단하는 것이 현재로서는 가장 확실한 C형 간염의 관리방법이긴 하나 약 반수정도의 환자는 산발형의 감염으로 뚜렷한 원인을 찾지 못하고 있다. 따라서 만성 C형 간염의 치료는 주요한 과제의 하나이며 최근 interferon 제제로 치료하여 ALT치의 정상화, 혈청 HCV RNA의 소실 및 간조직검사상의 상태개선이 가능하다는 국외의 희망적 보고들이 계속되고 있음은 다행한 일이며 이 보고내용에는 간편한 ALT치 검사가 치료경과 관찰 및 판정의 지표로 사용될 수 있음을 시사하고 있다. 저자는 1991년 5월부터 1992년 9월 사이에 영남대학교 의과대학 부속병원 내과에 내원하여 혈청 anti-HCV가 양성이며 임상적으로 최기간경변증 및 간조직검사상 만성간염 진단된 환자 16명에 대해 ${\alpha}$-interferon 300만 단위를 주 3회, 1-9(평균 4.0)개월동안 피하주사하고 그후 1-18(평균 7.0)개월후까지의 ALT치를 조사하였다. 치료중 대부분의 환자에서 ALT치의 감소가 있었으나 2명에서는 치료중 다시 상승되었다. 치료중단 후 6개월 이후에도 계속적으로 정상 ALT치를 보인 경우에는 9명중 4명(44.4%)였다. 대부분 환자에서 ALT치의 정상화는 치료시작후 1개월 전후에 관찰되었으며 치료중 ALT치의 파동이 있거나 늦게 정상화된 환자에서는 치료중단 후에도 ALT치의 파동이 있거나 정상 ALT치가 유지되지 않았다. 따라서 만성 C형간염 환자에서 ${\alpha}$-interferon의 치료는 간기능 개선에 도움이 되나 더 나은 성적을 위해서는 투여양과 기간을 치료에 따른 환자의 검사결과에 따라 변화시켜 사용하는 것이 도움이 될 것으로 여겨지고 이 치료에 전혀 반응이 없는 환자들에 대한 연구도 병행되어야 할 것으로 생각되며 또한 우리나라에서도 치료 후 장기간의 경과관찰, HCV RNA 검사 및 HCV genotyne에 관한 연구로서 만성 C형 간염에 대한 ${\alpha}$-interferon 치료효과를 더 정확히 평가해야 할 것으로 사료된다. 또한 최근에는 만성 C형간염의 ${\alpha}$-interferon 치료 후의 장기효과에 영향을 미칠 수 있는 여러조건들 즉 나이, 간경변의 유무, 치료전 NCV-RNA양 및 HCV와 genotype 등이 관심이 대상이 되기 시작하였으므로 향후 이점들에 대해서도 관찰이 시도되어야 할 것으로 사료된다.

CYP2C19 Genotype Could be a Predictive Factor for Aggressive Manifestations of Hepatocellular Carcinoma Related with Chronic Hepatitis B Infection in Thailand

  • Background: Chronic hepatitis B virus (HBV) infection related hepatocellular carcinoma (HCC) is a major health problem in the Asia-Pacific region including Thailand. Several factors have been proposed as contributing to hepatocarcinogenesis. This study was aimed to investigate the impact of CYP2C19 genotypic polymorphism in HCC related to chronic HBV infection in Thailand. Materials and Methods: A cross-sectional study was performed between April 2014 and January 2015. Chronic HBV patients with HCC (n=50) and without HCC (n=50) were included. Clinical information and blood samples of all patients were collected. The CYP2C19 genotype was determined by polymerase chain reaction-restriction fragment length polymorphism method, and was classified as rapid metabolizer (RM), intermediate metabolizer (IM) or poor metabolizer (PM). Results: The CYP2C19 genotype frequencies of RM, IM and PM in HBV patients were found to be 19/50 (38%), 25/50 (50%) and 6/50 (12%), respectively. The CYP2C19 genotype frequencies of RM, IM and PM in HBV with HCC patients were 21/50 (42%), 25/50 (50%) and 4/50 (8%), respectively. The distribution of CYP2C19 genotype was not different between patients with and without HCC. Interestingly, among HBV with HCC patients, the RM genotype of CYP2C19 tended to increase risk of aggressive manifestation (OR=2.89, 95%CI=0.76-11.25, P-value=0.07), compared with non RM genotype carriers. Conclusions: CYP2C19 genotype IM was the most common genotype in Thai patients with chronic HBV infection. In addition, genotype RM could be an associated factor for aggressive presentation in HCC related to chronic HBV infection.

페그-인터페론과 리바비린 병합치료 중 발생한 간질성 폐렴 및 범혈구 감소증 1예 (A Case of Interstitial Pneumonitis and Pancytopenia Following the Combination Therapy of Pegylated Interferon and Ribavirin)

  • The combination therapy of pegylated interferon and ribavirin is the mainstay of treatment for chronic hepatitis C patients. Anti-viral therapy is commonly associated with side effects such as headache, fever, myalgia, and arthralgia. However, anti-viral therapy can continue because these side effects are mostly mild and can be improved with supportive management. Anti-viral therapy should be stopped promptly if serious side effects, such as interstitial pneumonitis or hemolytic anemia occur, although those serious side effects are rare. There were a few case reports of interferon-related interstitial pneumonitis worldwide. In Korea, one atypical case report of interstitial pneumonitis has been reported, which followed the combination therapy of interferon-alpha and ribavirin in a patient with chronic hepatitis C. We present a case of interstitial pneumonitis and pancytopenia following the combination therapy of pegylated interferon and ribavirin in a patient with chronic hepatitis C.
