Effectiveness of inactivated hantavirus vaccine on the disease severity of hemorrhagic fever with renal syndrome

  • Background: An inactivated Hantaan virus vaccine (iHV) has been broadly used as a preventive strategy for hemorrhagic fever with renal syndrome (HFRS) by the South Korean Army. After the vaccination program was initiated, the overall incidence of HFRS cases was reduced in the military population. While there are about 400 HFRS cases annually, few studies have demonstrated the efficacy of the iHV in field settings. Therefore, this study aimed to evaluate the iHV efficacy on HFRS severity. Methods: From 2009 to 2017, HFRS cases were collected in South Korean Army hospitals along with patients' vaccination history. HFRS patients were classified retrospectively into two groups according to vaccination records: no history of iHV vaccination and valid vaccination. Vaccine efficacy on the severity of acute kidney injury (AKI) stage and dialysis events were investigated. Results: The effects of the iHV on renal injury severity in between 18 valid vaccinated and 110 non-vaccinated patients were respectively evaluated. In the valid vaccination group, six of the 18 HFRS patients (33.3%) had stage 3 AKI, compared to 60 of the 110 (54.5%) patients in the non-vaccination group. The iHV efficacy against disease progression ($VE_p$) was 58.1% (95% confidence interval, 31.3% to 88.0%). Conclusion: The iHV efficacy against the progression of HFRS failed to demonstrate statistically significant protection. However, different severity profiles were observed between the iHV and non-vaccination groups. Additional studies with larger populations are needed to demonstrate the effectiveness of the iHV in patients with HFRS.

Recent increase of human granulocytic anaplasmosis and co-infection with scrub typhus or korean hemorrhagic fever with renal syndrome in Korea

  • We report 17 patients with human granulocytic anaplasmosis between January 2015 and September 2018 at two tertiary university hospitals in Korea. Monthly incidence peaked in May and June. Among these patients, we identified three who were co-infected with scrub typhus, and one patient with hemorrhagic fever with renal syndrome.

Introduction of Vaccinomics to Develop Personalized Vaccines in Light of Changes in the Usage of Hantaan Virus Vaccine (Hantavax®) in Korea

  • The Ministry of Food and Drug Safety of Korea made an official announcement in March 2018 that the total number of inoculations of Hantaan virus vaccine ($Hantavax^{(R)}$) would change from 3 to 4. Some aspects of this decision remain controversial. Based on the characteristics of Hantaan virus (HTNV) and its role in the pathogenesis of hemorrhagic fever with renal syndrome, it might be difficult to develop an effective and safe HTNV vaccine through the isolate-inactivate-inject paradigm. With the development of high-through-put 'omics' technologies in the 21st century, vaccinomics has been introduced. While the goal of vaccinomics is to develop equations to describe and predict the immune response, it could also serve as a tool for developing new vaccine candidates and individualized approaches to vaccinology. Thus, the possibility of applying the innovative field of vaccinomics to develop a more effective and safer HTNV vaccine should be considered.

가을철 유행하는 급성열성질환의 혼합감염 양상 (The Behavior of Acute Febrile Illness with Incidence the Fall Mixed Infection)

  • 본 연구는 가을철 급성열성질환인 쯔쯔가무시병의 단독감염과 신증후군 출혈열과의 혼합감염 시 임상증상 및 혈액검사의 양상을 보고자 전라남도 일개종합병원에서 최근 5년간 쯔쯔가무시병 혈청학적 확진검사에서 항체역가가 1:80이상인 양성 환자 221명을 분석하였다. 쯔쯔가무시병 단독감염 환자는 183명이며, 신증후군 출혈열과의 혼합감염 환자는 38명이었다. 연령별 환자발생은 70세 이상에서 가장 많이 발생하였으며, 성별 분포는 남성이 70명(31.7%), 여성이 151명(66.8%)으로 여성에서 다소 높았다. 발생 시기는 10월과 11월에 전체 발생율의 70% 이상을 차지하였다. 대상자의 임상증상은 발열(p=0.028)과, 가려움증(p=0.000), 근육통(p=0.000)에서 단독감염보다 혼합감염 시 유의하게 높았으며, 혈액검사 상 AST(p=0.000), ALT (p=0.000), 혈액요소질소(p=0.002), 총 빌리루빈(p=0.000)수치가 단독감염보다 혼합감염 시 유의하게 높았다.

한탄바이러스 감염증 환자에 대한 갈근해기탕을 이용한 치험례 (A Case of Hantaan virus Inflammatory Symptom Treated by Galgunhegi-tang)

  • Hantaviruses are found worldwide and are known to cause two serious and often fatal human disease: hemorrhagic fever with renal syndrome(HFRS) and hantavirus pulmonary syndrome(HPS). The typical clinical prodrome consists of fever, chills, myalgia, headache, and gastrointestinal symptoms. Treatment usually involves maintenance of fluids, blood pressure, ventilation and electrolytes. We report a patient who had multisystem inflammatory symptom with Hantaan virus antibody positive accompanied by mild fever and myalgia. This case was diagnosed as HFRS. This patient was treated by Galgun hegi-tang. As a result of this treatment, symptoms were markedly improved.

  • Hemorrhagic fever with renal syndrome (HFRS), scrub typhus, murine typhus and leptospirosis have been the principal acute febrile diseases in Korea. To evaluate the seroepidemiologic patterns of acute febrile illness, sera collected from 2,423 patients in 1996 were examined for antibodies against Hantaan virus, Orientia tsutsugamushi, Rickettsia typhi, and Borrelia burgdorferi by indirect immunofluorescent antibody technique (IFA) and macroscopic agglutination test for Leptospira interogans. Seropositive cases against Otsutsugamushi, Rickettsia typhi, Leptospira interogans and Hantaan virus were 192 (7.9%), 193 (8.0%), 12 (0.5%) and 324 (13.4%), respectively. Male was more affected in HFRS and murine typhus contrasting to scrub typhus and leptospirosis in female. Most positive cases occurred during October and November for scrub typhus, and during November and December for HFRS. These results showed similar patterns with previous epidemical data for recent couple of years, and possibly implied no significant changes occurred in ecologic situations for acute febrile diseases in Korea.

1991~1993년 전남지방에서 발생한 렙토스피라증, 쯔쯔가무시병, 신증후출혈열에 관한 연구 - 보건기관에 보고된 자료를 중심으로 - (A Study on Leptospirosis, Tsutsugamushi Disease and Hemorrhagic Fever with Renal Syndrome in Chonnam in 1991~1993 - Based on notified cases to the Public Health Center -)

  • This study was performed on 302 cases of leptospirosis, tsutsugamushi disease and hemorrhagic fever with renal syndrome(HFRS) which occurred in Chonnam(Do) in 1991 through 1993. The results were as follows : 1. 81.8% of the patients with Leptospirosis and 54.5% of the HFRS patients were men while 61.4% of the patients with tsutsugamushi disease were women. 2. Most patients lived in rural areas(Gun), their educational level was elementary School or lower and their occupations were either farmer or jobless. 3. Peak ages were 40s for leptospirosis(36.4%) and 50s for tsutsugamushi disease and HFRS(32.9% and 36.4% respectively). 4. The high incidence areas of tsutsugamushi disease were northern, eastern and a diagonal line, from northeast to southwest, of the Chonnam area, and these are consistent with a mountainous district. 5. In monthly distribution leptospirosis was higher in Sep. and Oct., and tsutsugamushi disease and HFRS were higher in Oct. and Nov. 6. The first case is occurring and the last case shows up later than in past years.

Seroepizootiology of Hantavirus Infection in Indigenous Rodents in Korea, During 1995-2000

  • To better understand the reservoir host range and distribution of hantaviruses in small mammal populations in Korea, a serological survey was conducted on 1,375 wild rodents and 62 insectivores captured in seven provinces during the six-year period, 1995 to 2000. As determined by the indirect immunofluorescent antibody (IFA) test, 90 ($13.1\%$) of 685 Apodemus agrarius, 47 ($13.6\%$) of 345 Apodemus peninsulae, and 4 ($6.5\%$) of 62 Crocidura laciura were seropositive against the Hantaan virus, while 38 ($13.5\%$) of 282 Eothenomys regulus were seropositive against the Puumala virus. Serological evidence for hantavirus infection was not found in 50 Microtus fortis, six Micromys minutus, six Mus musculus, and one Cricetulus triton. Our serological data indicate that hemorrhagic fever with renal syndrome (HFRS)-related hantaviruses are widely distributed in indigenous rodents in Korea. Particularly noteworthy was the high seropositivity rates among Apodemus peninsulae and Eothenomys regulus captured in certain mountainous regions, suggesting that HFRS may be under-reported among nearby residents or among individuals who might visit such areas for recreational or occupational purposes.
