혈액투석환자의 수면장애에 영향을 미치는 요인 (Study on the Factors Affecting the Sleep Disorder of Hemodialysis Patients)

  • Purpose: The purpose of this study is to investigate the degree of sleep disorder and its related factors in hemodialysis patients. Method: The subjects of this study consisted of 210 hemodialysis patients. Data were analyzed by descriptive statistics, t-test, ANOVA, pearson's correlation coefficient, or stepwise multiple regression analysis by using SPSS/PC+ program. Result: Sleep disorder degree averaged 2.24. Stress of hemodialysis patients averaged 2.52. Depression degree of hemodialysis patients averaged 1.14. There was significant difference in sleep disorder according to age, marriage status, education, family income and help-givers. Sleep disorder was significantly associated with stress and depression. Stress, depression and marriage status explained 31.3% of the variance in the sleep disorder. Conclusion: In conclusion, the sleep disorder of hemodialysis patients is the nursing problem. The sleep disorder degree relates with stress, depression. Especially, it is strongly affected by stress among them.

혈액투석시 EMLA크림 처치와 Lidocaine 피내주사에 따른 통증정도의 비교 (A Comparison of Pain Reducing Effects of Topical EMLA Cream and Subcutaneous Lidocaine in Hemodialysis Patients)

  • This study was conducted to compare the severity of cannulation pain in hemodialysis patients after topical application of EMLA cream and local injection of lidocaine and evaluated side effects and problems accompanied by the former. Twenty patients, who were on hemodialysis from September 1 to October 15, 1994 at the Kangnam St. Mary's Hospital, Catholic University Medical College, were divided into two groups of ten. To conduct a cross over study, two groups were placed on four repeated methods with lidocaine followed by four repeated methods with EMLA cream and vice versa, respectively, while the severity of cannulation pain was being measured according to a Visual Analogue Scale with each methods. The results are follows : 1) The scale of pain was recorded as $4.56{\pm}1.38$ and $2.05{\pm}1.36$ points for methods with lidocaine and EMLA cream, respectively, indicating the less severe pain with EMLA cream. 2) Local side effects such as itching(4 cases, 5.0%)and pallor (5 cases, 6.3%)were observed with methods with EMLA cream but disappeared before the completion of hemodialysis. 3) Problems associated with local lidocaine were pain at the injection of anesthetic (27cases, 16.9%)and fear for needle insertion(6 cases, 3.8%). The most frequent problems with EMLA cream application were an inconvenience in use (11 cases, 6.9%)and tedious long pretreatment time(11 cases, 6.9%), those associated with inconvenience in cream applying procedures. 4) Twelve out of twenty patients(60.0%) responded with yes to a continued use of EMLA cream in spite of problems with cream application and economical difficulties in purchasing. These results indicate that 5% EMLA cream used as a local anesthetic in hemodialysis significantly reduces cannulation pain and lacks side effects, thus serving as a suitable method for the alleviation of cannulation pain and inconvenience in hemodialysis and the relief of psychological stress of nurses.

  • Purpose : This study wad aimed to estimate nursing cost for hemodialysis of secondary hospitals using Resource Based Relative Value Scale(RVRBS). Method : The first, calculated nursing workload for the hemodialysis of secondary hospitals. Second, measured each spent time according to 14 nursing behaviors. Third, computed Resource Based Relative Value(RBRV) scores and nursing expenses of hemodialysis nurse. Finally, estimated nursing cost for hemodialysis of secondary hospitals, Result & Conclusion : The mean RBRV scores for each nursing behaviors were from 218.9 to 383.9 point(mean=312.7). The high RBRV socres were 383.9(Complication during hemodialysis), 353.7(Access patient vascular), 345.7 (Patient Teaching). Nursing cost for hemodialysis was estimated 23,234(won).

  • Purposes: Quality of life is an important health outcome for hemodialysis patients. The purposes of this study were to identify the level of quality of life and to identify the predictors of quality of life among hemodialysis patients. Method: A descriptive correlational study was conducted. Data were collected from 103 hemodialysis patients at the hospitals in a community using structured questionnaire and medical record. Data were analyzed using descriptive statistics, t-test, ANOVA, correlation, and stepwise multiple regression. Results: Quality of life among hemodialysis patients was relatively lower than that of previous studies. In the final analysis, quality of life was predicted by presence of comorbidity, emotional health, gender, physical health, and knowledge of disease. These variables accounted for 45% of variance of the quality of life. The presence of comorbidity was the most significant predictor of quality of life among hemodialysis patients. Conclusion: Interventions to increase quality of life among hemodialysis patients such as health promotion program and educational program for dietary compliance are needed. These must be developed and applied.

Simulation of Cardiovascular System for an Optimal Sodium Profiling in Hemodialysis

  • The object of this study is to develop a mathematical model of the hemodialysis system including the mechanism of solute kinetics, water exchange and also cardiovascular dynamics. The cardiovascular system model used in this study simulates the short-term transient and steady-state hemodynamic responses such as hypotension and disequilibrium syndrome (which are main complications to hemodialysis patients) during hemodialysis. It consists of a 12 lumped-parameter representation of the cardiovascular circulation connected to set-point models of the arterial baroreflexes, a kinetic model (hemodialysis system model) with 3 compartmental body fluids and 2 compartmental solutes. We formulate mathematically this model in terms of an electric analog model. All resistors and most capacitors are assumed to be linear. The control mechanisms are mediated by the information detected from arterial pressoreceptors, and they work on systemic arterial resistance, heart rate, and systemic venous unstressed volume. The hemodialysis model includes the dynamics of urea, creatinine, sodium and potassium in the intracellular and extracellular pools as well as fluid balance equations for the intracellular, interstitial, and plasma volumes. Model parameters are largely based on literature values. We have presented the results on the simulations performed by changing some model parameters with respect to their basal values. In each case, the percentage changes of each compartmental pressure, heart rate (HR), total systemic resistance (TSR), ventricular compliance, zero pressure filling volume and solute concentration profiles are represented during hemodialysis.

혈액투석 환자의 수액관리 경험 (The Experience of Fluid Management in Hemodialysis Patients)

  • Purpose: The purpose of this study was to explore the experience of fluid management in hemodialysis patients by describing how they manage fluid intake and what affects fluid management. Methods: Purposive sampling yielded 11 patients who have received hemodialysis for one year or longer in one general hospital. Data were collected through in-depth interviews and analysed using Giorgi's phenomenological method. Data collection and analysis were performed concurrently. Results: The findings regarding how hemodialysis patients manage fluid intake were classified into four constituents: 'recognizing the need for fluid control', 'observing the status of fluid accumulation', 'controlling fluid intake and output', 'getting used to fluid management'. The factors that affect fluid management of hemodialysis patients were revealed as 'willpower', 'change in the mindset', 'support system', and 'emotional state'. Conclusion: The study results show that hemodialysis patients manage fluid intake through food and exercise as well as interpersonal relationships. These findings suggest that strategies in the development of nursing interventions for hemodialysis patients should be directed at assisting them in familiarization with fluid management based on an understanding of their sociocultural contexts.

장기혈액투석 환자를 위한 주기적 개별교육 프로그램 적용 효과 (The Effect of Periodical and Individualized Educational Program for Long-term Hemodialysis Patient)

  • Purpose: This study was conducted to provide an effective nursing intervention using an individualized educational program composed with knowledge, compliance, and physiologic parameters for long-term hemodialysis patients. Methods: A quasi-experimental study using a non-equivalent control group and pre- and post-test design was conducted with 40 hemodialysis patients at G university hospital in the J city from June to August, 2015. A data was analyzed using frequency, percentage, ${\chi}^2$ test, Shapiro-Wilk test, independent-samples t-test and repeated measures ANOVA using SPSS 21.0 program. Results: knowledge about hemodialysis and patient role behaviors were not significantly different between the two measures. In the biological index, there was significant difference between the groups by points in time and group in blood potassium, albumin, and Kt/v. However, there was no difference in gaining weight between hemodialysis, hemoglobin, and blood phosphorus. Conclusion: The individual training program in this study had an effect on changing some physiological indicators of long-term hemodialysis patients. Future research is warranted for developing various kinds of education program incorporating the findings of the study for the given population.

말기 신부전 환자의 근골격계 통증 발생 실태 -혈액투석 환자를 대상으로- (Prevalence of Musculoskeletal Pain in Patients With End-Stage Renal Failure Receiving Hemodialysis)

  • Purpose: The objectives of this study were to investigate the current status of the pain in and the dysfunction related to the musculoskeletal system experienced by patients with end-stage renal failure receiving hemodialysis. Methods: A questionnaire survey was given to 107 patients undergoing hemodialysis in M Hospital. The survey was conducted by using a brief pain inventory(BPI) to investigate the body parts in pain, degree of pain, difficulty in daily life, correlation with hemodialysis and the degree of satisfaction with their own health. Results: The investigation of the patients undergoing hemodialysis showed that 89(83.2%) of the 107 patients experienced pain in their musculoskeletal system. Their pain's incidence was highest in the knees(46.1%) and 68.5% of the patients reported that the most severe pain they had experienced within the past 24 hours was at a level between "moderate" and "severe." The limitation of daily life by pain is high most as 47.7% in the accomplishment of works. Conclusion: Most hemodialysis patients experience musculoskeletal related pain that affects their daily life. Hemodialysis patient management programs should include an assessment and management of such pain.

혈액투석환자의 근감소증 관련요인 (Factors Associated with Sarcopenia among Hemodialysis Patients)

  • Purpose : This descriptive survey aims to identify the prevalence of factors associated with sarcopenia among hemodialysis patients. Methods : The study subjects were 137 patients with chronic kidney failure undergoing hemodialysis in three artificial kidney centers in B and Y cities. Data were collected from August 1 to September 30, 2020, using the SARC-F (Strength, Assistant walking, Rising from a chair, Climbing stairs, Falls) questionnaire, Mini Nutritional Assessment-Short Form (MNA-SF), International Physical Activity Questionnaire (IPAQ-SF), Bioelectrical Impedance Analysis (BIA), and a grip dynamometer. The collected data were analyzed using t-tests, crossover analysis, and logistic regression using the IBM SPSS 23 program. Results : The prevalence of sarcopenia among hemodialysis patients, determined using the SARC-F questionnaire, was 16.1%. The associated factors of sarcopenia among hemodialysis patients were found to be gender (OR=6.44, p =.002), age (OR=1.07, p =.015), nutritional status (OR=10.37, p =.027), and albumin level (OR=0.10, p =.014). These findings are supported by an explanatory power of 46.3% (p =.597). Conclusion : The identified risk factors for sarcopenia in hemodialysis patients were; sex, age, nutritional status, and albumin level. The findings of this study can serve as clinical evidence for the development of an intervention program for preventing and managing sarcopenia in patients undergoing hemodialysis.

혈액투석환자 중 소양증군과 비소양증군의 특성 비교연구 (Comparative Research of Pruritus Group and Non Pruritus Group in Hemodialysis Patients)

  • Purpose: The purpose of this study was to compare characteristics of two groups (pruritus and non pruritus group), in hemodialysis patients. Method: The degree of ichting in 130 patients treated at the D Medical Center Hemodialysis Unit was examined by itching severity & sites tool, itching observation tool and itching NRS tool. The data were collected from October 1 to 12, 2007 and analyzed by student t-test and Mann-Whitney test. Result: Sixty three patients (48.3%) of hemodialysis patients had pruritus. The severity and number rating scale score of pruritus in one day before hemodialysis was higher than during and after hemodialysis. The frequency of dialysis and usage of anti-histamine drug were significantly lower in nonpruritis group than in pruritus group (F=5.209, p=.022, F=6.549, p=.010). The other general characteristics, hemodialysis-related characteristics and laboratory data were not significantly different between the two groups. The depression score was significantly higher in pruritus than in nonpruritus group (t=-2.505, p=.017). But there were no significant differences in hostility and anxiety between the two groups. Conclusion: Residual renal function and depression were associated with pruritus in hemodialysis patients. Efforts for preservation of residual renal function and emotional support may decreased severity and frequency of pruritus in hemodialysis patients.
