체중지지 트레드밀훈련이 편마비 환자의 보행과 서기균형에 미치는 영향 (Effect of Body Weight Support Treadmill Training on Gait and Standing Balance in Patients With Hemiplegia)

  • Body weight support treadmill training is a new and promising therapy in gait rehabilitation of patients with hemiplegia. The purpose of this study was to identify the effects of body weight support treadmill training on gait and standing balance in patients with hemiplegia. Eighteen patients with hemiplegia participated in the study. A 10 m-timed walk test, measurements of step length and standing balance score were administered. Intervention consisted of body weight support treadmill training five times a week for 2 weeks. The data were analyzed by paired t-test. Body weight support treadmill training scoring of standing balance, step length and 10 m-timed walk test showed a definite improvement. Body weight support treadmill training offers the advantages of task-oriented training with numerous repetitions of a supervised gait pattern. The outcomes suggest that patients with hemiplegia can improve their gait ability and standing balance through body weight support treadmill training.

트레드밀 보행훈련이 만성편마비 환자의 보행 속도와 보행 지구력에 미치는 영향 (Effect of Treadmill Training on Walking Velocity and Gait Endurance in patients with chronic hemiplegia)

  • Treadmill training is a new promising therapy in gait rehabilitation of patients with hemiplegia. The purpose of this study was to identify the effect of treadmill training on walking speed and gait endurance in patients with chronic hemiplegia. The subject of this includes twenty patients, who had suffered from chronic hemiplegia and were in the K rehabilitation center; each ten patients were randomly assigned to experimental or control group. Among twenty patients, one group of ten for experiment was treated with progressive speed increase treadmill ambulation traing besides conventional physical therapy(SITAT) while the rest ten for the controlled group was treated with conventional physical therapy(CPT) only, for 8 weeks alike. Before and after the foregoing 8 weeks training, walking velocity and gait endurance were measured to both groups. The data were analyzed by paired t-test. The results of this study are as follows; The SITAT and CPT showed the significant difference in walking velocity and gait endurance. As compared the rehabilitation of dependent varibles between the SITAT and CPT, SITAT showed the significant difference in walking velocity and gait endurance. The outcome suggest that patient with chronic hemiplegia can improve their walking velocity and gait endurance throught treadmill training.

동작관찰훈련 프로그램이 편마비환자의 보행 및 균형에 미치는 영향 (The Effects of Action-Observation Training Program on Gait and Balance of Patients with Hemiplegia)

  • Purpose : The purpose of this study is to evaluate the effects of Action-observational training program on gait and balance of patients with hemiplegia. Method : The subjects of this study are patients with hemiplegia; 10 patients who agreed with this research, were picked up. Participants were divided randomly into equal groups: experimental group that applied to action-observation training for at least 30 minutes/day for 6 weeks and control group that underwent general task-oriented training. Gait and balance were measured 10 meter walk test(10MWT), gait speed, berg balance scale(BBS) and timed up and go(TUG). The intervention were compared by measuring before and after. Result : There were significant improvements in the subscales of the gait and balance test of those who practiced with the Action-observational training program, while the control group showed no significant changes. Conclusion : Therefore, Action-observational training program is effective in improvement of to improve the gait and balance in patients with hemiplegia.

  • The purpose of this study was to investigate the effect of the involved lower limb weight bearing training on symmetrical weight supporting rate improvement and gait characteristics of patients with hemiplegia including their gait velocity, cadence, stride length, step length of the non affected side, step length of the affected side, foot angle of the non affected side, foot angle of the affected side, base of support, and so on. The subject of the study was 28 men and women patients with hemiplegia from 22 to 77 age, among patients who doctor diagnosed as hemiplegia due to stroke or traumatic brain injury, were possible to do independent gait more than 45m without others assistance, the flexion contracture of hip pint was less than $15^{\circ}$, did not have contracture for knee pint and one more than $5^{\circ}$ for ankle joint, did not have contraindication for exercise or gait did not show visual defect due to brain injury. The patients, the subject of the study, were classified into 14 patients of treatment. group applying continuous involved weight bearing with general therapeutic exercise and 14 patients of control group applying general therapeutic exercise, and then analyzed their gait before and after exercise. Temporal distance gait analysis(Boening, 1977) was used to analyze their gait, and ink foot-print was applied as one of measurement methods. However, it was statistically signifiant in the change rate(%) of gait characteristics, and treatment group's patients with hemiplegia had been highly changed in gait characteristics in comparison with control group. From the above-mentioned results, could find that continuous involved weight bearing training for patients with hemiplegia was effective to improve their gait ability and it could increase the ability in comparison with general exercise.

  • Purpose: Deficits in lower-extremity function and trunk control ability have a negative impact on individuals with hemiplegia. This case report aimed to describe the effect of trunk stability exercises using proprioceptive neuromuscular facilitation (PNF) on trunk control ability, balance, and gait in a patient with hemiplegia. Methods: A 77-year-old man with hemiplegia and trunk and lower extremity impairment participated in this four-week training intervention. Results: The patient demonstrated improvements in trunk control ability, balance, and gait performance. Outcome measures (Fugl-Meyer Assessment Lower Extremity (FMA-LE), Trunk Control Test, Berg Balance Scale, Timed Up and Go test, 10 Meter Walk test) were measured before and after the training program. Conclusion: The results of this case suggest that a trunk stability exercise using a PNF program may improve trunk control ability, balance, and gait in a patient with hemiplegia.

Effect of Action Observational Training on Gait in People with Stroke

  • Purpose: This study examined the effects of action observational training to improve the gait function for patients with stroke. Methods: The participants were divided into two groups: right hemiplegia group (n=12) and left hemiplegia group (n=12). All groups received conventional therapy for five sessions for 30 minutes, each for three weeks. Left and right hemiplegia group practiced additional action observational training for five sessions for 20 minutes each for three weeks. They participated in three weeks of action observational training coupled with immediate physical practice (intervention), followed by a final assessment. The duration of each action observation video sequence was 10 minutes, followed immediately by practice of the observed motor skill (10 minutes). The gait velocity, cadence, swing time, step length, and BOS (base of support) were examined using the GAITRite system. Results: The results of this study showed significant improvement in the gait function. The outcomes of the gait abilities from gait velocity, cadence, swing time, step length of the affected side, and BOS (base of support) were improved significantly in the right hemiplegia group (p<0.05). In the left hemiplegia group, there was no significant improvement in the gait velocity, cadence, and BOS except for the swing time and step length of the affected side. The left and right group comparisons between the groups were not significant (p<0.05). Conclusion: Action observation training improves the gait function. These results suggest that action observational training is feasible and suitable for stroke patients.

장애물 넘기 트레드밀 보행 훈련이 편마비 환자의 정적 및 동적 균형 능력에 미치는 영향 (Effects of Treadmill Gait Training with Obstacle-Crossing on Static and Dynamic Balance Ability in Patients with Post Stroke Hemiplegia)

  • PURPOSE: This study was conducted to determine the effects of treadmill gait training with obstacle-crossing on the static and dynamic balance ability of patients with post stroke hemiplegia. METHODS: Twenty-one patients with post stroke hemiplegia were divided into three groups as: treadmill gait training with obstacle-crossing (TOG, n=7), treadmill gait training without obstacle-crossing (TGG, n=7) and a control (CON, n=7). TOG and TGG performed exercise for 20 minutes, three times a week for 8 weeks. Static balance ability (stability typical, ST; weight distribution index, WDI; fourier harmony index, FHI; and fall index, FI) and dynamic balance ability (berg balance scale, BBS and timed up and go test, TUG) were measured before and after 8 -weeks in each exercise group. Statistical analyses were conducted using two-way ANOVA with repeated measures, a paired t-test, and multiple comparisons according to Tukey's HSD. RESULTS: FHI and BBS were significantly increased at TOG (p<.01) and TGG (p<.05) after 8-weeks compared to before treadmill gait training with obstacle-crossing. FHI and BBS were significantly increased at TOG compared with CON and TGG (p<.05). CONCLUSION: Treadmill gait training with obstacle-crossing was more effective than that without obstacle-crossing to improve posture control and independent daily life performance of hemiplegia patients.

Effects of Backward Walking Training with Task Orientation on the Functional Gait of Children with Spastic Hemiplegia

  • Purpose: This study examined the effects of backward walking training with task orientation on the functional walking ability of children with cerebral palsy. Methods: This study was a single-blinded, randomized controlled trial with a crossover design conducted at a single rehabilitation facility with cross-over to the other intervention arm following a two-week break. For a total of 12 children with spastic hemiplegia cerebral palsy, the forward walking training group (n=6) underwent training three times a week for three weeks, 40 minutes a day, and the backward walking training group (n=6) was also trained under the same conditions. To identify the functional walking ability, variables, such as the walking speed, stride length, and step length, were measured using a walk analyzer (OptoGait, Microgate S.r.l, Italy). Results: Both groups showed significant increases in walking speed, stride length, and step length (p<0.01). The backward walking group showed more significant improvement in the walking speed from pre- to post-test (p<0.05). The gait characteristics were similar in the two groups (stride length and step length) but the walking speed in the backward walking group showed a mean difference between the positive effects higher than the forward walking group. Conclusion: Task-oriented backward walking training, which was conducted on the ground, may be a more effective treatment approach for improving the walking functions of spastic hemiplegia children than forward walk training.

기능적 전기자극을 적용한 트레드밀 보행 훈련이 편마비 환자의 보행 속도와 보행 지구력, 에너지 소모지수에 미치는 영향 (Effect of Treadmill Training with FES on Walking Velocity, Gait Endurance, and Energy Expenditure Index of Hemiplegia Patients)

  • The purpose of this study was to investigate the effect of Treadmill Training with FES(TTF) on walking velocity, gait endurance, and energy expenditure index(EEI) of hemiplegia patients with foot drop. Two subjects with hemiplegia participated in this study. They took walking excercise 5 times per week for 8 weeks. One time excercise spent 30minutes. The theraputic effect was evaluated by how many seconds they needed to walk 10 meters, how far they could walk for 12 minutes, and how much they spent energy in walking for 12 minutes. Two cases were examined before, after 4 week, and after 8 week, walking training. The results of this study are as follows; 1) Walking velocity : Case 1 increased from 0.52m/sec before walking training to 0.83m/sec after 8 weeks. Case 2 increased from 0.58m/sec to 0.92m/sec. 2) Gait endurance : Case 1 increased from 383.23m to 625.53m. Case 2 increased from 410.19m to 693.47m. 3) EEI : For comfortable walking condition, Case 1 decreased from 0.98beats/min to 0.71beats/min, and Case 2 decreased from 0.93beats/min to 0.68beats/min. For maximum walking condition, Case 1 decreased from 0.93beats/min to 0.67beats/min, and Case 2 decreased from 0.91beats/min to 0.61beats/min. The findings suggest that hemiplegia patients can improve their walking velocity, gait endurance and energy expenditure index through TTF.

동작관찰훈련프로그램이 편마비환자의 보행 및 균형에 미치는 영향 (Effects of Action-observational training Program on Gait and Balance of Patients with Hemiplegia)

  • 본 연구는 만성뇌졸중 편마비 환자에게 적용한 동작관찰 훈련이 보행과 균형에 미치는 영향을 알아보고자 하였다. 연구에 동의한 만성 뇌졸중 편마비 환자 4명중 동작관찰훈련을 시행한 실험군 2명 일반과제지향훈련을 시행한 대조군 2명에게 총 4주간 하루 30분씩 훈련하였다. 훈련효과를 알아보기 위해 훈련전 보행은 10m검사를 시행하였고 균형은 버그균형척도로 검사하고 훈련 후 다시 재측정하였다. 이에 동작 관찰 훈련군이 일반과제지향훈련군에 비해 보행과 균형에서 유의미한 차이를 보였다. 이에 동작관찰훈련이 만성뇌졸중 편마비환자의 보행과 균형에 효과적인 프로그램임을 알 수 있다.
