Hematological and Serum Chemical Values in Pere David′s Deer and Wapiti (사불상 및 와피티 사슴의 혈액 및 혈청화학치)

  • The hematologic values and serum chemistry were examined healthy male 12 Pere David's Deer and 8 Wapiti at the Farmland Zoological Garden. In wapiti, the blood values collected from horn and jugular vein were compared. The results were as follows: 1. There were not any significant differences in hematologic values and serum chemistry of blood col. looted from horn and jugular vein in wapiti. 2. Efficacy and Tolerability of Weekly Docetaxel, Cisplatin, and 5-Fluorouracil for Locally Advanced or Metastatic Gastric Cancer Patients with ECOG Performance Scores of 1 and 2

  • Background: Docetaxel, cisplatin, 5-fluorouracil (DCF) given every three weeks is an effective, but palliative regimen and significantly toxic especially in patients who have a low performance score. Here, we aimed to evaluate the efficacy and tolerability of a weekly formulation of DCF in locally advanced and metastatic gastric cancer patients. Materials and Methods: 64 gastric cancer patients (13 locally advanced and 51 metastatic) whose ECOG (Eastern Cooperative Oncology Group) performance status (PS) was 1-2 and who were treated with at least two cycles of weekly DCF protocol as first-line treatment were included retrospectively. The weekly DCF protocol included $25mg/m^2$ docetaxel, $25mg/m^2$ cisplatin, and 24 hours infusion of $750mg/m^2$ 5-fluorouracil, repeated every week. Disease and patient characteristics, prognostic factors, treatment response, grade 3-4 toxicity related to treatment, progression free survival (PFS) and overall survival (OS) were evaluated. Results: Of the patients, 41 were male and 23 were female; the median age was 63 (29-82) years. Forty-one patients were ECOG-1 and 23 were ECOG-2. Of the total, 81.2% received at least three cycles of chemotherapy. Partial response was observed in 28.1% and stabilization in 29.7%. Overall, the disease was controlled in 57.8% whereas progression was noted in 42.2%. The median time to progression was 4 months (95%CI, 2.8-5.2 months) and median overall survival was 12 months (95%CI, 9.2-14.8 months). The evaluation of patients for grade 3-4 toxicity revealed that 10.9% had anemia, 7.8% had thrombocytopenia and 10.9% had neutropenia. Non-hematologic toxicity included renal toxicity (7.8%) and thrombosis (1.6%). Conclusions: In patients with locally advanced or metastatic gastric cancer who were not candidates for DCF administered every-3-weeks, a weekly formulation of DCF demonstrated modest activity with minimal hematologic toxicity, suggesting that weekly DCF is a reasonable treatment option for such patients.

Hematologic Features of Children with Inflammatory Bowel Disease (염증장병 환아의 혈액학적 소견)

  • Purpose: The aim of this study was to evaluate hematologic features in Korean children with inflammatory bowel disease (IBD) in order to determine appropriate management strategies. Methods: We retrospectively analyzed the complete blood count (CBC) profiles and assessed the incidence of anemia in 25 children with IBD (Crohn disease, 16; and ulcerative colitis, 9). The correlation between hemoglobin (Hb) and duration of illness and the activity index of IBD were also investigated. Results: The incidence of anemia was 76% in IBD, 88% in Crohn disease, and 56% in ulcerative colitis. Leukocytosis and thrombocytosis were only found in Crohn disease (56% and 25%, respectively). No statistically significant correlation was observed between Hb and duration of illness in Crohn disease and ulcerative colitis or between Hb and the activity index in Crohn disease and ulcerative colitis. Conclusion: The incidence of anemia found in this study was slightly higher than in previous reports. Additionally, there was no correlation between Hb and the duration of illness or the activity index. Therefore, even if the duration of illness is short, or the activity index is low, appropriate management of children with IBD should include careful evaluation for anemia.

Expression of MicroRNA-221 in Korean Patients with Multiple Myeloma (한국인의 다발성골수종 환자에서 MicroRNA-221의 발현)

  • Multiple myeloma (MM) is the leading cause of death among hematologic neoplasms. Recently, microRNA has been reported to be useful in the diagnosis of multiple myeloma. This study examined whether miR-221 could be used as a diagnostic marker for multiple myeloma. The study was performed on 20 patients with multiple myeloma without any other hematological diseases. MicroRNA extraction was performed using formalin-fixed paraffin-embedded (FFPE) tissues obtained from the bone marrow of patients with multiple myeloma. miR-15a, miR-16, miR-21, miR-181a, and miR-221 were selected as the microRNA target genes for multiple myeloma. The significance of microRNA was based on a fold change of <1.5. To quantify the fold changes, data normalized to the human gene, SNORD43, were used as the values of the patient group. Fold change values greater than 1.5 were defined as "overexpression", whereas values less than -1.5 were defined as "underexpression". Of note, 65.0% (13/20) of samples showed significant "overexpression" in the levels of miR-221 expression and plasma cells with a group of more and less than 30% in MM patients did not show any significance of plasma cell (P<0.05). The results of other studies showing a correlation between the expression of miR-221 and MM in Caucasians were confirmed. These results suggest that miR-221 may be a useful indicator for diagnosing patients with MM. In conclusion, miR-221 is useful in the diagnosis and determining the prognosis of multiple myeloma in Koreans.

Comparative evaluation of the safety and efficacy of recombinant FVIII in severe hemophilia A patients

  • Objective: This study compared the safety and efficacy of $Safacto^{(R)}$ versus $xyntha^{(R)}$ in patients with severe hemophilia A. Methods: Thirty-three male patients with severe hemophilia A were randomly divided into two groups. Seventeen patients received $Safacto^{(R)}$ and 16 patients received $Xyntha^{(R)}$ for four consecutive times. The dosage of FVIII was 40-50 IU/kg for each injection. Plasma level of FVIII activity was evaluated before every injection, 15 minutes after the injection and one month after the start of the trial. The rate of factor VIII activity, pain and joint motion were also assessed before and after the treatment. Results: Plasma level of FVIII clotting activity in $Safacto^{(R)}$ and $Xyntha^{(R)}$ were $1.96{\pm}0.5IU/dl$ and $1.63{\pm}0.5IU/dl$ and increased to $88.84{\pm}25.2IU/dl$ and $100.09{\pm}17.8IU/dl$, respectively (P<0.001). Pain score and range of motion improvement were $9.3{\pm}0.9$ and $8.7{\pm}0.1$ in $Safacto^{(R)}$ (P=0.17); and $9.4{\pm}0.8$ and $8.8{\pm}0.3$ in $Xyntha^{(R)}$ (P=0.35), respectively. No allergic or other unfavorable reactions was observed with either of the preparations. Conclusion: This study showed that $Safacto^{(R)}$ has a favorable efficacy and safety profile.

Analysis of the Impact on Community Health after Accidental Leak of Hydrofluoric Acid (일개 응급의료센터에서 분석한 불화수소산 대량 누출이 지역사회 건강에 미친 영향)

  • Purpose: The purpose of this study is to verify the influence of a massive hydrofluoric acid spill on community health through patients who claimed to have been exposed. Methods: We analyzed 2459 patients who visited our emergency department with the claim of exposure to hydrofluoric acid, and retrospective analyses were performed. We analyzed changes in numbers of visitors per day from the day of the accidental hydrofluoric acid spill, symptoms presented by the 1924 patients, and general characteristics. Comparisons of symptoms and hematologic characteristics were made between the initially set evacuation zone(1.3 km radius parameters from the spill) and the outer zone. Results: A total of 2,459 patients who claimed exposure visited our ED from 27 September 2012 to 23 October 2012, and there was a significant increase in the number of visiting patients from day 8 of the hydrofluoric acid spill. The most common complaints were a sore throat, 729(37.9%) and no specific symptom with health concern, 547 (28.4%). Statistically significant findings were pulmonary symptoms (p=0.001), nasal symptoms (p=0.001), diarrhea (p=0.023), and skin symptoms (p=0.007). In hematologic study, a statistically significant difference was observed in white blood cell count (p=0.018), creatine phosphokinase (p<0.001), erythrocyte sediment rate (p=0.013), and phosphorus (p<0.001). Conclusion: A significant increase in the number of patients was observed one week after the accidental spill of hydrofluoric acid. The most frequent symptoms were sore throat, headache, cough, and sputum. Statistically significant increase in creatine phosphokinase level and decrease in phosphorus level were noted in patients within the evacuation zone.

  • The hematologic values were examined from 74 healthy mixed breed dogs in the area of Seoul. The results obtained are summarized as follows; 1. Mean${\pm}$SD values and ranges of red blood cell(RBC) count were $6.16{\pm}0.92{\times}10^6/{\mu}l$ and $4.40{\sim}8.62{\times}10^6/{\mu}l$, of hemoglobin(Hb) content $14.90{\pm}2.42g/100ml$ and 8.7~19.2g/100ml, of packed cell volume(PCV) $45.47{\pm}6.16ml/100ml$ and 30~57ml/100ml, of mean corpuscular volume (MCV) $74.80{\pm}6.83fl$ and 54.08~90.90fl, of mean corpuscular hemoglobin (MCH) $24.41{\pm}3.91pg$ and 14.19~32.97pg, of mean corpuscular hemoglobin concentration (MCHC) $32.07{\pm}3.24g/100ml$ and 22.23~39.76g/100ml, respectively. 2. The RBC count value in the age group of 3~4 years was higher (p<0.05) than the total RBC count value. The Hb content value in the age group of less than 6 months was lower (p<0.01) but in the age group of 1~2 years and the age group of 3~4 years were higher (p<0.05, p<0.01) than the total Hb content value. The PCV value in the age group of less than 6 months was lower (p<0.05) than the total PCV value. The MCHC values in the age group of 1~2 years and the age group of 3~4 years were higher (p<0.01, p<0.05) than the total MCHC value. 3. Mean${\pm}$SD values and ranges of white blood cell (WBC) count were $11.26{\pm}3.05{\times}10^3/{\mu}l$ and $6.30{\sim}18.4{\times}10^3/{\mu}l$, of band neutrophil $2.97{\pm}1.44%$ and 1~10%, of segmented neutrophil $62.81{\pm}4.92%$ and 42~70, of lymphocyte $30.55{\pm}5.69%$ and 17~50%, of monocyte $2.49{\pm}0.84%$ and 1~5%, of eosinophil $1.81{\pm}1.175$ and 1~8%, respectively. 4. The WBC count value in the age group of 7~12 months was lower (p<0.05) but in the age group of 3~4 years was higher (p<0.05) than the total WBC count value. The band neutrophil values in the age group of 1~2 years and the age group of 3~4 years were higher (p<0.05, p<0.01) than the total band neutrophil value.

Novel influenza A (H1N1) 2009 infection in the pediatric patients with hematologic and oncologic diseases in the Yeungnam region in Korea

  • Purpose: Natural history and consequences of the novel 2009 influenza A H1N1(2009 H1N1) infection in immunocompromised pediatric patients are not yet fully understood. In this study, we investigated the clinical features and outcomes of the 2009 H1N1 infection in pediatric patients with hematological and oncological diseases. Methods: We retrospectively reviewed the medical records of 528 patients who had hematological and oncological diseases and who were treated at 7 referral centers located in the Yeungnam region. Among the 528 patients, 27 with definite diagnosis of 2009 H1N1 infection were the subjects of this study. All patients were divided into the following 3 groups: patients who were receiving chemotherapy (group 1), patients who were immunosuppressed due to a nonmalignant hematological disease (group 2), and patients who were off chemotherapy and had undergone their last chemotherapy course within 2 years from the influenza A pandemic (group 3). Results: All 28 episodes of 2009 H1N1 infection were treated with the antiviral agent oseltamivir ($Tamiflu^{(R)}$), and 20 episodes were treated after hospitalization. Group 1 patients had higher frequencies of lower respiratory tract infection and longer durations of fever and hospitalization as compared to those in group 2. Ultimately, all episodes resolved completely with no complications. Conclusion: These results suggest that early antiviral therapy did not influence the morbidity or mortality of pediatric patients with hematological and oncological diseases in the Yeungnam region of Korea after the 2009 H1N1 infection. However, no definite conclusions can be drawn because of the small sample size.

Survival, Hematologic and Histological Changes of File Fish Thamnaconus modestus Adult Exposed to Different Lower Temperature (저수온에 노출된 말쥐치 Thamnaconus modestus의 생존율, 혈액학적 및 조직학적 반응)

  • Temperature is one of the most important criteria considered in species preference for aquaculture. Acute drop in temperature during winter is a cause of disease and mass mortality in farmed fish. This study was carried out the low water temperature tolerance, oxygen consumption, hematologic and histological responses to use as basic data for the management of fish farming which frequently cause death due to winter water temperature drop. Low-lethal water temperature for 4 days of file fish Thamnaconus modestus (4day-LT50) was 6.97℃ (6.69~7.27℃). Oxygen consumption rate decreased with decreasing water temperature, showing a significant difference between water temperatures. SOD activity increased significantly at 6℃ experimental group than control group (10℃) (p<0.05), but CAT did not show any significant difference between experimental temperatures (p>0.05). Cortisol increased with decreasing experimental water temperature compared to control group. Histological changes in the liver include decreased blood vessels in the blood vessels, proliferation of acid cells, condensation of the nucleus, atrophy of pancreatic exocrine gland cells, and enzyme source granules.

The effects according to the timing of thoracic radiotherapy in limited stage small cell lung cancer (제한병기 소세포폐암에서 흉부 방사선 치료의 도입 시기에 따른 치료 효과의 비교 분석)

  • Background: Combination chemotherapy is now considered to be the cornerstone of small cell lung cancer (SCLC). management but the optimal management of limited SCLC is not well defined. The role of thoracic radiotherapy (TRT) is less well established. Recent meta-analyses reports revealed that TRT combined with chemotherapy produce "good" local control and prolonged survival. But other reports that survival was not changed. The liming, dose, volume and fractionation for TRT with the combined chemotherapy of SCLC remains unsettled. In this study, we analyzed the effects according to the timing of thoracic radiotherapy in limited SCLC. Method: All fifty one patients received cytoxan, adriamycin and vincristine(CAV) alternating with etoposide and cisplatin(VPP) every 3 weeks for 6 cycles were randomized prospectively into two groups: concurrent and sequential. 27 patients received 4500cGy in 30 fractions(twice daily 150cGy fractional dose) over 3 weeks 10 the primary site concurrent with the first cycle of VPP(concurrent gorup). 24 patients received 4000 to 5000cGy over 5 or 6 weeks after completion of sixth cycles of chemotherapy(sequential group). Results: 1. Response rates and response duration : Response rates were not significantly different between two groups(p=0.13). But response duration was superior in the concurrent group(p=0.03). 2. Survival duration was nor different between two groups(p=0.33). 3. Local control rate was superior in the concurrent group(p=0.00). 4. Side effects and toxicities: Hematologic toxicities, especially leukopenia, infection and frequency of radiation esophagitis were higher in the concurrent group (p=0.00, 0.03, 0.03). Conclusion: The concurrent use of TRT with chemotherapy failed to improve the survival of limited stage SCLC patients compared with the sequential use of TRT but response duration and local control rate were superior in the concurrent group. Frequency of radiation esophagitis, life threatening hematologic toxicities and infection were more frequent in the concurrent group than sequential group. So, the selection of an optimal schedule of chemotherapy combined with TRT that would lead to a major increase in survival with minimal toxicity is remained to be validated in large scale study in the future.
