One of the most significant feature of diagnostic ultrasonic instrument is to display information on the soft tissues in the body in real time. In this paper we carried out basic study on the digital moving image processing for tracing the movement of heart valves in echocardiograms. Digital moving image file was made from analog echocardiograms and it was remade as 256 gray-level images on each frame. The ROI(Region of interest) was placed on a heart valve region to process images efficiently. Images were processed by the use of image enhancement filters and morphology filters. The result shows that the processed images were more enhanced than original images. When a moving image is reconstructed by using these enhanced images, we can trace the movement of heart valves more easily. In this study we proposed the availability of the moving image reconstruction using enhancement images.
This paper reports 15 native valve endocarditis cases had surgical operation in the past 10 years at the department of Cardiovascular and Thoracic Surgery, Chonbuk National University Hospital. In this study, 10 cases out of 15 were in class I or II by the New York Heart Association functional classification. None of the cases had a history of taking addictive drugs. Five cases were congenital heart disease, three cases were rheumatic heart disease and two cases were degenerative heart disease. Thus 10 cases had the underlying disease. All cases had antibiotics treatment for 3 to 6 weeks before operation. In the culture test, only four cases were positive in the blood culture and one case was positive in the excised valve culture. Organisms on blood and valve culture were Streptococcus epidermis, Streptococcus viridans, Staphylococcus aureus and Staphylococcus epidermidis. In the 10 cases without ventricular septal defect, the aortic valve was involved in four, mitral in four, both in two and involved valves in the 5 cases with ventricular septal defect were tricuspid in three, pulmonic in two. Eight cases had operation because they showed moderate congestive heart failure due to valvular insufficiency and vegetation with or without embolism. Seven cases had operation because they showed persistent or progressive congestive heart failure and/or uncontrolled infection. Five cases with ventricular septal defect underwent the closure of ventricular septal defect, vegetectomy and leaflet excision of the affected valves without valve replacement. In the cases without ventricular septal defect, the affected valves were replaced with St. Jude mechanical prosthesis. Postoperative complications were recurrent endocarditis in two, embolism in one, allergic vasculitis in two, spleen rupture in one and postpericardiotomy syndrome in one. At the first postoperative day, one case died of cerebral embolism. At the 11th postoperative month, one case died of recurrent endocarditis and paravalvular leakage in spite of a couple of aortic valve replacement. In the survived cases[13 cases in this study , all cases but one became class I or II by the New York Heart Association functional classification.
The left ventricular filling flow is now considered as an indicator which can be used for early diagnosing of cardiovascular diseases. Because the understanding of left ventricular flow physics is critical for this purpose, the downstream flow characteristics of the artificial heart valve are investigated using particle image velocimetry (PIV) method. In this study, we investigated the wake characteristics of flows passing through three different artificial valves (St.Jude medical bileaflet mechanical valve. Bjork-Shiley monostrut mechanical valve and St.Jude medical Biocor bio valve). The downstream flow field has remarkably altered according to the different valves. SJM MHV has the flow field similar to the pulsating circular jet and BS MHV has oblique pulsating jet. SJM BHV shows the similar flow field of clinical data of normal heart.
From January 1986 to June 1993, 12 patients Aad required reoperation: 9 had undergone mitral and 3 aortic valve replacement. Five were male and 7 female, and ages ranged from 29 to 61 years. Replacement of the prosthetic heart valve was performed at a mean interval of 98 $\pm$ 1 months after the Hrst operation. In aortic valve replacement patients the mean interval was 115 $\pm$ 2 months and in mitral valve replacement patients 98 $\pm$ 4 months. Primary tissue failure was the most frequent reason of replacement (10 patients) followed by valve thrombosis (1 patient) and prophylactic replacement (1 patient) in order. The most pronounced pathology of the failed prosthetic heart valves seen in the primary tissue failure group was calcification, perforation, shrinkage and tearing of the cusps. There was one early operative death (8.3%) due to intractable low cardiac output and acute renal failure. Eleven early survivors had successful operative results and there was no late death.
The Carpentier-Edwards porcine xenograft valve was used in 21 patients at Seoul National University Hospital during the period between 1977 and 1979. Twenty-four Carpentier-Edwards valves were implanted along with 2 others. Three patients died within 30 days of operation, an operative mortality rate of 14.3%. Eighteen early survivors were followed up for a total 67.5 patient-years [mean, 45.0$\pm$32.0 months]. There were 2 late deaths with a linealized late mortality rate of 2.96%/patient-year; one died from cerebral bleeding [1.48% bleeding/patient-year] and the other from prosthetic valve endocarditis [1, 48% endocarditis/patient-year]. There was no case of thromboembolism. Two patients developed mitral regurgitation [2.96% failure/patient-year]. Symptomatic improvement was excellent. The actuarial survival rate and the probability of freedom from overall valve failure were 75.3$\pm$9.6% and 80.7$\pm$12.9% at 9 years after surgery respectively. During the period from October, 1968, through June, 1985, 1, 190 substitute heart valves were used in a total of 967 patients at Seoul National University Hospital; of which, 90.9% were either porcine aortic or bovine pericardial xenograft valves. For the evaluation of the xenograft tissue valves, the consecutive patients with lonescu-Shiley valve in the mitral, aortic and both positions, Angell-Shiley valve and Carpentier-Edwards valve were recently studied on the clinical ground. They were 531 patients, and 643 xenograft valves were used. The operative mortality rate was 6.97% and a linealized late mortality rate 2.94%/patient-year. A total of 490 early survivors were followed up for 917.6 patient-years [mean, 22.5 months], and 70% of patients completed the follow-up. The linealized incidences of complications were: 2.29% emboli/patient-year, 1.98% bleeding/patient-year, 1.20% endocarditis/patient-year, and 3.49% failure/patient-year. These clinical resutls are fully comparable with those in the major reports. The durability of the glutaraldehydepreserved xenograft heart valves remains as a great concern and a continuing debate, expecially for the group of patients in the pediatric age. The need of more durable material for the improved tissue valves was also discussed.
From January 1985 to July 1985, prosthetic heart valves were replaced in 5 patients at Keimyung University Dongsan Medical Center. The patients included three women and two men ranging in age from 22 to 41 years. Three mitral valve replacements, one aortic valve replacement and one double valve replacement [mitral and aortic valve replacement] were done at the first valve operations. Reoperation were performed 2 to 76 months after the first operations, Prosthetic valve endocarditis occurred in 3 patients, early in one and late in two and primary valve failure occurred in 2 patients. In operative findings, vegetation in prosthetic valve endocarditis and calcification in primary valve failure were found. All except one had relatively successful operative results. One died of early fungal prosthetic valve endocarditis due to relapsed prosthetic valve endocarditis with heart failure and block.
From June 1984 to February 1994, cardiac valve replacement was performed in 108 patients. The distribution of patients was ranged from 13 to 64 year-old age[mean 39.48 1.24] and 51 patients were male, 57 patients were female [male:female=1:1.1]. 64 patients had mitral valve replacement, 27 patients underwent aortic valve replacement and 17 patients were performed double[mitral & aortic] valve replacement. Total 125 artificial cardiac valves were used, mechanical valves were 51 valves and tissue valves were 74 valves. The duration of follow-up was 473.41 patient-year[mean 4.79 3.29 patient-year] and the information of follow-up was available for 99 patients[92%]. The actuarial survival rates including the operative mortality was 89.5% & 88.3 at postoperative fourth & ninth year. The probability of freedom from overall valve failure, thromboembolism and bacterial endocarditis were 77.5%, 89.2% and 95.6% at ninth year after cardiac valve replacement.
In this department 504 cases of valve replacement were done since 1968 to the end of October 1981. Since October 31, 1978 to the end of October 1981 ,333 Ionescu-Shiley bovine pericardial xenograft bioprosthetic cardiac valves were replaced in 265 patients. There were 149 males and 116 female. Ages ranged from 2 to 63 years with 25 cases under 15 years of age. Among 265 cases of Ionescu valve replacement there were 157 MVR, 36 VAR, 6 TVR, 45 MVR+ AVR, 16 MVR+TVR and 5 MVR+AVR+TVR cases with mortality of 5.7%, 8.3%, 16.7%, 8.9%, 18.8% and 20% for each group respectively. Over all mortality rate in 265 Ionescu valve replacement cases was 7.9% with 21 total deaths. Main causes of operative deaths were due to LCOS in 7, bleeding in 5, arrhythmia in 3, air embolism in 2,and heart block in 2 cases. There were 12 late complications with 6 deaths. Over all long-term survival rate was 89.8%. MVR showed the highest long-term survival rate with 92.4%, and MVR+AVR+TVR the lower with 80% lower with 80%.Average follow-up period was 14 month duration. Twenty five congenital anomaly cases were operated with Ionescu-valve replacement that consisted of 7 VSD+AI, and 5 Ebstein anomaly cases with over all operative mortality of 16% and late mortality of 14.3% among 21 operative survivors. There were 25 Ionescu valve replacement cases in pediatric patients under the age of 15 years, with 4 operative deaths. Fourteen MVR, 7 AVR, and 3 TVR cases were found. Even though long-term follow-up study was short in postoperative period with total of 33~.0 months among 244 operative survivors ranging one to 36 months, the late survivors showed beneficial long-term results two thromboembolic episodes in 244 patients were found. More cases and longer term follow-up study are warranted for valve replacement in pediatric and TVR cases with Ionescu-valves which have advantageous hemodynamic structures compared with other bio-prosthetic heart valves.
Computed tomography (CT) plays an important supplementary role in the evaluation of patients with heart disease. CT can be used to evaluate the aorta, pulmonary artery, pulmonary vein, cardiac chambers, coronary artery, valves and systemic veins (superior vena cava, inferior vena cava and hepatic veins). The "Learning Objectives" describe the normal anatomy and typical pathological conditions seen on axial scans and reformatted images from CT in patients with heart disease, focusing focus on frequent, fatal, and rare but characteristic diseases encountered in routine practice.

There has been increasing incidence of open heart surgery in infants in recent years. After 1990, 28 cases of infant open heart surgery below the 12month were performed. Most lowest weight was 2, 200g[15 days]. Operative mortality of open heart surgery in infant with weight below the 10kg was very high as a 25.7%.
