Characteristics of Patients with Surgical Closure of an Atrial Septal Defect during Infancy

  • Background: Surgical closure of an atrial septal defect (ASD) is infrequently indicated during infancy. We evaluated the clinical characteristics and outcomes of patients who underwent surgical ASD closure during infancy. Methods: A single-center retrospective review was performed for 39 patients (19 males) who underwent surgical ASD closure during infancy between 1993 and 2020. The median body weight percentile at the time of operation was 9.3. Results: During a median follow-up of 60.9 months, 4 late deaths occurred due to chronic respiratory failure. A preoperative history of bronchopulmonary dysplasia (BPD) was the only risk factor for late mortality identified in Cox regression (hazard ratio, 3.54; 95% confidence interval [CI], 1.75-163.04; p=0.015). The 5-year survival rate was significantly lower in patients with preoperative history of BPD (97.0% vs. 50.0%, p<0.001) and preoperative ventilatory support (97.1% vs. 40.4%, p<0.001). There were significant postoperative increases in left ventricular end-diastolic (p=0.017), end-systolic (p=0.014), and stroke volume (p=0.013) indices. A generalized estimated equation model showed significantly better postoperative improvement in body weight percentiles in patients with lower weight percentiles at the time of operation (<10th percentile, p=0.01) and larger indexed ASD diameter (≥45 mm/m2, p=0.025). Conclusion: Patients with ASD necessitating surgical closure during infancy are extremely small preoperatively and remain small even after surgical closure. However, postoperative somatic growth was more prominent in smaller patients with larger defects, which may be attributable to an increase in postoperative cardiac output due to changes in ventricular septal configuration. The benefits of ASD closure in patients with BPD are undetermined.

  • Between 1983 and 1993, 250 patients over 16 years of age who had undergone a surgical correction of a congenital cardiovascular disease were reviewed. 222 patients were divided into acyanotic group and 28 patients were cyanotic group. The most common defects were atrial septal defect [96 patients and ventricular septal defect [95 patients . There were 128 patients in the third decade, 71 patients under 20 years of age, 40 patients in the fourth decade and 11 patients over 40 years of age. The male to female ratio was 1.05:1. Operative mortality was 6.8% [4.1% in the acyanotic group and 26.8% in the cyanotic group and the most common cause of death was low cardiac output syndrome.

Resolution of Protein-Losing Enteropathy after Congenital Heart Disease Repair by Selective Lymphatic Embolization

  • With improving survival of children with complex congenital heart disease (CCHD), postoperative complications, like protein-losing enteropathy (PLE) are increasingly encountered. A 3-year-old girl with surgically corrected CCHD (ventricular inversion/L-transposition of the great arteries, ventricular septal defect, pulmonary atresia, postdouble switch procedure [Rastelli and Glenn]) developed chylothoraces. She was treated with pleurodesis, thoracic duct ligation and subsequently developed chylous ascites and PLE (serum albumin ${\leq}0.9g/dL$) and was malnourished, despite nutritional rehabilitation. Lymphangioscintigraphy/single-photon emission computed tomography showed lymphatic obstruction at the cisterna chyli level. A segmental chyle leak and chylous lymphangiectasia were confirmed by gastrointestinal endoscopy, magnetic resonance (MR) enterography, and MR lymphangiography. Selective glue embolization of leaking intestinal lymphatic trunks led to prompt reversal of PLE. Serum albumin level and weight gain markedly improved and have been maintained for over 3 years. Selective interventional embolization reversed this devastating lymphatic complication of surgically corrected CCHD.

최소 피부 절개술을 이용한 선천성 심장 질환 수술 (Minimal Skin Incision with Full Sternotomy for Congenital Heart Surgery)

  • 배경: 선천성 심장 질환에서의 최소 피부 절개에 대한 보고는 매우 적다. 저자등은 수술상처를 최소화하면서 통상적인 수술수기 및 시야를 확보할 수 있는 방법을 고려하였다. 대상 및 방법: 저자등은 1997년 4월부터 1997년 9월까지 선천성 심장 질환 환자 40명을 대상으로 최소 피부 절개술과 정중 전 흉골 절단을 통해서 개심술을 시행하였다. 환자 질병 분포는 성인 환자가 5명(남:여=1:4)이며 이중, 심방 중격 결손증 3명, 심실 중격 결손증 1명, 부분 심내막상 결손증 1명이었고, 소아 35명(남:여=17:18)에서는 심방 중격 결손증 4명, 심실 중격 결손증 30명, 발살바 동맥류 1명이었다. 정중 피부 절개는 흉골 두번째 늑간하부에서 검상돌기 1~2cm상방까지 실시하였다. 흉골 하부 절단에는 일반 전기톱을, 흉골 상부 절단에는 특수 전기톱을 피부 밑에 삽입하여 전장의 흉골을 절단하였다. 그리고, 흉골 견인은 좌우 & 상하 양방향에 2개의 견인기를 각각 직각으로 거치하여 수술시야를 확보하였다. 결과: 흉골길이대비 피부절개의 길이는, 성인에서는 55.0$\pm$3.5%로 절개길이가 평균 12cm(10~13.5cm)였고, 소아에서는 63.1$\pm$3.9%로 평균 7.3cm(5.2~11cm)였다. 모든 증례에서 체외 순환시 필요한 동정맥 삽관을 추가적인 서혜부 피부 절개 없이 직접 대동맥 및 상하공 대정맥에 시행할 수 있었으며 좌심방벤트관은 필요시 삽입하였다. 전 례에서 수술사망이나 합병증은 발생하지 않았으며, 최소 피부 절개술에 따른 창상 감염 및 피부 괴사, 혈종 형성, 출혈에 의한 합병증은 없었다. 결론: 이상의 결과로 최소 피부 절개술과 정중 전 흉골 절개를 이용한 선천성 심장 질환 수술은, 통상적인 체외 심폐 순환을 할 수 있으며, 외관상 미용의 효과가 뚜렷하였고, 다양한 선천성 심질환에서 안전하고 효과적으로 사용될 수 있다고 판단된다.

Surgical Repair of Ventricular Septal Defect after Myocardial Infarction: A Single Center Experience during 22 Years

  • Background: Surgical repair of post-infarct ventricular septal defect (VSD) is considered one of the most challenging procedures having high surgical mortality. This study aimed to evaluate the outcomes of the surgical repair of post-infarct VSD. Methods: From May 1991 to July 2012, 34 patients (mean age, $67.1{\pm}7.9$ years) underwent surgical repair of post-infarct VSD. A retrospective review of clinical and surgical data was performed. Results: VSD repair involved the infarct exclusion technique using a patch in all patients. For coronary revascularization, 12 patients (35.3%) underwent concomitant coronary artery bypass graft, 3 patients (8.8%) underwent preoperative percutaneous coronary intervention, and 9 patients (26.5%) underwent both of these procedures. The early mortality rate was 20.6%. Six patients (17.6%) required reoperation due to residual shunt or newly developed VSD. During follow-up (median, 4.8 years; range, 0 to 18.4 years), late death occurred in nine patients. Overall, the 5-year and 10-year survival rates were $54.4%{\pm}8.8%$ and $44.3%{\pm}8.9%$, respectively. According to a Cox regression analysis, preoperative cardiogenic shock (p=0.069) and prolonged cardiopulmonary bypass time (p=0.008) were independent predictors of mortality. Conclusion: The early surgical outcome of post-infarct VSD was acceptable considering the high-risk nature of the disease. The long-term outcome, however, was still dismal, necessitating comprehensive optimal management through close follow-up.

개심술 전후 방사성 동위원소를 이용한 심기능 평가에 관한 연구 -수술전 shunt 의 진단 및 교정수술후의 성적평가에 대하여- (Evaluation of Cardiac Function Using Radioisotope before and after Open Heart Surgery -Detection of Preoperative Cardiac Shunt and Postoperative Remnant Shunt by Nuclear Angiocardiography-)

  • In this investigation we undertook to evaluate the utility of radionuclide cardiac angiography in the detection of cardiac shunts before and after surgical correction. Time-activity curves of ventricles and lungs were evaluated after bolus intravenous injection of 99mTc-human serum albumin in 512 preoperative patients and 551 post-operative patients. Omitting 31 cases of technical failure due to poor bolus, we detected shunts in 459 cases of 481 preoperative evaluations, so the detectability was 95.4%. The cases which couldn`t be detected by this method had small amount of shunt. Also the degree of shunt detected by radioisotope methods were well correlated with oxymetry method. [r=0.89, p<0.01 ] In postoperative evaluations, 18 out of 411 patients with left to right shunt and 10 out of 140 right to left shunt were found to have remnant shunts with radionuclide cardiac angiography. Of the 28 cases with failed operation, 2 were confirmed in reoperation, 2 by cardiac catheterization, 2 by two -dimensional echocardiography. All except one .f these patients had membranous ventricular septal defects and those with left to right shunts had moderate to severe pulmonary hypertension and shunt amount. Also those had larger septal defects than control group. We consider that radionuclide cardiac angiography is a simple and noninvasive method which can show the preoperative diagnosis and postoperative follow up of cardiac shunts.

선천성(先天性) 심장병(心臟病)의 개심술(開心術) -6례(例) 수술경험(手術經驗) - (Open Heart Surgery for Six Cases of the Conegnital Heart Disease)

  • Six cases of congenital heart disease were operated on by means of cardiopulmonary bypass between December, 1975 and April, 1976. Two cases of ventricular septal defects (VSD), two cases of VSD, associated with ruptured aneurysm of sinus Valsalva, two cases of atrial septal defects (ASD) and one case of pulmonic stenosis with patent ductus arteriosus were operated. Sarns roller pumps and Bentley Temptrol oxygenators were used for extracorporeal circulation. Pump oxygenator was primed with Ringer's lactate solution, 5% dextrose in water, mannitol, and ACD blood. Flow rate ranged from 2.0 to $2.4L/M^2/min$. Bicarbonate was added to the oxygenator with estimated amount as 15 mEq/L/hr. Venous catheters were introduced into superior and inferior vena cava, and oxygenated blood was returned to the body through aortic cannula inserted into ascending aorta. Moderate hypothermia ($30^{\circ}C$) was induced by core cooling. Aorta was cross clamped for 15 minutes and released for 3 minutes, and repeated clamping when necessary. Atrial and ventricular septal efects were closed by direct sutures. Aneurysms of sinus Valsalva ruptured into the right ventricle were repaired through right ventriculotomy by d:rect closure with Dacron patch reinforcement. Cardiopulmonary bypass time varied from 66 to 209 minutes, and aorta cross clamping time ranged from 13 to 56 minutes. Postoperative bleeding was minimal except one case who needed for evacuation of substernal hematoma. Intra- and postoperative urinary output was satisfactory. Acid-base balance, partial pressure of $O_2$, electrolytes, and hematological changes during intra- and post-perfusion period remained at the acceptable ranges. No mortality was experienced.

말티스견에서 발생한 양대혈관 우심실기시 (Double Outlet Right Ventricle in a Maltese Dog)

  • 8개월령 말티스가 호흡곤란, 운동불내성의 증상으로 내원하였다. 흉부방사선상 환자는 심한심종대와 주폐동맥의 확장 소견을 보였고, 심초음파상 대동맥근이 우심실을 향하고 좌-우 단락을 가지고 있는 대동맥하 심실중격결손증이 확인되었다. 또한 중격결손에 의한 좌-우 단락성 혈류에 의해 폐혈관계에 과순환 소견과 폐성 고혈압에 의한 심한 폐동맥 역류가 (최고속도 4.7 m/s, 압력구배 ~88 mmHg) 확인되었다. 상기의 소견을 토대로 환자는 양대혈관 우심실기시로 진단되었다. 환자는 우심실의 혈량과부하를 줄이기 위한 이뇨제 처치(furosemide 1 mg/kg), 심장재구성을 늦추어주기 위해 spironolatcone (1 mg/kg) 및 enalapril (0.5 mg/kg) 처치와 폐의 과순환과 폐성 고혈압을 완화시켜주기 위해 sildenafil (1 mg/kg)를 처방하였다. 일주일뒤 재검에서 환자의 임상 증상은 크게 개선되었다. 현재 환자는 생존해 있고 정기적으로 모니터하고 있다.

교정형 대혈관전위증동반된 심혈관기형의 수술요 (Corrected transposition of the great arteries: surgical treatment of associated anomalies)

  • Corrected transposition of the great arteries [C-TGA] is one of the rare congenital heart disease in which there is both a discordant atrioventricular relationship and transposition of the great vessels. With this arrangement, systemic venous blood passes through the right atrium into the morphologic left ventricle and out the pulmonary artery. Pulmonary venous blood returns to the left atrium, flows into the morphologic right ventricle and out the aorta. Thus, in the rare case when no additional cardiac anomaly is present, a hemodynamically normal heart exists. But more often they are symptomatic as a result of one or several of the commonly associated defects. This paper describes 13 patients who underwent repair of one or more cardiac anomalies associated with corrected transposition at SN UH, from June 1976 through June 1984. 1.8 were males and 5 females, with ages ranging from 3 years to 27 years. 2. Segmental anatomy was {S,L,L} in 12, or {I,D,D} in 1. 3.Associated anomalies were ventricular septal defect in 10, pulmonary outflow tract obstruction in 6, tricuspid insufficiency in. 4, atrial septal defect in 3, subaortic stenosis in 1, mitral insufficiency in 1, and patent ductus arteriosus in 1. 4.None had complete heart block preoperatively, and 3 developed complete heart block intraoperatively. But one of them recovered sinus rhythm on the postoperative 7th day spontaneously. 5.There were 3 cases of hospital morality. But there was no morality since Dec. 1980. 6.Patients with single ventricle, hypoplastic ventricle or those who had palliative surgery alone are not included in this review.

막성주위형 심실중격결손중의 봉합시 경삼첨판륜 절개방법의 외과적 치료효과 (Effects on Surgical Repair of VSD by TATV)

  • 막성주위형 심실중격결손증(Perimembranous ventricular septal defect, PMVSD)은 심실중격결손증(ventriculal septal defect, VSD)의 가장 흔한 형태이며, 그 수술방법 또한 다양하다. 막성주위형 심실중격결손증(PMVSD)을 교정하는 대표적인 수술방법은 우심방을 절개하여 결손부위를 직접보고 펫취봉합하는 방법이다. 그러나 상부 경계가 잘 안보일때 혹은 건삭(ch(wda)의 VSD경계부위 부착의 원인으로 인하여 다발성 VSD로 보일때 외과 의사에 따라서 경삼첨판륜 절개방법(transannular approach of tricuspid valve, TATV)을 가끔 이용하고 있다. 그 러나 이 수술방법은 삼첨판륜을 절개하기때문에, 방실판막의 개폐작용에 문제가 생길 수 있다고 취급되어왔 다. 본 연구는 경삼첨판륜 절개방법(TATV)이 막성주위형 심실중격결손증(PMVSD) 수술시 안전한 방법인지를 알아보고자 하였다. 최근 5년간 단독형 막성주위형 심실중격결손증 96례를 대상으로 그중 28례(29%)에서 경삼첨판륜 절개방법 (TATlr)으로 VSD를 봉합하였으며, 수술후 3개월에서 33개월동안 추적관찰하여 다음과 같은 결론을 얻었다. 1. 수술시 나이는 4개월에서 38세였는데, 5세이상에서 17(6 %)례로 가장 많았다. 2. 수술전 폐1체 혈류비는 1에서 2.8이었고, 22fll(79%)에서 2이하였다. 3. 수축기 폐동맥압은 8례에서 30 mmllg미만, 17례에서 30-50 mmHg사이, 3례에서 50 mmHg이상으로 25례 (89%)에서 50 mmHg이하였다. 4. 삼첨판폐쇄부전은 수술전 12례에서 없었고, 6례에서 미세했고, 3례에서 경했고, 5례에서 중등도 2례에서 중증도였는데,수술후에는 4례에서 경했고, 6례에서 미세했고, 18례에서는 없어져, 대체로 수술후 감소하거나 없어지는 경향을 보였다. 5. 좌-우 단락양이 적은 막성주위형 심실중격결손증(PMVSD)에서 삼첨판맹낭(tricuspid valve pouch 19례), 우심실 유출로폐쇄(4례), 아급성심내막염(1례) 및 동반질환을 부가적인 수술적응으로 삼았다. 6. 수술후 관찰결과 사망률은 없었고, 잔존 심실중격결손증도 없었다. 이상의 결과로 경삼첨판륜 절개방법(TATV)은 삼침판맹낭을 가지고 있는 막성주위형 심실중격결손증 (PMVSD) 수술시 좋은 방법이며, 또한 막성주위형 심실중격결손증(PMVSD)에서 수술시 시야를 좋게하고 수술후 삼첨판폐쇄부전에 악영향을 미치지 않는 결과로 안전하고 효과적인 방법으로 사료된다.
