A heart sound algorithm, which separates the heart sound signal into four parts; the first heart sound, the systolic period, the second heart sound, and the diastolic period has been developed. The algorithm uses discrete intensity envelopes of approximations of the wavelet transform analysis method to the phonocard-iogram(PCG)signal. Heart sound a highly nonstation-ary signal, so in the analysis of heart sound, it is important to study the frequency and time information. Further more, Wavelet Transform provides more features and characteristics of the PCG signal that will help physician to obtain qualitative and quantitative measurements of the heart sound.
A descriptive exploratory design was used in this study to evaluate the effects of sponge bathing on physiological(heart rate, heart period, vagal tone, oxygen saturation, respiration) and behavioral responses in newly born 40 preterm infants from intensive care unit of S University Hospital in Seoul. Data has been collected from October, 1997 to March, 1999. The infants were between 27-33 weeks gestational age at birth, and were free of congenital defects. The subjects entered the protocol when they were medically stable (determined by initiation of feeding and discontinuation of all respiratory support) but still receiving neonatal intensive care. The infants' physiologic parameters were recorded a 10 - minute before, during, and after bathing. Continuous heart rate data were recorded on a notebook computer from the infant's EKG monitor. The data were digitized off-line on software(developed by Lee and Chang in Wavelet program) which detected the peak of the R wave for each heart beat and quantified sequential R-R intervals in msec(i.e. heart periods). Heart period data were edited to remove movement artifact. Heart period data were quantified as : 1) mean heart period; 2) vagal tone. Vagal tone was quantitfied with a noninvasive measure developed by Porges(1985) in Mxedit software. To determine behavioral status, tools were developed by Scafidi et al(1990) were used. Collected data were analyzed with the SPSS program using paried t-test, ANOVA, and Pearson correlation. The result were as follow. 1. The results of the ANOVAs indicated that vagal tone were signifcantly lower during bathing than baseline and post-bathing. There were significant differences in heart period and heart rate levels across the bathing. But the mean oxygen saturations and respirations were no differences. Also, there were no significant differences on behavioral sign, motor activity, behavioral distress, weight changes, morbidity, and hospitalization period. 2. To evaluate the relation between vagal tone and subsequent parameters, the two groups (the high group had 19 subjects and low group had 21subjects) were divided by the mean baseline vagal tone. Vagal tone measured prior to bathing were significantly associated with respiration before bathing, vagal tone during bathing, and the magnitude of change in both vagal tone. But, other subsequent reactivities were no differences in two groups. 3. Correlations were also calculated between vagal tone and the subsequent physiological reactivities from baseline through after- bathing. Correlations were significant between baseline vagal tone and baseline heart rate, between baseline vagal tone and baseline heart period, between baseline vagal tone and oxygen saturation after bathing. In summary, the bathing in this study showed a stressful stimulus on premature infants through there was significance in the physiological parameters. In addition, our study represents the documentation that vagal tone reactivity in response to clearly defined external stimulation provides an index of infant's status.
From Jan. 1962 to Aug. 1983, one hundred patients with cyanotic heart disease underwent various palliative operations at the department of thoracic and cardiovascular surgery, S.N.U.H. In the period from Jan. 1962 to Dec. 1973, in which the open heart surgery was not routinely performed, sixty-two operations including 2 cases of second shunt operation were performed in sixty patients, and all of them were tetralogy of Fallot except three cases. Various palliative procedures such as Glenn, Brock, Waterston and Blalock-Taussing operation were used in this period with overall mortality rate of 16%. In the period from Jan. 1980 to Aug. 1983, forty patients with cyanotic heart disease were operated and majority of them was complex anomalies. Only Blalock-Taussing operation was used in this period with a mortality rate of 20%. These two groups of patients were compared according to age, diagnosis and results of operations, and it appears that Blalock-Taussing operation is effective palliation for patients with cyanotic heart disease, especially with complex anomaly, with an acceptable mortality.
77 cases of Aortic Valve Replacement, which were composed of 64 rheumatic valvular heart disease and 13 combined congenital heart disease, were operated at Seoul National University Hospital for Aortic valvular disease during the period from June 1968 to December 1983. Among these 64 rheumatic aortic valvular heart disease cases, 8 patients were expired during and immediate after operation and overall mortality rate was 12.5%. For more precise remarks, these patients were divided into two periodic groups, 1st period [from 1968 to 1976] and 2nd [from 1977 to 1983] when annual open heart surgery were over 100 cases, and in 1st period three of four patients were died and in 2nd period five of sixty patients were died and its mortality rate was 8.3%. There were 12 cases of postoperative complication, which were 3 cases of remaining other valvular heart disease required MVR, 2 paravalvular leaks [one of them got Redo AVR], 4 thromboembolism or problem of anticoagulant therapy, 2 late death due to SBE with replaced valve failure and one functional AS with small sized valve. Operative death was affected by pump-time and aortic cross-clamping time, heart size, Ejection Fraction, LVEDP and symptom duration, and other many factors may influence the survival rate. Improved operative technique and myocardial protection and meticulous evaluation of the preoperative patient status will make the AVR safer.

On the other hand, the acquisition of pulse wave or photoplethysmogram(PPG) is very easy. However, the peak of PPG is not so sharp as ECG. This study tries to enhance the performance of peak detection in PPG signal. The method uses the average slopes around the main peak. The crossing point of the increasing and the decreasing slopes is selected as the peak point of heart rate period. The proposed method showed smoothed heart rate graph and reduced irregularity in heart rate values.
Objectives: Economic growth and development of medical technology help to improve the average life expectancy, but the western diet and rapid conversions to poor lifestyles lead an increasing risk of major chronic diseases. Coronary heart disease mortality in Korea has been on the increase, while showing a steady decline in the other industrialized countries. An age-period-cohort analysis can help understand the trends in mortality and predict the near future. Methods: We analyzed the time trends of ischemic heart disease mortality, which is on the increase, from 1985 to 2009 using an age-period-cohort model to characterize the effects of ischemic heart disease on changes in the mortality rate over time. Results: All three effects on total ischemic heart disease mortality were statistically significant. Regarding the period effect, the mortality rate was decreased slightly in 2000 to 2004, after it had continuously increased since the late 1980s that trend was similar in both sexes. The expected age effect was noticeable, starting from the mid-60's. In addition, the age effect in women was more remarkable than that in men. Women born from the early 1900s to 1925 observed an increase in ischemic heart mortality. That cohort effect showed significance only in women. Conclusions: The future cohort effect might have a lasting impact on the risk of ischemic heart disease in women with the increasing elderly population, and a national prevention policy is need to establish management of high risk by considering the age-period-cohort effect.
Alopecia is a minor complication following open heart surgery, but it is a cosmetic and psychotic problem to the patient. Especially the extensive long period of operation, prolonged mechanical ventilation and no change of patient head position during operative and recovery period may serve the localized scalp pressure effect, which causes postoperative alopecia. So, for prevention of the postoperative alopecia, adequate blood flow and pressure must be maintained with alternative change of head position during operative and recovery period. The author experienced one case of occipital alopecia following open heart surgery and reports with literature.
Heart rate variability (HRV) in electrocardiogram (ECG) is an important index for understanding the health status of heart and the autonomic nervous system. Most HRV analysis approaches are based on the proper heart rate (HR) data. Estimation of heart rate is thus a key process in the HRV study. In this paper, we report an innovative method to estimate the heart rate. This method is mainly based on the concept of periodicity transform (PT) and instantaneous period (IP) estimate. The method presented is accordingly called the "PT-IP method." It does not require ECG R-wave detection and thus possesses robust noise-immune capability. While the noise contamination, ECG time-varying morphology, and subjects' physiological variations make the R-wave detection a difficult task, this method can help us effectively estimate HR for medical research and clinical diagnosis. The results of estimating HR from empirical ECG data verify the efficacy and reliability of the proposed method.

Purpose: The purpose of this study was to examine and analyze the post operative fasting period of neonates in the intensive care unit (ICU) after receiving open heart surgery in order to provide optimal nutrition support for these neonates. The variables included biochemical nutrition indexes (albumin, total lymphocyte count, total cholesterol) and patient outcomes (duration of mechanical ventilation, ICU stay, hospital stay, infectious complication). Methods: The participants were 124 neonates in ICU after receiving open heart surgery, and the design of this study was to investigate their post operative fasting period retrospectively to analyze the biochemical nutrition indexes and patient outcomes according to post operative fasting period. Results: The results for 4 groups according to post operative fasting period showed that the group with less 48 hours of fasting had the best biochemical nutrition indexes and patient outcomes, followed by the 48-72 hour group, the 72-144 hour group, and the over 144 hour group. Conclusion: The results indicate that for new born babies receiving open heart surgery, the period of fasting after the operation should be minimized and tube feeding should be started as soon as possible.
