Total Anomalous Pulmonary Venous Drainage with A.S.D. (전페정맥 이상환류 (TAPVD)의 치험 1례)

  • This is a case report of total anomalous pulmonary venous drainage with Atrial septal defect, which was corrected surgically by intracardiac procedure under total cardiopulmonary bypass. This 9 years old girl, she complained mild cyanosis, exertional dyspnea, and frequent upper respiratory infection from 3 months age. The chest X-ray showed cardiomegaly and a "snow man appearance", she has systolic murmur with splitting of S2. The diagnosis was confirmed with right heart catheterization, the catheter was reached to right pulmonic vein passed through right atrium, right superior vena cava, innominate vein, left superior vena cava, and common venous trunk. On 28th, Dec. 1979, an anastomosis between common pulmonary venous trunk and left atrium, Pericardial patch closure of ASD, and ligation of left superior vena cava were performed with Extra-corporeal circulation. The postoperative course was uneventful and discharged with excellent general condition.Total Anomalous Pulmonary Venous Drainage with A.S.D. with A.S.D.

Surgical Treatment of Intracardiac Myxoma (심장 점액종의 외과적 치료)

  • Since November 1979, 20 patients have undergone excision of an intracardiac myxoma, which was located in the left atrium in 18[90%], in the right atrium in 1[5%], and in the right ventricle in 1[5%], There were 17 female and 3 male patients with a mean age of 43.5 years [range 23 to 68 years]. Only one patient was asymptomatic, the others were seen mostly exertional dyspnea, palpitation, sings of systemic illness. Diagnosis was confirmed by echocardiography in all cases and angiography in two cases, preoperatively, The myxomas were successfully removed in all patients, either by shaving them from the atrial septum or by excising a portion of normal atrial septum with tumor. All heart chambers were carefully explored for presence of multi-centric myxoma or tumor debris. We conclude that excision of intracardiac myxoma is curative and long term survival is excellent and long term clinical and echocardiographic follow-up are recommended since late recurrence, although rare, has been reported.

  • A 43-year-old man who had had a history of atrial septal defect (ASD) device closure 31 months previously presented with abrupt chest and back pain along with progressive cardiogenic shock and cardiac arrest. After resuscitation, he was diagnosed with cardiac tamponade. Diagnostic and therapeutic surgical exploration revealed left atrium (LA) perforation due to LA roof erosion from a deficient aortic rim. Device removal, primary repair of the LA perforation site, and ASD patch closure were performed successfully. The postoperative course was uneventful. The patient was discharged after 6 weeks of empirical antibiotic therapy without any other significant complications.

  • Congenital coronary arteriovenous fistula is a ~are condition, and with widespread use of cardiac catheterization, angiography and selective coronary arteriography is being recognized with increasing frequency. Surgical correction is strongly recommended to prevent the development of congestive heart failure,angina, subacute bacterial endocarditis, myocardial infarction, and pulmonary hypertension, as well as coronary aneurysm formation with subsequent rupture or embolization. I report a case of congenital coronary arteriovenous fistula of the left main coronary artery to the fight atrium in a 23 year old female, which is associated with bacterial endocarditis with right atrial vegetation.

Influence of Phenylephrine and Clonidine on The Coronary Flow in Rats (백서 관상 혈류에 미치는 Phenylephrine 및 Clonidine 의 영향)

  • The regulatory role of the post \ulcorner1-and \ulcorner2-adrenoceptors on cardiac function, particularly in coronary flow rate, was investigated in the isolated rat heart treated with 10-6 M propranolol. When introduced into the left atrium of the heart, phenylephrine[10-7-10-2 M] decreased coronary flow rate and increased mean coronary resistance in a dose related fashion, but did not affect heart rate. Methoxamine also elicited the increment of coronary resistance and the decrement of coronary flow rate, though the effects of methoxamine were weaker than those by phenylephrine. The effect of phenylephrine was inhibited by 1\ulcornerM prazosin and shifting the dose-response curve to the right. The effects of clonidine, a selective \ulcorner2-adrenoceptor agonist, were studied in the heart taken from reserpinized rats. Clonidine increased coronary resistance, decreased heart rate and coronary flow rate with a dose-dependent manner. These effects were abolished by 10-6 M yohimbine, a selective \ulcorner2-antagonist, and were not affected by 10-6M prazosin. Clonidine also decreased coronary flow and increased mean coronary resistance in electric paced heart. These effects were inhibited by rawoulscine, a selective ca-antagonist. These results indicate that the stimulation of both post \ulcorner1-and \ulcorner2-adrenoceptor causes coronary vasoconstiction. And it is inferred that this model of sympathomimetics-induced coronary vasospasm may provide a useful tool for investigating spasmolytic agents which are of benefit in the treatment of variant angina.

  • Though acute pulmonary thromboembolism is usually managed medically with the use of thrombolytics or anticoagulants, an emergent life-saving surgery would be required. In a case of acute pulmonary thromboembolism with acute severe right heart failure and deferment of it could result in fatal outcomes in a short time. In addition, the mortality is raised considerably if it is combined with right heart thrombi. Despite paradoxical thromboembolism via patent foramen ovale was reported, few report might be presented, in which showed the thrombus in right atrium has traversed atrial septal defect into left atrium and left ventricle like this case as the evidence of paradoxical thromboembolism. We report a case of acute pulmonary thromboembolism with acute right heart failure arising from deep vein thrombosis, developed immediately after low anterior resection for colon cancer in a 63-year-old male, who was managed successfully by emergent thromboembolectomy with cardiopulmonary bypass.

A case of recurrent hepatoblastoma : lung, heart and brain metastasis (폐, 심장, 뇌에 전이되어 재발된 간모세포종 1례)

  • Hepatoblastoma is a hepatic tumor predominantly occurring in children. The usual site of metastasis is the lung. There are only several reports worldwide on the distant metastasis of hepatoblastoma to the central nervous system in children. Only one reported case showed survival of a patient after multiple resections of a recurrent brain lesion. Involvement of the cardiovascular system has been reported in the medical literature. Lesions almost always involve the right-side of the heart. We report a case of recurrent hepatoblastoma at multiple sites, including brain, left atrium of the heart and lung in a 6-year-old girl who was partially treated in the past at the age of 1.5 years; the patient had been event-free for four and a half years.

  • Cardiac injury is a relatively uncommon entity, which calls for emergency surgical treatment. During the period from 1974 up to 1975, three cases of stab wound of the heart were treated in Department of Thoracic Surgery. Capital Armed Forces General Hospital among 70 chest injury cases. All of the cases had stab wounds on the heart by knife. Injured sites were found in two cases on the right ventricle, and one case on the right atrium and intrapericardial inferior vena cava. All patients were treated by thoracotomized and sutured with 000 silk for myocardial stab wounds. One of these died of cerebral hypoxia, who was resuscitated from cardiac arrest during operation. Pericardial tamponade signs were not definite except one case.

Penetrating Wounds of the Heart: A Report of Four Cases (심장손상 4례에 대한 치험)

  • A cardiac injury is one of the most dramatic and demanding emergencies than may be cared by the surgeon. Four patients who sustained penetrating wound of the heart have been experienced in the Depart-ment of Thoracic Surgery, Kyungpook National University Hospital. They were 3 males and one female. The age was between 21 and 25. All of this series had stab wounds on the heart by instruments of scissors, gimlet, kitchen knife and glass. Injured sites were two cases of right ventricle, one of the right atrium and one of indifinitive with pericardial laceration. Three of these patients have been successfully treated by emergency thoracotomy with 3-0 silk sutures for myocardial penetrating wounds. One of these patients occurred cardiac arrest during surgery and expired, who was in penetrated right ventricular injury through which massive bleeded. The classic clinical description of tamponade, Beck`s triad, seemed to be useful for making diagnosis of the tamponade but not entirely reliable as diagnostic criterion of the tamponade. In the studing of 3 tamponade cases in these series, the triad of Beck was presented in only two cases. Elevated venous pressure was thought to be a valuable diagnostic factor in pericardial tamponade.

  • In this study, we developed a whole cardiovascular system model combined with a Laplace heart based on the numerical cardiac cell model and a detailed arterial network structure. The present model incorporates the Laplace heart model and pulmonary model using the lumped parameter model with the distributed arterial system model. The Laplace heart plays a role of the pump consisted of the atrium and ventricle. We applied a cellular contraction model modulated by calcium concentration and action potential in the single cell. The numerical arterial model is based upon a numerical solution of the one-dimensional momentum equations and continuity equation of flow and vessel wall motion in a geometrically accurate branching network of the arterial system including energy losses at bifurcations. For validation of the present method, the computed pressure waves are compared with the existing experimental observations. Using the cell-system-arterial network combined model, the pathophysiological events from cells to arterial network are delineated.
