A Study on the Need for a Nutritional Care Service and the Role of a Dietitian in Silver Town (성인을 대상으로 한 실버타운 내 영양 서비스 및 영양사 역할에 대한 요구도 조사)

  • The purpose of this study was to investigate the need for a nutritional care service in silver town and identify awareness of the role of a dietitian in silver town using a structured questionnaire. The subjects consisted of 466 adults (216 males, 250 females), aged 20~59 years, in the Gyeongnam area. Although most of the subjects were aware of silver town and felt a necessity for it, only 24.9% of the subjects intended to live in silver town, whereas 55.6% of the respondents replied that they did not know whether they lived in silver town. Results on the importance of services offered in silver town indicated that medical service facilities, the interior and mood of silver town, the reliability of management, and meal service prepared by a dietitian were prioritized. Healthy food was the most valuable part of the meal service followed by taste. In total, 88.3% of the subjects answered that the meal service in silver town should be supervised by a dietitian. Clinical nutrition management was chosen as the most important job of the dietitian followed by sanitation management. These results could provide a better understanding of the personal needs of silver town, focusing on the meal service and the role of a dietitian.

An Analysis of Research Related to the Massage as the Nursing Intervention (간호중재로서의 마사지 적용연구 분석)

  • Purpose: The purpose of this study was to identified patterns of studies and to analyze the nursing research related to the massage intervention in Korea and to improve direction of nursing research in massage intervention in Korea. Methods: The research studies related to the massage were selected from the Korean Nurses Academic Society Journal and dissertation which were conducted experimental research design the last 7 years in Korea. The total numbers of the studies were 58. Result: The study subjects were post operational state in 34.5%, healthy adult in 17.2% studies. The others dealt with infant, cancer, dementia, hypertension, preterm neonates, elderly. Types of massage were whole body in 31.1%, foot in 22.6%, hand in 17.2%, back in 13.8% studies. And Meridian massage in 17.2%. Length of massage was below 5 minutes in 17.2%, 6~10 minutes in 29.4% and 11~20 minutes in 32.8% studies. Frequency of massage was only 1 time in 25.9%, 2~5 times in 31.1% studies. Massage produced psychological and physiological effects. Positive psychologic effects were anxiety relief, pain relief, mood enhancement and fatigue relief. Positive physiologic effects on body systems were blood pressure, heart rate, edema, skin temperature, immune system, blood glucose, excretion, PMS syndrome and growth and development. Conclusion: As a result, effects of massage were almost positive. Therefore massage is useful alternative nursing intervention. But more attention research is needed to identify the objectivity instrument to measure and repeat research.

Effect of Acupuncture-like Transcutaneous Electrical Nerve Stimulation on Plasma $\beta-endorphin$ and ACTH, Serum Cortisol and Urinary 17-OHCS Levels (저빈도-고강도 경피신경전기자극이 혈장 $\beta-endorphin$ 과 ACTH, 혈청 Cortisol 및 요중 17-OHCS 농도에 미치는 영향)

  • The purpose of this study was to examine the effect of electroanalgeia and $\beta-endorphin$ action by acupuncture-like (Lof/Hil) transcutaneous electrical nerve stimulation (TENS) applied to acupuncture points. Twelve healthy adult male aged between 19 ann 25 were randomly assigned to TENS group (n=6) and naloxone group (n=6). Subjects of both groups were strongly stimulated TENS with 4 pps and $200{\mu}s$ for 30 minutes on the LI 3 and LI 10 meridian points of dominant am. Naloxone group was injected naloxone hydrochloride before TENS application. The experimental pain threshold was measured by chronaxie meter CX-2 on the distal end of radius just before and after TENS application. The levels of plasma $\beta-endorphin$ and ACTH. serum cortisol and urinary 17-OHCS were analyzed by radioimmunoassay (RIA) kits before and after TENS application. In TENS group, there was a significant increase of experimental pain threshold (p<0.01), plasma $\beta-endorphin$ level (p<0.05), serum cortisol level (p<0,001) and urinary 17-OHCS levels (p<0.05) after TENS application. The plasma ACTH level was not significantly increased, but it showed an increasing tendency. In naloxone group, although there was a decreasing trend, ACTH and cortisol level did not show a significant change, but $\beta-endorphin$ and 17-OHCS level were significantly decreased (p<0.01). The result of this study stewed that acupuncture-like TENS induced analgesic effect, such that the levels of plasma $\beta-endorphin$, plasma ACTH, serum cortisol and urinary 17-OHCS were concomitantly increased with experimental pain threshold. It is suggested that the analgesic mechanism of the acupuncture-like TENS probably related to endogenous opioid component such as $\beta-endorphin$.

Effect of Microcurrent Stimulation at Hand Somatic Acupuncture Points on Experimental Pain Threshold (미세전류가 수부체성경혈점의 실험적 통증 역치에 미치는 영향)

  • The purposes of this study were 1) to examine the effects of microcurrent electrical neuromuscular stimulation 2) to compare surface electrode with needle electrode at somatic acupuncture points on experimental pain threshold measured at the distal end of the radius 3) to determine the changes in effect over time. A total of sixty healthy adult male and female subjects were assigned randomly to one of two experimental group or to a control group. Group 1(n=20) received MENS(Microcurrent Electrical Neuromuscular Stimulation) with surface electrode. Group 2(n=20) received HENS with needle electrode. Group 3(n=20) received no MENS. It measured experimental pain threshold at the wrist on pretreatment, 0 min after treatment. 15 min after treatment, 30 min after treatment in two experimental group. The results were as follows: 1. Experimental pain thresholds were higher in males than females(P<0.01). 2. Only the experimental group exhibited a significant increased in pain threshold after MENS treatment(P<0.05). 3. Surface electrode group increased significant pain threshold 0 min, 15 min after treatment, but greatly decrease 30 min after treatment. 4. Needle electrode group increased significant pain threshold 0 min, 15 min after treatment, but decrease 30 min after treatment. The results suggest that MENS applied to appropriate somatic acupuncture point can increase pain threshold. Further research is needed to assess the effects of greatly variety intensity of MENS of pain sufferes.

The Study of Thermo-physiological Responses with Near Infrared Lighted Garment at a Hot Environment (서열환경에서 근적외선 조사의복 착용시의 온열생리반응)

  • The purpose of this study was to assess the feasibility and the effects of near infrared lighted garments on thermo-physiological responses in human body. Seven healthy adult men were recruited for this study. All subjects were informed the contents and purpose of this study. The experiment was carried out in a climate chamber of $32^{\circ}C$, 60%RH with 'Rest', 'Exercise' and 'Recovery' period. The experimental garments consisted of briefs, undershirts(sleeveless), nightclothes, T-shirts, knee-trousers and socks. Subjects participated in two experiments, one was wearing near infrared lighted garments(NIR-O), the other was wearing regular garments(NIR-X). The order of experiment was randomized, and subjects wore experimental garments before 24 hours in order to benefit by near infrared light. Measurement items included rectal temperature ($T_{re}$), mean skin temperature ($\bar{T}_{sk}$), sweat rate, heart rate, oxygen uptake and subjective sensation. The results are as follows: As to the variation of rectal temperature and mean skin temperature, value of wearing NIR-X was higher than value of wearing NIR-O, indicating a significant level of difference (p<.001). Sweat rate under NIR-O and NIR-X condition were 575.35 g and 535.75 g, respectively. Heart rate value of NIR-X condition was higher than NIR-O. Oxygen uptake measured during experiment was the higher in NIR-X condition with significant difference (p<.001). In the subjective sensation, the value of NIR-O condition was higher than NIR-X condition without significantly difference.

  • On 6 healthy adult males aged 20-30 years, oxygen pulse was observed during carrying sand bags weighing 10, 20 and 30 kg on a level treadmill running with speeds of 3, 4 and 5 km/hr. The results obtained were as follows. 1. Oxygen pulse during load carrying on a level treadmill was proportionately increased with an increase of work load, average oxygen uptake per minute. Regression equation of oxygen pulse (y: $m\iota$/beat) on the average oxygen uptake (x:$\iota$/min) required for each grade of work was expressed as y=3.34x+5.99, $s_{y.x}=0.858$). 2. Oxygen pulse reached the highest value 2-5 minutes after the start of load carrying, and thereafter it decreased gradually to some extend as the load carrying was continued. Rate of decrease in the oxygen pulse was greater in lighter works. 3. In lighter works requiring less than $2.0\iota/min$ of oxygen uptake, no difference in oxygen pulse could be seen between methods of carrying even when the same amount of load was carried. In heavier works requiring more than $2.0\iota/min$ of oxygen uptake, oxygen pulse was larger in case of carrying a load by embrace, in both hands and on head than on back or on shoulder.

Title of Article: Current status of viral disease spread in Korean horn beetle, Allomyrina dichotoma (Coleoptera: Scarabeidae)

  • The current market size of insect industry in Korea is estimated at 300 million dollars and more than 500 local farms are related to many insect industry. One of the strong candidates for insect industry is Korean horn beetle, Allomyrina dichotoma. Early this year, we reported a viral disease extremely fatal to A. dichotoma larvae. While we were proceeding a nationwide investigation of this disease, it was informed that similar disease symptom has been occurred occasionally during past over 10 years. The symptom can be easily confused with early stage of bacterial infection or physiological damage such as low temperature and high humidity. A peroral infection with the purified virus to healthy larvae produced a result that only 21% of larvae survived and became pupae. Although some of the survived adult beetle was deformational, many of them had no abnormal appearance and even succeeded in mating. Later, these beetles were examined if they were carrying the virus, and all except one were confirmed as live virus carrier. This implies that these beetles may fly out and spread the disease to the nature. We found the evidence for this possibility by collecting a few wild A. dichotoma larvae which were virus infected, near two local farms rearing A. dichotoma larvae. So far, transovarial transmission of this virus to the eggs, or horizontal transmission to other commercially reared insects is not known yet.

Chest Radiographic Parameters of Mediastinal Hemorrhage in Patients with Traumatic Aortic Injury Patients (외상성 대동맥손상 환자에서 관찰한 종격동 출혈의 흉부방사선 소견)

  • Background: Traumatic rupture of the aorta is a life-threatening injury that must be diagnosed as rapidly as possible and treated immediately. The chest X-ray is a valuable tool for screening traumatic rupture of the aorta in blunt chest trauma. And various chest radiologic parameters are being used as diagnostic tools for aortic injury. The purpose of this study is to identify chest radiographic parameters that may assist in the detection of traumatic rupture of the aorta and to compare these findings with those of other reports. Methods: This study involved 30 adult patients with traumatic rupture of the aorta seen at the emergency department of the Asan Medical Center from 1997 to 2004. The control subjects were 30 healthy patients with neither lung nor cardiovascular disease. We retrospectively assessed over 14 parameters on chest X-rays. Results: In 11 of the 14 parameters, there were significant differences between the study group and the control group. There was no significant difference in the M/C ratio (mediastinumto-chest width ratio) between the two groups, and neither the left nor the right paraspinal interface was statistically significant (p value>0.05). Our study indicates that new criteria for the MC ratio and for the paraspinal interfaces are needed for screening traumatic aorta injury. The other radiographic parameters for traumatic rupture of the aorta need to be further assessed through a prospective study.

Serum Lipids by Gender, Age and Lifestyle in Korean Adults (일부 한국성인의 성, 연령 및 생활습관에 따른 혈청지질 농도)

  • The purpose of this study was to measure serum lipids concentrations by gender and age, and to investigated the relation of serum lipids levels to cigarette smoking alcohol intake exercise and menopausal status in Korean adults. The subjects consisted of 463 healthy adult(29- men and 173 women) volunteers who were visiting the health Promotion Center in the Asan Medical Center from July 1, 1994 to July 22, 1994 Subjects were ambulatory free-living people and aged 20- 80 years, Anthropometric measurement was performed and serum triglyceride(TG), HDL-cholesterol and total cholesterol(TChol) concentrations were measure by automatic system(Hitachi 736-40). Data on life styles were obtained using structured questionnaires issued by a trained interviewer, All data were statistically analyzed by PC-SAS(Version 6.04) Serum concentrations of triglyceride and total cholesterol was higher(p<0.05) in men than in women, and serum HDL-cholesterol was lower(p<0.05) in men than in women. The prevalence of hyperlipidemia(TG>170 mg/dL and TChol>220 mg /dL) was 12.5% in men and 4.1% in women. The levels of serum triglyceride, total cholesterol and BMI increased with age in women. The distribution curves of serum triglyceride, total cholesterol and LDL-cholesterol of women were located in lower increased more rapidly with aging than in men, The distribution curves of BMI of women increased with aging after the thirties, The BMI of women was located in higher levels than of men in their sixties. The serum concentration of triglyceride and total cholesterol of male current-smokers and non- smokers were significantly(p<0.05) higher than those of ex-smokers. The concentrations triglyceride the male current0drinkers were higher(p<0.05) than those of male non-drinkers. In non-smokers and non- drinkers, serum TG concentration were lower (p<0.05) than those of current-smokers and current-drinkers. In exercisers, the serum triglyceride concentration was significantly(p<0.05) lower than in non- exercisers. In postmenopausal woman BMI and serum concentrations of TChol, TG and LDL-Chol were significantly by age and gender and smoking alcohol drinking, exercise and menopausal staus might be a better indicator to predict serum lipids in both genders.

A Study on Wearing Test of Protective Clothing against a Toxic Substance (유해물질 차단을 위한 방호복의 착용실험에 관한 연구)

  • The purpose of this study was to search for comfortable protective clothing for waste incinerator workers. The experimental protective clothing came in two types: one whose outer side made use of polypropylene film, and the inner side, a non-woven rayon fabric processed with charcoal with a 10% density (CF): and one whose outer side made use of polyolefine and polyamide films, and the inner side, a non-woven polypropylene fabric (NNCF). Experiments were conducted on five healthy adult women whose average age was 21. These experiments were conducted at a climate chamber, in which the temperature and relative humidity were set below $28{\pm}1^{\circ}C\;and\;50{\pm}10%$, respectively. Measured were the rectal temperature, the skin temperature, the sweating rate, the weight loss, the heart rate, the blood pressure, the temperature, and the relative humidity of a microclimate and subjective sensation. These were measured within a period of 60 min, consisting of a 20-min stable period, a 20-min exercise period (walking exercise for 2 miles/hr on a treadmill), and a 20-min recovery period. Through this experiment, the differences between the human body's physiological reactions to CF and NNCF clothing, and the human body's comfort levels when wearing these, were determined.
