국소약물송달제제가 성인형 치주염의 치료에 미치는 효과 (Effects of Controlled-Release Local Delivery Drugs on the Treatment of Adult Periodontitis)

  • The present study was performed to assess and compare the clinical and microbiological effects following local application of 2% minocycline gel or 0.1% chlorhexidine subgingival irrigation to augment scaling and root planing in patients with moderate to advanced chronic adult periodontitis. 32 healthy patients with moderate to advanced chronic adult periodontitis were enrolled in the study. In each patient, the quadrants that had 2 or more teeth with $5{\sim}8mm$ probing pocket depth and radiographic evidence of alveolar bone loss were selected and divided into test side and control side according to the split-mouth design. All patients received standardized oral hygiene instructions at the beginning of the study and all remaining teeth received scaling and root planing until 0 week. The 2% minocycline gel was applied to periodontal pocket at 0, 1, 2, 3 week in the test side. The 0.1% chlorhexidine solution and the normal saline were irrigated subgingivally for about 30 seconds in the positive control side and negative control side respectively. The clinical and microbiological analysis carried out at 0, 4, 8, and 12 weeks . The results of this study were as follows; 1. In saline irrigation group, there was no adjunctive effects in probing pocket depth reduction, sulcular bleeding index and no significant changes in relative proportions of subgingival bacteria. 2. The chlorhexidine irrigation as an adjunct to scaling and root planing results in reduction in the plaque index and sulcular bleeding index, but there was not statistically significant. The relative proportion of spirochetes was significantly reduced, but the proportion of motile rods was no significant reduction. 3. The minocycline gel delivered subgingivally as an adjunct to scaling and root planing provide significant benefit in reducing probing depths and sulcular bleeding index compared to saline and chlorhexidine irrigation groups. 4. The relative proportions of spirochetes and motile rods were significantly reduced and the proportions of cocci and non-motile bacteria were correspondingly increased in the minocycline gel group. In conclusion, minocycline gel delivered subgingivally as an adjunct to scaling and root planing induces clinical and microbial responses more favorable for periodontal health than saline and chlorhexidine subgingival irrigation.

치근면 활택술후 2% minocycline gel의 치료효과 (Effects of 2% minocycline gel as an adjunct to scaling and root planing on the treatment of adult periodontitis)

The results of this st udy were as follows; 1.2% minocycline gel delivered subgingivally as an adjunct to scaling and root planing provided benefit in reducing sulcular bleeding index and pocket depth than the use of normal saline. 2. The relative proportion of cocci and non-motile bacteria was increased in the test and control groups with time, and there was no statistically significantdifference between two groups. 3. The proportion of spirochetes was slowly reduced in the control group, but, inthe test group, they were remarkably reduced from the 4th week, and there was a statistically significant difference between two groups. 4. In both groups, the relative proportion of motile rods was notably decreasedat the beginning of the study, and remained until 12th week in the test group,but, in the control group, they were slowly increased from the 4th weekand finally similar to that of the initial examination. In conclusion, local application of 2% minocycline gel may be effective in the clinical and microbiological aspects as an adjunct to scaling and root planing in periodontal disease sites.

한국 성인의 TEMPA 표준치, 손기능과 근력에 관한 연구 (Normative Data of Upper Extremity Performance Test for Elderly (TEMPA) for Korean Adult Population and Characteristics of Hand Function and Strength)

  • 목적 : 본 연구의 목적은 한국 성인의Upper extremity performance test for elderly (TEMPA) 표준치를 제시하고, 수집된 자료를 바탕으로 성인의 손 기능의 특성과 근력의 차이를 연령별, 성별로 알아보는 것이다. 연구방법 : 연구에 참여한 대상자는 20세에서 59세 사이의 손 기능에 장애가 없는 성인 138명이었다. 손 기능의 특성과 근력을 평가하기 위하여 모든 대상자에게 TEMPA, Jarmar dynamometer, pinch gauge를 시행하였다. 결과 : 성인의 손 기능에는 연령별, 성별 차이가 통계적으로 유의미한 차이를 보이지는 않았지만, 40대와 50대에서 수행 속도가 감소하는 경향을 보였다. 성인의 근력은 40대에서 가장 높은 수준의 근력을 보였으며 50대에서 감소하는 경향을 보였지만 대부분의 결과에서 유의미한 차이는 없었다. 결론 : 작업치료사는 중년 성인의 손기능과 근력에서 나타나는 감소의 전조 증상에 대한 관심과 그 기능 수준을 유지하기 위한 교육 프로그램을 제공하는 것이 필요할 것이다. 본 연구를 통해 한국 성인의 TEMPA 표준치가 확립되었다. 이 표준치가 임상적 평가 및 근거 기반연구에 있어 유용한 자료가 되기를 기대한다.

성인여성의 식습관과 영양섭취상태와 골밀도 및 골무기질함량과의 관계 (The Relationship between Food Habit, Nutrient Intakes and Bone Mineral Density and Bone Mineral Content in Adult Women)

  • Bones are important parts in sustaining the shape of the body, but they are also metabolic organs which undergo bone remodeling by constant bone resorption and formation. Osteoporosis, the typical metabolic bone disease, is characterized by a reduction in bone mineral density (BMD). Women more than men are at risk fir osteoporosis-related fractures, especially in the lumbar spine, wrist, and hip region. Risk of fracture depends on one's BMD, which open determined by the peak bone mass value achieved at skeletal maturity and followed by subsequent age-and menopause-related bone loss. Genetic and environmental factors are known to play a key role in bone metabolism and diet is considered as one of the important environmental factors. The purpose of the present study was to assess the status of BMD and bone mineral content(BMC) to clarify the relationships between dietary intakes and the risk of osteoporosis in adult women in Taegu. Subjects were 130 healthy females in between 20 and 69 years of age. BMD and BMC of the lumbar spine(venebrae L2-4) of the subjects were measured by dual energy X-ray absorptiometry. The average age of the subjects was 47.4${\pm}$11.7 years old, the average weight was 57.2${\pm}$8.4kg, the average age of menarche was 16.6${\pm}$1.9 years old and the average age of menopause was 48.4${\pm}$5.3 years old. The nutrient intakes of the subjects measured by the convenient method were generally lower than the level of RDA. The result of nutrient intake assessed has shown that the average energy intake was 1701${\pm}$316kca1 which is 85.1% of the RDA and the average calcium intake was 485.4${\pm}$172.3mg which is 69.3% of the RDA. The intakes of protein, vitamin A, vitamin B$_1$, niacin were greater than the RDA, whereas the remaining nutrient intakes were lower than the RDA. The average BMD of the subjects was shown to be 1.06${\pm}$1.09g/$\textrm{cm}^2$. The highest BMD of 1.24${\pm}$0.14g/$\textrm{cm}^2$ was noticed in the subjects of 30s compared to 20s, 40s, 50s, 60s. The BMD values were compared by the relative body weight(R3W) of the menopause subjects, and it was found that the underweight group had significant lower BMB while the rest of the groups did not have any differences in BMD. The most strongly correlated nutrient with BMD among the menopause subjects appeared to be calcium. The women whose Ca intakes were higher than 500mg showed the significantly higher BMD than those with Ca intakes lower than 500mg. This study suggests that the most effective way to prevent osteoporosis and to reduce the incidences of fractures seems to be minimizing bone loss through the adequate intake of calcium as well as avoiding underweight, especially in menopausal women. 흰가루병에 대해 성체식물 저항성을 지닌 봄보리에서 $^{14}CO_2$ 동화와 $^{14}C-$동화산물의 대사 ([ $^{14}CO_2$ ] Assimilation and Metabolism of $^{14}C-$Assimilates in Whole Plants of Spring Barley In Relation to Adult-Plant Resistance to Powdery Mildew)

  • 흰가루병에 대하여 감수성인 봄보리품종 Peruvian과 성체식물 저항성품종 Asse의 4엽기에서 흰가루병에 감염되었을 때 $^{14}CO_2$동화와 $^{14}C-$동화산물의 대사변화를 연구하였다. 건전식물에서는 Peruvian과 Asse간에 $^{14}CO_2$동화와 $^{14}C-$동화산물 대사에서 차이가 어 없었다. 감염된 엽수가 늘어남에 다라 이들 두 품종에서 $^{14}CO_2$ 동화와 동화산물의 이행량은 감소하였다. 감염정도가 같을때도 Asse보다 Peruvian에서 $^{14}CO_2$동화가 더 감소되었다. 흰가루병이 감염되지 않은 제4엽에서도 $^{14}CO_2$고장이 줄어들었고, Peruvian에서 더 줄었다. 아래 3엽이 감염되었을 때 감염되어 있지 않는 상위 제4엽에서 fructose, glucose등의 탄수화물로 $^{14}C$가 결합되는 것을 억제하였고 Asse보다 Peruvian에서 억제가 더 컸다. 감염된 제3엽에서도 탄수화물내에 $^{14}C$이 줄어들었으나 Peruvian에서 $^{14}C$로 표식된 fructose, glucose함량이 더 높았다. 모든 식물기관에 $^{14}C$-유기산합성이 촉진되었고 Asse보다 Peruvian에서 합성이 더 뚜렷하였다. 감염된 제3엽이나 건전제4엽에서 $^{14}C$이 아미노산으로 결합되는 것이 증가하였으나 엽초로 이동은 감소되었다. 이들 잎에서 어떤 $^{14}C$-아미노산은 Asse보다 Peruvian에 더 많이 축적되었다.

Morphologic Alterations in Amygdala Subregions of Adult Patients with Bipolar Disorder

  • Objectives Previous studies have revealed inconsistent results on amygdala volume in adult bipolar disorder (BD) patients compared to healthy controls (HC). Since the amygdala encompasses multiple subregions, the subtle volume changes in each amygdala nucleus might have not been fully reflected in the measure of the total amygdala volume, causing discrepant results. Thus, we aimed to investigate volume changes in each amygdala subregion and their association with subtypes of BD, lithium use and clinical status of BD. Methods Fifty-five BD patients and 55 HC underwent T1-weighted structural magnetic resonance imaging. We analyzed volumes of the whole amygdala and each amygdala subregion, including the anterior amygdaloid area, cortico-amygdaloid transition area, basal, lateral, accessory basal, central, cortical, medial and paralaminar nuclei using the atlas in the FreeSurfer. The volume difference was analyzed using a one-way analysis of covariance with individual volumes as dependent variables, and age, sex, and total intracranial volume as covariates. Results The volumes of whole right amygdala and subregions including basal nucleus, accessory basal nucleus, anterior amygdaloid area, and cortico-amygdaloid transition area in the right amygdala of BD patients were significantly smaller for the HC group. No significant volume difference between bipolar I disorder and bipolar II disorder was found after the Bonferroni correction. The trend of larger volume in medial nucleus with lithium treatment was not significant after the Bonferroni correction. No significant correlation between illness duration and amygdala volume, and insignificant negative correlation were found between right central nucleus volume and depression severity. Conclusions Significant volume decrements of the whole amygdala, basal nucleus, accessory basal nucleus, anterior amygdaloid area, and cortico-amygdaloid transition area were found in the right hemisphere in adult BD patients, compared to HC group. We postulate that such volume changes are associated with altered functional activity and connectivity of amygdala nuclei in BD.

성인의 치매 지식, 치매 두려움 및 치매 예방행위 의도가 치매 예방교육 요구에 미치는 영향요인 (Factors Influencing Adult Dementia Knowledge, Fear of Dementia, and Intention of Dementia Prevention Behavior on the Demand for Dementia Prevention Education)

  • 본 연구는 성인의 치매 지식, 치매 두려움 및 치매 예방행위 의도가 치매 예방교육 요구에 미치는 영향요인을 분석하고자 수행되었다. 연구대상자는 충남, 충북에 거주하고 있는 19~64세 성인으로 연구의 목적을 이해하고 연구에 참여할 것을 동의한 265명을 대상으로 설문조사하였다. 수집된 연구 자료는 빈도, 백분율, 평균, 표준편차, t-test, ANOVA, 다중 회귀분석을 이용하여 분석하였다. 연구 결과 성인의 치매 지식과(p=.055) 치매 두려움은(p=.302) 치매예방교육 요구에 유의한 영향을 미치지 않는 것으로 나타났고, 치매 예방행위 의도가(β=.329, p<.001) 치매 예방교육요구에 유의한 영향을 미치는 것으로 나타났다. 이 같은 연구결과를 토대로 치매의 이해와 치매관리사업, 치매조기검진과 치매예방 건강수칙, 치매예방을 위한 만성질환 관리, 치매예방을 위한 운동, 치매예방을 위한 건강한 식생활, 치매예방을 위한 인지증진프로그램 등 국가의 치매 예방 홍보 및 국가 차원의 대응 노력과 더불어 지역사회를 중심으로 치매예방 프로그램에 적극 참여하여 올바른 치매 예방행동을 실천하고 예방할 수 있는 기회를 충분히 제공해야 한다고 사료된다.

사무직근로자의 요추부 굴곡근 및 신전근의 등속성 근력평가 서울시 일부지역의 21세 $\sim$ 49세 남자직장인을 중심으로 (Isokinetic Evaluation of the Trunk Flexors and Extensors for the White Collar Workers in Adult Males)

  • After warming-up exercise for 20 minutes, Isokinetic measurement of trunk strength for flexor and extensor was done by using Cybex 6000 TEF Unit on 91 healthy male white workers from 22years old to 49 years old, and compared each other. 20 repetitions of trunk extension-flexion were done at $120^{\circ}$/sec angular velocity. After resting for 1 minutes, Four repetitions at two different angular velocities($60^{\circ}$/sec, $120^{\circ}$/sec) were done with 30 seconds of resting interval between each angular velocity. The purpose of this study is to obtain the isokinetic normative strength values for trunk extensors and flexors, and is to know the correlation between age, height, weight of subjects and data from isokinetic trunk strength measurement, and is to provide a guideline for exercise program of male white collar workers The collected data were analyzed by ANOVA, Duncan's Multiple Range Test, and Pearson correlation coefficiency in PC-SAS program. The results obtained were as follow; 1. There is significant positive-correlation with the statistic value between weight and peak torque of trunk muscles at two different angular velocities($60^{\circ}$/sec, $120^{\circ}$/sec)(p<01), between height and peak torque of trunk muscles at two different angular velocities($60^{\circ}$/sec, $120^{\circ}$/sec) except peak torque of trunk flexor at $60^{\circ}$/sec(p<01). 2. There is nagitive-correlation between age and peak torque of trunk muscles at two different angular velocities($60^{\circ}$/sec, $120^{\circ}$/sec), there is significant differences with statistic value between age and peak torque of trunk extensor at $120^{\circ}$/sec(p<.01). 3. Mean peak torque and mean peak torque % by body weight of trunk extensor is 1.1 times higher values than trunk flexor at $60^{\circ}$/sec. 4. There is the increase in peak torque angle of trunk flexor with increasing of age, and the decrease in peak torque angle of trunk flexor with increasing of age at two different angular velocities($60^{\circ}$/sec, $120^{\circ}$/sec). there is significant differences with statistic value in peak torque angle of trunk flexor at $120^{\circ}$/sec(p<.01). 5. There is significant decrease in endurance ratio of trunk extensor with increasing of age at $120^{\circ}$/sec(p<.01). In conclusion, peak torque of trunk extensor is 1.1 times higher values than trunk flexor in healthy male white collar workers.

초음파 검사를 시행한 자궁근종 환자 258명의 사상 체질 의학적 분포 연구 (Study on Sasang constitutional medical distributions of 258 patients with uterine myoma who underwent ultrasonography)

  • Objectives: The purpose of this study was to examine whether Sasang medical constitutional distributions of patients with uterine myoma are significantly different from those of healthy women in Korea. Methods: The subjects for this study were 258 patients who were diagnosed with uterine myoma by ultrasonography and classed through a questionnaire for the Sasang Constitution Classification II (QSCC II) among patients who entered Kyunghee Bogung oriental medical clinic from June 7th, 2011 to March 7th, 2012. Results: 1. The average size of myoma of subjects was $5.16{\pm}2.48$ and the average age was $37.1{\pm}7.2$ years old. The average age of menarche was $13.13{\pm}1.44$ years old and average frequency of abortion was $0.65{\pm}1.07$. There were no significant differences by constitution in size of myoma, age, marital status, age of menarche, history of giving birth or history of abortion. 2. The overall average BMI of all subjects in this study was 21.4. By constitution, average BMIs were $24.2{\pm}3.2$, $20.5{\pm}2.0$ and $20.2{\pm}2.2$ for Taeumin, Soyangin and Soeumin, respectively. There was a significant difference by constitution. 3. Sasang medical constitutional distributions of subjects was as follows: Taeumin 28.3% (73 patients), Soyangin 35.7% (92 patients), Soeumin 36.0% (93 patients). When compared with those of healthy Korean adult females who did not have uterine myoma (44.4%, 14.8% and 40.9%, respectively), the proportion of Soyangin was significantly higher. Conclusions: We found that Sasang medical constitutional distribution of subjects differs from those of healthy Korean women without uterine myoma and in particular, among Soyangin is significantly higher.

초음파에 의한 전완 국소 온도와 정중 운동 신경전도 속도의 변화 (Alteration of Forearm Local Temperature and Median Nerve Conduction Velocity by Therapeutic Ultrasound in Healthy Adult Subjects)

  • PURPOSE: Previous studies have documented the lack of ultrasound's non-thermal effects on nerve conduction using frequencies of 1 MHz and 870 kHz. The purpose of this study was to determine the biophysical effects of continuous ultrasound on median local forearm temperature and motor nerve conduction velocities using frequencies of 3.0 MHz. SUBJECTS: Twelve healthy subjects (6 males, 6 females, age $22.30{\pm}2.41$ yrs, weight $61.33{\pm}10.16$ kg, height $167.58{\pm}8.04$ cm) without a history of neurological or musculoskeletal injury to their dominant arm volunteered for this study. METHODS AND MATERIALS: Each subject received a total of five treatments, one each at .0, 0.5, 1.0, 1.5, 2.0 W/$cm^2$ of 3 MHz continuous ultrasound on the anterior surface of the middle area of dominant forearm for 10 minutes. Dependent measures for forearm local temperature and median motor nerve conduction velocity (MNCV) were taken pretreatment and immediately post-treatment. One-way ANOVA were used for each dependent measure. RESULTS: The posttreatment forearm local temperature were differed significantly (p<0.001) between intensities of ultrasound. The posttreatment forearm local temperature of the ultrasound treated with 1.0 w/$cm^2$, 1.5 w/$cm^2$ and 2.0 w/$cm^2$ were significantly higher than 0.5 w/$cm^2$ and 0.0 w/$cm^2$ of ultrasound (p<0.05). The posttreatment median MNCV were differed significantly from the respective pretreatment velocities (p<0.001). The MNCV of the ultrasound treated with 0.0 w/$cm^2$ and 0.5 w/$cm^2$ were significantly (p<0.05) slower than that observed pretreatment, while the three ultrasound intensities produced significantly increased posttreatment MNCV: 1.0 w/$cm^2$ and 1.5 w/$cm^2$ and 2.0 W/$cm^2$. The posttreatment MNCV at 2.0 w/$cm^2$ and 1.5 w/$cm^2$ was significantly faster than that at 0 w/$cm^2$, 0.5 w/$cm^2$ and 1.0 w/$cm^2$ (p<0.05), the MNCV at 1.0 w/$cm^2$ was significantly faster than that associated with 0 w/$cm^2$ and 0.5 w/$cm^2$ of ultrasound (p<0.05). CONCLUSIONS: The decreased median motor forearm local temperature and MNCV of the ultrasound treated with 0.0 w/$cm^2$ and 0.5 w/$cm^2$ were attributed to the cooling effect by ultrasound transmission gel. Local forearm temperature and nerve conduction velocity were directly related to the intensity of ultrasound. Alterations in MNCV from ultrasound on healthy nerves appeared to be related to temperature changes induced by thermal effects of ultrasound.
