  • The cultural assets of the palaces royal tombs remains institution expanded the convenient facilities for the disabled that can help the disabled to watch, to propel the convenient facility expanding business according to the condition of each facility. However, there has been limitations for the disabled facility users in viewing the cultural assets, as the facilities were not installed or were not installed properly. Due to the uniform installation of the convenient facilities that did not consider the features of the cultural assets, the conservation value of the cultural assets have been depreciated and the convenient use standards of the facility users such as the disabled were simultaneously unsatisfactory. Thus, this study maintains the pre-existing value of the palaces royal tombs remains institution that is the Korean indigenous traditional heritage, and uses a part of the improvement model development study on the palaces royal tombs remains institution of the disabled convenience enhancement to understand the current convenient facility installation and relevant services to find the restricted factors in using the facilities and the main problems. The problems were categorized into 13 types of questions, using the repeated problems occurred within the investigated facilities to look at the actual condition of the facilities, which showed that the facilities were in adequate in the aspects of convenient facility installation and relevant service.

정신의료기관 평가와 인증 기준의 공간 및 요구사항 연구 (A study on the space and requirements of evaluation and certification criteria for psychiatric institutions)

  • Purpose: The study aimed to extract the space mentioned in the criteria for evaluation and certification of psychiatric institutions in which the most specific standards for medical services of psychiatric institutions are prepared, and to organize and analyze the requirements. Method: The implied space is derived by analyzing the evaluation and certification criteria. The derived space is re-classified by departments of the hospital, and the requirements are organized and analyzed. Results: First, a total of 41 spaces were derived. The derived spaces can be considered as spaces to be treated as important in psychiatric institutions. Second, as a result of reclassifying the derived space by department, 10 spaced for the hospital as a whole, 10 for wards, 12 for central/outpatient departments, 5 for pharmaceutical departments, 2 for catering facilities, and 2 for facilities. Third, if organized by psychiatric institution, there is a total of 40 spaces for psychiatric hospitals according to certification criteria, and according to evaluation criteria, there is 38 spaces for psychiatric hospitals and departments and 19 spaces for the clinics. The difference between the certification and evaluation criteria of psychiatric hospitals is insignificant. In order to promote the qualitative improvement of psychiatric institutions, it is necessary to gradually strengthen from the certification criteria. Fourth, the requirements reflecting the characteristics of a psychiatric institution were very limited to emergency exits and treatment rooms. It is necessary to prepare the space requirements for other rooms in consideration of the behavior of the mentally ill. Implications: The derived spaces and requirements can serve as practical reference materials for practitioners preparing to receive certification and evaluation of psychiatric institutions. In addition, it has meaning as a basic data for estimating the level of space and requirements of psychiatric institutions that are currently required in Korea.

노인보건의료의 현황과 법 제도적 개선방안 (Improvement Devices on the Law and Institution and Current Situation of Health and Medical Treatment for the Aged)

  • 인구가 고령화되면서 전체 인구의료비 가운데 고령인구에 지출되는 의료비의 비중은 증가하여, 건강보험 의료급여 등의 재정을 압박하는 경향은 갈수록 심화될 전망이다. 노인성 질환으로 인해 의료비용 부담이 증가함으로서 사회적 문제로 발전됨에 따라 노인보건의료의 현황을 분석하고, 현행 관련법제도를 살펴보고 문제점을 도출하고자 하였으며, 외국의 노인보건실태와 동향을 토대로 시사점을 도출하여 우리나라에의 법제도적 개선방안을 제시하고자 하였다. 그 결과 현행 노인보건의료보장의 관련법 체계의 문제점, 현행 노인장기요양보험법상의 문제점을 도출하였고, 법적 개선방안으로서는 건강보험의 보장성 강화, 장기 요양보험료 재정의 건전성 확보문제, 노인복지와 장기요양보험제도의 연계와 상호보완 기능을 강화, 치료요양에 대한 노인장기요양보험제도와 노인복지법의 중복성 문제, 등급판정체계의 개선, 재가서비스 지원강화 등 노인의 특수성을 반영하여 의료서비스의 질적 수준의 개선이 필요하다고 보고 노인장기요양법제의 개선과제를 제기하는 등 노인의료서비스의 지원확대 방안을 제시하였다. 노인을 대상으로 하는 중증응급 의료서비스의 질을 제고하기 위해서는 적정 응급의료자원의 확보뿐만 아니라, 그 효율적 운영체계의 마련도 필요하다. 아울러 급증하고 있는 노인의료비에 대응하기 위해서 "노인의 의료확보에 관한 법률"을 제정할 필요성을 제시하였다.

Situation Analysis of Existing Facilities for Screening, Treatment and Prevention of Cervical Cancer in Hospitals/Primary health Centers of Delhi-NCR Region, India

  • Cervical cancer, the second most common malignancy all over the world, is associated with HPV infection. In a developing country like India, lack of early detection and treatment facilities is the main cause for its high burden. Therefore, through our study we e tried to present the current scenario of existing facilities for the detection and treatment of cervical cancer in hospitals and primary health centers (PHCs) of Delhi-NCR region. Data were collected from 312 healthcare facilities including public and private hospitals and PHCs of all nine districts from Delhi-NCR region. Healthcare providers including gynecologists, medical officers, women health care providers and paramedical staff were interviewed, using a questionnaire; the facilities for screening, diagnosing, and treating cervical cancer in each institution were recorded, using a previously designed checklist. Our study has shown that the basic facilities for the detection and treatment of cervical cancer are abhorrently lacking in Public hospitals and PHCs as compared to the Private hospitals in Delhi-NCR region. This study demonstrates that there is an urgent need for more investment in the diagnosis and treatment of cervical cancer facilities in public and rural healthcare facilities of Delhi-NCR region.

의료기관인증이 병원 근로자가 지각하는 환자안전과 질, 직무만족 및 조직몰입에 미치는 효과 -일개 종합병원을 대상으로- (The impact of healthcare accreditation on patient safety and quality, job satisfaction and organizational commitment: about a general hospital)

  • 본 연구는 의료기관인증이 병원 근로자가 지각하는 환자안전과 질, 직무만족 및 조직몰입에 미치는 효과를 평가하기 위한 단일집단 사전 사후 검사 설계연구이다. 대상자는 서울시내 일개 종합병원에서 1년 이상 근무한 405명을 대상으로 하였다. 시험처치는 2012년 4월부터 준비를 시작하여 2012년 11월 의료기관평가인증원으로부터 조사를 받은 의료기관인증이며, 자료 분석은 SPSS(version 18.0) 프로그램을 사용하여 paired t-test와 One-Way ANOVA로 검정하였다. 연구결과, 연구결과, 의료기관인증 후 병원근로자가 지각하는 환자안전과 질(t=7.28, p=.000), 직무만족(t=7.13, p=.000) 및 조직몰입(t=3.44, p=.001)은 통계적으로 유의하게 증가하였다. 따라서 의료기관인증은 환자안전과질, 직무만족 및 조직몰입을 증진시키는 데 효과가 있다고 볼 수 있다.

환자 정보를 빅 데이터화 하기 위한 유헬스케어 서비스 관리기법 (U-healthcare Service Management Scheme for Big Data of Patient Infomation)

  • 최근 현대인의 식습관에 의해서 질병의 예방, 관리, 건강증진 등을 제공하는 유헬스케어 서비스의 트랜드가 급속하게 변화하고 있다. 그러나, 유헬스케어 서비스를 제공받는 사용자의 질병정보가 관리서버에 저장되지 않거나 저장되더라도 분석되지 못하는 상황이 발생되어 사용자의 의료서비스에 불편을 주고 있다. 본 논문에서는 유헬스케어 서비스를 제공받는 사용자의 질병 정보를 빅 데이터화하여 시간과 장소에 상관없이 사용지의 빅 데이터 정보를 통해 사용자의 의료 서비스를 원활하게 하는 유헬스케어 서비스 관리기법을 제안한다. 제안 기법은 사용자의 생체신호 및 건강정보를 측정하고 유무선 통신을 통해 데이터를 의료기관에 전송하며 의료기관에서는 사용자의 의료정보를 빅데이터화하여 사용자의 의료 정보를 분석한 후 다시 사용자에게 피드백 하여 사용자의 질병을 원격 관리한다.

  • 본 연구는 접근성과 의료서비스 품질, 대외적 이미지를 높임으로써 향후 지방의료원이 지역 내 대표의료기관으로서 경쟁력을 확보하고 경영효율성을 향상시키는데 그 목적이 있다. 조사대상은 604명으로 하였으며, 분석방법은 ANOVA, 공분산 구조방정식을 실시하였다. 결론을 보면, 의료기관 선택요인에 대한 고객들의 만족도나 선호도가 인구통계학적 특성별로 유의한 차이가 있다는 결과는 앞으로 지방의료원이 의료서비스 전문화 전략을 수립할 때 주요 고객집단의 욕구에 대한 면밀한 검토가 이루어져야 할 필요가 있음을 시사하고 있다. 또한 지방의료원이 진료비 수준에 대한 치료효과와 타 의료기관에 비해 저렴한 진료비가 경쟁우위를 확보할 수 있는 중요요인으로 이에 따른 이미지 부각과 의료서비스의 질의 향상을 극대화하여 의료소비자들을 유인하는 전략이 필요하다.

디지털헬스케어 시장과 O2O서비스 소비자분쟁 및 보호방안 (A Study on the Consumer Disputes and Protection Measures of the Digital Healthcare Market and O2O Service)

  • The O2O services in the healthcare sector have only been in full swing for about three years, and unlike existing O2O consumer goods, the scale and scope of the dispute are more complicated due to restrictions on medical treatment. In this study, O2O service platform operators and medical institutions' roles and responsibilities were redefined as a countermeasure for resolving disputes in healthcare O2O services and the laws for changing the transaction environment. A change in institutional mechanisms was proposed. This study looked at the types of consumer disputes related to healthcare O2O services as insufficient information problems, problems in the course of medical service implementation, problems with immunity provisions for platform operators, cancellations, and non-compliance with refunds. All the information generated during transactions in the healthcare sector was extensive in scale and included the most sensitive information among personal information, stressing the importance of ensuring security. The area that started in the O2O range before the medical institution visit also proposed a plan to establish a system for the delivery of proven information as a pre-medical person. The scale and growth will grow faster, given that consumers can experience the information they want anytime, anywhere they want. However, the platform broker's role, a link player, will become more important because consumers who use the service will have their first meeting with non-face-to-face product providers. On the other hand, service providers may have side effects of misleading consumers by providing false information or misleading consumers through exaggerated advertisements. The O2O service market is expected to expand beyond distribution and dining out to the entire industry. However, since it is challenging to check accurate statistics on the detailed market, various disputes and consumer protection measures will be required for each detailed market, and comprehensive leading solutions will be essential in the future.

의료기관 인공지능 챗봇 이용자의 인구사회학적 특성과 챗봇의 사회적 실재감 및 신뢰감의 관련성 연구 - 성별과 연령 중심으로 (The association between the social presence and trust of chatbots and the sociodemographic characteristics of artificial intelligence chatbots users in general hospitals : focusing on sex and age)

  • Objectives: This study explores the impact of age groups on social presence and trust among users of medical artificial intelligence chatbots. Furthermore, we investigate the existence of gender differences within these relationships. Method: We collected data through a survey from people who had interacted with general hospital chatbot services, either by making reservations or seeking consultations. Multiple linear regression analysis was conducted to examine the relationship between general characteristics of study population and social presence and trust of artificial intelligence chatbots. Additionally, we conducted stratified analysis to confirm the presence of gender differences within these relationship. Results: Among 300 participants, those aged 50 and older had higher social presence of artificial intelligence chatbots and greater trust of artificial intelligence chatbots (social presence, 𝛽=0.543, p=0.003; trust, 𝛽=0.787, p=0.000). In stratified by sex, women aged 50 and older had higher social presence and trust of artificial intelligence chatbots compared to those in their 30s age group (social presence, 𝛽 = 0.925, p=0.002; trust, 𝛽=0.645, p=:0.007). However, there was no statistically significant relationship between age and chatbot social presence and trust in men. Conclusion: This study demonstrates that advanced age plays a significant roles in users' social presence and trust in medical artificial intelligence chatbots. Futhermore, our findings reveal gender differences with women aged 50 and older showing the most substantial levels of social presence and trust. Therefore, it is expected that this finding can serve as valuable evidence to enhance the satisfaction of medical institution service users, offering crucial insights into the effective utilization of chatbot services.

  • This paper proposes a portable electrocardiograph and smart device-based heart health monitoring and risk notification system. The proposed system consists of a portable electrocardiograph and a smart device for a system user, and a web-based monitoring system for observers. This system can improve the convenience and efficiency of measurement by using a light-weight portable electrocardiograph and a smart device. In addition, any authorized person such as caregiver or family member who is not related to medical institution can monitor users'heart health in real-time using the web-based monitoring system. Therefore, a user and authorized remote observers can efficiently monitor and manage user's heart health in daily-life even without any medical institution's help, and can preemptively deal with any possible dangerous situations, such as degeneration of a cardiac disorder and sudden cardiac death.
