빛공해 분야의 환경영향평가 지침 표준화 연구 (Study on the Standard Guideline of Environmental Impact Assessment Focusing on Light Pollution)

  • 인공조명은 필수적인 부분이지만, 불필요하거나 과도한 빛의 사용으로 인해 빛공해를 유발시킨다. 빛공해는 전력낭비, 인체의 건강상 악영향, 생태계 파괴 등과 같은 부정적인 영향으로 국 내외에서 법적인 규제를 하고 있다. 하지만 현재는 조명기구 설치 이후의 방법으로만 관리하기 때문에 사전에 빛공해가 유발될 우려가 있는 지역을 효과적으로 관리하고 영향을 저감시키기 위한 빛공해분야 영향평가지침을 제안하고자 한다. 영향평가작성지침을 수립하여 작성자 및 검토자의 혼란을 방지하고 중장기적으로 국내의 빛공해 관리를 효과적으로 수립하는데 활용하고자 한다.

뇌졸중 환자의 체간조절, 균형능력에 기계적 승마치료가 미치는 영향 (Effects of Mechanical Horseback Riding Training on Trunk Control and Balance function in Stroke patients)

  • 본 연구는 기계적 승마치료가 뇌졸중 환자의 체간조절, 균형능력에 미치는 영향을 평가하기 위해 실시되었다. 본 연구에 15명의 뇌졸중 환자가 참여하였다. 연구 대상은 뇌줄중 환자를 실험군과 대조군으로 각각 7명, 8명씩 무작위로 배정하였다. 모든 환자들은 보존적 물리치료를 받았으며, 실험군에게 추가적으로 기계적 승마치료를 30분동안 시행하였다. 연구는 2013년 3월 4일부터 5월 3일 까지 약 8주간 진행되었다. 본 연구의 평가 도구는 체간조절 검사, 뇌졸중 자세조절 검사, 버그 균형 척도, 일어나서 걷기 검사였다. 측정은 중재 전, 후로 실시하였다. 중재 후 군내에서 실험군과 대조군의 체간조절 검사, 뇌졸중 자세조절 검사, 버그 균형 척도, 일어나서 걷기 검사의 중재 전, 후 값에서 유의한 차이를 보였다(p<.05). 실험군은 중재 전, 후 차이값이 체간조절 검사, 뇌졸중 자세조절 검사, 버그 균형 척도, 일어나서 걷기 검사에서 각각 $27.85{\pm}19.03$, $6{\pm}4.04$, $19.28{\pm}4.27$, $16.68{\pm}4.61$ 향상되었고, 대조군에서는 각각 $17.87{\pm}18.3$, $2{\pm}2.07$, $7.62{\pm}6.75$, $3.21{\pm}1.75$ 향상되었다. 두 군간의 중재 후의 평가 비교에서 중재 후 체간조절 감사, 뇌졸중 자세조절 검사, 버그 균형 척도, 일어나서 걷기 검사에서 유의한 차이가 있었다(p<.05). 이는 기계적 승마치료가 뇌졸중 환자의 체간조절 능력, 균형 능력에 긍정적인 영향을 미치는 것을 의미하여 앞으로 이러한 연구결과를 일반화시키는 것이 필요할 것이라고 사료된다.

간이 영양상태 평가법에 의한 급식서비스 이용 노인의 영양 위험도와 관련 요인 (Nutritional Risk and its Related Factors Evaluated by the Mini Nutritional Assessment for the Elderly who are Meal Service Participants)

  • This study is designed to assess the prevalence at risk of malnutrition according to the Mini Nutritional Assessment (MNA) and evaluate the factors influencing on the nutritional risk of the elderly. Three hundred and nine elderly (110 men and 199 women: mean age =74.1) who participated in meal service in the Chung-buk province were investigated. Mean MNA total score was 21.9 and women had significantly lower MNA scores than men (respectively, 21.5 and 22.8). In the mean time mean MNA-SF (Short Form) score was 10.7, respectively 10.6 for the women and 11.0 for the men, with the difference being statistically significant. The MNA classified 33% of the elderly as well-nourished, 61.7% as at risk of malnutrition and 5.3% as overt malnourished. However, MNA-SF categorized the examinees 40.2% as good and 59.8% at nutritional risk. Those who identified as malnourished elderly had significantly lower mean BMI, mid-arm and calf circumference, poorer functional abilities (ADL, IADL) , lower MAR and food habits scores, and higher number of nutrient $\leq$ 75% of RDA than those with at risk of malnutrition and well nourished. Also socioeconomic status such as educational level, self-rated economic status, poverty level, and marital status significantly influenced nutritional status. Similar effect was observed in self-rated nutritional status and health status, dental status, appetite change according to MNA score. Stepwise multiple regression analysis indicated that weight loss was the most predictive item in the total MNA and MNA-SF score. It was found that items such as mobility, living status (home vs institution) , mode of feeding, and pressure sores were inappropriate for assessment of the elderly who are able to participate meal service program. Also, some modifications of items in MNA are needed in order to apply to Korean elderly. Even though the MNA seems to be an useful tool to screen those old people at risk of malnourished, a lot of work is still to be done with this assessment tool to secure its reliability.

Risk Assessment of Triclosan, a Cosmetic Preservative

  • Triclosan (TCS) is an antimicrobial compound used in consumer products. The purpose of current study was to examine toxicology and risk assessment of TCS based on available data. Acute toxicities of oral, transdermal and inhalation routes were low, and phototoxicity and neurotoxicity were not observed. Topical treatment of TCS to animal caused mild irritation. TCS did not induce reproductive and developmental toxicity in rodents. In addition, genotoxicity was not considered based on in vitro and in vivo tests of TCS. It is not classified as a carcinogen in international authorities such as International Agency for Research on Cancer (IARC). No-observed-adverse-effect level (NOAEL) was determined 12 mg/kg bw/day for TCS, based on haematoxicity and reduction of absolute and relative spleen weights in a 104-week oral toxicity study in rats. Percutaneous absorption rate was set as 14%, which was human skin absorption study reported by National Industrial Chemicals Notification and Assessment Scheme (NICNAS) (2009). The systemic exposure dosage (SED) of TCS has been derived by two scenarios depending on the cosmetics usage of Koreans. The first scenario is the combined use of representative cosmetics and oral care products. The second scenario is the combined use of rinse-off products of cleansing, deodorants, coloring products, and oral care products. SEDs have been calculated as 0.14337 mg/kg bw/day for the first scenario and 0.04733 mg/kg bw/day for the second scenario. As a result, margin of safety (MOS) for the first and second scenarios was estimated to 84 and 253.5, respectively. Based on these results, exposure of TCS contained in rinse-off products, deodorants, and coloring products would not pose a significant health risk when it is used up to 0.3%.

Risk Assessment of Ethylhexyl Dimethyl PABA in Cosmetics

  • Ethylhexyl dimethyl para-aminobenzoic acid (PABA) is an oily yellow liquid derivative of water-soluble PABA commonly used in sunscreen. Ethylhexyl dimethyl PABA is widely used as an ingredient in many cosmetics at an average concentration of 1.25% (0.5-2.0%) in Korea. Previous studies, including those involving animals, have demonstrated that ethylhexyl dimethyl PABA is toxic to the following four organs: testis, epididymis, spleen, and liver. In addition, experiments using human keratinocytes found that ethylhexyl dimethyl PABA inhibits cell growth and DNA synthesis at low concentrations, and halted the cell cycle of MM96L cells (human melanoma cell line) at the G1 phase. Despite limited clinical data in humans, many studies have confirmed increased mutagenicity of ethylhexyl dimethyl PABA following exposure to sunlight, which suggests that this molecule is likely to contribute to onset of sun-induced cancer despite protecting the skin through absorption of UVB. For risk assessment, the no observed adverse effect level (NOAEL) chosen was 100 mg/kg bw/day in a 4 weeks oral toxicity study. Systemic exposure dosage (SED) was 0.588 mg/kg bw/day for maximum use of ethylhexyl dimethyl PABA in cosmetics. Based on the risk assessment and exposure scenarios conducted in this study, the margin of safety (MOS) was calculated to be 180.18 for a sunscreen containing 8% ethylhexyl dimethyl PABA, which is the maximum level allowed by the relevant domestic authorities.

유수식 연속노출장비를 이용한 과불화화합물(PFOS, PFOA)이 송사리 (Oryzias latipes) 알의 초기발생과정에 미치는 영향 연구 (Toxicity of PFCs in Embryos of the Oryzias latipes Using Flow though Exposure System)

  • Perfluorinated chemicals (PFCs) is a kinds of persistent organic pollutants, and have the potential toxicity of which is causing great concern. In this study, we employed Oryzias latipes embryos to investigate the developmental toxicity of perfluorooctane sulfonate (PFOS) and perfluorooctanoic acid (PFOA)s compound using flowthrow system for 14 day. O. latipes embryos were exposed to solvent control, 20, 40 and 80 mg/L of PFOS and 62.5, 130, 260 mg/L of PFOA respectively. After exposure, hatchability, mortality, total length and heart beats were examined. Hatching rates were reduced approximately 27% in the 80 mg/L PFOS-treated group and 17% in the 62.5, 130 mg/L PFOA-treated groups. Heart beats in the PFOS-treated groups were reduced at 7 day but, PFOA-treated groups were increased heart beats. 80 mg/L PFOS treated group showed significant reduction in growth (total length) level to 90% of control. But PFOA did not showed significant effect on growth. In the 14 days $LC_{50}$ of PFOS and PFOA was 22.74 mg/L and 173 mg/L, respectively. The overall results indicated that the early stage of O. latipes might be a reliable model for the testing of developmental toxicity to perfluorinated chemicals.

제약단지 인접 지역 지표수의 잔류 의약물질 생태위해성평가 (Ecological Risk Assessment of Pharmaceuticals in the Surface Water Near a Pharmaceutical Manufacturing Complex in Korea)

  • Objectives: Limited information is available on the presence and associated ecological risks of pharmaceutical residues in aquatic environments near pharmaceutical manufacturing areas in Korea. In this study, we investigated the current state of pharmaceutical contamination and its associated ecological risks in streams near a pharmaceutical manufacturing complex. Methods: Seven pharmaceuticals (acetaminophen, clarithromycin, diclofenac, diphenhydramine, ibuprofen, mefenamic acid and roxithromycin) were measured in water samples collected from the streams near a pharmaceutical manufacturing complex. A predicted no-effect concentration (PNEC) was derived using either the assessment factor method or species sensitivity distribution method. In addition, a hazard quotient for each pharmaceutical was calculated by dividing its measured environmental concentration by its PNEC. Results: Samples collected downstream from the wastewater treatment plant (WWTP) had higher concentrations of pharmaceuticals than those collected from the reference site (upstream). Moreover, pharmaceutical concentrations were greater in ambient water than in the final effluent from the WWTP, which suggested that non-point sources were contributing to the contamination of the ambient water environment. Some of the target pharmaceuticals exhibited a hazard quotient >1, indicating that their potential ecological effects on the aquatic environment near the pharmaceutical industrial area should not be ignored. Conclusion: This study demonstrated that the pharmaceutical manufacturing area was contaminated with residual drugs, and that there was a possible non-point source near the WWTP effluent discharge area. The results of this study will aid in the development of management plans for pharmaceuticals, particularly in hotspots such as pharmaceutical industrial sites and their vicinities.

Application of a newly developed software program for image quality assessment in cone-beam computed tomography

  • Purpose: The purpose of this study was to apply a newly developed free software program, at low cost and with minimal time, to evaluate the quality of dental and maxillofacial cone-beam computed tomography (CBCT) images. Materials and Methods: A polymethyl methacrylate (PMMA) phantom, CQP-IFBA, was scanned in 3 CBCT units with 7 protocols. A macro program was developed, using the free software ImageJ, to automatically evaluate the image quality parameters. The image quality evaluation was based on 8 parameters: uniformity, the signal-to-noise ratio (SNR), noise, the contrast-to-noise ratio (CNR), spatial resolution, the artifact index, geometric accuracy, and low-contrast resolution. Results: The image uniformity and noise depended on the protocol that was applied. Regarding the CNR, high-density structures were more sensitive to the effect of scanning parameters. There were no significant differences between SNR and CNR in centered and peripheral objects. The geometric accuracy assessment showed that all the distance measurements were lower than the real values. Low-contrast resolution was influenced by the scanning parameters, and the 1-mm rod present in the phantom was not depicted in any of the 3 CBCT units. Smaller voxel sizes presented higher spatial resolution. There were no significant differences among the protocols regarding artifact presence. Conclusion: This software package provided a fast, low-cost, and feasible method for the evaluation of image quality parameters in CBCT.

단국대학교병원 EHR 기반 MOA CDM 구축을 위한 용어 매핑 사례와 시사점 (The Case and Implications of Terminology Mapping for Development of Dankook University Hospital EHR-Based MOA CDM)

  • Purposes: The Common Data Model(CDM) is very important for multi-institutional research. There are various domestic and international CDM construction cases to actively utilize it. In order to construct a CDM, different terms from each institution must be mapped to standard terms. Therefore, we intend to derive the importance and major issues of terminology mapping and propose a solution in CDM construction. Methodology/Approach: This study conducted terminology mapping between Electronic Health Record(EHR) and MOA CDM for constructing Medical Record Observation & Assessment for Drug Safety(MOA) CDM at Dankook University Hospital in 2022. In the process of terminology mapping, a CDM standard terminology process and method were developed and terminology mapping was performed by applying this. The constructions of CDM mapping terms proceeded in the order of diagnosis, drug, measurement, and treatment_procedure. Findings: We developed mapping guideline for CDM construction and used this for mapping. A total of 670,993 EHR data from Dankook University Hospital(January 1, 2013 to December 31, 2021) were mapped. In the case of diagnosis terminology, 19,413 were completely mapped. Drug terminology mapped 92.1% of 2,795. Measurement terminology mapped 94.5% of 7,254 cases. Treatment and procedure were mapped to 2,181 cases, which are the number of mapping targets. Practical Implications: This study found the importance of constructing MOA CDM for drug side effect monitoring and developed terminology mapping guideline. Our results would be useful for all future researchers who are conducting terminology mapping when constructing CDM.

Dietary Protein Restriction on Growth and Immuno-biochemical Response of Crossbred Calves during Post-ruminant Phase of Life

  • Sixteen crossbred (Bos indicus${\times}$Bos taurus) calves were randomly distributed in two groups (NP and LP) of eight calves each to study the effect of restricted (75%) protein supply on growth and immuno-biochemical response as an indicator of production and health of under-nourished animals during 3 to 9 months of age. The normal requirement of protein was provided to group NP and a less of 25% to group LP through calculated amount of concentrate and roughage in their daily ration. Assessment was made for weekly change in live weight, periodic alteration in blood metabolites and immunological status at six months of age in calves. An initial (during 3 to 6 months of age) depression (p<0.05) in growth was seen in low protein fed group (LP) compared to NP, which became non-significant in the later period of life (6 to 9 months of age). There was no significant effect on haemoglobin, total protein, albumin and globulin concentration except that of urea, which was decreased significantly (p<0.05) in animals fed on low protein diet ($19.83{\pm}1.25$ vs $25.93{\pm}1.29mg/dl$). The treatment effect that was seen in different periods of life was not uniform for other parameters except for urea, which showed a regular depression in LP compared to NP. The assessment of immunological status by indirect haemagglutination (IHA) test against Pasteurella multocida (P52 strain) was considerably (p<0.05) reduced in animals on LP ration compared to those on NP. It is thus argued that with poor nutrition (low protein) and state of compromised immunological response the production and health of the animals will be adversely affected.
