주상복합아파트 거주자의 질병자료 내용 분석 (Contents Analysis on Medical Reports of High-Rise Condominium Residents)

  • The purpose of this study is to figure out the relationship between the residence stories in high-rise condominium and residents' disease patterns throughout the dweller's medical reports. Research basic data are obtained from medical fee request of National Health Insurance Corporation. Data are limited to 'A' high-rise condominium and a medical treatment time to 3 years (2004. 1-2006, 12). Data for analysis are composed of total 346,286 medical records, 43,159 disease records, and 8,999 personal records. Data are stored by sex, age, building story, residence story, visiting year and month, treatment days, main disease type (KCD-4). Treatment number, disease type and asthma in disease records and personal records are statistically analyzed by residence story considering age. Findings are as follows: 1) Women have more medical treatments than men, 40-50 age group is more treated, and the residents of 6-25 stories are more received medical treatments. According to KCD-4, diseases of the respiratory system and diseases of the eye and adnexa are relatively treated higher than other diseases. 2) The diseases of he respiratory system, the eye and adnexa, the skin and subcutaneous issue, the ear and mastoid process, and the asthma have not relation to the high-storied residence through the data of disease records and personal records. But the analysis on the data of children, age 7 and less, showed a significant relation. to conclude, there is no relationship between the residence of high-stories in the condominium and residents' disease patterns, but there is a little probable to the relationship in the pre-school child.

모바일 개인건강기록(Personal Health Records: PHR) 어플리케이션의 이용이 소비자 건강행태에 미치는 영향 (The Effects of a Mobile Personal Health Records (PHR) Application on Consumer Health Behavior)

  • 본 연구는 모바일 개인건강기록(Personal health record: PHR) 어플리케이션의 장단점을 분석하고 그 어플리케이션의 사용이 소비자 건강정보행태에 미치는 영향을 파악하고자 하였다. 본 연구를 위해 총 27명의 대학생들이 3개월 동안 PHR 어플리케이션을 사용하였으며, 그 경험을 바탕으로 서면인터뷰를 실시하였다. 내용분석 결과, 시각적 인터페이스를 통한 예방 차원의 건강관리 및 건강 향상을 위한 동기부여와 구체적 가이드라인의 제공, 가족과의 데이터 공유 및 간병인(caregiver)의 환자 건강관리지원, 무엇보다도 환자와 의료전문가와의 상호작용 강화 등이 PHR의 혜택으로 부각되었다. 반면 근력운동과 같은 콘텐츠의 미비 내지는 타 기기와의 호환성 미흡 등이 문제점으로 지적되었다. PHR의 도움으로 건강을 향상시키고자 하는 소비자들은 감정적 지원을 받는가 하면, 수면장애극복, 금주, 금연, 그리고 체중감량 및 식습관의 변화 등 건강행태를 변화시키고자 하는 동기부여가 생겼으며, 실제로 건강행태의 변화를 성취함으로써 자기효능이 증가하기도 하였다. 본 연구는 국내 연구에서는 도입단계인 PHR에 대한 연구를 시도했다는 것에 학문적 의의가 있으며, 헬스케어분야의 패러다임 변화에 대응하기 위한 건강 및 의료정보서비스를 모색하는데 통찰력 있는 시사점을 제공한다.

  • 개인 맞춤형 식이 운동 콘텐츠 제공을 위해 1개 3차병원에서 제공하는 PHR(Personal Health Records)을 기반으로 건강관리 스마트폰 어플리케이션을 개발하였다. PHR의 상병명에 근거하여 각 질환과의 상관관계를 적용한 식이 운동 적합률 알고리즘을 통해 개인의 질환을 관리하기 위한 맞춤형 콘텐츠를 제공하며, 식사량과 운동량을 기록하여 섭취 소비한 칼로리를 기록하는 기능을 제공한다. 또한 사용자의 위치정보를 근거로 한, 음식점 위치 정보 및 해당 메뉴, 그에 따른 영양분석에 대한 정보를 제공하는 개인에게 적합한 콘텐츠를 이용하고 기록할 수 있는 서비스로써, 사용자의 상태와 편의성을 고려하여 유헬스 서비스를 제공할 수 있다.

연령과 사상체질에 따른 개인건강기록(PHR)에 기반한 아토피피부염 환자의 특성 (Characteristics of Atopic Dermatitis Patients Based on Personal Health Records (PHR) according to Age and Sasang Constitution)

  • Objectives The purpose of this study is to identify the characteristics of atopic dermatitis patients based on the personal health records (PHR) according to age and Sasang Constitution, and to find basic data for daily life management using the personal health record. Methods We analyzed the characteristics of atopic dermatitis patients based on PHR (symptoms, emotional state, SCORAD Index, five viscera weakness questionnaire, etc.), and Sasang Constitution collected for 6 weeks in 34 patients (or guardians). Results Each item of the SCORAD Index score in children and adolescents showed that children and adolescents have significantly higher edema than adults. However, repeated measurement of the SCORAD Index showed a significant positive improvement over time regardless of age and constitution. In terms of the symptoms of atopic dermatitis, there was a significant correlation between age and swelling, quality of life and effusion, pruritus (itch) and sleep disturbance in relation to the general characteristics of the subject. In the five visceral weakness scores, adults had significantly higher spleen weakness, liver weakness, kidney weakness scores than in children and adolescents. All results showed a high tendency to have a high lung weakness score, which was more pronounced in children and adolescents. Conclusion This study is meaningful to confirm the correlation between SCORAD Index items related to the symptoms of atopic dermatitis, and to identify trends for specific items by ages and Sasang Constitution.

대소변 개인건강기록의 임상연계 활용 연구 (A Study on the Clinical Utilization of Personal Health Records of Stool and Urine in Korean Medicine)

  • Objectives : In this study, we analyze the medical significance of feces symptoms so that the daily records of the feces of individuals can be not only used as a measure of individual health monitoring in daily life, but also more actively connected to the medical treatment of the Korean Medicine (KM). Methods : Categories and clinically significant attributes for symptoms of Urination and defecation in the KM ontology DB are determined, and connected to KM related dialectical indicators by experts' common criteria including Viscera and Bowels [臟腑], eight principles [八綱], Qi Blood fluid and humor phlegm-retained fluid static blood [氣血津液痰飮瘀血], six excesses [六淫]. Results : The analysis of the symptoms of feces in the Korea Medicine ontology shows that the symptoms of stool in categories of 'stool stiffness', 'blood swelling', 'discomfort' are highly ranked among the overall clinical symptom categories. In the case of urine symptoms, symptoms corresponding to 'urine color,' 'urine discomfort,' and 'urine volume' are the top rankers among other total clinical symptoms. In the case of stool, the relationship between the symptom of stool and the categories of spleen, stomach, and colon is increased as the weighted symptom is considered. The relationship between the symptom of urine and the categories of the small intestine and the bladder is increased in the same way. Conclusions : This study could help better utilize the personal generated health records of feces in clinical practice of Korean Medicine.

개인건강기록 (PHR)을 이용한 허약아 정보 수집 연구 (Study on Weak Children Information Collection Using Personal Health Record (PHR))

  • Objectives The Korean Medicine (KM) PHR platform is a personalized healthcare service which allows individuals to keep and manage their own health records. When parents are reporting for their children from their memories, there is high possibility of recall errors. In these cases, it could be useful for doctors to collect the patient's symptoms through PHR platform. In this study, we aimed to investigate the clinical significance of the PHR by using the KM PHR platform in the pediatric clinic. Methods The PHR platform was used to collect child health information from parents and child care teachers. The collected data were analyzed in comparison with the results of screening by pediatrician. Results A total of 58 children were recruited, 44 of which health information were collected from their parents and their child care teachers. The remaining 14 children's health information were collected from their parents only. As a result the parents tended to perceive their children weaker than the child care teacher. Compared to other organs, there was a only significant difference in the heart weakness score and spleen weakness score in the comparison of the weak and healthy children. Conclusions Although the study was conducted on a small group of subjects, and used PHR platform developed specifically for adults to indirectly input child's symptoms, and analyzed their health information, there was a difference in health records between information providers. Development of PHR platform for children is needed to collect more reliable information.

  • 의료서비스와 IT 기술간의 융합으로 환자 개인의 건강정보가 전자의무기록(EHR)의 보급과 함께 빠르게 전자화되고 있다. 이와 함께 유헬스사회에 접어들면서 전자화 된 환자의 건강기록들을 진료 이외의 공중보건 및 의학 분야의 연구, 의료서비스 향상을 위해 사용하고자 하는 2차이용의 요구가 증가하고 있다. 개인건강정보의 2차이용으로 의학 분야의 발전의 매우 유익한 일이지만 부주의하게 개인의 건강정보를 이용하는 경우 환자 개인의 프라이버시 손상이 발생, 더불어 2차이용융 통한 연구나 서비스 발전에도 제한이 발생할 수 있다. 하지만 아직 개인건강정보를 이용한 2차적 이용에 대해 체계적인 연구나 논의가 없는 것이 현실이다. 따라서 본 논문에서는 개인건강정보의 2차이용과 관련하여 국내외의 법안들을 살펴보고 이를 비교 분석하여 앞으로 개인의 프라이버시를 존중하고 더불어 의료분야 서비스 있는 방향을 제시하고자 한다.

  • 개인건강기록 서비스는 환자에게 건강기록 관리와 중요한 의료파일 관리 그리고 응급상황 연락과 같이 편리하고 사용하기 쉬운 해결책을 제시해준다. 이러한 중요한 장점에도 불구하고 개인건강기록 서비스는 데이터의 보안에 관련된 소비자 입장에서는 피할 수 없는 중요한 도전을 제기하고 있다. 개인건강기록 기술이 헬스케어와 융합되면서 사용자 개인정보 침해가 발생하고 사용자의 민감한 의료정보가 유출되는 문제가 증가되고 있다. 본 논문에서는 개인건강기록 서비스의 취약점과 반드시 해결해야할 다양한 보안 측면을 분석한다. 또한, 개인건강기록 사용자와 애플리케이션 서비스 제공자 관점에서 보안 요구사항을 기술하였으며, 개인건강기록 보안 요구사항을 만족하고 보안위협을 대응할 수 있는 보안 메커니즘에 대해 연구하였다.

Sharing and Privacy in PHRs: Efficient Policy Hiding and Update Attribute-based Encryption

  • Personal health records (PHRs) is an electronic medical system that enables patients to acquire, manage and share their health data. Nevertheless, data confidentiality and user privacy in PHRs have not been handled completely. As a fine-grained access control over health data, ciphertext-policy attribute-based encryption (CP-ABE) has an ability to guarantee data confidentiality. However, existing CP-ABE solutions for PHRs are facing some new challenges in access control, such as policy privacy disclosure and dynamic policy update. In terms of addressing these problems, we propose a privacy protection and dynamic share system (PPADS) based on CP-ABE for PHRs, which supports full policy hiding and flexible access control. In the system, attribute information of access policy is fully hidden by attribute bloom filter. Moreover, data user produces a transforming key for the PHRs Cloud to change access policy dynamically. Furthermore, relied on security analysis, PPADS is selectively secure under standard model. Finally, the performance comparisons and simulation results demonstrate that PPADS is suitable for PHRs.

Application of Standard Terminologies for the Development of a Customized Healthcare Service based on a PHR Platform

  • The personal health record platform can store and manage medical records, health-monitoring data such as blood pressure and blood sugar, and life logs generated from various wearable devices. It provides services such as international standard-based medical document management, data pattern analysis and an intelligent inference engine, and disease prediction and domain contents. This study aims to construct a foundation for the transmission of international standard-based medical documents by mapping the diagnosis items of a general health examination, special health examination, life logs, health data, and life habits with the international standard terminology systems. The results of mapping with international standard terminology systems show a high mapping rate of 95.6%, with 78.8% for LOINC, 10.3% for SNOMED, and 6.5% when mapped with both LOINC and SNOMED.
