건설공사 발주자의 계획, 설계, 공사단계 안전보건관리 역할 분석 (Analysis on Construction Clients' Role for Safety and Health Management in Plan, Design, and Construction Stage)

  • The duty of construction clients in WSH (Workplace Safety and Health) system was included in the amendment of Occupational Safety and Health Act (enforced on 16 January 2020), which was estimated the shift of paradigm in the prevention of construction accidents. The purpose of this paper is to introduce the analysis results of construction clients' role in the construction project, which were performed by authors over the recent years in order to impose the duty on construction clients, and to suggest their role according the plan, design, and construction stage. Utilizing the systematic literature review process based on Meta analysis, the related papers were selected. For the selected papers, related domestic and foreign regulations, and other prominent report, the construction clients' role was analyzed by reflecting the experts' advice. Results show that the construction clients should control the designer and contractor for implementing the WHS system during the whole process of the construction project. They should supply sufficient source and time to ensure the workers' safety. In the plan stage, the key role of construction clients is to identify intensively controlled hazard and risk reduction plan and to transfer the results. In the design stage, their key role is to select the designer with the capacity in WSH and to assist the designer for the safety design. The main key role of construction clients in the costruction stage is to select the contractor with specialty in WSH including a contract reflecting the WSH requirement and to check implementation of WSH plan, WSH cost, WSH education, and accident report. In addition, it is thought that the construction clients' participations in the site WSH activity and adjustment of safety and health problem among contractors can be effect in the prevention of construction accidents.

  • In 2015, Ministry of Health and Welfare of Korea announced a research and development plan of using Korean healthcare data to support decision making, reduce cost and enhance a better treatment. This project relies on the adoption of BigData technology such as Apache Hadoop, Apache Spark to store and process HealthCare Data from various institution. Here we present an approach a design and implementation of OLAP server in Korean HealthCare BigData platform. This approach is used to establish a basis for promoting personalized healthcare research for decision making, forecasting disease and developing customized diagnosis and treatment.

국내 보건복지 장기계획 수립 현황 (Current Status of Health and Welfare Long-Term Plans in Korea)

  • Korea is undergoing a rapid environmental change in health and welfare. Therefore, the law mandates the establishment and implementation of plans in accordance with the changes. A total of 49 long-term plans related to health and welfare were specified by the National Law Information Center, the Korean representative legal information website managed by the Korea Ministry of Government Legislation. Of the 49 long-term plans, 10 plans (20.4%) were not yet fully constructed. Eight out of 10 non-constructive plans have been put into force for more than a year, but these plans still require further systematic planning and development. The complete construction of long-term plans is substantial to account for the changes in South Korean health and welfare. In addition, a systematic plan with solidarity and continuity between the mutual plans should be established in planning.

건강증진을 위한 산업장에서의 금연프로그램 적용의 일 연구 (A Study on the Application of a Health Promotion Program about No Smoking in Industry)

  • This study was conducted to diffusion of the health promotion program about no smoking in industry. No smoking program model composed of assessment, plan, implementation and evaluation was developed through the method of literature review and applied to 27 smokers of the management team and dept. of the 1st production in one industry, from October 27 to November 3D, 1993. For the analysis, descriptive statistics and paired t-test were used. The result of this study are summarized as follows: 1. The object was composed of no smoking and reduced smoking. The achievement rate of object was $48\%$. 2. As a result of the implementation of no smoking program, $11\%$ of no smoking rate was obtained. 3. After application of no smoking program, the health age was a little lower but statistical significations was not found. 4. Health education and support of family were effective in health promotion program. Complementary measures about indivisual activity for no smoking were required in no smoking program. In conclusion, subject's response about no smoking program was high and health education and support of family were effective in health promotion· program. And nurses were expected to do their roles as a planner, intervener, manager and supporter in health promotion services.

중대재해처벌법 시행에 따른 안전보건경영시스템 개선방안 연구 - H건설사 중심 (A Study on the Safety and Health Management System Improvement Plan according to the Implementation of the Serious Accident Punishment Act - Focused on H Construction Company)

  • 연구목적: 중대재해처벌법 시행에 따른 건설사 안전 관리 시스템 개선 방안을 제시한다. 연구방법: H건설사에서 이행 중인 중대 재해 처벌 법 대응 방안에 대한 안전보건관리체계를 안전보건진단을 통해 실무중심의 재해 예방활동의 효과를 분석하였다. 연구결과: 중대재해처벌법과 안전보건경영시스템의 연계로 재해율 감소 효과 및 안전보건경영 체계의 고도화 등이 분석되었고 중대재해처벌법 시행 전·후의 안전 활동 종합결과 분석을 통한 안전보건경영시스템 개선방안을 제시하였다. 결론: 중대재해처벌법 시행에 따른 사업주의 의식 개선, 재해예방을 위한 투자 확대, 재해예방활동의 정량화 등을 통한 실질적인 성과측정의 효과가 발현되었다.

건강증진학교 수행 및 효과평가 (Implementation and Evaluation of a Health Promoting School Program)

  • Objectives: The purpose of this study was to develop strategies for creating health promoting school and to evaluate the implementation and its effect on the school. Methods: Based on WHO's Health Promoting School, we developed 5 strategies and implementing process for creating health promoting school and undertook The implementation process included workshops to raise awareness of the health promoting school, establishment of a school health team, review of the health needs of its community, development of an action plan, and practice of activities and reflect on program. Seventeen teachers (male 35.3%, female 64.7%) and 248 students (male 51.2%, female 47,6%) had completed pretest and posttest. Results: It was increasing in school's social environment, community relationship, personal skill and health service among components of HPS. However, there were no significant differences in self-reported health status and health behaviors among teachers. Although there were no significant changes in self-esteem, self-efficacy and BMI, there were significantly increasing in health knowledge, physical activities and fruit and vegetable intakes among students of a pilot school. Conclusion: Strategies for a creating health promoting school were appropriate to implement in school and they would also make a contribution to improve health knowledge and health behaviors in elementary school students.

식품영양안전 정책 현황 및 발전방향: 1차 당류 저감 종합계획을 중심으로 (Current status and development direction of the food nutrition safety policy: The first phase of sugar intake reduction plan)

  • Objectives: The purposes of this study are to refer existing and past domestic and foreign policies and to establish a comprehensive plan to reduce sugar consumption and utilize it as fundamental data to establish and implement relevant policies. Methods: Korean government analyzed domestic and foreign policies and average sugar intake to develop draft of sugar intake reduction plan. Based on the plan, objectives, sections and agendas of the plan were set after taking into account of their feasibility. Results: The policy has three directions: to help sugar intake reduction in all population groups, to create an environment which helps people to make informed decision on lower sugar choices and to lay scientific and statistical foundation for sugar intake reduction plan. The goal is to restrict average sugar intake from processed food to less than 10% of total energy intake per day. Three core strategies are to help reduce intakes of sugar in all population groups, to create an environment which helps people to make informed decision on lower sugar choices and to lay a foundation for sugar intake reduction plan. Each strategies has 2~4 projects. Conclusions: This study is expected to serve as a catalyst for carrying out the first phase of sugar intake reduction plan to help reduce sugar consumption.

소방 방화복 교차오염 저감 및 관리체계 개선을 위한 델파이 연구 (Delphi Study on the Reduction of Cross-contamination and Improvement of Management System on Firefighting Protection Suit)

  • Objectives: This study evaluates and recommends the priority of policy implementation to improve the fire protection clothing management system used by firefighters and the reduction of cross-contamination from contaminated clothing at the scene of a fire. Methods: It consisted of 7 experts and conducted three interviews and two modified Delphi surveys. Through the results of previous research and interviews with experts, a plan to reduce cross-contamination of fire suits and improve the management system was first derived. An improvement plan was presented in the four areas including resources, management, fire protection related work, and laws and regulations, and the priority of policy implementation was derived by analyzing the importance and practicality of the policy at the same time. Results: As a result of the analysis, the first priority was education on the health effects of pollutants at the disaster scene for firefighters, and the second priority was the addition of SOP for the primary decontamination of on-scene personal protective equipment in preparation for the health effects of the disaster scene, and education for fire suppression and rescue workers. The next step was to improve the management system of personal protective equipment such as fire suits and develop a training course for systematic operation. Conclusions: This findings could be used in the implementation of mid- to long-term firefighting policies for the systematic operation and establishment of a systematic management system for personal protective equipment such as fire protective suits.

Effects of Iranian Economic Reforms on Equity in Social and Healthcare Financing: A Segmented Regression Analysis

  • Objectives: One of the main objectives of the Targeted Subsidies Law (TSL) in Iran was to improve equity in healthcare financing. This study aimed at measuring the effects of the TSL, which was implemented in Iran in 2010, on equity in healthcare financing. Methods: Segmented regression analysis was applied to assess the effects of TSL implementation on the Gini and Kakwani indices of outcome variables in Iranian households. Data for the years 1977-2014 were retrieved from formal databases. Changes in the levels and trends of the outcome variables before and after TSL implementation were assessed using Stata version 13. Results: In the 33 years before the implementation of the TSL, the Gini index decreased from 0.401 to 0.381. The Gini index and its intercept significantly decreased to 0.362 (p<0.001) 5 years after the implementation of the TSL. There was no statistically significant change in the gross domestic product or inflation rate after TSL implementation. The Kakwani index significantly increased from -0.020 to 0.007 (p<0.001) before the implementation of the TSL, while we observed no statistically significant change (p=0.81) in the Kakwani index after TSL implementation. Conclusions: The TSL reform, which was introduced as part of an economic development plan in Iran in 2010, led to a significant reduction in households' income inequality. However, the TSL did not significantly affect equity in healthcare financing. Hence, while measuring the long-term impact of TSL is paramount, healthcare decision-makers need to consider the efficacy of the TSL in order to develop plans for achieving the desired equity in healthcare financing.

Current Systems of National and Regional Nutrition Surveys and Future Direction

  • The National Nutrition Survey in Japan (NNS-J) started in 1945 and has provided information on dietary intake and health status of Japanese citizens to the public and policymakers for more than half a century. We summarized several relevant issues on the survey in this report : the current framework of the NNS-J in accordance with the Nutrition Improvement Law, utilization of the survey for nutrition and health policy in Japan, the Health Promotion Law recently enacted in 2003, the national plan for health promotion and disease prevention (Health Japan 21), and possible measures to improve the survey systems under the new law. We also mentioned implementation structures of regional health and nutrition surveys, because the Health Promotion Law designates an active role of local governments on promoting health for their citizens, which will enhance the needs for appropriate assessment of health and nutrition conditions in each community as well as the monitoring at the national level.
