급성기 중풍환자의 위험요인, 경고증상, 전조증상 및 설진, 맥상, 변증과 NIHSS의 관련성 연구 (The Relationship between Risk Factors, Warning Signs, Tongue Diagnosis, Pulse Pattern, Pattern Identification and National Institute of Health Stroke Scale in Acute Stroke Patients)

  • Objectives : The aim of this study was to assess the relationship between risk factors, warning signs, tongue diagnosis, pulse pattern, pattern identification and National Institute of Health Stroke Scale in acute stroke patients. Methods : We studied patients hospitalized within 4 wks after their ictus who were admitted at the Internal Medical Department at Kyunghee Oriental Medical Center, Kyunghee University East-West Neo Medical Center, Kyungwon University Songpa Oriental Medical Center, Kyungwon University Incheon Oriental Medical Center, Dongguk University Ilsan Oriental Medical Center from April 2007 to August 2009. We analyzed the relationship between risk factors, warning signs, tongue diagnosis, pulse pattern, pattern identification and National Institute of Health Stroke Scale in acute stroke patients. Results : 1506 subjects were included into the final analysis. 1. In the risk factors, the NIHSS mean score of atrial fibrillation was significantly higher than non-atrial fibrillation. 2. In the warning signs, the NIHSS mean scores of weakness, loss of eyesight, dysarthria, and sensory loss were significantly higher than in the non-warning signs. 3. There were no significant differences in lifestyle, tongue fur color, pattern identification between groups. 4. In the tongue color, the NIHSS mean score of red was significantly higher than pale or pale red. 5. There were significant differences statistically between forceful/weak, fine/not fine, slippery/not slippery pulse and NIHSS score. Conclusion : The above results show the relationship between risk factors, warning signs, tongue diagnosis, pulse pattern, pattern identification and National Institute of Health Stroke Scale in acute stroke patients. These results can be utilized in the future as a basis material.

잠재프로파일 분석을 활용한 한국 노인 라이프스타일 유형화와 영향요인 분석 (Predicting Healthy Lifestyle Patterns in Older Community Dwelling Adults: A Latent Profile Analysis)

  • 목적 : 본 연구는 고령자의 라이프스타일이 어떤 형태로 유형화되는지에 대해 라이프스타일 잠재 집단 유형을 분석하고 각 집단의 유형별 특성을 파악하여 고령자의 건강과 삶의 질 증진을 위한 기초자료를 마련하기 위해 수행되었다. 연구방법 : 본 연구에는 횡단연구방법이 사용되었다. 2019년 4월부터 5월까지 고령자의 라이프스타일 유형을 파악하기 위해 만 65세 이상의 국내 지역사회 거주 노인 184명을 대상으로 설문조사가 이루어 졌다. 수집된 설문자료를 활용하여 잠재프로파일분석(LPA)을 실시하였고, 도출된 각 유형별 특성과 영향요인을 확인하기 위해 χ2 검정, 다항로지스틱회귀분석 등을 활용하였다. 결과 : 연구결과, 고령자의 라이프스타일은 중 첫 번째 영역인 신체적 활동부분에서는 '소극적 운동 참여형(31.1%)', '저강도 운동 집중형(54.5%)'과 '균형적 운동 참여형(14.5%)'인 3개의 잠재집단으로 분류되었다. 활동 참여의 경우 '비활동형(12%)', '생활유지형(61%)', '활동적 노년형(27%)'인 3집단으로 분류되었으며, 마지막 식이습관에 대한 경우 '전반적 영양부족형(13.5%)'과 '균형적 영양 섭취형(86.5%)' 2집단으로 분류되었다. 또한 라이프스타일 유형이 고령자의 건강과 삶의 만족도에 미치는 영향을 파악하기 위해 다항로지스틱회귀분석을 실시한 결과, 활동적·균형적 라이프스타일에 속할수록 삶의 질과 건강 수준이 전반적으로 높은 곳으로 확인되었다. 또한 이러한 유형의 예측요인에서 성별, 교육수준, 거주지역 등이 주요하게 작용하는 것으로 나타났다. 결론 : 고령자가 보다 다양한 활동에 균형적으로 참여하고, 활동적인 일상생활을 수행할 때 건강과 삶의 만족도가 증진됨이 분석되었다. 따라서 본 연구결과를 토대로 고령자의 라이프스타일 유형에 맞춘 실증적·정책적 개입 방안을 제안하였다.

경기지역 여고생과 여대생의 배변양상에 영향을 미치는 생활습관 요인 조사 (A Study on the Defecation Pattern and Lifestyle Factors of Female High School and College Students in Gyeonggi Province)

  • The aim of this study was to investigate the dietary and lifestyle factors related to bowel pattern of female high school and college students in Gyeonggi Province. The total of 202 self-administered questionnaires (high school students 77 ; college students 125) were analyzed. All respondents were devided into four groups based on their self-reported pattern of defecation: 18 subjects ($9.0\%$) comprised the normal group, 73 ($36.1\%$) the constipation group, 73 ($36.1\%$) the constipation/diarrhea group, and 38 ($18.8\%$) the diarrhea group. Regularity of exercise was significantly higher in the college students than high school students, and times spending on a chair were longer in the high school students than college students. Most students ($72.7\%$) reported that they had stress. Fourty point six percent of the subjects reported that they had 3 meals per day, which tended to be higher in the normal and diarrhea group than constipation and constipation/ diarrhea group. Most students (69.7) skipped breakfast which was lower in the normal group than the other groups. The most preferred dietary fiber food was korean cabbage kimchi. Fifty-eight point four percent of the subjects reported that they had irregular bowel movement. Bowel movement was more irregular in the high school students than college students, and in the constipation group than the other groups. Of the subjects, $77.7\%$ had defecation frequency between three per week and three per day. Those who spent within 10 minutes for defecation were $79.6\%$, and those had difficulty in evacuating were $76.0\%$. High school students and those with constipation and constipation/diarrhea had a significantly lower defecation frequency, longer time spent at the toilet, and greater difficulty in evacuating than college students and normal and diarrhea group. The percentage of those who had feelings of residue in the intestine after defecation was $92.5\%$, and it was greater in the constipation or diarrhea group than in the normal group. Most students ($93.5\%$) reported that they had abdominal pain or discomfort. These results suggest that decreasing times spending on a chair, decreasing stress, keeping 3 meal per day at regular hours, and increasing dietary fiber intake are associated with desirable bowel pattern.

일 대학 간호과 여학생의 월경곤란증 (A Study on the Dysmenorrhea in College Female Students)

  • This study was undertaken to obtain the incidence of dysmenorrhea and differance of dysmenorrhea according to the general characteristics, lifestyle, and menstrual pattern in women community college students. For the data collection, self-administered questionnaire survey was made from April 7, 1999 to April 14, 1999 among the 204 women community college students. The resultant data were processed by SAS program for frequency, proportion, and Chi-square test. The results of this study are as follows ; 1) Prevalence rate of dysmenorrhea was 81.9%. Among women who had dysmenorrhea, 42.6% of them had family history on dysmenorrhea, 47% of them experienced the dysmenorrhea 'monthly', 89.2% of them had experienced dysmenorrhea on the first-second day, 38.5% of them responded that the most painful region was 'low abdomen', 61.5% of them responded that they used 'analgesics' to soothe dysmenorrhea, 92.3% of them responded that they had analgesics without doctor's prescription, and 42.6% of them responded that they experienced digestive system related symptoms during menstrual period. 2) The incidence of dysmenorrhea was significantly different by ordering of sisters, blood type, and body shape. 3) The incidence of dysmenorrhea was no significantly different by life style. 4) The incidence of dysmenorrhea was no significantly different by menstrual pattern.

비만도에 따른 대학생의 혼자 식사 및 함께하는 식사 시의 식행동 비교 (Comparison of Eating Behavior between Commensality and Solo-eating of University Students by BMI)

  • The objective of this research was to explore the influences of 'having a meal with someone' on individuals' eating pattern. Eating is not a simple matter of energy intake but also serves to anchor daily routines being cultivated by people and society. This study was conducted using a cross-sectional eating behavior survey of university students (N = 893, 380 men, 513 women) aged 20 to 24 years. Results were analyzed and presented as frequencies, means and ${\chi}^2$-test with SPSS 14.0. Differences in dietary habits by commensality and solo-eating were observed; Students who ate alone, spent 15 min for a meal and ate convenience food items when they didn't feel hungry. Compared to students who ate alone, those who ate together with someone spent 30 min for a meal and ate more amount of food. Eighty percent of respondents ate more various menus in commensality than solo-eating. They felt lonely when they ate alone and preferred to eat together. In conclusion, university students start to decide and select their own meals by themselves after junior and high school food services which are fixed with regard to menu and the amount. Dietary habits of Koreans rapidly changed concomitant with social changes over the past half century. Governments and health experts recognize that unbalanced meals cause lifestyle-related diseases, in particular obesity. Our research findings will contribute to more comprehensive efficient nutrition education programs in order to prevent obesity and other lifestyle-related diseases in early stages of adulthood.

국민건강증진을 위한 영양 부문의 2000년 잠정목표와 전략 (Provisional National Nutrition Targets and Strategies for Health Promotion of Koreans)

  • This study attempts to propose a priority of national nutrition targets and strategies for health promotion by the year 2000 in Korea, as a part of the task set for national health promotion objectives and strategies. Among all of the important health issues raised, ten were chosen, nutrition was one priority area. In the first part, the current status of the nutrition-related health problems and risk factors are reviewed, in conjunction with the newly arisen health phenomena, such as changes in prevalence of lifestyle disease and causes of death, changes of food consumption patterns in our country. In the second section this study suggests six feasible national nutrition targets, eight implementing strategies and current major tasks on the basis of the assessment of present status and in consideration of the other health promotion goals and strategies, with reference to that of other developed countries. The main targets and strategies are suggested as follows ; Firstly, the national nutrition monitoring and surveillance system should be established for identifying the nutritional problems for our people, and current National Nutrition Survey is a strong need for improvement to a more comprehensive and reliable one. Secondly, effective administrative mechanism should be operation at national level for the development of nutrition policy. Ministry of Health and Welfare (MOHW) as well as local health department must be remarkably renewed and strengthened the nutrition section. And it is recommended that MOHW organize and operate “The Council of Nutrition”, in which all government authorities related with foodstuffs and nutrition would incorporated. The Council of Nutrition would act as an adjustor as well as a coordinator in nutrition related policy-making. Thirdly, healthy eating pattern will be supported by activities of introducing a nutrition labeling for providing consumers with the necessary information and skills for food selection. Fourthly, nutrition education, and nutrition intervention programs will be carried out in various settings such as health centers, schools, and clinical fields and workplace. Fifthly, the current dietary guidelines shall be continuously improved in detail, and publicly circulated to particular levels of people by age group and by health condition. And finally, researches and epidemiological studies particularly in regard to diet for development of chronic diseases are needed for more investigation and up-to-date national health and nutrition data should be collected with the support and cooperation from the various medical professional teams . 공중보건의 역할변화와 보건소의 기능 (The Function of Health Center in the New Public Health Age)

  • Public health system and public health practice have changed over the past decades as the result of social and epidemiologic changes. New public health concept emphasizes leadership, strategical thinking, systematic planning and effective performance to identify and solve complex health problem In Korea, the role of Health Center has been expanded rapidly. However there is strong suspicion that Health Center can achieve their mission. This paper aims at understanding what is the needed functions of Health Center and which of its aspects needs improvement. The main results of this study are summarizes as follows. District Health Law does not address the core functions of Health Center. The staffs of Health Center have difficulties in prioritizing their services. The recent attempt to restructure Health Center and change delivery pattern of public health services ended with only limited success. To effectively confront threats to the public's health, the three major function of Health Center must be ${\circled}1$ modifying individual behavior and lifestyle, ${\circled}2$ improving social and economic conditions, and ${\circled}3$ reforming health policies. Better results do not come from setting new functions only; they come from understanding and improving the processes that will then leads to better outcome. We recommend that policy-makers focus economic evaluation of public health programs, building and spreading of the scientific evidence of programs, linkage of public health research and public health program. The criteria of delegation of public health service to private sector is urgently needed. Making community health information data available on a routine basis to providers, managers and researchers of public health services helps promote the efficiency of the overall operation of public health system.

취약지역 빈곤아동의 인구사회학적 특성별 건강행동 변화단계의 분포 (The Stages of Change Distribution for Health Behaviors among Low Income Children in Underserved Area)

  • Objectives: The purpose of this study was to identify the stages of change distributions for nine major health behaviors among poor children in underserved area, and to provide basic information for developing specific objectives and strategies for health education program. Methods: The health behavior data were obtained with a self-administered questionnaire and analysed for 3,081 poor children in 106 local children's centers nationwide. Stages of change classification were based on self-reported responses to five statements. The health behaviors included were balanced diet, safety behavior (helmet and protective device use while riding), regular exercise, smoking, alcohol use, drug use, sleep pattern, weight management and stress management. Results: Poor children were more likely in precontemplation and less frequently in action and maintenance stages for a safety behavior. 33.1% of respondents were precontemplators, 29.9% contemplators, and 6.5% in preparation stage. Only 4.3% and 9.6% were in action and maintenance stages, respectively. Gender differences were apparent. Boys were more likely to be precontemplators for nine health behaviors. Children from the lowest socioeconomic status and disrupted family were more likely to be in precontemplation for weight management and exercise behaviors. Children living with both parents were more likely to be in the action and maintenance stages for nine health behaviors. Stages of change for balanced diet and smoking were significantly related with those for other eight health behaviors. And the stage of exercise adoption was found to be significantly related with those for other seven health behaviors. Conclusion: Safety education must receive high priority in health education program for low income children. Balanced diet smoking cessation and regular exercise could be a possible gateway behavior toward healthier lifestyle practices.

Life-Style Habits in a High-Risk Area for Upper Gastrointestinal Cancers: a Population-Based Study from Shanxi, China

  • Background: Cancer is a burden on humanity and ranks as a leading cause of morbidity and mortality in China. Shanxi province has its unique cancer patterns and the burden is increasing. In this study, we aimed to assess the pattern of dietary habits and life-style in Shanxi, a high-risk area for upper gastrointestinal cancers in China and further evaluate the trends in cancer incidence and mortality based on registered data. Materials and Methods: Data on lifestyle, diet, physical activity were obtained from the household health survey at Zhongyang from 2013 to 2015. Cancer diagnoses were reported to Shanxi Center for Disease Control and Prevention (SCDCP). Population-based cancer incidence data and mortality data of 2012 were collected from the SCDCP. All incidence and death rates were expressed per 100,000 populations. Univariate analysis was performed using the Chi-squared test or Fisher's exact test. Results: Overall, deficiencies in fresh fruits and vegetable food, and intake of hot food, salted food, or pickled food are serious problems in Shanxi, especially in rural areas. Upper gastrointestinal cancers were the most commonly diagnosed cancers, and the incidence in rural areas is higher than those in urban areas. Cervical cancer is the most common cancer for females. Moreover, the agespecific incidence exhibited an increased trend before 40 years old. Consistent with the previous literature, our epidemiological investigation results suggest that lifestyle, nutrition deficient, and infections were major risk factors for upper gastrointestinal cancers or cervical cancer in Shanxi. Facing a serious situation, we further explored defensible recommendations for the general public in order to promote changes in environments that support healthful eating and physical activity habits, to reduce cancer risk. Conclusions: Our results present the current cancer trends in Shanxi and its related etiologic risk factors and provide a theoretical basis to guide public health efforts to prevent and control cancers in the province.

운동에 영향을 끼치는 피트니스 앱 요인 연구 (What Makes People Walk Using a Fitness App?)

  • Purpose An increase in the number of fitness apps across the world is a testament to the growing interest in health. As focus on medical services shifted from treatment to prevention, this type of app plays an essential role in promoting one's health and exercise management. Yet little has been known about practical effects of fitness apps in existing literature thus this study searches for factors that affect daily use of fitness apps and examine the relationship between app usage and exercise patterns. Design/methodology/approach This study surveyed people who used Cashwalk, the pedometer app for a minimum of a week to analyze reasons why they used the app. Particpants' exercise patterns were also recorded. By examining survey results of fitness app users and developing a research model based on the Customer Value Theory, this study provides real-life factors to the fitness and smart health care industry that will adopt a proactive lifestyle pattern. Findings Empirical analysis proved properties such as the complexity of fitness apps, monetary rewards, exercise monitoring and the enjoyment of using fitness apps contribute to using fitness apps on a daily basis. Furthermore, more daily use of fitness apps was shown to have a greater impact on exercise. This study also confirmed that social comparison does not affect the daily use of fitness apps and the amount of exercise, and the habitual walking was proved to directly affect the amount of exercise.
