Dioxins are chemical substances that are not properly decomposed degradation under natural photochemical, biological and chemical conditions in the environment, and bio-accumulate through the food chain, so pose a risk of causing adverse effects to human health and the environment. Ninety seven percent of human exposure to dioxins comes through meat, fish, dairy consumption and so on. Pork is the largest proportion meat aspect of daily intake in the Korean food. Pigs are three times more than Cattle in terms of breeding population and production amount in the world, and the amount is increasing every year. Therefore, it is necessary to monitor the exposure level of dioxins in pig's body. Also, it is required to evaluate the presence of these chemical substances in the pig's feed grain such as corn, soybeans, and tallow. The purpose of this study was to evaluate PCDD/Fs in pig's feed to search the origin of PCDD/Fs. Feed samples obtained were wheat from East Europe, corn from South America and America, soybean meal from Korea, America, South America and India and tallow from Korea. The preparation of samples was based on the EPA method 1613. Instrumental analysis was based on the use of high resolution gas chromatography coupled to high resolution mass spectrometry (HRGC/HRMS). The concentrations of 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD) and 1,2,3,7,8-pentachlorodibenzo-p-dioxin (PCDD), which are the most toxic potent dioxins, were not observed in any of the samples. However OCDD was detected in most of the samples. The TEQs in the feed samples were ranged from 0 pg TEQ/g to 0.09841 pg TEQ/g. Especially, the high level of TEQs was observed in the samples of wheat from East Europe and soybean meal from Korea.
The relationship between level of milk composition and conditions of ovary and uterus were analyzed in Holstein cows at seven farms participating in a reproductive herd health management program. Milk data were taken from 503 early lactating cows between 30 and 60 days in milk with reproductive examination with ultrasonography from september 1999 to August 2000. Milk fat, protein and solid-not-fat concentration in the herds were $3.70{\pm}1.08%$, $2.97{\pm}0.35$, and $8.41{\pm}0.61%$, respectively. The reproductive disorder relative to normal cows had higher risk in the cows that the level of protein was lower than 2.70%. Also, the higher milk fat than 4.50% were associated with a higher risks in the uterine disease and follicular cysts. Therefore, the cows with the fat to protein ratio of > 1.30 had higher risks for reproductive disorder such as cystic ovarian diseases, inactive ovaries and endometritis. These results indicated that cows diagnosed with reproductive disorder were energy deficient prior to reproductive disorder diagnosis. Consequently, milk fat and protein analyses may be used serve as a monitoring tool for condition of ovary and uterus in early lactating cows
Purpose: Duodenal stump leakage (DSL) is a potentially fatal complication that can occur after gastrectomy, but its underlying risk factors are unclear. This study aimed to investigate the risk factors and management of DSL after laparoscopic radical gastrectomy for gastric cancer (GC). Materials and Methods: Relevant data were collected from several prospective databases to retrospectively analyze the data of GC patients who underwent Billroth II (B-II) or Rouxen-Y (R-Y) reconstruction after laparoscopic gastrectomy from 2 institutions (Sir Run Run Shaw Hospital, Zhejiang University School of Medicine, and HwaMei Hospital, University of Chinese Academy of Sciences). The DSL risk factors were analyzed using univariate and multivariate analysis regression. Results: A total of 810 patients were eligible for our analysis (426 with R-Y, 384 with B-II with Braun). Eleven patients had DSL (1.36%). Body mass index (BMI), elevated preoperative C-reactive protein (CRP) level, and unreinforced duodenal stump were the independent risk factors for DSL. DSL was diagnosed in 2-12 days, with a median of 8 days. Seven patients received conservative treatment, 3 patients received puncture treatment, and only 1 patient required reoperation. All patients recovered successfully after treatment. Conclusions: The risk factors of DSL were BMI ≥24 kg/㎡, elevated preoperative CRP level, and unreinforced duodenal stump. Nonsurgical treatments for DSL are preferred.
Background: Promoter hypermethylation is a frequent epigenetic mechanism for gene transcription repression in cancer and is one of the hallmarks of the disease. Cadherin EGF LAG seven-pass G-type receptor 3 (CELSR3) contributes to cell contact-mediated communication. Dysregulation of promoter methylation has been reported in various cancers. Objectives: The objectives of this study were to investigate the CELSR3 hypermethylation level in oral squamous cell carcinomas (OSCCs) using methylation-sensitive high-resolution melting analysis (MS-HRM) and to correlate CELSR3 methylation with patient demographic and clinicopathological parameters. Materials and Methods: Frozen tissue samples of healthy subjects' normal mucosa and OSCCs were examined with regard to their methylation levels of the CELSR3 gene using MS-HRM. Results: MS-HRM analysis revealed a high methylation level of CELSR3 in 86% of OSCC cases. Significant correlations were found between CELSR3 quantitative methylation levels with patient ethnicity (P=0.005), age (P=0.024) and pathological stages (P=0.004). A moderate positive correlation between CELSR3 and patient age was also evident (R=0.444, P=0.001). Conclusions: CELSR3 promoter hypermethylation may be an important mechanism involved in oral carcinogenesis. It may thus be used as a biomarker in OSCC prognostication.
Effective and systematic sanitation management programs are necessary to prevent foodborne disease outbreaks in school foodservice operations. The purpose of this study was to identify the elements to improve in order to ensure the safety of school food service by evaluating sanitation management practices implemented under HACCP-based programs. The survey was designed to assess the level of hygiene practices of school food service by using an inspection checklist of food hygiene and safety. Fifty-four school foodservice establishments considered as poor sanitation practice groups from two year inspections by Seoul Metropolitan Office of Education were surveyed from September to December in 2005. Inspection checklists consisted of seven categories with 50 checkpoints; facilities and equipment management, personal hygiene, ingredient control, process control, environmental sanitation management, HACCP system and safety management. Surveyed schools scored $68.0{\pm}12.42$ points out of 100 on average. The average score (% of compliance) of each field was 10.7/20 (53.3%) for facilities and equipment management, 7.4/11 (67.2%) for personal hygiene, 7.4/11 (74.1%) for ingredient control, 22.4/32 (69.8%) for process control, 8.9/12 (73.8%) for environmental sanitation management, 4.2/7 (59.7%) for HACCP systems management, and 7.2/8 (89.7%) for safety management, respectively. The field to be improved first was the sanitation control of facilities and equipment. The elements to improve this category were unprofessional consultation for kitchen layout, improper compartment of the kitchen area, lacks of pest control, inadequate water supply, poor ventilation system, and insufficient hand-washing facilities. To elevate the overall performance level of sanitation management, prerequisite programs prior to HACCP plan implementation should be stressed on the school officials, specifically principals, for the integration of the system.
This study was performed to investigate the dose-response relationship between average daily cadmium dose (ADCD) from rice and the occurrence of urinary cadmium (U-Cd) in individuals eating that rice. This was a retrospective cohort designed to compare populations from two areas with different levels of cadmium contamination. Five-hundred and sixty-seven participants aged 18 years or older were interviewed to estimate their rice intake, and were assessed for U-Cd. The sources of consumed rice were sampled for cadmium measurement, from which the ADCD was estimated. Binary logistic regression was used to examine the association between ADCD and U-Cd (cut-off point at $2{\mu}g/g$ creatinine), and a correlation between them was established. The lowest estimate was $ADCD=0.5{\mu}g/kg\;bw/day$ [odds ratio (OR) = 1.71; with a 95% confidence interval (CI) 1.02-2.87]. For comparison, the relationship in the contaminated area is expressed by $ADCD=0.7{\mu}g/kg\;bw/day$, OR = 1.84; [95 % CI, 1.06-3.19], while no relationship was found in the non-contaminated area, meaning that the highest level at which this relationship does not exist is $ADCD=0.6{\mu}g/kg\;bw/day$ [95% CI, 0.99-2.95]. Rice, as a main staple food, is the most likely source of dietary cadmium. Abstaining from or limiting rice consumption, therefore, will increase the likelihood of maintaining U-Cd within the normal range. As the recommended maximum ADCD is not to exceed $0.6{\mu}g/kg\;bw/day$, the consumption of rice grown in cadmium-contaminated areas should not be more than 246.8 g/day. However, the exclusion of many edible plants grown in the contaminated area from the analysis might result in an estimated ADCD that does not reflect the true level of cadmium exposure among local people.
BACKGROUND/OBJECTIVES: Inflammation is known to be a risk factor for metabolic diseases. This study aimed to develop a Food-based Index of Dietary Inflammatory Potential (FBDI) and examine its association with metabolic biomarkers. SUBJECTS/METHODS: This study analyzed the raw data from the 2012-2014 Korean Genome and Epidemiology Study data of 17,771 people. To analyze the relationship between foods consumed by Koreans and inflammation, we conducted a correlation analysis between 51 food groups and hs-CRP levels. The FBDI was developed from 17 food groups selected by multiple regression method. We examined whether FBDI was associated with metabolic markers (waist circumference, blood pressure, fasting glucose, triglyceride, and HDL-cholesterol) in the 6th Korea National Health and Nutrition Examination Survey (KNHANES). We used binary logistic regression analysis to examine the association. RESULTS: The FBDI model included seven of the anti-inflammatory food groups and three of the pro-inflammatory food groups. The FBDI formula was calculated by multiplying the intake of food group by ${\beta}$-coefficients derived from the multiple regression model based on the correlation analysis. The FBDI was significantly associated with waist circumference (P < 0.001), blood pressure (P < 0.001), triglyceride level (P < 0.001), and HDL-cholesterol (P < 0.001) level among adults aged 20-64 years in the KNHANES. The prevalence of metabolic syndrome was 2.618 times higher in the group with the highest FBDI than in the group with the lowest one (95% confidence interval: 1.778-3.856, P for trend < 0.001). CONCLUSIONS: This study established an FBDI reflecting food intake patterns of Koreans, which showed a significant relationship with the prevalence of metabolic syndrome.
Purpose: Normal pancreas has low uptake rate in $^{18}F$-FDG PET scan. However, it is possible to diagnose malignancy of pancreatic cancer which has high uptake rate. Many studies approve a high prevalence of diabetes in pancreatic cancer and if the blood glucose level (BGL) is over the normal range, FDG uptake will be decreased and there will be inconvenience for patients from the delay time to reduce the high BGL or could cause difficulty to arrange the schedule. Therefore, we studied the relation of BGL and image quality in pancreatic cancer on PET. Materials and Methods: A hundred patients had PET scan. The prevalence of pancreatic cancer and diabetes were evaluated using SPSS ver. 17. The fasting BGL of patients were examined and sorted as diabetes mellitus (DM) group and Non-DM group. For the evaluation, patients were divided into 3 groups (Non-DM, DM; BGL${\geq}7.0mmol/L$, and DM; BGL<7.0 mmol/L). The ROI was drown on Liver and Lung for the PET imaging analysis. Results: Fifty three male and forty seven female were in the patients. The average age was $60.1{\pm}13.5$. There were 36 patients (male: 22, female: 14, 36%) who had pancreatic cancer with DM. There were 15 patients who showed over 7.0 mmol/L in their fasting BGL and 85 patients who showed under 7.0 mmol/L in their fasting BGL. Among the Non-DM, DM; BGL${\geq}7.0mmol/L$, and DM; BGL<7.0 mmol/L, there was not a statistical significance (p>0.05). Conclusion: The prevalence of pancreatic cancer was comparatively high in this study. If the fasting BGL was slightly over the normal BGL, we believe it will not give a severe disturbance when the patients have PET scan. Furthermore, the examination schedule doesn't need to change and the inconvenience from the delay of patients preparation will be reduced.

Objectives: The association of air pollution levels and land-use types with changes in the prevalence of allergic diseases (allergic conjunctivitis, allergic rhinitis, asthma, and atopic dermatitis) was investigated for seven metropolitan cities in Korea Methods: Data on daily hospital visits and admissions (of those under 19 years old) for 2003-2012 were obtained from the National Health Insurance Cooperation. Meteorological data on daily mean temperature, humidity, and air pressure were obtained from the Korea Meteorological Administration. Daily mean or maximum concentration data for five pollutants ($PM_{10}$, $O_3$, $NO_2$, $SO_2$, and CO) as measured at air quality monitoring sites operated by the Ministry of Environment were used. We estimated excess risk and 95% confidence intervals for the increasing interquatile range (IQR) of each air pollutant using Generalized Additive Models (GAM) appropriate for time series analysis. Results: In this study, we observed a significant association between the IQR increases of air pollutants and the prevalence risk of allergic diseases (allergic conjunctivitis, allergic rhinitis, asthma, and atopic dermatitis) in all metropolitan cities after adjusting for temperature, humidity, and air pressure at sea level. Among the air pollutants, $NO_2$ and $PM_{10}$ were associated with the prevalence of asthma, and $O_3$ was associated with only allergic conjunctivitis in regression analysis. However, in GAM analysis considering land-use, $O_3$ and $SO_2$ were associated with allergic conjunctivitis, PM10, O3, NO2, and CO were associated with allergic rhinitis, and $PM_{10}$, $O_3$ and $NO_2$ were associated with asthma in industrial area. Conclusion: This study found a significant association between air pollution and the prevalence of allergic related diseases in industrial areas. More detailed research considering mixed traffic-related air pollution (TRAP) and conducting meta-analyses combining data of the all cities is required.
It is well known that smoking habit is hazardous to health, especially for juvenile. The present study on smoking behavior of high school students in Seoul has two major objectives. The first objective is to find out the smoking behavior of high school students in Seoul. Toward this objective, individual's smoking experience has been examined as ever smoking and never smoking. The second objective is to determine the variables associated with their smoking behavior at the individual, family and school environment levels. For the data collection, the survey was carried out for the four high schools in Seoul from September 15 through October 15, 1982. The major findings are summarized as follows: 1. Smoking behavior of the students 1) Out of 1,278 respondents, 30.2% of them were found to be current smokers and 29.3% of them were former smokers. This implies that around 60% of school students in Seoul have experienced smoking. 2) A significant differences in the current smoking rates between two types of the school students were shown as 19.3% for day-time school and 42% for night-time school. 3) In terms of the current smoking behavior, the students who don't live with parents were higher in smoking rate than those of the living with parents. 2. Attitudes and knowledge about smoking 1) Attitudes of students toward smoking in high school days were shown that around 17% of them agreed with it and around 64% of them disagreed with it. 2) Around 99% of the respondents answered that their smoking is harmful for health. A source of the information about negative effects of smoking on health was 'Radio and TV' (23.9%) as the most influential, 'school teacher' (20.9%), 'Newspaper' (18.2%) and so on. 3. Behavioral analysis for the current smokers 1) The factors affected for motivation in the first smoking were 'curiosity' (59.7%), 'temptation of friend' (19.7%), 'resistance feeling, (7.1%), 'merely interest and pleasure' (6%) respectively. 2) The time of the first smoking was 'third grade of Junior-high school' (31.5%) as highest, 'first grade of Senior-high school' (23.7%) and 'second grade of Junior-high school' (14.7%). 3) An average daily number of cigarettes consuming of current smokers was seven cigarettes. 4. Family and school-mates influences on individual's smoking behavior 1) The data revealed a significant relationship between student's smoking and their parent's smoking behavior. Around 75% of the students whom both parents are smoking have experienced cigarette smoking. It was found that the individual's smoking behavior was influenced by his sibling. Around 65% of the students whom brothers are smoking have experienced cigarette smoking. 2) The 'Smoking-Index' of friendship network or a group explained individual's smoking behavior in the group. The result of dyad analysis of smoking behavior in the friendship network showed that a high score of 'Smoking-Index' tended to be explained an adoption of smoking behavior at the individual level in the group. on the other hand, a low score of 'Smoking-Index' explained non-smoking behavior in the group.

1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
