Health Impact Assessment as a Strategy for Intersectoral Collaboration

  • Objectives: This study examined the use of health impact assessment (HIA) as a tool for intersectoral collaboration using the case of an HIA project conducted in Gwang Myeong City, Korea. Methods: A typical procedure for rapid HIA was used. In the screening step, the Aegi-Neung Waterside Park Plan was chosen as the target of the HIA. In the scoping step, the specific methods and tools to assess potential health impacts were chosen. A participatory workshop was held in the assessment step. Various interest groups, including the Department of Parks and Greenspace, the Department of Culture and Sports, the Department of Environment and Cleansing, civil societies, and residents, discussed previously reviewed literature on the potential health impacts of the Aegi-Neung Waterside Park Plan. Results: Potential health impacts and inequality issues were elicited from the workshop, and measures to maximize positive health impacts and minimize negative health impacts were recommended. The priorities among the recommendations were decided by voting. A report on the HIA was submitted to the Department of Parks and Greenspace for their consideration. Conclusions: Although this study examined only one case, it shows the potential usefulness of HIA as a tool for enhancing intersectoral collaboration. Some strategies to formally implement HIA are discussed.

여성 가족부양자의 건강상태 및 가족부양의 부정적.긍정적인 영향 (Health Status of Women Caregivers and Negative and Positive Impacts of Family Caregiving)

  • The purpose of this study was to examine the health status of the daughter and daughter- in-law caregivers who care for a cognitively and/or functionally impaired elderly, individual to identify factors that were related to reported health outcomes, and to investigate the negative and positive impacts of family caregiving. Data was collected from 120 daughter and daughter-in-law caregivers and care-recipients using face to face interviews. Most caregivers were daughters- in-law (77.5%) and most care-recipients were female (88.3%). Sixty-eight percent (n=81) of caregivers reported depressive symptomatology. General health also deteriorated by caregiving. Caregivers reported several negative impacts (difficulties): care-recipients' problematic behaviors, deterioration of their own health, pressure from social norms related to family caregiving in Korea, intrafamily conflict, and economic problems. Contrary to the popular belief, caregivers reported diverse positive impacts of family caregiving (68.3%): a sense of filial responsibilities, recognition from elderly, family members, relatives, and society, education for the children, and familial harmony. Higher depression score was predicted by lower family income, the presence of cognitive impairment of care-recipients, and higher level of social conflict of caregivers. Poor general health of caregivers was predicted by older caregivers' age, lower competing roles of caregivers, and poor emotional health. While not seeking to deny the negative aspects of family caregiving, it is also necessary to understand positive aspects of family caregiving to see complete picture of caring for an elderly family member.

  • During the last decade, Health Impact Assessment (HIA) has been discussed worldwide as being an important tool for the development of healthy public policy. HIA has been advanced as a means of bringing potential health impacts to the attention of policy makers, particularly in sectors where health impacts may not otherwise be considered. HIA, a systematic assessment of potential health impacts of proposed public polices, programs, and projects, offers a means to advance population health by bringing public health research to bear on questions of public policy. In Korea, health-related items under current EIA (Environmental Impact Assessment) system can only be found in the categories of hygiene and public health. However, environment and public health are not adequately connected and also health is underestimated even though health is an important objective component for the implementation of Environmental Assessments (EA). As a result, health is not well integrated within criteria for investigating the impacts on environment. This study examines linkages for HIA from the related and relatively well-developed field of Prior Environmental Review System (PERS) which is similar to SEA and EIA in Korea.

지구온난화 최소화를 위한 신재생 에너지들의 잠재환경영향 (Identification of Potential Environmental Impacts among Renewable Energy Technologies Promising to Minimize Global Warming)

  • Global warming, which is one of the most serious challenges, has been the subject of intense debate and concern for many scientists, policy-makers, and citizens for at least the past decade. To protect the health and economic well-being of current and future generations, we must reduce our emissions like carbon dioxide. Alternatives to achieve an energy future without serious global warming are to change to clean and renewable sources of energy like the wind, the sun lights, rivers, the biomass, hydrogen, and oceans. To identify some of the key and new environmental impacts associated with renewable energy and hydrogen energy, we set up the new conceptual methodology. Specifically, new identified environmental and health impacts are related with the usage of hydrogen energy. When comparing with fossil fuel, the renewable energies can reduce the release of carbon dioxide when they are used except hydrogen produced from fossil fuel. However, all renewable energy technologies are not appropriate to all applications or locations. Our results suggest that all of alternatives to replace fossil fuel can release the several global and local impacts although they seems to be smaller than the impacts from fossil fuel. Therefore, the quantitative and detail analysis to assess environmental impacts of the alternative energies might be useful to make our decision for the future energy against the global warming.

농촌노인의 건강증진행위 관련 요인 - 신체적 건강의 매개효과를 중심으로 - (The Impacts of Physical Health on Health Behaviors of The Korean Rural Elderly)

  • The purpose of this study is to examine the impacts of the physical health on health behaviors of the rural elderly. In this study, physical health is measured self-rated health and farmer symptoms. The specific questions addressed in this study are: What are the general tendencies of health behavior, self-rated health and farmer symptoms? What are the direct impacts and indirect impacts of respondents' characteristics, self-rated health and farmer symptoms on health behaviors of the rural elderly? For this purpose, survey data was gathered from 881 rural elderly who live in a village. The statistical methods used for data analysis were descriptive statistics, correlations, and path analysis with spsswin 12.0 program. The major findings of this study are as follows: The level of health behaviors and self-rated health is low and the level of farmer symptoms is high. In general, it can be said that respondents of this study have the low level of physical health and health behaviors. A path analysis shows the relation of variables, which influence on health behaviors. Economic status, education status and farmer symptoms play direct and positive effects on health behaviors. Especially, the effect of farmer symptoms is more important than the others. Sex and age are indirectly significant on health behavior. The results obtained from the study confirm that objective physical health, like as farmer symptoms, plays important roles in health behavior of the rural elderly. It means that the Korean rural elderly need many facilities and services to promote their physical health.

일부 여대생의 구강건강이 삶의 질에 미치는 영향 (Oral health related quality of life of women college students)

  • The purpose of this study was conducted to evaluated the effects of oral health related quality of life and impacts of oral health related quality of life of early adult group through a questionnaire OHRQoL(Oral health related quality of life). The study subjects were 224 women college students. The results of this study were as follow; 1. Physical aspects score was $41.98{\pm}7.78$, social aspects score was $33.04{\pm}7.25$, psychological aspects score was $32.50{\pm}7.50$ in effects of oral health related quality of life. 2. Physical aspects score was $25.49{\pm}4.86$, social aspects score was $20.65{\pm}4.47$, psychological aspects score was $19.84{\pm}5.53$ in impacts of oral health related quality of life. 3. The better oral health was the higher effects of oral health related quality of life(pE0.05). No missing tooth(pE0.01), use of oral health device(pE0.05), no experience oral health education(pE0.05) group was higher impacts score of oral health related quality of life. 4. The significantly correlated among all items on effects of oral health related quality of life. Therefore it suggested that college students oral health program, need precision oral exam and oral health education.

Occupational Injury Prevention Research in NIOSH

  • This paper provided a brief summary of the current strategic goals, activities, and impacts of the NIOSH (National Institute for Occupational Safety and Health) occupational injury research program. Three primary drivers (injury database, stakeholder input, and staff capacity) were used to define NIOSH research focuses to maximize relevance and impact of the NIOSH injury-prevention-research program. Injury data, strategic goals, program activities, and research impacts were presented with a focus on prevention of four leading causes of workplace injury and death in the US: motor vehicle incidents, falls, workplace violence, and machine and industrial vehicle incidents. This paper showcased selected priority goals, activities, and impacts of the NIOSH injury prevention program. The NIOSH contribution to the overall decrease in fatalities and injuries is reinforced by decreases in specific goal areas. There were also many intermediate outcomes that are on a direct path to preventing injuries, such as new safety regulations and standards, safer technology and products, and improved worker safety training. The outcomes serve as an excellent foundation to stimulate further research and worldwide partnership to address global workplace injury problems.

노인의 본인인지 구강건강상태와 우울감 (Association of self-perceived oral health and depression in the elderly: the sixth Korea national health and nutrition examination survey(the 6th KNHANES))

  • Objectives: The aim of the study was to investigate the association of self-perceived oral health and depression in the Korean elderly. Methods: The subjects were 1,329 elderly in Korea from the sixth Korea National Health and Nutrition Examination Survey (the 6th KNHANES). The dependent variable was depression that continued more than two weeks. Model I for the impacts of depression on self-reported oral health, Model II for the impacts of depression on chewing problem, Model III for the impacts of depression on speaking problem was evaluated. PASW Statistics 18.0 version was used for frequency analysis, chi-square test and logistic regression analysis. Results: Female elderly were much more prone to depression than male. Female had lower monthly compensation, less medicaid, chewing problem, speaking problem, and less education. For these reasons, they tended to have more depression than male (p<0.001). Self-perceived oral health impacts on depression included poor self-reported oral health(p<0.01), poor chewing problem(p<0.01) and poor speaking problem(p<0.05). On the other hand, male did not show a statistically significant association. Conclusions: The study showed the self-perceived oral health related quality of life had a significant influence on depression in the elderly. The continuing lifelong oral health care can prevent depression in the later life because oral health care improvement can enhance the self-perceived oral health status.

한국에서의 건강영향평가 도입 방안 (Introduction Scheme of Health Impact Assessment in Korea)

  • Korea has 30 years of experiences in environmental impact assessment (EIA). Although EIA includes sanitation-public health factor, considering health impacts, among 74 unit projects of 17 sections, health impacts haven't been properly considered or have been ignored in many cases. The increasing awareness on the importance of health impacts has triggered this study to seek an optimal introduction scheme of health impact assessment (HIA). The processes of EIA already include screening, scoping, analysis, impact assessment, consultation, document review, decision making and monitoring, in which they would be the essential parts of HIA. In this context, integrating HIA into the existing EIA process could be the most effective way to use the benefits in both legal and procedural processes existed and to avoid the confusion and overlapping since the close relationships between environment and health impacts might be. Furthermore, it is desirable that the existing sanitation-public health factor should be substituted by and extended to environment-health factor with sufficient determinants to properly consider health impacts. When considering the first step of HIA, the prospective and qualitative approach is suitable more than the retrospective and quantitative one due to the lack of database accumulated. Similarly, an approach based on epidemiology and toxicology could analyze the limited evidences and impacts related to human disease, whereas one based on socio-science and psychology could provide the effective means available for predicting how the people and community will act by the change of surroundings. Checklist approach with various and comprehensive health determinants focused on prospective and qualitative methods will be very useful for more convenient and progressive dissemination of HIA. Various checklist approaches of toolkits could be found from HIA documents elsewhere, for example Westminster Toolkit, and they would be helpful to figure out how to develop common procedures and health determinants for checklist, in which the unique characteristics on korean cultural and political context compared to abroad should be carefully considered since checklist would be the most basic and essential part of HIA. After the establishment of checklist and procedural processes, the pilot projects should be conducted. Main purpose of pilot projects is to apparently prove the effectiveness and profitability of HIA. Pilot projects should be implemented to decide the effectiveness and suitability of HIA for future projects, programs and policies, and should be provided as the positive cases that can be achieved through the proper implementation and progress.

건강영향평가를 통한 아동 복지서비스 개선: 청주시 드림스타트 센터 사례 (Improvement of Child Welfare Service Using Health Impact Assessment: A Case of the Cheongju Dream Start Center)

  • Objective: This study aimed to introduce Health Impact Assessment using the case of a HIA on 2008 Dream Start Project in Cheongju. Methods: We followed the typical procedure of HIA recommended by the Merseyside Guidelines on HIA. In scoping, the steering committee decided 5 key domains of child health to be assessed: prenatal care, vaccination, nutrition, access to health care, and child abuse and also the methods of collecting the evidence. The HIA appraisal team collected information from various sources including literature, community survey, and focus group interviews. The HIA appraisal team also synthesized the collected information in terms of the nature of health impacts and equity and made recommendations accordingly. Results: Positive impacts were expected in prenatal care, vaccination, and nutrition, while negative impacts were expected in access to health care. The impact of Dream Start on child abuse was uncertain. Several recommendations were made and submitted to the Dream Start team for their consideration. About 2 years later, we found many of them were implemented in 2009 Dream Start project. Conclusion: HIA was found to be applicable and effective to make decision makers in the welfare sector consider health in their work.
