Under the Korean mental health circumstances where familistic culture is predominant and social resources are limited, siblings are an important presence that effect chronically mentally ill people's lives. Despite this fact, our society in some aspect overlooks their importance. Therefore, this study is focused on the role of siblings as caregivers of chronically mentally ill adults and is conducted to understand the relevant process and types of how siblings fulfill the role of caregivers. In order to achieve this goal, data was collected from nine study participants through individual interviews and focus group interviews and its contents were analyzed according to Grounded Theory. The results revealed that the siblings' experiences as caregivers of chronically mentally ill people were mainly categorized as 'carrying painful memory, responsibility, and concern about the obscure future of the mentally ill sibling and finding a caring role different from those of parents.' Moreover three types of sibling caregivers were recognized: 'reality-adapting, obligation fulfilling' type, 'sacrificial self-responsibility fulfilling' type, and 'growing guardianship creator' type. This research aimed to vividly deliver the voices of research participants and proposed social support and permanent planning services based on the experiences and desires unique to siblings.
Background: Carbapenem-resistance is rapidly evolving among the pathogenic microbes in intensive care units (ICUs). This study aimed to determine annual trend of carbapenem-resistance in the ICU for 4 years, since the opening of a university-affiliated hospital in South Korea. Methods: From 2005 to 2008, microbial samples from consecutive 6,772 patients were screened in the ICU. Three hundred and ninety-seven patients (5.9%) and their first isolates of carbapenem-resistant pathogens were analyzed. Results: The percentage of patients infected with carbapenem-resistant organisms increased constantly during the initial three years (2.3% in 2005, 6.2% in 2006, 7.8% in 2007), then it declined to 6.5% in 2008. Acute Physiology and Chronic Health Evaluation (APACHE) III score at admission was $58.0{\pm}23.5$, the median length of the ICU stay was 37 days, and the mortality rate was 37.5%. The sampling sites were endotracheal suction (67%), catheterized urine (17%), wound (6%) and others (10%). Bacteria with carbapenem-resistance were Pseudomonas aeruginosa (247 isolates, 62%), Acinetobacter baumannii (117 isolates, 30%), Enterobacteriaceae (12 isolates, 3%), and others (21, 5%). Of note, peak isolation of carbapenem-resistant microorganisms in medical ICU was followed by the same epidemic at surgical ICU. Conclusion: Taken together, carbapenem-resistant pathogens are of growing concern in the ICU.

Recently, river maintenance is change due to concern for the environment increases. Thus, the river restoration and river environment is best part of river maintenance. In case of Korea, existing river is improvement straightly for flood control and transportation. When the stream channel is straightly, maintain stability is important. Thus, construction of levees along the river. The various river structures for the purpose of flood control and transportation are inhibit factors of longitudinal and lateral connectivity. Connectivity is defined as the maintenance of lateral, longitudinal, and vertical pathways for biological, hydrological, and physical processes. Long-term point of view, increased connectivity is very important for a healthy ecosystem composition. As the first step of river restoration, this study described theory and concept of river continuum and the numerical model was applied to a real topography to simulate the flow analysis with or without segregated and blocked space in the Mankyung river. The results of this study can be utilized to develop the watershed connectivity assessments methods in order to the river restoration.
Herpes simplex virus type 2 (HSV-2) infection has been a public health concern worldwide. It is the leading cause of genital herpes and a contributing factor to cervical cancer and human immunodeficiency virus (HIV) infection. No vaccine is available yet for the treatment of HSV-2 infection, and routinely used synthetic nucleoside analogs have led to the emergence of drug resistance. The small molecule $Retro-2^{cycl}$ has been reported to be active against several pathogens by acting on intracellular vesicle transport, which also participates in the HSV-2 lifecycle. Here, we showed that Retro-2.1, which is an optimized, more potent derivative of $Retro-2^{cycl}$, could inhibit HSV-2 infection, with 50% inhibitory concentrations of $5.58{\mu}M$ and $6.35{\mu}M$ in cytopathic effect inhibition and plaque reduction assays, respectively. The cytotoxicity of Retro-2.1 was relatively low, with a 50% cytotoxicity concentration of $116.5{\mu}M$. We also preliminarily identified that Retro-2.1 exerted the antiviral effect against HSV-2 by a dual mechanism of action on virus entry and late stages of infection. Therefore, our study for the first time demonstrated Retro-2.1 as an effective antiviral agent against HSV-2 in vitro with targets distinct from those of nucleoside analogs.
The purpose of this study was to investigate the elementary school dietitian's status and recognition of nutrition education (NE) in Incheon. A cross-sectional study was carried out using a self-administered questionnaire and subjects were 100 elementary school dietitians. The results are as follows. As for training in NE, $61.2\%$ of the dietitians attended training in NE. After training in NE, $86.5\%$ of the dietitians who attended training in NE were more concerned about NE. Also $59.5\%$ of the dietitians gave students NE and most of them did as a weekly printout 2-4 times per month. There was a significant difference in experience of NE for teachers between subgroups by experience of training in NE; while $48.1\%$ of the dietitians with training in NE gave teachers NE, $20.0\%$ of the dietitians without training in NE gave teachers NE. The main reason for not giving NE was too much work load and low concern of school administration. Also $96.4\%$ of the dietitians answered that NE is necessary in elementary school and the main reason for being necessary was correction of unbalanced diet and good table manner, As for proper time to start NE for children, $51.8\%$ of dietitians answered 'kindergarten' and $45.8\%$ of them answered 'lower grade of elementary school'. As for effective type for NE, $59.5\%$ of the dietitians answered 'NE as a part of other subject' and $23.8\%$ of them answered 'NE as a separate subject'. Also $79.5\%$ of the dietitians answered 'teacher' as the suitable person for NE. Most of the dietitians recognized menu formation as the ideal major work load and office work as the most time-consuming work load. As to job satisfaction, most of them were dissatisfied with office work and NE. Therefore, it is nationally supported for elementary school students' health and well-being that school dietitians as NE specialists give NE with minimizing their office work and developing a standardized NE program.

One of the most important things in the researches on diseases is to attach more importance to prevention of a disease and preservation of health than to treatment of a disease, also to foods rather than to medicines. In this context, the most significant concern in examining a patient is to find the presence of disease, and, if any, to diaguose the type of disease, after which a pharmacotherapy is followed. In this paper, various diagnosis methods of Oriental medicines are discussed. And ocular inspection, the most important method among the 4 disease diagnoses of Oriental medicines, is studied. Observing a person's shape and color has been the major method for ocular inspection, which usually has been dependent upon doctor's intuition as of these days. We are developing an automatic system which provides objective basic data for ocular inspection. As the first stage, we applied the signal processing techniques to automatic feature extraction of faces for ocular inspection. Firstly, facial regions are extracted from the point of frontal view, which was followed by extraction of their features. The experiment applied to 20 persons showed that frontal face regions are perfectly extracted, as well as their features, such as eyes, eyebrows, noses and mouths. Future work will seek to address the issues of morphological operation for a few unfinished extraction results, such as combined hair and eyebrows.
The US railroad network carries 40% of the nation's total freight. Railroad bridges are the most critical part of the network infrastructure and, therefore, must be properly maintained for the operational safety. Railroad managers inspect bridges by measuring displacements under train crossing events to assess their structural condition and prioritize bridge management and safety decisions accordingly. The displacement of a railroad bridge under train crossings is one parameter of interest to railroad bridge owners, as it quantifies a bridge's ability to perform safely and addresses its serviceability. Railroad bridges with poor track conditions will have amplified displacements under heavy loads due to impacts between the wheels and rail joints. Under these circumstances, vehicle-track-bridge interactions could cause excessive bridge displacements, and hence, unsafe train crossings. If displacements during train crossings could be measured objectively, owners could repair or replace less safe bridges first. However, data on bridge displacements is difficult to collect in the field as a fixed point of reference is required for measurement. Accelerations can be used to estimate dynamic displacements, but to date, the pseudo-static displacements cannot be measured using reference-free sensors. This study proposes a method to estimate total transverse displacements of a railroad bridge under live train loads using acceleration and tilt data at the top of the exterior pile bent of a standard timber trestle, where train derailment due to excessive lateral movement is the main concern. Researchers used real bridge transverse displacement data under train traffic from varying bridge serviceability levels. This study explores the design of a new bridge deck-pier experimental model that simulates the vibrations of railroad bridges under traffic using a shake table for the input of train crossing data collected from the field into a laboratory model of a standard timber railroad pile bent. Reference-free sensors measured both the inclination angle and accelerations of the pile cap. Various readings are used to estimate the total displacements of the bridge using data filtering. The estimated displacements are then compared to the true responses of the model measured with displacement sensors. An average peak error of 10% and a root mean square error average of 5% resulted, concluding that this method can cost-effectively measure the total displacement of railroad bridges without a fixed reference.

This study was undertaken to evaluate the relation between obesity and dietary life of children aged 4-7 divided into two groups based on height for age, weight for age and weight for height. Controls were selected based on sex and age similar with those of cases. Cases and controls were selected using the combination of weight for height, weight for age and height for age by the World Health Organization standard. Cases were 23 children judged as obese. Controls were 37 judged as normal. Whether their mother had a job or not, showed relative risk 1.35(95% C1 0.32-5.64). In obese group, employmental statue of mother was 17.4% compared with 13.5% in control group. In mother's concern for cooking pattern, the relative risk was 5.64 and 95% CI was 1.70-18.66 in the item of "We consider the color arrangement when we serve foods" which was the highest rate. The item having the lowest relative risk was "We cook the meal by ourselves with spending time" which of the relative risk was 0.52 and 95% CI was 0.16-1.65. The item having a great significance in the dietary habit of subjects were "They beat the tablewear with the chopstick"(RR:1.64, 95% CI:0.22-12.73) and "I talk with food in my mouth"(RR :1.11, 95% CI:0.39-3.15), and the other items didn't show significancy. Number of food eating per day for male was 30.0$\pm$10.93 in obese group where as 23.2$\pm$9.80 in control group. 22.3$\pm$4.56, 21.8$\pm$10.91 were taken obese group and control group respectively in female. In the survey for general habit in life, the item of "We has a time for conversation with our family regularly" was high correlation to obesity and item of "We made our children change clothes and go to toilet themselvesj has slight relation to obesity. From above, we observed normal and obese children had different factors such as mother's attitude for preparing meals, table manners and habit of living. So we should take a continuous interest in children's dietary life in order to correct the wrong dietary habit and to protect from future problems.
The zoonotic transmission of viral diseases to humans is a serious public health concern. Pigs are frequently a major reservoir for several zoonotic viral diseases. Therefore, periodic surveillance is needed to determine the infection rates of zoonotic diseases in domestic pigs. Hepatitis E virus (HEV), rotavirus, sapovirus (SaV), and norovirus (NoV) are potential zoonotic viruses. In this study, 296 fecal samples were collected from weaned piglets and growing pigs in 13 swine farms, and the viral RNA was extracted. Partial viral genomes were amplified by reverse transcription-polymerase chain reaction (PCR) or nested-PCR using virus-specific primer sets under different PCR conditions. HEV-3, rotavirus A, and SaV genogoup 3 were detected from 11.5, 2.7, and 3.0% of the samples, respectively. On the other hand, NoV was not detected in any of the samples. Genetic analysis indicated that the nucleotide sequences of swine HEV-3 and rotavirus A detected in this study were closely related to those of human isolates. However, swine SaV was distant from the human strains. These results suggest that HEV-3 and rotavirus A can be transmitted from pigs to humans. Therefore, strict preventive measures should be implemented by workers in the swine industry to prevent infections with HEV-3 and rotavirus A excreted from pigs.
We can and must improve the diagnostic images using available knowledge and technology. At the same time we must strive to reduce the patient's integral and entrance radiation dose. Reducing the integral dose to the patient during the radiologic procedure is a primary concern of the patient, especially the pediatric patient, the radiologist and the technologist. A 100cm focal film distance generally is used for most over-table radiography. The early x-ray tubes and screen film combinations required long exposures, which often resulted in motion artifacts. But nowaday, we have the generators and x-ray tubes that can deliver the energy necessary in a very short time and the receptors that can record the information just as rapidly. And, we performed this studies to evaluate the patient exposure dose and the image quality by increasing focal film distance in diagnostic radiography. There are many factors which affected to exposure factor, but we studied to verify of FFD increase, only. Effect of increasing the focal film distance to a 140 cm distance was tested as follows; 1. The focal film distances were set at 100, 120, and 140cm. 2. A 18cm acryl(tissue equivalent) phantom was placed on the table top. 3. An Capintec 192 electrometer with PM 05 ion chamber was placed at the entrance surface of the phantom, and exposure were made at each focal film distances. 4. The procedure was repeated in the same manner as above except the ion chamber was placed beneath the phantom at the film plane. 5. Exit exposure were normalize to 8mR for each portions of the experiment. Based on the success of the empirical measurements, a detailed mathematical analysis of the dose reduction was performed using the percent depth dose data. The results of this study can be summerized as followings ; 1) Increasing FFD from 100 cm to 140 cm, we would create a situation that would have a significant effect on the overall quality of radiograph and achive the 17.42% reduction of entrance dose and the 18.95% reduction of integral dose that the patient receives. 2) Thickness of Al step wedge for equal film density increased with the long distance. 3) Increasing FFD, Magnification of image was lowered. 4) Resolution of image also increased with the FFD. As the results described above, we strongly recommend using the long FFD to provide better information for our patients and profession in abdomen radiographic studies.
