Health Care Reform in OECD Countries : A Comparative Policy Analysis (OECD 국가의 보건의료개혁 : 역사적 전망과 정책적 과제)

  • The paper analyzes OECD health systems from the perspective of historical institutionalism. Criticizing the dependence of Korea's national health program on Pacific countries such as Japan and the U.S., it suggests that European experiences of national health programs can be a model of the Korean health system in the future. Based on an inquiry into Italian and British cases of national health systems, the author emphasizes (1) the role of local governemts in national health programs, and (2) the integration of a national health program with public health programs.

Effects of a Self-regulation Program on Blood Pressure Control and Self-Care in Patient with Essential Hypertension (자기조절프로그램이 고혈압환자의 혈압 및 자가간호행위에 미치는 효과)

  • Purpose: This study was done to verify the effects of a self-regulation program for management of hypertension. Method: Thirty patients with hypertension registered in a community health center were selected as the experiment group, and control group were patients in another community health center, matched for age and gender. The self-regulation program included daily blood pressure checks, periodic counseling, and health education. A self-check digital device with instructions was provided for self-monitoring of blood pressure, and the participants were interviewed before they took part in the program. Results: The first hypothesis was supported: There will be a greater reduction in both systolic and diastolic blood pressure for patients with hypertension who participate in the self-regulation program compared to patients in the control group. The second hypothesis was also supported: Patients with hypertension who participate in the self-regulation program will perform self-care activities better than those in the control group. Conclusion: The findings indicate that a self-regulation program reduces systolic and diastolic blood pressure and improves self-care in patients with hypertension. It is recommended that this self-regulation program be used in community health clinics for management of hypertension and prevention of complications.

A Comparison of Oral Health Behavior and Oral Health Outcomes between Cooperative and Non-Cooperative Groups following Implementation of an Oral Health Care Program (치위생 과정에 근거한 구강예방프로그램 적용 후 협조군과 비협조군 간 구강건강상태 및 행동 비교)

  • The purpose of this study was to reveal analyze the relationship between status of participation in an oral health care program and oral health outcomes among patients in Korea, and to evaluate the results to provide evidence regarding the feasibility of widespread implementation of the program. Patients were designated as either cooperative or non-cooperative with the oral health care program and were assigned to each group accordingly. Modified dental hygiene process (M-DHP) of the oral healthcare program was modified to form the dental hygiene process. The study included 48 patients at a dental clinic in Busan, Korea. Questionnaires were used to collect information on oral health behavior (OHB), clinical examination was used to record bleeding on probing (BOP) and O'Leary index, and phase microscopy was used to identify microorganisms. Differences between groups were evaluated using repeated measures ANOVA. Our results showed that the group cooperative with the oral health care program showed greater improvement in OHB, BOP, and O'Leary index than the non-cooperative group. Second, patient satisfaction with the M-DHP was very high, particularly for content and the friendly nature of the staff. The cooperative group showed greater improvement in oral health than the non-cooperative group for all metrics. Our results suggest that this low-coste program, if implemented, would be actively accepted and utilized in dental clinics.

The Effect of Tailored Health Care Program Based on Sasang Constitution for Vietnamese (사상체질에 기반한 베트남 맞춤형 건강관리 프로그램 효과)

  • Objectives This study aimed to develop the tailored health care program based on Sasang constitution for Vietnamese and evaluate the effect of them. Methods This research was executed from Sep. $1^{st}$ 2012 to Nov. $30^{th}$ 2012 in Vietnam Hanoi National Hospital of Traditional Medicine. All of 50 vietnamese subjects participated in the tailored health care program for 8 weeks after constitutional diagnosis. During the applicable period of the 8 weeks program, follow-up visits was conducted after 1 week, 4 week, and 8 week from the first visit, and Quality of life, Anxiety, Fatigue and Sleep Quality were measured. Results The tailored Health Care Program was applied to the subjects for eight weeks. The quality of life scores of Physical Functioning (p=0.006), Pain (p=0.003) and Physical Component Score (p=0.009) were significantly increased, and the sleep quality of subjects was improved after the tailored health care program is applied. Conclusions The tailored health care program based on Sasang constitution affected to some items about the quality of life and sleep quality of the subjects. however, this result can not be generalized to all Vietnamese, and various follow-up studies are needed.

  • This purposes of this study is to develope policies on the facilities for the elderly care in Korea. Methodology of this study adopts comparative analysis on the facilities for the elderly care in 6 countries which have experienced various problems on the elderly. These 6 countries are Japan, Sweden, America, Australian, England and Germany. Major issues for comparative analysis are the standard of care facilities, residence condition, finance of facilities establishment and expense, operating system, management, and the law and administrative structure about the facilities for the elderly care. The elderly people need various kinds of welfare services such as medical care facilities, nursing home facilities, home care facilities etc.. Thus the public policies for the aging population nations are compose of income maintenance program, health and medical care services and social welfare services. The policies of facilities for elderly care are very important since these policies include the characteristics of income maintenance, me\ulcorner미 care program and welfare program. This study willsupply basic data for the development of facilities for the elderly care in Korea, especially conceming the system and institutional device of the facilities.

Effects of the Latest Robotic Horse-riding for Low Back Pain : Narrative Review

  • Purpose: The purpose of this study is to review health-care program using the recent robotic horse-riding technology and its clinical effects for chronic low back pain. Research design, data and methodology: Recent clinical articles were determined under three inclusion criteria for in-depth review: 1) article that is published within 1 year, 2) article that includes the detailed explanation of health-care program using robotic horse-riding, 3) the article that deals with chronic low back pain during more than 6 months. Results: As a result, the finally-determined two articles demonstrated the clinical effects of robotic horse-riding statistically on pain intensity, low back muscle strength, spinal alignment, and fear-avoidance belief. Conclusions: After in-depth review, I concluded that health-care program using robotic horse-riding for chronic low back pain needs to be provided at low-intensity (e.g. less than 6km/h horse walking program) in the beginning of health-care for improving their motor control ability, then, at the increased intensity for strengthening core muscles.

The Effect of Self-help Programs for Hypertensives in Community Areas (고혈압환자를 위한 자조관리 프로그램의 효과)

  • This study was conducted to identify the effects of the comprehensive nursing aided self-help program for hypertensives. The program educates hypertensives in community areas about blood pressure, self-care, health promoting behaviors, and life satisfaction. For this purpose a one group time series design was used. Sixteen subjects were evaluated from S Community Health center of B city. Self-help programs developed by the author were given to the subjects. The program consisted of group education on hypertension and self-care including blood pressure self-monitoring, recording of self-care activities, and encouraging and reinforcing self-help. The whole program was carried out from October to December in 2002. Data was analyzed by repeated measure ANOVA, and post-hoc Turkey. The results were as follows: There were significant relationships between changes in systolic pressure(F=10.638. P=.000), diastolic pressure(F=6.783. p=.013), self-care(F=13.506. p=.000), and health promoting behavior(F=9.067. p=.001) at the 6th week and the 9th week. There were no significant relationships between changes in life satisfaction at the 6th week or 9th week. From these results, it can be concluded that the self-help program is an effective nursing strategy to promote self-care of hypertensives in community areas.

Evaluation Study of Home Health Care Program Operated in G Public Health Center (인천광역시 일 보건소 대도시 방문보건사업의 평가 연구)

  • Purpose: The purpose of this study was to evaluate the achievements of the project, and also to find out its strengths and weaknesses. Method: This evaluative study employed system theory and analytic techniques by using criteria which were relevance, appropriateness, adequacy, progress, effectiveness and satisfaction for input, process, output, and outcome of program. Study subjects were participants in the home health care program implemented in G public health center in Inchon metropolitan city in 2003. Results: Input factors including recruitment personnel, and support organization development were not adequately met for the program. However. the goal and objectives of the project were really appropriate for the community needs as well as government's policy. For the Process evaluation, home health care record form and computer data base had not progressed as scheduled, but overall program activities were finished on time. However, cooperation between organizations in the health center during service activity were not supportive. Managerial ability of program charged personnel about coordination and integration of team members was not affirmative. Output and outcome evaluation showed that people improved self care ability were 221 (17.5%). and 71 (5.62%) of subjects were moved into category of possible self-care. Client's satisfaction for this project showed a high degree. Conclusions: Based on the above results support organization and staff personnel for this program should be developed. Also, a community network of resources should be established and case management services should be focused continuously in community based home health care.

The Local Council Members' Attitudes to the Health Policy (지방의회의원들의 보건정책에 대한 인식)

  • In order to understand the local council members' attitudes to the health policy, we conducted mail surveys using self-administered questionaire for 2 months(February and March. 1995). The study subjects were 2.312 local council members in Korea, but only about 11% among whom. 257 persons, responded to 2 times mail survey. This response rate revealed that the local council members was not interested in health care fields. The main results were as follows; The respondents thought that the economic and income development was most important among 15 regional policy agendas and the health care was the 5th or 7th important agenda. They. who had more health needs of and poor access to health care, tended to think that the health care was more important. They considered lobbying to and persuading the civil servants as the best method to tackle the local health care policy agenda. The respondents, who had poor access to health care facilities. tended to set the highest priority for the expansion of public and private health care resources. They expected that the election of local governor would activate the public health program more than thought that the program was implemented more actively than other region. The main opinion of respondents was that the central government had to take over planning and financing for the public health program, and the basic local government had to implment the program and budgeting. The majority of respondents agreed the private dominant medical care delivery system and nation-wide uniformed financing mechanism. Over 60% of them suggested that they were ready to suffer environmental pollution inducing health hazards for the purpose of regional economic and income development. About 75% of them favour the campaign for antismoking regardless of reducing local government's revenue from sale tax.

Home visiting oral health program of Longterm home care service and the change of some pathogenic microorganism counts in denture (재가장기요양기관의 방문구강보건교육 전·후 의치의 일부 병원성 미생물의 집락수 변화)

  • Purpose: This study is a repetitive comparative analysis of a qualitative case study that carried out a home visiting oral health education program. Method: This study conducted an interview survey through medical examination by interview before and after the home visiting oral health education and measured some pathogenic microorganism counts in the denture. Results: There was a positive change in the self oral care ability of the elderly at home after home visiting oral health education, including the behavior of self management of dentures, and some pathogenic microorganism counts in the dentures. Conclusion: The home visiting oral health education of home care service centers can improve oral health care for the elderly at home.
