The purpose of the study was to evaluate the educational content which had been given by midwifery training program. It was hoped that this result would help. It was sponsored by com-munity health worker plan effective health education. College of Nursing Ewha Womans University and The Korean Nurses Academic Society during the November 19 thru 24, 1979. It was carried out on July through on September 1980, and involved 22 community health workers. The results were as follows: 1. Most of the community health workers came from Seoul & Pusan areas and have been working at the hospitals. There were 31.82% of Head Nurses, 27.2% of Staff Nurses, 22.73% Nurse Supervisons, 13.6% of Nurse Directors and 4.5% of educational coordinator for Nurses. These participant had nurse-midwifery lincences by 63.64%. None of there had just midwifery lincences. 2, Age structures of the study population shows 31.82% of whom are.26-30 years and 22.73% of whom are 36. 40 years of age. This shown that seniority proportion is higher than the younger. There are 31.82% of 1-5 years, 27.27% of 6-10 year and 11-15 years, respectively by work career. 3. There are 54.55% of the institutions have opened their own midwifery training course for their nursing staff members. Because of lack of the facilities, shortage of instructors, and problems of administrative process. 4. According to the institution which opened for midwifery training courses, the participant was responsible for “midwifery”“Infant care”“MCH”“practice of midwifery”“Nursing adjustment”and“F. P.”5. During the midwifery couse, there were 8 institution who used the textbook and 4 institution who did not. Least of there referned to content matinals which was given by the sponsored. 6. There are 7 insititues who kept their training courses with other professional helps such as physicians., professiors and nurses. Some problems are pointed out by respondents such as“conflict with residents”“poor suportive administration”and“lake of manpower”. 8. The participant showed that they learned new knowledge as trends during this programs for there quality work so it need (one or twice times) a year. But they suggested that it needed more emphasis on the“maternal health care”and“role of the nurse-midwifery”. 9. The analysis of the results are as follows within the 6 areas which are given by the sponsored: There are highest ranks between“basic theory & family planning”“role of midwifery & nursing practice”. In the prenatal care the highest rank ware related to“health risk”on“idenify of risk symtoms”. In the health care areas which related to delivery, the responsers were related to“general conditions”or“high risk criteria”. In the health care area which related to high risk maternity care. In the neonatal health care, the highest rank was related to”health assessment of normal infant”. In the infant health care the responses was related to“abnormal symptoms”and“risk symptoms”. Actually, the participants show that they are more interested in“role of midwifery”“health assessment”and “high risk maternity care”are which emphasised on health promotion, health maintenance & disease preventive. 1) The midwifery training program need higher education for midwifery on a regular basis. 2) Within the open institution of midwifery training program, the nurses must be supported by their own institution and administry of social welfare must give systematic support. Also non-open institution must be open very soon. 3) All health workers including the residents & other workers, must cooperate for their phased common good of impovement of the maternity health. 4) Administration agonies & education institutions must provide the curriculum facilitis and administration systems which are needed for training of nurse-midwifery.
Objectives : This study was performed to investigate health care system recognition and influential factors using the data from the "2017 Health Care Experience Survey". Methods : Data on 7,000 participants in the Health Care Experience Survey were drawn and statistically examined using a t-test, ANOVA, and multiple regression analysis. Results : First, the significant factors of health care service satisfaction were education, income, region, chronic diseases, unmet medical needs, satisfaction with doctors and institutions, and the health care system's reliability and importance. Second, the influential factors of willingness to pay additional health insurance premium were age, occupation, income, health status, chronic diseases, unmet medical needs, satisfaction with health care institutions, limit to utilization of medical services, necessity of health care reform, and the health care system's reliability, satisfaction, importance. Conclusions : Since the additional burden for improving the health care has been negative to the socially disadvantaged, there should be efforts to provide stable health care funding for financial stability of the health insurances by considering public opinions and reaching social consensus.
Background: The long-term care (LTC) group has higher rates of chronic disease and disability registration compared to the general older people population. There is a need to provide integrated medical services and care for LTC group. Consequently, this study aimed to identify medical usage patterns based on the ratings of LTC and the characteristics of benefits usage in the LTC group. Methods: This study employed the National Health Insurance Service Database to analyze the effects of demographic and LTC-related characteristics on medical usage from 2015 to 2019 using a repeated measures analysis. A longitudinal logit model was applied to binary data, while a linear mixed model was utilized for continuous data. Results: In the case of LTC ratings, a positive correlation was observed with overall medical usage. In terms of LTC benefit usage characteristics, a higher overall level of medical usage was found in the group using home care benefits. Detailed analysis by medical institution classification revealed a maintained correlation between care ratings and the volume of medical usage. However, medical usage by classification varied based on the characteristics of LTC benefit usage. Conclusion: This study identified a complex interaction between LTC characteristics and medical usage. Predicting the requisite medical services based on the LTC rating presented a challenge. Consequently, it becomes essential for the LTC group to continuously monitor medical and care needs, even after admission into the LTC system. To facilitate this, it is crucial to devise an LTC rating system that accurately reflects medical needs and to broaden the implementation of integrated medical-care policies.
In order to investigate the basic data for the visiting nursing care plan of Health Center in Korea, the questionnaire survey in regard to health care needs and health problems of the subjects with visiting nursing care was carried out on 131 subjects with visiting nursing care. The results were as follows : 1. The subjects consisted of 38.2% in male and 61.8% in female. 77.1% of the subjects had no job. 2. In the health problem, the subjects symptoms were 31.2% of hypertension, 20.6% of arthritis, and 19.1% of diabetes and other chronical illness. Utilization of medical care facilities were 61.8% of private clinics or general hospitals and 29.0% of Health Center. 3. 10 areas of health care needs that subjects wanted were disease management(19.5%), demand for welfare concerns(15.7%), health promotion and disease prevention(14.2%), information for medical institution(12.3%), health management for the aged(10.8%), hospice care(8.0%), prevention of dementia(8.0%), care for mental hygiene(6.7%), management for drinking, smoking and drug abuse(3.3%), home care nursing and rehabilitation nursing(1.2%).
The purpose of this study is to estimate cesarean section rate in Korea and analyze characteristics of health care institution which affect regional variation in the rate. We have searched vaginal and cesarean section deliveries among Diagnosis Related Group dat based upon insurance claim bills which have been submitted to Korean Insurance Corporation for two years since March, 1985. The results are as follows: 1. Out of all delivery cases of 87,500, cesarean section rate was 16.3% (14,299 cases). 2. Cesarean section rate varied according to size and ownership of health care institutions. In above 6- bed sized hospitals, the rate was at about 20% higher than small sized institutions, but rather in hospitals that have more than 500 beds, it was somewhat low. Classified by the hospital ownership, the rate was low at 18.4% in hospitals of religious organization and highest at private or corporate hospitals. 3. This study shows large regional variation in cesarean section rate; there are two times differences between region with the highest and lowest rate. Strongly related factors in that variation was the ownership of health care institution and urbanization variables. Low level of cesarean section rate in a region is explained by high proportion of delivery cases at institutions of religious organization and at insitutions in county level site. This result shows that apart from medical conditions of patients, indications of cesarean section differs from health care providers, and especially ownership of institution strongly affect them. Cesarean section rate in Korea is supposed to be at high level and development of utilization review programs to keep appropriate cesarean section rate is needed.
The relaxation of the regulation in selection of medical institution allows patients to use their own judgement in choosing proper institution for their diseases. Since the change of the regulation, there should have been many changes in medical institution selection behavior. The analysis of the change in disease specific selection pattern is critical because there be an optimal selection criteria that ensure the efficient and effective utilization of medical resources. This study analysis the institution selection factors by comparing the choice among the cases of acute diseases, the cases of chronic diseases, inpatient services, outpatient services, and emergency medical service. The comparisons performed in terms of size, class and other characteristics of medical institutions. For the study the nationally surveyed database was used and the data were analyzed using logistic regression procedure. The results indicates that the primary care facilities were not properly utilized. This study speculates that the reason for the undesirable pattern of utilization is that the roles of primary care facilities in the healthcare delivery system was not clearly defined. Based on the results, the medical policy implications are discussed.
Objectives : The purpose of this study was to examine factors affecting the effective demand of workers for dental treatment in a bid to provide some information on the improvement of oral health. Methods : The subjects in this study were 255 workers who were engaged in the pharmaceutical industry. A self-administered survey was conducted, and the collected data were analyzed by a spss(statistical package for the social science) win 12.0 program to find out their demographic characteristics, oral health awareness, oral health behavior and factors for the choice of a dental institution. Results : 1. Concerning the level of oral health awareness, 47.0 percent were at a low level(0 to three scores), and 35.2 percent were at an intermediate level(four to six scores). 2. As to oral health behavior involving a daily mean toothbrushing frequency, they brushed their teeth 2.65 times per day on the average. The most widely utilized oral hygiene supply was mouth rinse. Regarding oral health status, 53.7 percent weren't in good dental health, and just 23.0 percent got a dental checkup on a regular basis. 45.8 percent of those who didn't get a dental checkup cited time constraints as the reason. 26.0 percent received oral health education, and 91.4 percent were aware of the necessity of a corporate incremental dental care system. 3. Marital status, age and the reliability of dental institutions made differences to their choice of a dental institution. 4. As for the relationship between oral health awareness and the factors for the choice of a dental institution, oral health awareness had a statistically significant positive correlation to the reliability of dental institutions. Conclusions : The above-mentioned findings suggest that oral health education enables workers to have a correct knowledge on oral health, to change their own oral health behavior and to make the right choice of a dental institution in consideration of their oral characteristics. They should be urged to get a regular dental checkup not to develop chronic oral diseases so that they couldn't have to receive first-aid dental treatment. To promote the oral health of workers, oral health care personnels available should be utilized, and the incremental dental care system should be more vitalized to cut down on their effective demand for dental treatment.
Health care conditions in Korea are gradually improving along with the economic and social development. However, the volume of disease is still great, especially in rural areas. This study attempts, therefore, to initiate a comprehensive proposal of rural health care delivery system. The proposal is constructed three parts, problem of health care system, medical cost, medical education system. The proposal consist of the following components: I. The health care system 1. health sub-center is required to be locate in "Myun" the basis administrative unit of local government for delivering primary health care. But, in the viewpoint of medical economics, the primary health care is operated cautiously. 2. Health center is desirable to provide health services in coordinating the health sub-center and other private health institution. 3. The secondary health care is performed in regional combination hospitals, and the attitude that doctors accomodate this system is required. II. The medical cost, Insurance In the expenditure of medical care, the method of a third person's payment is required absolutely. III. The medical education system. 1. The medical education system (process) is changed from the medical education to regional doctor education. 2 In the nurse education system. nursing technical high school is resurrected.
Objectives: This study aims to find status of the oral health care, the oral health education program utilization and the oral health guidance according to the experience status of oral health education of special school teachers. Methods: The study subjects were 133 teachers at special schools in Seoul, Gyeonggi, Chungbuk and Jeonbuk regions. Results: 32.8% of the male and 67.2% of the female teachers have the oral health education experience. Barriers for the dental treatment to teachers with the education experience show in the order as 'disabled children's noncooperation' 49.2%, 'economic reasons' 37.7% and 'medical institution's rejection' 6.6% and, to teachers without the education experience, 'disabled children's noncooperation' 45.8%, 'economic reasons' 19.4% and 'not emergency situation' 13.9%(p<0.01). Regarding the necessity of arranged institution to manage the oral health, 91.8% of teachers with the oral health education answer as 'necessary'(p<0.05). Regarding the barriers on the performance of oral health program 27.9%, the majority of teachers with the education experience answer as 'insufficient administrative & financial support' and 36.1%, the majority of teachers without the education experience answer as 'insufficient understanding and expertise on the oral health education'. 86.9% of the teachers with the education experience and 62.5% of the teachers without the education experience are found to 'guide students on the oral health'(p<0.01). Conclusions: It was considered that various oral health education programs positively influential to the oral health care and education for disabled children should be developed according to the status of oral health education experience of special school teachers.
