독거 여자노인의 식품공급프로그램 실시 후 음식섭취빈도, 식태도 및 영양위험정도 변화 (The Changes of Dish Consumption Frequencies, Dietary Attitudes and Health-Nutrition Risk for Single Living Female Elderly on Food-Aid Program)

  • This study aimed to estimate the effect of the home based food supplying program in single living female elderly on the basis of depression, Health and Nutrition Risk Index, dietary attitude and dish consumption frequencies. The Food Supplying Program was carried out for 8 months for 27 single living female elderly from lower income status in Bucheon city. Main supplied foods were consisted with soymilk, vegetables, fruits and fish. The effect of the food supplying program were analyzed for 22 elderly who completed the program (group supplied with food : GSF) compared to the group not supplied with food (GNSF). The results of this study were as follows: Degree of depression estimated with depression score and Health-Nutrition Risk Index were decreased and the scores of dietary attitude and behaviors were increased for GSF after 8 months. However these improvements were not shown in the GNSF. GSF showed significantly increased consumption frequencies in soybean milk, tangerine, stir fried anchovy and yogurt, where as GNSF was observed with the decreasing trend in most of the dishes. In conclusion, home based food supplying program for single living female elderly not only increased the consumption frequencies of some dishes but also improved depression rate, Health and Nutrition Risk Index and dietary behavior concomitantly.

성남지역 노인 영양교육 프로그램의 효과 분석 (The Analysis of Effect on Nutrition Education Program for the Elderly in Sung-nam Area)

  • Elder people in Korea was affecting the nutritional status by following factors : low energy intakes, low food diversity, and poor quality of nutrition. Management a nutrition education program was planned to change the elder's nutrition knowledge and improve their nutritional status. There are seven kinds of indicator - knowledge for health questionnaire (before and after education) - we have investigated elderly nutrition education group in Su-Jung ku, Sung-Nam city. The items of surveyed was general characteristics and anthropometric measurement of the elder people, their mini dietary assessment index score, nutritional risk, nutrition knowledge test, and it's valuation comparisons between the before and after every education we did. According to the results of mini dietary assessment index score, the mean was 22.7 at the maximum 30 points and $94.2\%$ of respondents got more than nomal group. The nutritional risk score was the highest in 'high risk' group. Also the results of nutrition knowlede test showed that the mean increase $35.1\%$ of respondents. Therefore, significant improvement results showed by nutrition education programs in elders. These results suggests that the educating nutrition programs fur elder's encouraging eating behavior themselves and changing their knowledge in nutrition.

노인 영양개선 전략 연구 : 건강 관련 요인 및 영양위험지표 분석 (Strategies to Improve Nutrition in the Elderly : An Analysis of Health Related Factors and the Nutritional Risk Index of the Elderly)

  • The elderly are the most rapidly growing segment of the population in Korea and the largest consumers of expensive medical care. It is reasonable to believe that improving the nutritional status would be beneficial to reducing morbidity and to impro- ving the quality of their lives. This study was conducted to assess the health status and the Nutritional Risk Index of the elderly, and to provide the basic data for the Elderly Nutrition Improvement Program at the Public Health Center. One hundred and forty seven(76 males. 71 females) aged 60 years and over living in Suwon, were assessed with anthropometric measurements, body fat analysis, blood pressure measurements, and a set of questionnaires about health and other related variables. Over half of them had less than a junior middle school education and were considered low income. The percentage of overweight and obese subjects was 33% by the BMI(Body Mass Index), and the prevalence of hypertension was 28$\%$(males) and 31$\%$(females). They had a lot of self-recognized health problems, the male elderly complained about more than the female elderly. In the case of psychological health status, however, the female elderly showed a higher proportions of depression than the male elderly(p$\ll$0.05). In social health status, the elderly had good relationships with friends and collegues, whereas they had poor relationships with their families. They had many nutritional risk factors, and smoking was the most prevalent risk factor for the male elderly and anemia was for the female elderly. The results of this study suggest that Elderly Nutrition Improvement Programs should be planned that can be easily followed. It would be helpful to design a program focusing on individual phychological and social health status, this would increase the efficiency of the program.

여자노인의 영양상태 평가 및 건강관리를 위한 Nutritional Risk Index (NRI) 비교 분석 (The Analysis of Geriatric Nutritional Risk Index (GNRI) for Nutritional Assessment and Health Care in Elderly Women)

  • Nutritional assessment for the elderly can identify health status and morbidity. However, development of Nutritional Risk Index (NRI) remains limited for elderly because of difficulties in understanding physiological mechanism of elderly. This study was performed to analyze and develop Nutritional Risk Index for Korean elderly Women (Geriatric Nutritional Risk Index, GNRI). Based on literature review, factors for NRI were identified and indices were assessed by a cross-sectional survey. The survey involved Korean elderly women (${\geq}$60, n = 94) in Gwangju area, and sociodemographics, lifestyle characteristics, health conditions, dietary intakes based on 24h- recall, anthropometric measures (wt, ht, BMI, waist, hip, WHR, body protein, body fat, abdominal fat, and triceps skinfold thickness), and clinical biochemistry parameters (systolic blood pressure, diastolic blood pressure, cholesterol, HDL-cholesterol, triglyceride, total protein, albumin, prealbumin, hemoglobin, hematocrit, fasting blood glucose, HbAlc, ferritin, Zn, Ca, Na, K, Vit E, Vit $B_{12}$, folate, C-reactive protein) were examined relation to nutritional risk index. Based on literature review and data analyses, three NRIs were categorized (NRI I, NRI II, NRI III) and used for further analysis. NRI I was related to having metabolic syndrome, NRI II was related to serum albumin and body weight, and NRI III was related to food habit and health concerns. Abdominal fat (%) of elderly was correlated with each NRIs. NRI II was correlated with nutritional deficiency and higher tendency of inflammatory response, and NRI III was correlated with nutritional status which tend to be lower on aging (protein, folate, Vit $B_{12}$). NRI can serve as a useful tools in assessing health risk and nutritional status. Some modification of items in NRI and validity study are need to apply to Korean elderly.

Increased Risk of Cardiovascular Disease in Obese Female University Students with Binge Eating Episodes

  • Since binge eating is known to be related to increased risk of body weight fluctuations, it may be associated with increased health risks. This study was conducted to investigate the risks of binge eating on the cardiovascular system of female university students in Korea. Sixty-five female university students who were interested in a weight control program were recruited from a university campus. After personal interviews were conducted using a semi-structured questionnaire, 36 individuals were classified as binge eaters and 29 as non-binge eaters according to modified criteria of the proposed DSM-IV by the American Psychiatric Association(APA). All subjects participate in anthropometric and clinical measurements to evaluate the level of obesity and cardiovascular risks. Binge eating subjects showed no significant differences in obesity index compared to non-binge eating subjects. However, they had a higher level of low-density lipoprotein (LDL) cholesterol and a higher atherogenic index. They also showed close correlations with general obesity and cardiovascular risk factors. Body mass index(BMI) was the main explanatory index related to cardiovascular risk factors according to the stepwise regression analysis. Furthermore, obese binge subjects had higher levels than non-obese binge subjects or non-binge subjects for total cholesterol, LDL-cholesterol, triacylglycerols, atherogenic index, and systolic blood pressure. The findings strongly suggest that obese young women having binge eating episodes might display a greater risk for cardiovascular disease tan that shown for obese non-binge eating women.

한국노인에서 심장혈관계 질환 위험인자에 대한 비만지표인 체질량지수, 허리-엉덩이둘레비, 및 허리둘레의 타당도 비교 (Comparing Validity of Body Mass Index, Waist to Hip Ratio, and Waist Circumference to Cardiovascular Disease Risk Factors in Korean Elderly)

  • The purpose of this study was to compare the validity of obese index among body mass index (BMI), waist to hip ratio (WHR), and waist circumference (WC) and to determine which is the best in relation to cardiovascular disease (CVD) risk in Korean elderly more than 65 ages. Data from the 1998 Korean Health and Nutrition Survey were used (n=1017). Anthropometric indices and CVD risk factors were measured, and chi-square test, analysis of variance following duncan's multiple range test, partial correlation analysis, and Receiver Operator Characteristic (ROC) curves were used in the analysis. Anthropometric values were decreased in both male and female when ages were goes up. In female elderly, it specially showed the characteristics of upper body fat and systolic blood pressure risk (p<0.05). Among life style factors the current smokers were prevalent in obese male (p<0.05), but not prevalent in female having obese or upper body fat. Also, person with upper body obesity have more exercise than that of normal group (p<0.01). Mean BMI values of the current smoker was lower than that of normal group in both sexes (p<0.01). Mean BMI value of person with other risk factors were higher than that of normal groups (p<0.05). Among 7 CVD risk factors in partial correlation analysis, WC had the highest correlation coefficient in 5 in male, whereas BMI in 4 in female. In ROC analyses of 12 risk factors and health conditions, the largest area under curve of obese indices for risk factors were WC>WHR>BMI in male and BMI>WHR>WC in female. The optimal cutoff values of each index (BMI : WHR : WC) for one or more risk factors were 19.02 : 0.84 : 71.3 in male and 19.04 : 0.88 : 85.6 in female. In conclusion, Most Korean elderly showed non-obese and abdominal obesity likewise other Asians. Also CVD risk factors were prevalent in Korean elderly within normal limits of obese indices. Therefore the upper body fat indices reflected in the aged whose muscle mass is replaced by fat must be used as an indicator of CVD risk together with BMI. Although WHR was the worst index based on partial correlation analysis and so located between BMI and WC in ROC curve analysis in both sexes, it need to be use with WC to screen the cardiovascular risk group.

A Study on Health Risk Behavior Factors and Chronic Disease Risk Factors

  • This study classified subjects aged 30 to 64 into normal group, impaired fasting glucose group, and diabetes mellitus patient group based on data from the 6th period of the National Health and Nutrition Examination Survey (2013-2015). Skipping breakfast, lack of physical activity, inadequate sleep time, inadequate weight, excessive drinking, and current smoking are classified as a low health risk group when three or less items are present, and a high health risk group when three or more items are included. By classification, each item included in the physical measurement and biochemical analysis factors and health risk behavior factors was comparatively analyzed. As a result, in the normal group, impaired fasting glucose group, and diabetes group, the average age was higher in the group with high health risk factors than the group with low risk factors, and the male ratio was higher. Body mass index, waist circumference, blood pressure, triglyceride showed a significantly higher result. In the normal group, the fasting blood glucose level and total cholesterol level were also higher in group 2. Therefore, it is thought that it is necessary to control health risk behaviors through lifestyle changes in the normal group, fasting glucose disorder group, and diabetes group.

창원지역 일부 비만아의 체격지수와 생화학적 지표에 관한 연구 (Analysis of Anthropometric and Clinical Data in Obese Children in Changwon)

  • In this study, we investigated the effects of obesity index on the risk factors of chronic diseases in obese children. The subjects were male and female children aged 9 to 12. The average obesity index was 147.7$\pm$13.8% in boys and 147.6$\pm$16.5% in girls. The average percentages of body fat was significantly higher in boys(33.9$\pm$5.7%) than in girls(30.7$\pm$4.4%). It was found that 26% of subjects had abnormal serum GOT and GPT values. Percentages of subjects at risk of cardiovascular disease(CVD) based on corresponding criteria of TG, T chol, HDL chol, and LDL chol were 28.6%, 28.6%, 45.5% and 42.7%, respectively. The risk of CVD was increased with the degree of obesity, which was not significant. The AI(atherogenic index) was higher in boys(4.6$\pm$2.3) than in girls(3.6$\pm$1.2). The serum T chol and LDL chol levels of subjects whose mother are in thirties were significantly higher than those of subjects whose mother are in fourties. The T chol levels of subjects who had mother with job were significantly higher compaired to those of subjects who had housewife mother. The results indicated the urgent need of nu tritional management for the obese children to prevent further devel opment of childhood obesity and hyperlipidemia. Based upon this study, devising method and media for the mother nutritional education as well as for the children nutritional education is required for better growth and health promotion of primary school children.

High early pregnancy serum 25-hydroxy vitamin D level, within a sub-optimal range, is associated with gestational diabetes mellitus: a prospective cohort study

  • BACKGROUND/OBJECTIVES: Low early pregnancy serum 25-hydroxy vitamin D (25[OH]D) levels can increase gestational diabetes mellitus (GDM) risk, although inconsistent findings related to that association have been reported. This study examined the association of serum vitamin D with GDM and the possible influencers on this association. SUBJECTS/METHODS: This study included 259 pregnant women within the Seremban Cohort Study (SECOST). Blood samples at < 14 weeks of gestation were drawn to determine serum 25(OH)D levels. GDM diagnosis was made at 24 to 32 weeks of gestation using a standard procedure. Association between serum vitamin D and GDM was tested using binary logistic regression. RESULTS: Nearly all women (90%) had mild (68.3%) or severe (32.2%) vitamin D deficiency (VDD). Non-GDM women with mild VDD had a significantly higher mean vitamin D intake than GDM women with mild VDD (t = 2.04, p < 0.05). Women with higher early pregnancy serum vitamin D levels had a greater risk of GDM. However, this significant association was only identified among those with a family history of type 2 diabetes mellitus (T2DM) and in women with a body mass index indicating overweight or obese status. CONCLUSIONS: The high prevalence of VDD in this sample of pregnant women underscores the need for effective preventive public health strategies. Further investigation of this unexpected association between serum vitamin D level and GDM risk in predominantly VDD pregnant women and the potential effects of adiposity and family history of T2DM on that association is warranted.

한국 남성에서 식사의 질과 관상동맥질환 발생 위험에 대한 환자-대조군 연구 (A Case-Control Study on Dietary Quality and Risk for Coronary Heart Disease in Korean Men)

  • The purpose of this study was to investigate the association between dietary quality and the risk of coronary heart disease (CBD) among Korean men by a case-control study. Methods: The cases were 108 male patients with a first acute myocardial infarction or a new diagnosis of angina pectoris who were admitted to a university teaching hospital in Seoul. Controls were 142 age-matched patients admitted to the departments of ophthalmology and orthopedic surgery at the same hospital. Dietary information was assessed by a nutritionist using a semiquantitative food frequency questionnaire (FFQ), and dietary quality index (DQI) scores were calculated. Results: The intakes of total fat and cholesterol and body mass index (BMI) in cases were significantly higher than those in controls. The mean DQI scores were 8.6 for the cases and 9.4 for the controls. A higher DQI, which was indicative of a better quality diet, was inversely associated with the risk of CHD when comparing the highest to lowest quatiles, but borderline significant (odd ratios 0.60, 95% confidence interval 0.25-1.39, P for trend = 0.05). Conclusions: Our findings suggest that better dietary quality may reduce the risk of CHD among Korean men.
