Purpose: The purpose of this study was to investigate the impact of action observation training (AOT) on the balance and cognition of the elderly with dementia. Methods: Twenty-four participants were randomly assigned to experimental and control groups (12 participants each). The 5-week intervention involved 45 minutes sessions, three times a week. Both groups did 30 minutes of general physical exercises, followed by the experimental group watching a 5 minutes video featuring functional movements and imitating them for 10 minutes. The control group watched a scenic video and did functional training for 10 minutes, mimicking the experimental group. All participants were evaluated using the Timed Up and Go (TUG) test, Functional Reaching Test (FRT), Berg's Balance Scale (BBS), and Korean version of Mini-Mental State Examination (K-MMSE) before and after the intervention. A paired t-test was conducted to compare the within-group change before and after the intervention. Two-way repeated measures ANOVA was performed to compare the between-group difference. The statistical significance level was set to p=0.05 for all variables. Results: The experimental group showed significant within-group changes in the TUG test, FRT, BBS, and K-MMSE (p<0.05). The control group showed a significant change in FRT and K-MMSE (p<0.05). A significant difference was observed between the experimental group and the control group regarding the change in the TUG test, BBS, and K-MMSE after the interventions (p<0.05). Conclusion: The findings of this study suggest that the AOT and repetition of actual movements, led to more significant improvements in balance and cognitive abilities compared to the control group that observed scenic landscapes.
This study observed on habitual methods of toilet training for infants and toddlers in Korea thru 300 mothers at four medical institutions Ewha Woman′s University Hospital, Ewha Maternal and Child Health Center, Severance Hospital and Seoul National University Hospital) with a design to analyze beginning time, duration of period, methods employed, motives, equipments used, special terminology used for the subject training. The main purpose of this study was to generalize the proper methods of toilet training, and also to contribute a better psychological education for the mother and child. The results obtained from this study were as follows; 1. Majority of (67.3%) observed were in the age category between 30 and 40 years, and dominant numbers (64.7%) were housewives with high level of education and from middle class family background. 2. In the most cases (85%), toilet training was carried out by mothers(including wives who had a job) while more than half of mothers (53.7%) maintained their important motive for the training was "due to having high regard for cleanliness" 3. As for the time of beginning toilet training: finding indicated that starting period was decided (70%) at inconsiderate desertion of each mother. whereas, only minority group (30%) represents the cases where mother started the training when they consider tile child was physically and psychologically randy. Also greater number (77.7%) started bladder training prior to that of bowel. 4. It is noticeable that in course of training a large number of mothers (48.3%) applied strict training method when the child proper talenting, and the more rigid and strict in tile training. the more malformation of personality of the infant and toddler were seen after the training period (P<0.01). 5. Over the half of the total cases denoted (bowel 54.3%, bladder 67.7%) starting period before one year and in most cases (bowel 79.3%, bladder 729)the training was accomplished within 12 month, and therefore it was noted that earlier start(before 1 year) shortened the training period (with 12 month) . There was no significant difference between male and female infants in both starting period and duration of period in bladder training, however, in bowel training there was a tendency that female started earlier(7-12 months needed, 51.4%) than tile male (13-18 months needed, largest number 41.4%), and also in cases of female the period for needed for training were shorter than the cases of male. 6. Many a number (bowel 50.3%, bladder 97.7%) employed the method of continuous talenting at regular interval in accordance with that of child′s habit formed before training. Equipment used were various kinds, however, pieces of paper for male (45.5%) and piss pot or bedpost (42.3%) for female were common, on the other hand, "Eung-ga" for defecation (52.3%) and "Shii" for the urination (95.3%) were most standard expression that used during the training period.
The United States has a unique health care system, which is unlikely any other health care systems in the world. The major part of basic functional components of the system -financing, insurance, delivery, and payment- is in private hands. A market-oriented economy invites the participation of numerous private entities that are interested in carrying out the key functions of health systems. Due to this central feature, U.S.health care is not delivered through a network of interrelated components designed to work together coherently. For lack of standardization, the various components of the system fit together only loosely. The involvement of numerous players in the key functions leads to duplication, overlap, inadequacy, inconsistency, and waste, which add to the complexity and also make the system inefficient. Hence, cost containment remains an elusive goals. Moreover, the system falls short of delivering equitable services to all americans, though consumption of health care services is the largest in the world. On the other hand, United States leads the world in the latest and the best in medical technology, medical training, and research. It offers some of the most sophisticated institutions, products, and processes of health care delivery. This article discuss the characteristic features of the U.S. health care system. and its performance, trying to seek its implication on Korean health care system.
Objectives: Health screening in Korea is very active in both the public and private sector. However, primary prevention for health promotion has not been activated yet. Quaternary prevention is the prevention of unnecessary medical interventions or the prevention of overmedicalization. Methods: Data was collected after a search of the literature focusing on keyword. The curriculum guidelines for family medicine residents were collected from the homepage of the American Academy of Family Physicians. Results: This quaternary prevention is just beginning. The first step to enhance the health promotion services in the community is to identify the barriers pertaining to the delivering of health promotion activities. These barriers are related to the patient, the physician, attitudes, health promotion programs and the healthcare system. The second step is to establish new changes, such as expansion of insurance coverage, improvement of medical payment system, provision of consumer-oriented services, connection and integration of providers, and the service provider education and training. Conclusions: In order to enhance the health promotion services in the community we need to identify the barriers and to establish several changes to overcome them.

Objectives: The purpose of this study was to review the disaster preparation and response programs and the status of disaster preparation in public health center. Methods: In depth interview was performed in September 2017 using 5 open questions to the persons who are in charge of disaster response services in 5 public health centers of different levels in Korea. The questions included general characteristics of public health center, disaster programs and future issues. The research hired a quality method. Results: In general, the persons in charge recognized the cooperative agency of local government in disaster management. There were no disaster preparation programs developed by the public health centers. Most of the preparation were passive activities such as emergency support, crisis management on communicable disease and quarantine, participation in biological disaster response training, and education etc. The persons in charge emphasized necessity of disaster preparation programs. Conclusions: Disaster preparation and responsiveness is an evolving issue in public health centers in Korea. Medical support system and communicable disease management system are being set up in the national level. A comprehensive system covering health management, nutritional support, mental health, environment management of shelter, and volunteers supports on public health center level needs to be developed along with a easy-to-follow manual.
Proprioception training has been considered a secondary method to facilitate postural control ability. This study investigated the effects of two different proprioception training methods - the proprioceptive neuromuscular facilitation (PNF) and visual feedback-based joint position and force reproduction (VF) - on postural control advancements. Sixteen healthy people volunteered for this study, and they randomly grouped two. Each group participated in the PNF and VF training for three weeks. We evaluated each subject's proprioception levels and balance ability before and after the training. We used a clinometer and electromyogram (EMG) for VF training. The joint position reproduction test was also used to evaluate the position and force aspects of the proprioception level. We analyzed the trajectory of the center of pressure (COP) while subjects were standing on the firm floor and balance board with one leg using a pressure mat. The improvement of the position aspect of the proprioception level of the VF group (4.93±4.74°) was larger than that of the PNF group (-0.43±2.08°) significantly (p=0.012). The improvement of the anterior-posterior COP velocity of the PNF group (0.01±0.01 cm/s) was larger than that of VF group(0.002±0.01 cm/s) significantly (p=0.046). Changes of position error in the PNF group (rho=0.762, p=0.028) and tibialis anterior force reproduction error in the VF group showed a significantly strong relationship with balance ability variables. These results showed that different PNF and VF have different effects on improving two aspects of proprioception and their relationship with the balance ability. Therefore, these results might be useful for selecting proprioception or balance rehabilitation considering the clinical and patients' situation.
The purpose of this study was to investigate the effect of Treadmill Training with FES(TTF) on walking velocity, gait endurance, and energy expenditure index(EEI) of hemiplegia patients with foot drop. Two subjects with hemiplegia participated in this study. They took walking excercise 5 times per week for 8 weeks. One time excercise spent 30minutes. The theraputic effect was evaluated by how many seconds they needed to walk 10 meters, how far they could walk for 12 minutes, and how much they spent energy in walking for 12 minutes. Two cases were examined before, after 4 week, and after 8 week, walking training. The results of this study are as follows; 1) Walking velocity : Case 1 increased from 0.52m/sec before walking training to 0.83m/sec after 8 weeks. Case 2 increased from 0.58m/sec to 0.92m/sec. 2) Gait endurance : Case 1 increased from 383.23m to 625.53m. Case 2 increased from 410.19m to 693.47m. 3) EEI : For comfortable walking condition, Case 1 decreased from 0.98beats/min to 0.71beats/min, and Case 2 decreased from 0.93beats/min to 0.68beats/min. For maximum walking condition, Case 1 decreased from 0.93beats/min to 0.67beats/min, and Case 2 decreased from 0.91beats/min to 0.61beats/min. The findings suggest that hemiplegia patients can improve their walking velocity, gait endurance and energy expenditure index through TTF.
Background: Unstable surface-based core training can significantly enhance core strength, but no studies have compared the effects of balance pad- and sling-assisted exercises. Objects: To study the effects of unstable surface-based balance pad- and sling-assisted core strength exercises on muscle activity. Methods: Twenty male students aged 20-25 years participated in this study. The effects of three types of core strength exercises, performed with a sling or balance pad, on the activities of three muscles, i.e., the right musculus obliquus externus abdominis (EO), right erector spinae (ES), and right gluteus maximus (GM), were examined. Results: 1) In the glute bridge exercise, the percentage of maximum voluntary contraction of the EO, ES, and GM were significantly different between the balance pad- and sling-assisted exercises. The relative contribution of the ES and GM activities to all muscle activity were not significantly different between the two training types, whereas that for EO showed a significant difference. 2) There was no significant difference in the percentage value of maximum voluntary contraction (%MVC) among the EO, ES, and GM during the "leg-lifting with flat support" exercise, and there were no significant difference in the relative contributions between the two training types. 3) In the "side bridge leg separation exercise", the %MVC of the ES, EO, and GM were significantly different between the two training types. Conclusion: Sling training for core muscles was generally better than balance pad assist training. The majority of physiotherapy patients require core training. Our results could guide physiotherapists in the choice of targeted exercises for these patients.
Background: The purpose of this study was to investigated the effects of threshold resistance inspiratory muscle training on respiratory function in chronic stroke patients. Design: Randomized Controlled Trial Methods: Eighteen patient with stroke were randomly assigned to the experimental group (n=9) and control group (n=9) all testing and training. The experimental group underwent threshold resistance inspiratory muscle training with resistance adjusted of maximal inspiratory pressure, 60 breathing a day and general physical therapy 30 minutes a day, 5 times a week for 4 weeks. The control group was taken general respiratory muscle training and general physical therapy for 4 weeks in the same way. Respiratory function, walking ability were evaluated before and after the intervention. Statistical significance of the results were evaluated by ANCOVA between control group and experimental group after intervention. Results: There was a significant increase in FVC and FEV1 in the experimental group in pulmonary function tests (p<0.05). There was a significant difference with the maximum inspiratory pressure and the maximum inspiratory flow rate between experimental and control group (p<0.05). There was no significant difference with the maximum inspiratory capacity between experimental and control group (p>0.05) but the maximum inspiratory capacity of experimental group some increased than that of control group. Conclusion: These finding gave some indications that the threshold resistance inspiratory training may benefit on pulmonary function in people with stroke, and it is feasible to be included in rehabilitation interventions with this population.

Purpose: The purpose of the study was to find the strategies of mental health screening in school. Based on the literature review, we discuss the importance of screening students in schools for mental health problems. Methods: Data from the 2008 Korean Mental Health Screening in Schools(2008-KMHSS) are used to estimate the outline of this screening. We administered the questionnaire for satisfaction of 2008-KMHSS for students(N=1,280), parents(N=2,672), school nurses(N=75), teachers(N=685), district personnels(N=6), and mental health center staffs(N=37). Also we interviewed a part of them by telephone and e-mail. And we reviewed the tools and methods for screening students for emotional/behavioral problems. Results: Mental health screening in schools is a very important, yet worrisome, agenda that is in its very early stages. From the 2008 Korean Mental Health Screening in Schools, 9,588 students(12.9%) needed more evaluation in the first stage. Of these, 6,910(72.1%) completed the second stage screening. In this sample, 1,975(28.6%) utilized the mental health services in school or community. 38.3% of students and 43.7% of their parents notified the 2008-KMHSS. But only 12.1% of students and 10.9% of their parents dissatisfied with the screening. 9.9% of teachers and 22.7% of school nurses dissatisfied with the screening. Among them the school nurses were mostly dissatisfied, and they complained work burden from KMHSS. Mental health center staffs complained similar issues. The Children's Problem-behavior Screening Questionnaire(CPSQ) and Adolescents' Mental-health & Problem-behavior Screening Questionnaire(AMPQ) were compatible to screen students in schools for mental health problems in first stage. Conclusion: Mental health screening in schools needs careful planning and implementation. For successful mental health screening in schools, several elements need to be considered: careful planning, collaboration, staff training, and integrative mental health programs and services in community or schools.
