지역사회 여성 주민의 건강보험제도를 활용한 건강검진 및 암검사 수검 특성 (The Factors Associated with Health and Cancer Screening Using Preventive Programs from Health Insurance among Women of a Community)

  • This study, performed to analyze the factors associated with health and cancer screening using preventive programs form health insurance among the women of a community, through a survey of about 923 women in Euijungbu-city. The subjects of the study were selected by a proportional cluster sampling method. The self-reported questionnaire was intended to fine factors associated with health screening and cancer screening. The results of this study were as follows: 1. In the case of health screening using health insurance, 14.1% of the subjects turned out to have been screened once or more in their respective life-time. Reasons given for non-participation in the screening were : 'lacking screening information', a belief that' it's not useful' and a belief that they' weren't sick'. 2. The factors associated with health screening behavior were age, educational level, number of doctor visits, BMI and health promotion behavior(p<0.01, p<0.05). Also, the factors associated with health screening behavior were cue to action and health status, and the predictors on health screening behavior were age and health promotion behavior(p<0.01, p<0.05). 3. In the case of cancer screening through the health insurance, 7.4% of the subjects turned out to have been screened once or more respectively in their life-times. Reasons given for non-participation in the screening were : 'lacking screening information', a belief they 'weren't sick' and that it's not useful'. 4. The factors associated with cancer screening behavior were age, educational level, income, alcholol intake, exercise, number of doctor visits and BMI(p<0.01, p<0.05). Aditional factors associated with cancer screening behavior were cue to action, health belief score and health status. Predictors for cancer screening behavior were: age, health belief score, screening attitude and health status(p<0.01, p<0.05). As indicated by the above results, a lack of information was an important factor for a lack of participation in screening. Age and cue to action were also important factors in promoting the cancer screening rate. Therefore, a dissemination of information about cancer screening contributes to the promotion of a screening rate, and cooperation between health insurance and local health cancer facilitates to be public the community-based cancer screening program.

농촌지역 중년남녀의 암 조기검진 수검유무에 따른 건강증진행위 비교 (Comparison of Health Promotion Behavior in Middle aged Rural Residents by Cancer Screening Participation)

  • Purpose: The aim of this study was to compare health promotion behavior in middle-aged rural residents by cancer screening participation. Methods: Data were collected from 508 rural residents during the period from April 6 to June 6, 2009 using structured questionnaires, and analyzed using SPSS/WIN 12.0. Results: The cancer screening rate was 50.2%. The rate was significantly different according to sex, educational level, marital status, private insurance, family cancer history, smoking, drinking, perceived health status, and health concern. Men showed the highest screening rate in gastroscopy for stomach cancer, and women in pap smear test for cervical cancer. Pap smear test for cervical cancer showed the highest regular screening rate (4.3%). The average score of health promotion behavior was $2.65{\pm}0.35$. Health promotion behavior was significantly different according to cancer screening participation, health responsibility, stress management, and self actualization. Conclusions: These results suggested that there may be differences in health promotion behavior among middle-aged rural residents according to their cancer screening participation. A further study is necessary to find effective interventions for the non-cancer screening group.

  • This study was aimed at preparing basic data required for establishment of a cancer screening program by examining the status of cancer screenings performed by women in a city and the factors that had an influence on cervical and breast cancers screening behavior of women. In order to find out the status of cervical and breast cancers screening and the factors having an influence on cervical and breast cancers screening behavior, 1,113 women were selected as study area and subjects by a random cluster sampling method, and the subjects were answered by questionnaires. The major results were as follows: 1. In the cases of the breast and the cervical cancers, 16.7% and 55.7% of the subjects turned out to have had one or more screenings respectively in their life-time. Also the rate of screening group of cervical and breast cancers was 16.7%, the rate of screening group of only cervical cancer was 38.9%, and the rate of non-screening group was 44.4%. 2. As to the screening pattern of cervical and breast cancer, there were significantly for age, income, occupation, married status, spouse, the factors associated with health promotion (doctor visiting, exercising), married age, number of children, and breast feeding(p〈0.05). 3. The factors associated with screening behaviors for cervical and breast cancers were age, income, occupation, married status, doctor visiting, exercising, married age, number of children, and breast feeding(p〈0.05). Based on the above-mentioned results obtained by this study, it was anticipated that this study may be playa vital role as basic data for the development and execution of cervical and breast cancers screening program for women in a community. And the analysis, done on the basis of the status of the cancer screening, of the influence factors on cervical and breast cancers screening behavior showed that for the development of a cancer screening program, the factors like age, social-economic class, married status, health promotion behavior, and reproductive characteristics should be considered.

국가 암 조기검진 대상자의 암 검진 수검의도 관련 요인 (Factors Associated with Cancer Screening Intention in Eligible Persons for National Cancer Screening Program)

  • Objectives: To identify factors associated with cancer screening intention using the Theory of Planned Behavior (TPB). Methods: Among 55,920 eligible persons for National Cancer Screening Program (NCSP) in J city, 1,100 individuals were contacted. Of these, 797 were interviewed using a structured questionnaire. Thirty-six responses were excluded due to incomplete data. The remaining 761 completed questionnaires were analyzed to find factors associated with cancer screening intention. Results: Cancer screening intention was significantly associated with behavioral attitude (p<0.01) and subjective norm (p<0.01), but not with perceived behavioral control (p=0.29) in the TPB model. These three constructs explained 29.7% of cancer screening intention in multiple linear regression analysis. External factors such as socio-demographic status, health and health behavior variables explained 8.9% of screening intention. Among them, household monthly income, past cancer screening experience, exercise and daily eating habit were significantly associated with screening intention. Conclusions: Cancer screening intention may be influenced by focusing attitude, subjective norm in TPB model and other external factors. However, further studies are warranted to identify factors influencing cancer screening intention and behavior.

결혼이민여성의 자궁경부 세포진 검사 참여행위와 관련요인에 관한 연구 (Pap Smear Screening Participation Behavior and Related Factors in Married Immigrant Women)

  • Purpose: This study's aim was to investigate participation in Pap smear screening among married immigrant women and their behavior and its related factors. Methods: A survey was conducted among 165 married immigrant women in Gyeongsangnam-do from January 1 to March 20, 2010. Results: Results in this study showed that 51.5% have never participated in Pap Smear screening. Among those who have participated in the Pap Smear screening, 10.9% had regular screening, while 37.6% had irregular screening. Pap smear screening behavior was significantly different according to the experiences in health checkup ($x^2$=34.009, p<.001)', whether or not there was a hospital the woman regularly visited ($x^2$=7.768, p=.021) and perceived barriers (F=3.214, p=.043). Conclusion: For improvement of Pap smear participation, this study proposes to develop various nursing intervention programs which can improve perceived barriers in regards to the related variables. In addition, it is necessary to come up with a strategy to advertise the participation in Pap smear screening in a systematic and continuous manner in order to raise womens' awareness and to make married immigrant women realize the importance of regular Pap smear screenings.

유방암 조기검진에 대한 단계별 행동과 관련 요인 -국가암조기검진 대상자를 중심으로- (The Factors Associated with Changes in the Stage of Breast Cancer Screening Behavior among the Woman who are Eligible for the Korean National Cancer Screening Program)

  • Objectives : This study was performed to evaluate the relationships between psychosocial characteristics and changes in the stage of breast cancer screening behavior. Methods : The 474 study subjects were randomly sampled from 21,459 women(age range, 40-70 years) who were eligible for the Korean National Cancer Screening Program in 2006 in Jinju, Gyeongsangnam-do. The information, including behaviors and sociodemographic characteristics, attitudes, subjective norms and self-efficacy, was collected by trained interviewers via home visits. The breast cancer screening stages were grouped as precontemplation, contemplation, action, maintenance and relapse, according to Rakowski. Results : Of the 474 women, 18.8% were in the precontemplation stage, 23.3% were in the contemplation stage, 13.1% were in the action stage, 36.6% were in the maintenance stage, and 8.2% were in the relapse stage. The distribution of stages was associated with attitude, subjective norms and self-efficacy(p for trend<0.01). To investigate the overall relationship between the variables, we conducted a linear structural equation model analysis based on the theory of planned behavior. The subjective norms and self-efficacy influenced the stage of the women's screening behavior. Conclusions : We should target on self-efficacy about the screening behavior of women by performing timely, thoughtful interventions. The support from family members, friends and other people is crucial for women to undergo breast cancer screening and to improve the breast cancer screening rate.

Factors Affecting Cancer Screening Intention and Behavior of the Korean Elderly

  • Background: In this study we investigated factors influencing cancer screening intention and behavior to develop measures to increase the rate of cancer screening in the Korean elderly. Materials and Methods: Participants included 425 elderly subjects 65 years of age or older from D city, South Korea. The health behavior characteristics and cancer screening-related and theory of planned behavior (TPB) factors influencing the participant attitudes on cancer screening were examined to identify determinants significantly affecting cancer screening intentions and behavior. Results: Predictive factors influencing cancer screening behavior included smoking, exercise, cancer concerns, preference for the type of cancer screening, prior experience with the National Cancer Screening Program, perception of the National Cancer Screening Program, behavioral control with respect to cancer screening and cancer screening intentions. The factors influencing cancer screening behavior were different from those for cancer screening intentions. Conclusions: Increasing the cancer screening intentions of the elderly is necessary to raise the rates of cancer screening. Additionally, identifying the inhibitory factors that serve as obstacles to cancer screening in the elderly and changing screening intentions into actual screening behavior is necessary. This study provides a reference for developing and applying policy measures and intervention strategies to increase the cancer screening rates of the elderly in Korea.

지역사회 여성 주민을 대상으로 한 대상중심형 위암 조기검진 교육 프로그램의 개발 및 효과평가 (Effectiveness of a Community-based Program Using Tailored Stage-matched Messages to Promote Screening for Stomach Cancer)

  • Objectives: Recent studies have shown that tailored messages for cancer screening to the beliefs and stage of cancer screening behavior of individual women increases the take-up probability. Many studies on cancer screening have used the Transtheoretical Model (TTM) to identify variables associated with cancer screening behavior. This study was carried out to identify the cognitive-behavioral factors associated with stomach cancer screening among women aged 40 years and over, and to develop and evaluate a tailored educational program for stomach cancer screening by stages of change. Methods: Building on the TTM constructs, we conducted a quasi-experimental study(N=283) to test the effectiveness of a tailored educational program for endoscopic stomach cancer screening. We carried out pre and post tests in the experimental group(N=162) and the control group(N=121), and the experimental group was subdivided into an on-line group(N=81) and an off-line group(N=81) by educational methodology using e-mail and the postal service. We used the chi-square test, trend test, and paired t-test to test the effectiveness of the program for stomach cancer using a tailored stage-matched messages. Results: To examine the effectiveness of the program for stomach cancer screening by the tailored stage-matched messages, the stage-matched materials were offered to the experimental group(N=162) four times for 4 weeks. The stage-matched materials consisted of the four types for stomach cancer. The tailored message was effective in changing the cognitive-behavioral factors, such as experience process, behavior process, con opinion for stomach cancer, self-efficacy, and the behavioral stages for stomach cancer screening. The stomach cancer screening adherence was higher for the stage-matched materials using postal mail than for those using e-mail. Conclusion: To improve the stomach cancer screening rate, the use of tailored messages for stomach cancer screening will be generated using an expert system. Therefore the implementation of tailored educational program will be supported a partnership between public and private health organizations and increasing awareness of the necessity of community-based interventions.

중년여성의 규칙적인 자궁경부암 검진 이행관련 요인 (A Study on Regular Cervical Cancer Screening Behavior among Middle-aged Women)

  • Purpose: To identify the status of regular cervical cancer screening practices among middle-aged women, the associations between regular practice and research factors, and the predictive model and factors effecting such behavior was studied. Method: Two hundred women, aged 40 to 60, were selected by convenience in one urban area of Seoul. They were asked about their regular attendance for screening, knowledge of cancer and screening, health belief, health self-determination index and certain personal factors. Result: Approximately 54.5% of the women had periodic screening tests every 6 months to 2 years. Their knowledge of cervical cancer and health belief were at the medium level of each scale, but their health self-determination scores (HSDI) were low. Some influencing factors, and their cancer odds ratio were identified through univariate regression analysis. These variables were included in a predictive model, and this model proved to have enough fit and classification power (83.5%). In this model, the financial state, self-belief and self-determination scores were found to be significant. Conclusion: Middle-age women's intrinsic motivation for healthy behavior was found to be low in those who felt to be in a poor financial state, had higher perceived barriers, lower perceived benefits and a lower prevalence of undergoing regular screening test.

한국 여성의 유방엑스선 촬영에 영향을 미치는 요인 : 건강신념 모형을 중심으로 (Analysis of Factors Related to Mammography Screening Behavior Women: Use of Health Belief Model)

  • The purpose of this study was to explain mammography screening behavior of Korean women using components of Health Belief Model. A total of 310 women aged 30 and older participated in the study. Of the participants. 155 women who obtained a mammogram during the data collection period were classified as the mammography group and the other 155 who had never had a mammogram were classified as the non-mammography group. The researcher developed instrument used in the study. which included demographic variables and questions measuring the concepts of the HBM components. Trained data collectors administered the questionnaire employing a face-to-face survey method in the waiting areas of hospitals and health promotion centers. The study results indicated that age. knowledge. perceived susceptibility. and self-efficacy were identified as the significant variables in explaining Korean women's mammography screening behavior, whereas, perceived severity, perceived benefits, and perceived barriers were not significantly related to mammography screening behavior of the participants. The findings from this study can be used to guide the design and implementation of health education and health promotion programs in order to promote mammography utilization among Korean women.
