Background Free flap surgery for head and neck defects has gained popularity as an advanced microvascular surgical technique. The aims of this study are first, to determine whether the known risk factors such as comorbidity, tobacco use, obesity, and radiation increase the complications of a free flap transfer, and second, to identify the incidence of complications in a radial forearm free flap and an anterolateral thigh perforator flap. Methods We reviewed the medical records of patients with head and neck cancer who underwent reconstruction with free flap between May 1994 and May 2012 at our department of plastic and reconstructive surgery. Results The patients included 36 men and 6 women, with a mean age of 59.38 years. The most common primary tumor site was the tongue (38%). The most commonly used free flap was the radial forearm free flap (57%), followed by the anterolateral thigh perforator free flap (22%). There was no occurrence of free flap failure. In this study, risk factors of the patients did not increase the occurrence of complications. In addition, no statistically significant differences in complications were observed between the radial forearm free flap and anterolateral thigh perforator free flap. Conclusions We could conclude that the risk factors of the patient did not increase the complications of a free flap transfer. Therefore, the risk factors of patients are no longer a negative factor for a free flap transfer.
Purpose: To identify Osteoporosis and the related risk factors in middle-aged women, the descriptive survey was done. Method: The subjects were measured in 465 residents who were 40-60 aged healthy women. They underwent ultrasound measurement and health examination in G city's Health Center in Gyeonggi-do, Korea from July 2000 to March 2001. The self-reported questionnaire consisted of total 17 items about risk factors related to osteoporosis. The evaluation of bone density was based on ultrasound measurements of right heel. All data were analyzed by SAS-PC Program. Result: Mean age of the subjects was 46.7. Mean T score related to bone density was -1.30, ranged from -3.52 to 3.06. In diagnosis classification according to T score, 74.8% of subjects was normal, 12.9% was osteopenia, 12.3% was osteoporosis. Among risk factors, there were significant differences by age (t=15.35. p=0.000), parity (F=12.81, p=0.000), menopause status (t=22.05, p=0.000), period after menopause (F=5.20, p=0.006). The higher frequency of delivery, postmenopausal and longer period after postmenopause of subjects had the lower the bone density. Conclusion: It would be necessary to develop and apply the community-based health promotion program for middle-aged women to prevent osteoporosis.
Purpose: Unbalanced diets and decreased physical activity have contributed to increased prevalence of obesity and metabolic syndrome in adolescents. We have performed a systematic review and data analysis to examine the association between dietary pattern and metabolic syndrome risk factors in adolescents. Methods: We searched the PubMed and BioMedLib databases for appropriate articles published during the past 10 years and selected 6 articles. The studies reviewed applied factor analysis or cluster analysis to extract dietary patterns. For data analysis, we examined the association between dietary patterns and the prevalence of metabolic syndrome risk factors using data of 3,168 adolescents (13 to 18 years) obtained from 4 consecutive Korean Nutrition Health and Nutrition Examination Surveys (1998, 2001, 2005, and 2007 to 2009). Results: Our systematic review confirmed that western dietary patterns are positively associated with metabolic syndrome risk factors such as obesity and elevated triglycerides, while traditional dietary patterns were negatively associated. Data analysis found that the number of adolescents aged 16 to 18 years who had "Rice & Kimchi" dietary pattern decreased, while the number having western dietary patterns increased during the 1998 to 2009 time frame. There were no changes in the dietary patterns in adolescents aged 13 to 15 years. The risk of elevated serum triglycerides and reduced serum high density lipoprotein cholesterol was high in the "Rice & Kimchi" dietary pattern compared to the other dietary pattern groups. Conclusion: Because adolescents' dietary patterns are changing continuously and have long-term effects, further studies on the dietary patterns of adolescents and their health effects into adulthood are necessary.

The purpose of this study was to investigate the relationship between oral cancer and such factors as smoking and drinking pattern, oral health status, dietary intake pattern, socio-economic status. Oral cancer patients and other disease patients who visited Yonsei University Dental Hospital from May to September in 2000 were selected as the study subjects. The numbers of cases and controls were 41, 108, respectively. Two groups were matched with age and sex for case control study. Oral examination and questionnaires survey was performed by the dentist. To assess the strength of associations between oral cancer and other variables, chisquare tests were performed. The results were as follows : 1. The durations of smoking and alcohol drinking were not related significantly with oral cancer. But the doses of smoking and alcohol intake increased the risk of oral cancer significantly(OR=2.52, 4.11, p<0.05). 2. Denture wearing, the number of missing teeth and spicy and salty food, coffee, tea and fresh fruit intake frequency did not significantly increase the risk of oral cancer. But low education level, residency in rural area increased risk of oral cancer significantly(p<0.01).
Human papillomavirus (HPV) is a major cause of cervical cancer. More than 100 HPV genotypes have been identified; however the distribution varies geographically and according to ethnicity. The purpose of this study was to investigate the prevalence and distribution of HPV subtypes among Northeast Thai women. Subjects included 198 cases of SCCA and 198 age-matched, healthy controls. HPV-DNA was amplified by PCR using the consensus primers GP5+/6+ system followed by reverse line blot hybridization genotyping. The prevalence of high-risk HPV infection was 21 (10.1%) and 152 (76.8%) in the controls and in the cases, respectively. High-risk HPV significantly increased the risk for cervical cancer with an OR of 42.4 (95%CI: 22.4-81.4, p<0.001) and an adjusted OR of 40.7-fold (95%CI: 21.5-76.8, p <0.001). HPV-16 was the most prevalent HPV type in the SCCA (56.2%) followed by HPV-58 (17.8%) and HPV-18 (13.6%); whereas HPV-58 (46.4%) was a prominent genotype in the controls followed by HPV-16 (39.3%) and unidentified HPV types (25.0%). These findings indicate that HPV infection remains a critical risk factor for SCCA; particularly, HPV-16, HPV-58 and HPV-18. In order to eradicate cervical cancer, sustained health education, promoted use of prophylactics and a HPV-58 vaccine should be introduced in this region.

As prior studies indicate that chronic diseases are mainly attributed to health behavior, preventive health care rather than treatment for illness needs to improve health status. Since chronic conditions require long-term therapy, health care expenditures to treat chronic diseases have been substantial burden at national level. In this point of view, this study suggests that the health promotion program should be based on Knowledge Based System Using Data Mining Technique, we developed a predictive model for preventive healthcare management on diabetes mellitus. Generally, in the outbreak of diabetes mellitus there is a difference in lifestyle and the risk factors according to gender. So we developed a predictive model in accordance with gender difference and applied the Logistic Regression Model based on Data Mining process. The result of the study were as follow. The lift of the last predictive model was an average 2.23 times(male model : 2.13, female model 2.33) more improved than in the random model in upper 10% group. The health risk factors of diabetes mellitus are gender, age, a place of residence, blood pressure, glucose, smoking, drinking, exercise rate. On the basis of these factors, we suggest the program of the health promotion.
Objectives: To propose a risk-adjustment model from insurance claims data, and analyze the changes in cesarean section rates of healthcare organizations after adjusting for risk distribution. Methods: The study sample included delivery claims data from January to September, 2003. A risk-adjustment model was built using the 1st quarter data, and the 2nd and 3rd quarter data were used for a validation test. Patients' risk factors were adjusted using a logistic regression analysis. The c-statistic and Hosmer-Lemeshow test were used to evaluate the performance of the risk-adjustment model. Crude, predicted and risk-adjusted rates were calculated, and compared to analyze the effects of the adjustment. Results: Nine risk factors (malpresentation, eclampsia, malignancy, multiple pregnancies, problems in the placenta, previous Cesarean section, older mothers, bleeding and diabetes) were included in the final risk-adjustment model, and were found to have statistically significant effects on the mode of delivery. The c-statistic (0.78) and Hosmer-Lemeshow test ($x^2$=0.60, p=0.439) indicated a good model performance. After applying the 2nd and 3rd quarter data to the model, there were no differences in the c-statistic and Hosmer-Lemeshow $x^2$. Also, risk factor adjustment led to changes in the ranking of hospital Cesarean section rates, especially in tertiary and general hospitals. Conclusion: This study showed a model performance, using medical record abstracted data, was comparable to the results of previous studies. Insurance claims data can be used for identifying areas where risk factors should be adjusted. The changes in the ranking of hospital Cesarean section rates implied that crude rates can mislead people and therefore, the risk should be adjusted before the rates are released to the public. The proposed risk-adjustment model can be applied for the fair comparisons of the rates between hospitals.
Purpose: This study was performed to identify the socioeconomic factors, the health behavior factors and dietary pattern that have an influence on abdominal obesity, as measured by using the waist circumference in children and adolescents. Methods: This study used data collected from 1998, 2001, 2005 Korean National Health and Nutrition Examination Survey (NHANES). Final sample included 3,596 subjects from 10 to 19 years old who had completed necessary health examinations, health behaviors survey, and nutrition survey. Results: 12 dietary patterns emerged from factor analysis with different factor loading. Logistic regression analysis of the factors that affect abdominal obesity showed that the education level, BMI(Body Mass Index) and dietary pattern, 'Kimchi' including Korean cabbage and radish affected the abdominal obesity in children and adolescents. Conclusions: Abdominal obesity is a risk factor for morbidity and mortality, and it is associated with chronic diseases, including cardiovascular disease and diabetes. Based on the findings, it is essential to modify dietary pattern for preventing abdominal obesity, which is a condition associated with the incidence of chronic disease in adults.

This study was conducted to investigate the risk factors affecting dental caries in children by using the 2018 children's oral health survey data. The study was conducted on 20,235 children who were 12 years of age. The survey items examined general characteristics, dental dietary behavior, the oral condition, and the behavioral factors related to oral health. The results showed that gender, region, economic level, subjective oral health condition, dental dietary behavior, oral condition and oral health-related behavior were all risk factors. Those children with oral conditions particularly showed a higher risk of the dental caries symptoms of dental calculus, dental bleeding, tooth pain and white spot teeth. The oral health-related behaviors were found to be tooth brushing less than two times a day, the risk of not using a handle to hold dental floss and not using dental floss. Our results showed that countries or communities can diagnose and manage dental well-being early on for children with the highest sensitivity of dental health and they need to continue to establish a dental well-being management system for the oral health care of children. In addition, oral health education should be expanded, which can improve oral health care habits of children and adolescents. Further, an oral health policy system for improving community programs to prevent dental and community utilization is needed.

Background: The present objective was to verify whether burnout (emotional exhaustion [EE], depersonalization [DP] and low professional efficacy [PE]) is a risk factor for long-term sickness absence (LTSA; 30 consecutive days) from the teaching role. Methods: This was a prospective cohort study with two years of follow-up that investigated 509 elementary and high-school teachers. Burnout was identified by Maslach Burnout Inventory. Poisson regression with robust variance was used to adjust for possible confounders. Results: The incidence of LTSA was 9.4%. High EE levels were associated with LTSA in the crude analysis, but the association lost statistical significance after adjustments (for sex, age, perception of work-life balance, general self-rated health, chronic pain and depression). High DP levels were associated with this outcome, even after all adjustments (relative risk = 1.80; 95% confidence interval: 1.05-3.09). Low PE levels were not related to LTSA. Conclusion: The results reinforce the need to improve teachers' work conditions to reduce burnout, particularly DP, and its consequences.
