예비 간호인력 대상 다학제 전문직 간 교육 중재 연구의 체계적 문헌고찰: 동아시아권 국가 연구를 중심으로 (Systematic review on interprofessional education for pre-licensure nursing student in East Asia)

  • Purpose: This study aimed to identify and evaluate interprofessional education (IPE) interventions for healthcare professional students in East Asian countries. Methods: The reporting of this study followed the Preferred Reporting Items of Systematic Reviews and Meta-Analysis guidelines. A literature search was conducted using seven electronic databases: PubMed, EMBASE, CINAHL, Scopus, Web of Science, ERIC, and ProQuest Dissertations & Theses Global. Joanna Briggs Institute Critical Appraisal Checklists were also used to appraise the quality of the included studies. The outcomes of IPE interventions were classified based on a modified Kirkpatrick model. Results: This review included 30 studies predominantly conducted in Singapore, South Korea, and Taiwan. The prevalent research design was a one-group pre-posttest design, and most IPE interventions occurred as single events. Approximately 70% of the studies involved students from two healthcare professions, mainly nursing and medicine. Simulations, group discussions, and lectures have emerged as the most common teaching methodologies, with almost half of the studies leveraging a combination of these techniques. The IPE content primarily focused on interprofessional teamwork, communication, and clinical patient care situations; these included the management of septic shock. The effectiveness of the IPE interventions was mainly evaluated through self-reported measures, indicating improvements in attitudes, perceptions, knowledge, and skills, aligning with Level 2 of the modified Kirkpatrick model. Nonetheless, the reviewed studies did not assess changes in the participants' behavior and patient results. Conclusion: IPE interventions promise to enhance interprofessional collaboration and communication skills among health professional students. Future studies should implement rigorous designs to assess the effectiveness of IPE interventions. Moreover, when designing IPE interventions, researchers and educators should consider the role of cultural characteristics in East Asian countries.

일부지역 청소년의 구강건강지식 오개념에 관한 연구 (A Study on Adolescents' Misconceptions about Oral Health Knowledge in Some Areas)

  • 본 연구는 청소년의 구강건강지식에 대한 오개념과 관련요인을 파악함으로써 올바른 구강건강지식 개념형성을 위한 방안을 마련하는데 기초자료를 제공하기 위하여 실시하였다. 연구대상자의 구강건강지식 수준은 비교적 많은 문항에서 학생들이 바른 개념을 형성하고 있었으나, 칫솔손잡이, 잇몸약 사용, 수돗물 불소 조정사업에서 오개념이 형성되어 있었다. 구강건강지식 수준은 전체 평균 10.78점을 나타내었으며, 집단 간 오차범위 내에서 차이를 보이지 않았다. 구강건강지식 오개념의 집단별 연관성은 성별과 학력에서 차이를 보이지 않았으나 구강보건교육 이수 학생 보다 올바른 개념이 형성되어 있는 것으로 나타났다(p<0.001). 구강건강지식을 얻는 경로는 인터넷, 대중매체(35.4%)에서 습득 비율이 가장 높았고, 학교(9.6%)에서 습득 비율이 가장 낮았으며, 구강건강지식 수준은 학교에서 10.56점으로 가장 낮았다. 따라서 청소년의 바른 구강건강행동을 위한 학교 구강건강교육 활성화 및 교육기회를 부여할 수 있도록 구강건강교육자 양성이 매우 중요함을 알 수 있다.

초등학교 고학년의 구강건강신념과 구강건강행동간의 관련성에 관한 연구 (Study on Relation between Dental Health Beliefs and Dental Health Behavior in the High Grade Students of Elementary School)

  • 사회심리적인 모형인 구강건강신념모형이 예방적인 측면에서 구강건강행위에 영향을 미치므로 구강건강신념을 구성하는 요소와 구강건강관련 행동간의 관련성을 규명하고자 초등학교 고학년 490명을 대상으로 개별자기기입방식에 의한 설문조사를 실시하였다. 수집된 자료는 통계프로그램 SPSS 14.0을 활용하여 T-test, 일변량 T검정, One-way ANOVA, 피어슨 상관분석을 하여 나타난 결과는 다음과 같다. 1. 일반적 특성에 따른 구강건강신념은 유의한 차이가 없었으나 구강보건행동은 하루 칫솔질 횟수에서 여자가 남자보다 유의하게 조금 더 높았으며, 연간 치과의료기관 방문 횟수는 고학년일수록 유의하게 낮았다(p < 0.01). 2. 학생들의 구강건강신념과 구강건강행동의 상관관계에서 하루 칫솔질 횟수는 감수성, 장애도와 부의 미미한 상관성을 나타내었다. 그리고 연간 치과의료기관 방문횟수는 감수성, 심각성과 정의 미미한 상관성을 나타났었고, 하루 우식성식품 섭취횟수는 감수성, 심각성, 장애도와 정의 미미한 상관성를 나타내었으나 유익성과는 부의 미미한 상관성을 나타내었다. 3. 구강건강신념간의 상관관계는 감수성을 높게 느낄수록 심각성과 장애도는 높게 나타나지만 유익성은 낮아지며, 심각성을 높게 느낄수록 장애도는 높아지는 반면에 유익성은 낮아졌다. 또한 유익성이 높을수록 중요도는 높아지는 반면 장애도는 낮아졌다. 이상의 결과에서 알 수 있듯이 자신의 구강질환에 대한 인식의 정도나 구강상태를 심각하게 생각할수록 하루 우식성식품 섭취횟수와 연간 치과의료기관 방문횟수는 많았으나 칫솔질 횟수는 낮았다. 이는 칫솔질을 적게 하거나 우식성식품 섭취횟수와 연간 치과의료기관 방문횟수가 많을수록 자신의 구강상태가 나쁠 것이라는 생각을 하고 있으며, 치과진료에 대한 장애를 많이 느끼는 것으로 나타났다. 위의 결과를 미루어보아 구강건강신념과 구강건강행동은 서로 상호 관련성이 있음을 알 수 있으나 강한 상관성은 나타내지 않았다. 따라서 자기주도적 행동이 시작되는 초등학교 고학년의 올바른 구강건강신념 및 구강건강행동을 유도하기 위해서는 보다 적극적인 전문가에 의한 지속적이고 체계적인 학교구강보건 교육프로그램이 강화되어야 하겠다.

병원과 지역사회에 근무하는 정신보건간호사의 전문직 자아개념 (Professional Self-concept of Psychiatric Mental Health Nurse Practitioners in Hospitals and Public Health Centers)

  • This study was conducted to investigate and compare the degree of professional self -concept (PSC) of the psychiatric mental health nurse practitioners (PMHNP) in hospitals and public health centers and to identify the factors predicting PSC of them, in order to provide basic data for developing PSC increasing program PSC. The 355 PMHNP were sampled from the university or general hospitals. mental hospitals, community mental health centers and public health centers across the country. The scales used in this study were PSCNI by Arthur (1990), PSI by Heppner & Petersen (1982) and the Index of work satisfaction by Slavitt et al. (1978). The results of the study were as follows : 1. The average item score of PSCNI of PMHNP in hospitals was $2.83\pm0.27$, and that of PMHNP in public health centers was $2.76\pm0.28$. There was significantly different between two groups (p=0.0202) 2. A comparison of the scores for the dimensions of the PSCNI were as follows ; the mean item score of professional practice of nurses in hospital was $2.90\pm0.30$, and that in public health centers was $2.83\pm0.35$. There were significant differences between two groups in the scores of professional practices (p=0.0315), leadership (p=0.0071) and skills (p=0.0231). 3. There were significant differences between two groups according to education (p=0.0057) with no significant interaction effect of group and education. 4. Job satisfaction (JS) was the highest factor predicting PSC of PMHNP in hospitals $(34.5\%)$, and problem solving inventory score (PS) was the highest factor predicting PSC of PMHNP in public health centers $(33.6\%)$. JS and PS accounted for $42.6\%$ in PSC of PMHNP in hospitals. and PS, JS, age, marital status, religion, and career accounted for $57.6\%$ in PSC of PMHNP in public health centers. In the light of these results. to gam the professional self-concept. nurses should be educated continuously through role modeling in clinical nursing and research. Also, nurse educators and administrators need to develop programs and policies to increase professional self-concept of nurses, particularly of community PMHNP.

유치원과 어린이집 교사의 COVID-19 구강건강관리의 영향 (Impact of the COVID-19 pandemic on oral healthcare among children and teachers in kindergarten and daycare centers)

  • 연구목적: COVID-19로 인해 유치원과 어린이집에서의 교육 방식이 변화되고 있다. 본 연구의 목적은 COVID-19 이전과 COVID-19 시기의 유아 구강건강관리 실태와 유치원 및 어린이집 교사들의 구강건강관리에 대한 인식을 조사하는 것이다. 연구방법: 연구 대상은 만 3-5세 아동을 담당하는 유치원 및 어린이집 교사 189명으로 온라인 설문조사를 통해 자료를 수집하였다. 통계분석은 빈도분석과 카이제곱 검정을 이용하였다. 연구결과: COVID-19 이전보다 COVID-19 이후 아동의 점심식사 후 칫솔 사용 횟수, 구강보건교육 및 정기 구강검진 횟수, 보육교사의 구강보건교육 경험률이 감소했다. 또한 유치원 및 어린이집 교육자들은 보건소 구강보건담당자가 가장 바람직한 구강보건교육자라고 응답하였다. 결론: COVID-19 팬데믹 이후 유치원과 어린이집에서 영유아의 구강건강관리가 취약해졌다. 구강위생과 구강건강교육을 통해 이를 회복시키려는 노력이 필요하다. 또한 구강건강교육자로서 치과위생사의 역할을 확대할 필요가 있다.

현장중심형 치과기공사 배출을 위한 교육과정 개편 방안에 관한 연구 (A study on the reorganization of the curriculum for produce of field-oriented dental technicians)

  • Purpose: The objectives of this study is to collect the opinions of experts in the field of dental lab technology, and to present a plan for reforming the curriculum for producing clinical-based dental technicians. Methods: 71 experts were selected for this study. Experts were grouped into clinicians, educators, and policy-makers. First of all, the purpose and method of this study were explained to experts. After receiving the consent to participate in the research, their opinions were investigated. The survey was conducted through a questionnaire created based on their opinions and opinions collected. Results: The results showed that they were relatively satisfied with the curriculum. In addition, it was found that they hope to increase the proportion of mandatory education of digital dentistry and clinical practice. Emphasis was placed on expanding opportunities for on-the-job training as well as on time for hands-on training. As for admission capacity, the highest opinion was to keep it as it is, followed by the need for reduction. Conclusion: It seems necessary to minimize the difference between the curriculum and the clinical field in order to produce field-oriented dental technicians. To this end, it is necessary to reorganize the operation of field-oriented subjects and to increase the practice time to improve practical skills.

Translating Evidence into Practice in Low Resource Settings: Cervical Cancer Screening Tests are Only Part of the Solution in Rural India

  • Background: The majority of women in rural India have poor or no access to cervical cancer screening services, although one.quarter of all cervical cancers in the world occur there. Several large trials have proven the efficacy of low-tech cervical cancer screening methods in the Indian context but none have documented the necessary components and processes of implementing this evidence in a low-resource setting. Methods: This paper discusses a feasible model of implementation of cervical cancer screening programme in low-resource settings developed through a pilot research project carried out in rural Tamilnadu, India. The programme used visual inspection of cervix after acetic acid application (VIA) as a screening tool, nurses in the primary care centres as the primary screeners and peer educators within Self-Help Women groups to raise community awareness. Results: The uptake of screening was initially low despite the access to a screening programme. However, the programme witnessed an incremental increase in the number of women accessing screening with increasing community awareness. Conclusions: The investigators recommend 4 key components to programme implementation in low-resource setting: 1) Evidence-based, cost-effective test and treatment available within the reach of the community; 2) Appropriate referral pathways; 3) Skilled health workers and necessary equipment; and 4) Optimisation of health literacy, beliefs, attitudes of the community.

동영상 촬영과 피드백을 이용한 실습교육이 보건의료전공 대학생의 술기수행능력에 미치는 효과: 메타분석 (Effectiveness of Education Program Using Video Recording and Feedback on Skill Competency for Students of Majors in Health Care: A Meta-Analysis)

  • Purpose: A meta-analysis was conducted to evaluate effectiveness of education programs using video recording and feedback in the improvement of competency in clinical skills for health care majors. Methods: Six databases were searched and inclusion criteria were randomized controlled trials (RCTs) or non-randomized controlled trials (NRTs) reporting level of skill competency using numerical measurements. Data analysis and synthesis were performed using Comprehensive Meta-Analysis software and Revman program. Results: Of 1,568 records, 11 studies met inclusion criteria. Statistically significant effectiveness of education programs using video recording and feedback was identified. A low risk of bias was detected among both RCTs and NRTs. Meta-analysis showed that the intervention groups had more effective improvements in skill competency (standardized mean difference [SMD]: 0.74; 95% CI: 0.33~1.16). Results of subgroup analysis showed higher effects when interventions dealt with one skill, used self-reflection with expert feedback, and included instruction from instructor in the education programs. Conclusion: Findings suggest that schools for health care majors should actively adopt video and feedback based skill training allowing educators to design effective programs. Potential is higher for students to achieve higher competency when they train with one skill at a time, use of instruction and receive feedback from experts.

건강행위시행 변화단계에 따른 의사결정의 균형: 운동, 금연, 유방조영술 검진, 질회음근 강화운동을 중심으로 (A study of the Stage of Change and Decisional balance : Exercise Acquisition, Smoking Cessation, Mammography Screening and Kegel's Exercise Acquisition in Korea)

  • This study was carried out to assess the perception of decisional balance of Korean subjects about 4 health behaviors and to identify the influencing factor of decisional balance for exercise acquisition, smoking cessation, mammography screening and Kegel's exercise acquisition. All are representative health behaviors nurses can intervene in Korea based on the Transtheoretical model. Convenient samples of 2,484 subjects (191; exercise, 169; smoking cessation, 1903; mammography screening and 221; Kegel's exercise) were selected from cities and counties over 9 provinces throughout Korea, and the data was collected from January 1, 1999 to February 29, 2000. The research instrument were the Decisional Balance Measure for Exercise (Marcus & Owen., 1992), Smoking Cessation (Velicer et al., 1985), Mammography Screening (Rakowski et al.,1992) and Kegel Exercise (Lim, 1999) and Stage of Change Measure for Exercise (Marcus et al, 1992), Smoking Cessation (DiClemente et al., 1991), Mammography Screening (Rakowski et al.,1992) and Kegel's Exercise (Lim, 1999). The data was analyzed by the SAS Program. The results are as follows; 1. According to the stage of change measure, 2,484 subjects were distributed in each stage of change for four health behaviors: 1,233 subjects (49.8%), 745 subjects (30.2%), 113 subjects (4.7%), 156 subjects (6.5%), and 216 (8.7%) belonged to the pre- contemplation stage, contemplation stage, preparation stage, action stage and maintenance stage. They were all series of stages of change in their efforts to do health behavior. 2. Factor analysis identified 3 factors (1 of Pros, 2 of Cons) for the exercise, 4 factors for smoking cessation (2 of Pros, 2 of Cons), 2 factors (1 of Pros, 1 of Cons) for the mammogram screening and 2 factors (1 of Pros, 1 of Cons) for Kegel's exercise of decisional balance. 3. The analysis of variance and multiple comparison analysis showed that for all 4 samples, the Cons of changing the problem behaviors outweighed the Pros for subjects who were in the pre- contemplation stage, The opposite was true for subjects in action and maintenance stage. 4. Through the discriminant analysis, it was found that one factor of Pros for exercise, one factor of Cons for smoking cessation, 1 factor of Cons for mammogram screening and one factor of Cons for Kegel's exercise were the more influencing factors, than others in discriminating the stages of change. Results are consistent with the applications of the Transtheoretical model, which have been used to understand how people change health behaviors. This results provide some evidence that subject's report of his/her health behavior corresponds to beliefs about usefulness of related health behaviors. The results of this study have implications for patients' health education and health intervention strategies. The findings of this study give useful information for nursing educators for 4 health behaviors, especially the factors relating to decision making in the different stages of change.

치과위생사의 업무 실태 및 비중에 관한 연구 (A Study on the Current State and Weight of Dental Hygienists' Works)

  • With the change of medical environment, people are taking interest not just in treatment but in the prevention of oral diseases. As people's interest in oral diseases increases, the number of dental institutions is increasing continuously and this is heightening the necessity and importance of professional dental hygienists equipped with expert knowledge of oral health. Thus the present study purposed to survey and analyze the current state and weight of works carried out by dental hygienists, and to propose dental hygienists' works desirable in the medical environment changing its focus from treatment to prevention. The results of this study are as follows; 1. In the results of dividing dental hygienists' works into oral examination and preliminary examination, preventive works, assistance to dental treatment, oral health education, hospital administration and management, and surveying the performance rate of these works, the work of the highest performance rate in preliminary examination was collection of patients' systemic history (87.6%). It was also scaling (96.9%) in preventive dental treatment, preparation for treatment (96.5%) in assistance to dental treatment, education on the toothbrushing method (92.3%) in oral health education, and tool disinfection and management (72.2%) in hospital administration and management. 2. When the performance rate was surveyed according to age, institution, and work experience, the performance rate by work area was highest in dental hygienists aged between 27~28, those working at a dental clinic, and those with 4~6 years' work experience. 3. The weight of dental hygienists' works was high in order of assistance to dental treatment (59.1%), observation of oral health condition and preliminary examination (12.4%), oral health education (12.4%), hospital administration and management (10.4%), and preventive dental treatment (5.8%), but in the results of surveying perception on the importance of works, the importance was high in order of oral health education (34.7%), preventive dental treatment (29.0%), observation of oral health condition and preliminary examination (19.3%), assistance to dental treatment (11.2%), and hospital administration and management (5.8%), showing that the weight of works carried out current was different from perception on the weight of works regarded as important. 4. To the question on parts to be developed in response to the change of medical environment, the most frequent answers were the establishment of their roles as preventive dental practitioners (75.7%), and the establishment of their roles as oral health educators (74.9%). This was consistent with the works that dental hygienists gave the highest weight to, and suggested that dental hygienists regarded preventive dental treatment as most important. Development of abilities to give counsel to patients was 55.2%, acquisition of theoretical knowledge of dental treatment 42.1%, improvement in quality as a hospital manager 28.2%, and acquisition of skills to assist dental treatment 23.2%. These show that, even with regard to the development of future oriented dental hygienists' capacities, assistance to dental treatment was perceived less important as it was with regard to the importance of work.
